1.Clinical Observation on Female Urethral Injury.

Female urethral injury are very uncommon and encountered during traffic accident, trauma, instrumentation, operation and delivery. Treatment modalities are determined by the level of urethral injury. The result was better in the cases of rapid operative treatment than delayed one. In this report the clinical findings, patterns of injury and surgical managementof 4 cases were reviewed.

2.Heart Diseases in Korea.

Statistical observation was made on 2,639 cases with heart disease which has been seen in the Deparment of Internal Medicine, National Medical Center, in Korea, during the past 15 years ranging from January 1959 to December 1973. The results were as follows; 1. During the last 15 years, 2,639 cases with heart disease were observed, which represents about 12.9% of total 20,434 medical In-patients, and variable incidence for the consecutive years was noted. 2. The disease occurred most frequently in group aged under thirty and its incidence decreased in the fifth and sixth decades. 3. The incidence in male was 49.8% and in female 50.2%, the ratio of which was almost same. 4. The most common etiologic form of heart disease was hypertensive heart disease which occurred in 43.2% of all cardiac patients, corresponding to 5.57% of all In-patients. The incidence was most frequent in the sixth decade and decreased in the fifth and fourth decades with the male preponderance. 5. The second common heart disease was rheumatic heart disease, 31.6% of all cardiac patients, corresponding to 4.08% of all In-patients. It affected the mitral valve in most of the cases, whereas involvement of the aortic valve was rare. 6. Coronary heart disease, the third common heart disease, occurred in 5.1% of all cardiac patients, and most frequently in sixth and seventh decades. it affected male 2.5 times more frequently than female. 7. Postpartum heart failure and beriberi heart disease showed 3.8% and 3.1% respectively. They occurred more frequently in younger female, but the frequency was much decreased recently. 8. Pericarditis occured in 2.6% of all cardiac patients and tuberculosis was still a common cause of pericarditis. 9. Congenital heart disease was seen in 1.7% of the cases and patent ductus arteriosus, tetralogy of Fallot, atrial septal defect and ventricular septal defect occupied 76% of all congenital heart disease. 10. Cor pulmonale was present in 1.6%, arrhythmia 1.3%, cardiomyopathy 1.3%, thyroid heart disease 0.8%, endocarditis 0.7% and syphilitic heart disease 0.2%.

3.REDUCTION MAMMOPLASTY USING CENTRAL PARENCHYMAL PEDICLE WITHOUT VERTICAL SCAR.

There are several factors to be considered while performing reduction mammoplasty in patients having giant breast with severe breast ptosis. These are degrees of breast ptosis, location of newly created nipple areolar complex, preservation of sensation, symmetry of both breasts, amount of breast tissues to be resected, and finally the postoperative scar. Among the above factors, postoperative scar seems to be the major problem that patients complain about. So several methods have been investigated in time to minimize the scar. Authors have performed 3cases of reduction mammoplasty in patients with severe ptotic large breast by applying the method of umbilicus creation in abdominoplasty, so that only periareolar and submammary scars remained without a vertical scar. The age were between 33 and 43 years old. Mean follow up period was about 6.5 months. Amount of the resected mammary tissues was about 750-1350g. Periareolar deepithelialization was made about 8cm diameter for better cirulation to the nipple areolar complex and wider contact area thus enhancing the stabilization and survival of the nipple areolar complex. Only 1 case with partial deepithelialization of the areola occurred but completely healed within 1 month. Patients were pleased with natural shape and size of the breast and the sensation of the nipple areolar complex was restored completely in all cases after surgery. Authors have performed reduction mammoplasty without leaving the vertical scar which considered to be the major problems of the classical methods. Other problems such as insufficient reduction, flattening of the breast and severe wrinkles around the nipple were also solved. This method not only improve the aesthetic importance but also minimize the functional complications such as sensory loss and difficulty in lactation.

4.A case of breast sparganosis.

A 29-year-old Korean woman visited the Department of Surgery in MizMedi Hospital with a palpable itching mass on the right breast that had existed for the past 7 months. She had no history to eat either frogs or snakes, but had the history of drinking impure water. Sonography revealed a serpiginous hypoechoic tubular structure associated with partial fat necrosis in breast parenchymal layer and subcutaneous fat layer. It also revealed oval cystic lesions. At operation, an ivory white opaque ribbon-like worm that measured 16.5 cm in length and 0.5 cm in width was extracted. Anti-sparganum specific serum IgG level in the patient's serum (absorbance = 0.71), measured by ELISA, was found to be significantly higher than those of normal controls (cut off point = 0.21). Sonography and ELISA appear to be helpful to diagnose sparganosis. Breast sparganosis is rarely found throughout the world.

5.Application of pulsed Doppler ultrasound for the evaluation of small intestinal motility in dogs.

