Numerous cause of the carpal-tunnel syndrome have been reported, but we could not find a reference for tuberculosis of the median nerve causing this syndrome. Therefore, we are presenting the case report of this rare condition as another cause of the carpaltunnel syndrome.

2.Monostotic Fibrous Dysplasia of the Cervical Spine: A Case Report

Fibrous dysplasis is a condition characterized by the metastatic replacement of the medullary component of predominantly long bone with fibrous tissues. This change is responsible for the characteristic features, including bony abnormalities, pathologic fractures and pain. The monostotic form of the fibrous dysplasia occurs more freguent than the polyostotic form. The monostotic fibrous dysplasia does not exhibit the laboratory abnormality, extra-skeletal manifestation or the endocrinopathies associated writh polyostotic disease. Monostotic fibrous dysplasia occurs most frequently in the metaphysis of long bones and the ribs. Only the seven previous cases of vertebral involvement (three cervical, one thoracic, three lumbar spine) in monostotic fibrous dysplsia have been reported. Authors experienced a case of monosotic fibrous dysplasia of the seventh cervical vertebrate, so we report a case with the review of the seven previous reported cases of the monosotic fibrous dysplasia of the spine.

3.A Clinical analysis of the Fracture of the Humeral Shaft

The humerus is anatomically and phyelologically unique, such as, firstly, it is a non weight bearing bone, secondly, it has greatest range of motion, thirdly, when at rest while the person is standing, the axis of bone hangs vertically and is influenced by gravity, and conservative methods usually used in treatment. However, in the event of closed reduction failure, open fracture, multiple fracture, and old age, operative method may be employed. Authors reviewed and analzed 84 humeral shaft fractures which had been treated at the Department Orthopedic Surgery, Paik Hospital from Dec. 1979 to May 1988. The results were as follows : l. Among the 84 patients, male were 63 and female were 21. 2. The major causes of injury were a traffic accident. 3. The most frequent type and site of fracture was comminuted and middle one third. 4. The incident of radial nerve injury was 17 cases. 5. Of 84 cases, 22 cases were treated conservatively and 62 cases operatively. 6. The most common complication was non-union of 14 cases, of which 10 cases were treated with plate and bone graft and 4 cases with only bone graft.

4.Comparison of Effects and Side Effects of Amphotericin B Deoxycholate Infusion over 6 versus 24 Hours in Children with Cancer.

PURPOSE: This study was performed to compare the effects and side effects of amphotericin B deoxycholate infusion over 6 versus 24 hours in febrile children with cancer. METHODS: Twenty-four children with cancer who were mostly neutropenic, febrile and suspected to have fungal infection and treated with amphotericin B deoxycholate from March 2001 to July 2002 at the Yeungnam University Hospital, Department of Pediatrics were enrolled for a cross-over study. All patients received 0.5 mg/kg amphotericin B deoxycholate infusion for average 7 days by two different infusion rates, one group by continuous infusion over 24 hours and the other by rapid infusion over 6 hours with at least 4 weeks of washout period. The side effects related to the infusion rates, fever, chilling or rigor, vomiting, headache, serum and urine electrolytes, beta2-microglobulin and creatinine clearance were monitored. The effect of amphotericin B deoxycholate infusion were studied by the days required for defervescence. RESULTS: There were no significant differences (P=0.11) in the effects between continuous and rapid infusion. But less side effects of chilling or rigor (P=0.01) were observed when amphotericin B deoxycholate was infused continuously. The serum phosphate (P=0.05) and magnesium (P=0.04) were lower and creatinine clearance was more reduced (P=0.01) when it was infused rapidly. CONCLUSION: Continuous infusion over 24 hours of amphotericin B deoxycholate could reduce the nephrotoxicity, such as hypophosphatemia, hypomagnesemia, low creatinine clearance and chilling reactions related to the rapid infusion without compromising the effects of amphotericin B deoxycholate in children with cancer.

5.Prevalence of Obesity and Cardiovascular Risk Factors in Survivors of Childhood Acute Lymphoblastic Leukemia.

