3.Synaptic connectivity and ultrastructrue of the masseteric muscle spindle afferent trigeminals according to geometric location in the trigeminal motor nucleus of the cat.

The purpose of this study was to investigate any relationship between the geometric factors of synaptic contacts of muscle spindle afferent terminals and masseteric motor neurons in the trigeminal motor nucleus. Terminals from the masseteric muscle spindle afferents were stained with intra-axonal injection of HRP and were examined electronmi-croscopically with serial sections at the central and peripheral regions of trigeminal motor nucleus of the cat. The number of terminals examined were 76 in peripheral and 105 in central region. The results obtained were as follows. 1. Most of the labeled terminals showed simple synaptic connectivity. Each terminals in peripheral and central region made synaptic contact with 1 to 5 neuronal profiles. Two or three labeled terminals were occasionally seen to make synaptic contact with the same dendrite. 2. The average number of postsynaptic proximal dendrite per labeled terminal was higher in the central region than in the peripheral region. In contrast, that of postsynaptic distal dendrite per labeled terminal was higher in the peripheral region than in the central region. 3. The average diameter of postsynaptic dendrites in the central region was larger than that in the peripheral region. This imply terminals in the peripheral region contacted with further distal part of the distal dendrite than that in the central region. These results indicate that synaptic connectivity associated with the spindle afferents from masseteric muscle is different according to their geometric location within the trigeminal motor nucleus and suggest that there will be precise interrelationship between the morphology, pattern of synaptic connectivity and functions of muscle spindle afferents.

5.Locally Infiltrative Glomus Tumor: A case report.

Malignant glomus tumor is a rare neoplasm arising from the glomus body, comprising categories of locally infiltrative glomus tumor, sarcoma arising in benign glomus tumor and de novo glomangiosarcoma. We report one case of locally infiltrative glomus tumor arising in nail bed of the right thumb of a 50 year-old woman. The tumor showed the architectural features of glomus tumor but cytologically bizarre, pleomorphic features without mitosis and infiltrative growth pattern to adjacent connective tissue and nerve. Immunohistochemical results were consistent with glomus tumor. This category of malignant glomus tumor must be distinguished from other round cell tumors because of its good prognosis.

Iridocorneal endothelial syndrome is a disease characterized by corneal endothelial degeneration, corneal edema, peripheral anterior synechia, iris stromal change, and secondary glaucoma. This syndrome is considered to have three major variations: Chandler's syndrome, progressive (essential) iris atrophy, and Cogan-Reese syndrome. We experienced three cases of iridocorneal endothelial syndrome with transient intraocular pressure elevation.
9.A Case of Primary Cutaneous Mucinous Adenocarcinoma.

We report a case of primary cutaneous mucinous adenocarcinotna in a 64-year-old female. The tumor was about 2cm in size, forminga round dome-shaped alopecic scalp mass which had gradually increased in size over about 5 years, Histalogic examination revealed that the tumor was divided into numerous compartments by strands of fibrous tissue. In each compartment, abundant amounts of pale-staining mucin surrounded nests or cords of moderately anaplastic epithelial cells. We couldn't find any evidence of internal neoplasms as a source of metastasis. We resected the tumor with approximately a 1 cm margin and then performed a split thickness skin graft.

10.Heart Rate Variability and Lipid Profile in Patients with Major Depressive Disorder.

OBJECTIVES: The analysis of heart rate variability (HRV) is a useful non-invasive tool to investigate the autonomic nerve function. Previous studies on the relationship between HRV and depression have been reported controversial results. Similarly, the correlation between the serum lipids and depression is debatable. The purpose of this study is to examine the relationship between heart rate variability, lipid profile and depression. METHODS: A total of 42 patients with major depressive disorder (MDD) and 32 age and sex-matched normal subjects who had no previous history of major medical and mental illnesses were recruited for this study. A structured-interview was used to assess the general characteristics and psychiatric illness. HRV measures were assessed by time-domain and frequency-domain analyses. Psychological symptoms were measured using the Hamilton rating scale for anxiety (HAM-A), Hamilton rating scale for depression (HAM-D). In addition, the evaluation for lipid profile was performed by blood test. RESULTS: In serum lipid profile test, MDD group showed higher cholesterol (197.68+/-42.94 mg/dL vs. 176.85+/-34.68 mg/dL, p=0.044), TG (139.45+/-92.54 mg/dL vs. 91.4+/-65.68 mg/dL, p=0.018), LDL (130.03+/-33.18 vs. 106.62+/-27.08, p=0.004) level than normal control group. In HRV time domain analyses, the standard deviation of the NN interval (SDNN) was decreased in MDD group than normal control group, but was not significant (32.82+/-14.33 ms vs. 40.36+/-21.40ms, p=0.078). ApEn (Approximate Entrophy) was significantly increased in MDD group than normal control group (1.13+/-0.11 vs. 0.91+/-0.18, p<0.001). ApEn was correlated with LDL level (r=0.277, p=0.028), HAM-D scores (r=0.534, p<0.001) and HAM-A scores (r=0.470, p<0.001). CONCLUSION: MDD patients showed increased ApEn, one of the HRV measurement. And this ApEn was correlated with LDL, HAM-D and HAM-A scores. In this study, the analysis of ApEn would be a useful test of MDD.
