5.Effect of Various Fungi on the Aflatoxin Productivity in the Culture of Asp. Flavus.

This study was attempted to know that the interactions of various fungi, and methionine and MgSO4 introduced as the substrate of culture media for fungi were affected to produce aflatoxins by Asp. flavus. 5 different fungi were isolated from the fermented soybean mash and were cultured in Chemically Defined medium (C.D. media) and soybean mash at 25 degrees for 10 days. (1) It was confirmed that Asp. flavus produced aflatoxins in the C.D. medium and soybean mash, but that Asp. niger, Asp. oryzae, Asp. awamori and Asp. terreus did not produced them respectively. (2) Asp. flavus cultured with Asp. niger did not produce aflatoxins in C.D. medium, but produced in soybean mash, in other hand, Asp. flavus with other fungi except Asp. niger produced aflatoxins in C.D. medium and soybean mash. (3) The growth of fungi were more prosperous in the separate culture than in the mixed culture. (4) In the C.D. medium added 20% of cultured medium of Asp. niger, Asp. flavus did not produce aflatoxins but other cultured medium did not prohibit the production of aflatoxins by Asp. flavus. (5) On the contrary, MgSO4 increasing the productivity of aflatoxins by Asp. flavus in the C.D. medium methionine known as one of precurser of aflatoxins did not affected the increasing productivity with significance.

6.Effect of Mirtazapine on Memory Function of Patients with Posttraumatic Stress Disorder.

OBJECTIVE: This study was conducted to evaluate efficacy on symptoms and memory function of mirtazapine treatment of posttraumatic stress disorder. METHODS: Thirty Vietnam veterans were collected for this study, among whom fifteen were PTSD patients and fifteen were combat control subjects. We used Mississippi Scale for CombatRelated PTSD, Combat Exposure Scale (CES), Hamilton Depression Rating Scale (HDRS) and Clinician Administered PTSD Scale (CAPS). Digit Span, Paired Association Learning Test (PALT) and Rey-Osterrith Complex Figure Test (CFT) were assessed for memory function. We also evaluate HDRS, CAPS and memory function tests intervals of baseline, 2 week and 6 week in PTSD patients during mirtazapine treatment. RESULTS: There were significant differences between PTSD and Non-PTSD veterans in Mississippi Scale for Combat-Related PTSD, CES, HDRS and CAPS. Significant difference was found in memory function tests between PTSD and Non-PTSD veterans. PTSD veterans showed significant improvement in HDRS and CAPS at 2 week and 6 week during mirtazapine treatment and in memory function tests at 6 week during mirtazapine treatment. There was no significant correlation between symptoms and memory function. CONCLUSION: These results suggest that mirtazapine improve symptoms and memory function of PTSD. There was no significant correlation between PTSD sysmtoms and memory function.

7.Clinical Effectiveness and Nephrotoxicity of Aerosolized Colistin Treatment in Multidrug-Resistant Gram-Negative Pneumonia.

BACKGROUND: Colistin (polymyxin E) is active against multidrug-resistant Gram-negative bacteria (MDR-GNB). However, the effectiveness of inhaled colistin is unclear. This study was designed to assess the effectiveness and safety of aerosolized colistin for the treatment of ventilator-associated pneumonia (VAP) caused by MDR-GNB. METHODS: In this retrospective longitudinal study, we evaluated the medical records of 63 patients who received aerosolized colistin treatment for VAP caused by MDR-GNB in the medical intensive care unit (MICU) from February 2012 to March 2014. RESULTS: A total of 25 patients with VAP caused by MDR-GNB were included in this study. The negative conversion rate was 84.6% after treatment, and acute kidney injury (AKI) occurred in 11 patients (44%, AKI group). The average length of MICU stay and colistin treatment- related factors, such as daily and total cumulative doses and administration period, were not significantly different between groups. In-hospital mortality tended to be higher in the AKI group (p = 0.07). Multivariate analysis showed that a body mass index less than 18 was an independent risk factor of mortality (odds ratio [OR] = 21.95, 95% confidence interval [CI] 1.59-302.23; p = 0.02). Notably, AKI occurrence was closely related to the administration of more than two nephrotoxic drugs combined with aerosolized colistin (OR = 15.03, 95% CI 1.40-161.76; p = 0.025) and septic shock (OR = 8.10, 95% CI 1.40-161.76; p = 0.04). CONCLUSIONS: The use of adjunctive aerosolized colistin treatment appears to be a relatively safe and effective option for the treatment of VAP caused by MDR-GNB. However, more research on the concomitant use of nephrotoxic drugs with aerosolized colistin will be necessary, as this can be an important risk factor of development of AKI.

Short umbilical cord syndrome, also known as the limb-body wall malformation complex and the body stalk anomaly, is a poorly defined sporadic group of congenital anomaly charaterized by a complex set of disruptive abnormalities having in common the failured closure of the ventral body wall. This disorder is charaterized by a short or absent umbilical cord and disruption of the lateral body wall, spine, limbs, face, and cranium, isolated or in combination. Recently, we present a case of short umbilical cord syndrome which found in a term baby, so we report a case of short umbilical cord syndrome with brief review of literature.

10.A Case of the Foutrh Branchial Cleft Cyst Causing Respiratory Difficulty in Neonatal Period.

It is commonly believed that the branchial anomalies are persistent remnants of the embryologic branchial apparatus. These anomalies are originated from branchial cleft and most commonly involved in second branchial deft. Clinically they present palpable mass, discharge from fistula, recurrent infection due to obstruction of Gstula tract. As respiratory symptoms, they may present hoarseness, preceeded by or associated with upper respiratory infection. However, it has not been reported that they caused airway obstruction in newborn infants. Wc experienced a case of the fourth branchial deft cyst in a 2 day old infant who showed respiratory difficulty due to airway obstruction.