The purpose of this study was to verify whether small intestinal peristalsis could be observed and quantitatively assessed using pulsed-Doppler ultrasound. Pulsed-Doppler ultrasound was used to evaluate small intestinal peristalsis after a meal in ten normal dogs and ten sedated dogs. The small intestinal peristalses were measured 0, 1, 3, 6, 9, 12, and 24 hours after a 24-hour fast and after feeding. The number of small intestinal peristalsis were 0.133/min, 0.100/min, 0.033/min, 0.167/min, 0.070/min, 0.067/min, and 0.100/min in the fasted dogs, and 1.667/ min, 0.933/min, 1.133/min, 1.234/min, 1.933/min, 1.533/ min, and 0.533/min in fed dogs, respectively. In the dogs sedated with xylazine HCl, the number of small intestinal peristalsis was significantly reduced (p<0.01). However, in the dogs treated with ketamine HCl and acepromazine, the number of small intestinal peristalsis remained unchanged. Therefore, it can be concluded that pulsed-Doppler ultrasound allows graphic visualization of the intestinal movements, which can be subjected to qualitative and quantitative analysis, and may be suitable for a non-invasive study of small intestinal motility.

7.The Efficacy of Emergency Computed Tomography in Patients with Chest Trauma.

BACKGROUND: Trauma is the third leading cause of death, irrespective of age, and the leading cause of death in persons under 40 years of age. Computed tomography (CT) is an effective technique in the initial emergent evaluation of the abdomen and head following blunt trauma. Most chest indies are not seen on conventional chest radiography, or may be underestimated. But routine use of CT in the initial emergent evaluation of chest trauma is controversial. CT, however, has been shown to be useful in the diagnosis of unsuspected chest injuries and in directing therapeutic interventions. This review discusses the efficacy of emergent CT in patients with blunt trauma on the chest. METHODS: To evaluate the efficacy of CT of the thorax, a retrospective study comparing early thoracic CT scanning with initial chest roentgenogram(CXR) was carried out on 121 patients with blunt trauma on the chest. The interval between CXR and CT was less than 1 hour. RESULTS: Among 121 patients, 42 patients had normal initial chest roentgenogram in whom 19 patients showed normal CT findings. Sensitivities of diagnosing pneumothorax and pleural effusion by CXR were low (49.3%and 45.1%respectively), whereas 65.7% (44/67) of patients had thoracostomy only by CXR. CONCLUSIONS : Although sometimes abused in patients with chest trauma, CT of the thorax is an effective method of detecting thoracic injuries and provides accurate information regarding their pattern and extent in the initial emergent diagnosis.

8.A Statistical Study of Pediatric Dermatoses During Last 10 Years (1975 ~ 1984).

11,609 of childhood new outpatients seen in Department of Dermatology of Seoul National University Hospital from 1975 to 1984 were analyzed statistica,lly. The study results were summarized as follows: l. 11, 609 out of 48, 188 new outpatients were under 15 years old age, amounting to 24. l %. 2. The 10 most common dermatoses were atopic dermatitis, vitiligo, urticari, hemangiorna, contact dermatitis, fungal disease, molluscum contagiosum, scabies, impetigo and nevocellular neuvs. The most freguent age groups were 2 to 6 years amounting to 47 2p, af total outpatients. 4. The peak incidence occurred in August, while the lowest incidence occurred in November and October. 5. Dermatoses which showed increasing tendency were nevocellular nevus and vitiligo, those of decreasing tendency were contact dermatitis and impetigo,

9.A Case fo Generalized Syringoma.

Syringoma is not a uncommon tumor of the skin. It is a benign adenoma, of intraepidermal eccrine ducts. It occurs predominantly in women at puberty or adolescence. The lesions usually are multiple and small, skin colored or slightly yellowish payules, usually only 1 or 3mm in size. The lesions usually are limited to the lower eyelids but may occur on cheek, axilla, abdomen, neck, anterior chest and vulva. Rarely these are generalized in distributior. A 23 years old healthy man has had widespread lesions over the face, anterior chest and abdomen. The lesions appeared at the age of 13 on the anterior chest and abdomen. These increased in number and extended slowly to the face On visiting our department, there were numerous, angular or round, skin colored papules on his face and anterior chest as well as yellowish brown colored papules on his abdomen. Diagnosis was confirmed by clinical and histopathological findings as generalized syringoma.

10.Clear visual field ureterorenoscopy by using ureteral catheter.

Uereterorenoscopy has become a useful diagnostic tool for the evaluation and treatment of the ureteral diseases such as the filling defect upper tract hematuria and abnormal cytology. Keeping a clear visual field in ureterorenoscopy is an essential for the reducing of complication such as ureteral perforation. We used alternative suction with 4 Fr. ureteral catheter through side hole of ureterorenoscopic sheath in 32 cases without serious complications. We believe that this method may help not only keeping clear visual field but also doing ureterorenoscopy safely, while use ureteral catheter as a safety guide wire.
