1.Tibia Fracture on Removal of Interlocking Medullary Nail: A report of 3 cases.

Interlocking medullary nails have been widely used in the treatment of long bone fractures especially in tibia and femur. But with increasing use of nails, many preoperative, intra-operative and post-operative complications have been reported. We have experienced 3 cases of very rare complication, fracture of tibia shaft which occurred during removal of interlocking medullary nail after complete healing of original tibial fracture. We report these rare complications with literature review.

2.Impairment Grading in Orthopedic Certificates.

In orthopedic practice, patients sometimes ask certain kinds of medical documents certifying their disability grading. But many orthopedic surgeons are not accustomed to issue such medical certificates. So we want to review the way of evaluation and grading of disabilities for such different kinds of medical certificates(for examples: disability grading certificate for use in traffic accident, industrial accident, liability of national compensation, private insurance, the handicapped, military service and others) and compare the differences between them.

3.Clinical Results of Supracondylar Dome Osteotomy for Cubitus Varus and Valgus Deformities in Adults.

BACKGROUND: Cubitus varus and cubitus valgus deformities are common complications of distal humeral fractures in children. We evaluated the usefulness of supracondylar dome osteotomy as a treatment option for adults with cubitus varus or valgus deformity developed during childhood. METHODS: Ten patients who had received supracondylar dome osteotomy and stabilization with plates to treat cubitus varus or valgus deformity between July 2006 and August 2013 were included in this study. Their mean age at the time of surgery was 36.50 ± 10.22 years. The mean follow-up duration was 54.80 ± 32.50 months. We evaluated humerus-elbow-wrist angles (HEWA), improvements in the lateral prominence index (LPI) or medial prominence index (MPI), Mayo elbow performance scores (MEPS), and overall results in accordance with the Banerjee criteria. RESULTS: For the six patients with cubitus varus, the mean postoperative HEWA, mean correction angle, and mean improvement in LPI were 9.72°± 3.95°, 27.67°± 10.75°, and 6.92% ± 3.40%, respectively. For the four patients with cubitus valgus, the mean postoperative HEWA, mean correction angle, and mean improvement in MPI were 14.73°± 2.97°, 11.55°± 3.26°, and 11.33% ± 6.39%, respectively. There was no significant difference between postoperative and preoperative mean MEPS. The subjective ulnar nerve symptoms were alleviated in all patients. The overall results were excellent in six and good in four patients. CONCLUSIONS: This study suggests that supracondylar dome osteotomy with secure fixation using double plates may be useful in correcting cubitus varus or cubitus valgus deformity, yielding good functional outcomes in adults.

4.Succinylcholine Induced-Ocular Position under the General Anesthesia.

Succinylcholine, 1.5mg/kg, was injected preoperatively and postoperatively under the general anesthesia in 21 strabismus patients. We measured ocular position induced by succinylcholine under the general anesthesia and compared it with ocular position of conscious state from photograph. Regression analysis was performed to find the predictive relationships between preanesthetic and preoperative succinylcholine-induced ocular position and postoperative succinylcholine-induced ocular position and ocular position at postoperative 1 week. The results were as follows: 1) X: Preoperative succinylcholine induced ILD(interlimbal distance) under the anesthesia. Y: Preanesthetic ILD. Y=0.71X+10.6, R=0.82(p<0.025). 2) X: Postoperative succinylcholine induced ILD. Y: Postoperative 1 week ILD. Y=0.76X+9.14. R=0.74(p<0.005). The succinylcholine induced-ocular positions were highly correlated with those of conscious state.

5.Microorganisms in Conjunctival Sac, Lacrimal Sac and Nasal Cavity.

Microorganisms of conjunctival sac, lacrimal sac, and nasal cavity were studied in 86 patients with obstruction in the lacrimal system. We analyzed the data of bacteriologic culture and its antibiotic sensitivity. The results were as follows; 1. Staphylococcus epidermidis was the most common microorganism in conjunctival sac, lacrimal sac, and nasal cavity. 2. There was no correlation among the organisms in conjunctival sac, lacrimal sac, and nasal cavity. 3. Staphylococcus epidermidis and Staphylococcus aureus were most sensitive tocephalothin and cefatoxim.

6.A Cystic Mesothelioma in the Inguinal Area.

The cystic mesothelioma is a very rare tumor which has a clinically and histologically benign nature. Here in reported is the case of a cystic mesothelioma presented as a palpable mass of the inguinal area in a 28-year-old male. Ultrasound showed a cystic tumor at the inguinal canal, and the other physical and laboratory examinations were within normal limits. Grossly, the tumor consisted of cysts containing clear serous fluid and focally solid areas. Microscopically, the tumor was encapsulated with fibrocollagenous wall, and the tumor cells were cuboidal or polygonal epithelial cells with single or multiple layers and had clear cytoplasm. Some areas showed thyroid follicle-like structures. The content of follicle-like structures showed eosinophilia in the H&E section, but positive in mucin stain. Neither cytologic atypia nor mitoses were present. Immunohistochemical staining revealed positive reaction for keratins of low molecular weight, while negative for the thyroglobulin and CEA. These findings suggested mesothelial in origin. We concluded that this tumor was primary rather than metastatic, because he had no evidence of a tumor in gastrointestinal, genitourinary tracts and scrotum.

9.Two Cases of Excyclotropia in Superior Oblique Palsy Treated with Barada-Ito Operation.

Anatomic torsion of globe around the sagittal axis occurs in a large variety of strabismus conditions which are infinitively more common than paralysis of cyclovertical muscles. The anterior half of the superior oblique fibers are probably more effective intorters. The posterior superior oblique fibers are more effective depressors. Anterior shift of the superior oblique tendon has been advocated for treatment of torsional aspect of the superior oblque palsy. We experienced two cases of symptomatic excyclotropia in unilateral superior oblique muscle palsy without substantial vertical deviation. We operated paretic superior oblique muscle with Harada-Ito operation which is anterior and lateral displacement of the anterior fibers of the superior oblique tendon. In two patients with unilateral superior oblique palsy, the excyclotorsional deviations were reduced to 15 degrees to 2 degrees of excyclotorsion in primary position of the gaze and from 15 degrees to 2 degrees of excyclotorsion in downward gaze. In another case the cyclodeviation was reduced from 7 degrees to 0 degree in downward gaze.

10.Intervention of Nonsuicidal Self-Injury in School Counseling

Recently, nonsuicidal self-injury (NSSI) has been increasing considerably in school counseling. According to the Youth Counseling and Welfare Development Institute, the number of counseling support for NSSI adolescent in 2018 was more than three times higher than the previous year, and the average time when NSSI behavior first appeared was 12.43 years old. As such, adolesnect NSSI continues to increase in recent years, and the age is also gradually decreasing. Nevertheless, research on the motives and characteristics of NSSI among domestic adolescents is still insufficient. This may be attributed to the difficulty of conducting research because self-injury behavior is secretly performed. However, it is also true that this reality has another limitation in the school field, which urgently requires counseling intervention and prevention of students' mental health problems. In addition, counseling for self-injury in the school scene is the biggest cause of exhaustion for counselors because they are under very great stress to cope with repeated self-harm in the dual role of counselors and teachers in school situations. Therefore, this study examines the causes and characteristics of involuntary self-injury through previous research analysis, and examines various difficulties experienced as a school counselor, as well as the effective intervention method of non suicidal self-injury in school counseling.