PURPOSE: This study was performed to survey the prevalence of obesity, hypertension, hyperglycemia and dyslipidemia in survivors of childhood acute lymphoblastic leukemia (ALL). METHODS: Retrospective Cohort study of 45 subjects (median age, 15.3 years; median interval after completion of therapy, 5 years) who were diagnosed and treated with ALL and 30 subjects (median age 14 years; median interval after completion of therapy, 5.3 years) with other cancers (control group) at Yeungnam University Hospital from January 1983 to December 1998 was performed. The ratio of male to female was 1.4: 1 in the ALL group and 1.7: 1 in the control group. Body mass index (BMI), blood pressure, fasting serum glucose and lipids were analyzed. Fourty three of the 45 subjects (96%) with ALL received cranial irradiation and 30 control subjects did not. RESULTS: Only two of 45 (4.4%) were found to be obese (BMI < 95 percentile for age and sex) which was similar to the control group (2/30 subjects, 6.7%). Hypertension was found in only 1 subject (1/45 subjects, 2.2%) with ALL. Dyslipidemia (at least one of high cholesterol, high triglyceride or low high density lipid-cholesterol) was detected in 12 of 39 subjects (30.7%) with ALL and in 8 of 22 subjects (36.4%) with other cancers. No one of either group had hyperglycemia. CONCLUSION: Young survivors of childhood ALL do not seem to be at increased risk for obesity, hypertension and hyperglycemia, but seem to be at increased risk for dyslipidemia.

6.A Case of Multiple SCCs in situ and BCC Induced Probably by Chronic Exposure to Arsenic-containing Pills (Hwan-Yak).

Not legally, arsenic has been used for various medical purposes. Chronic exposure to arsenic can induce different kinds of cutaneous diseases, including arsenic keratosis, Bowen's disease, squamous cell carcinoma (SCC), basal cell carcinoma (BCC), and other pigment disorders. Also, arsenic induces internal malignancies, including tumors of the pulmonary and urogenital systems. To prevent these malignancies, the use of arsenics should be strictly prohibited. We describe a 72-year-old man who developed SCC in situ and BCC after chronic exposure to arsenics.

7.Adult Onset Still's Disesae: A Case Report.

Adult-onset Still's disease (AOSD) is a systemic inflammatory disease of an unknown etiology, and AOSD is characterized by high fever, skin rash, arthritis, lymphadenopathy, hepatomegaly, sore throat and serositis. The typical skin rash of AOSD patients is an evanescent, erythematous maculopapular lesion. There is no single diagnostic test; therefore, the diagnosis is based upon the clinical criteria. Treatment consists of anti-inflammatory medications such as aspirin, NSAIDs, corticosteroids, anti-rheumatics and immunosuppressants. We report here on a case of AOSD in which a 53 year-old female patient presented with a high fever, a typical rash, arthritis, a sore thoat and mild splenomegaly.

8.Complete Repair of Coarctation of the Aorta and a Ventricular Septal Defect in a 1,480 g Low Birth Weight Neonate.

Although outcomes of neonatal cardiac surgery have dramatically improved in the last two decades, low body weight still constitutes an important risk factor for morbidity and mortality. In particular, cardiac surgery in neonates with very low birth weight (< or =1.5 kg) is carried out with greater risk because most organ systems are immature. We report here on a successful case of early one-stage total repair of coarctation of the aorta and a ventricular septal defect in a 1,480 gram neonate.

10.Two Cases of Pneumocystis Carinii Pneumonia in Renal Allograft Recipients.

Renal allograft recipients are at risk for Pneumocystis carinii pneumonia (PCP) within the first year following transplantation and during treatment for graft rejection. We experienced two cases of PCP in renal allograft recipients. The first case was a 39-year-old female who had received renal allograft 7 years before. At the time of traosplantation, she was a carrier of hepatitis B surface (HBs) antigen. After transplantation, she had been received the rnaintenance dose of cyclosporine and oral prednisolone. Three months before adrnission, dosage of prednisolone was increased because of the increased serum creatinine level and gene-ralized edema. A week before admission, syrnptom of exertional dyspnea, dry cough, and fever was developed. Chest X-ray film showed streaky interstitial infiltration in both lung fields and chest CT showed diffuse ground-glass appearance. Rroncho- alveolar lavage revealed positive Grocott's methenamine silver stain for numerous clumps of pneumocystis carinii cysts. Despite the aggressive treatment, she died of respiratory and hepatic failure and GI bleeding. Another case was a 40-year-old male who had received renal allograft S years before. He had been received maintenance immune suppressive therapy with cyclosporine and oral prednisolone. He was admitted for evaluation of hypertension and elevated serum creatinine level. After several days of admission, he complained fever, dry cough and dyspnea. X-ray film showed pneumonic infiltration and the bronchial brushing and washing fluid revealed the Pneumocystis carinii cysts that were stained by methenamine silver. He was treated with the full dose of trimethoprim-sulfamethoxazole and clindamycin. Sacrificing the renal allograft, he recovered from Pneumocystis carinii pneumonia.
