1.Effect of systemic temperature on hippocampal neuronal survival in transient global ischemia animal model.

It has been known that the hypothermia have protective effect on neuronal survival after ischemic damage. We performed this study to evaluate the effect of the small changes in postischemic body temperature on the histopathological change of hippocampus in the transient global cerebral ischemia model. Mongolian gerbils were subjected to this study. Nine animal subgroups were investigated, including naive gerbils who underwent sham operation or carotid artery occlusion with postischemic rectal temperature maintained at 32.5, 34.5, 36.5 and 38.5C respectively. Carotid occlusion was maintained for 10 minutes and then reperfusion started. During ischemia, body temperature was maintained 36.5degrees C in all animals. For one hour after ischemia, body temperature was maintained constant at 36.5degrees C in the normothermia group, 38.5 degrees C in the hyperthemia group, 34.5 degrees C in mild hypothermia group, and 32.5 degrees C in moderate hypothermia group respectively. Seven days after the operation, the surviving animals were decapitated and perfusion fixated. After preparing coronal brain slices, viable neurons in hippocampal region were counted using cresyl violet staining. There were significant differences in the hippocampal neuronal survival in normothermia and hyperthermia groups compared with shamoperated group(P<0,01), and neuronal damages in mild and moderate hyperthemia groups were not significantly different from sham operated. Survival rate at postischemic 7th day was also significantly lower in hyperthermia group. We could confirm the protective effect of hypothermia on ischemic neuronal damage by histopathological study. Also hyperthemia was observed to aggravate neuronal death, Careful control of body temperature might have clinical effect in ischemic stroke.

PURPOSE: p53 mutations are one of the most common genetic alterations in human lung cancer. Although the prognostic value of mutant p53 is still debated, it is widely accepted as a relatively early genetic event in the development and progression of lung cancer. Moreover, there are growing reports about an association between smoking and p53 mutation, suggesting that the p53 gene could be a target of the smoking associated carcino- genesis in the lung cancer. MATERIALS AND METHODS: Surgically resected 89 primary non-small cell lung cancers were obtained from May of 1995 to May of 1997. p53 expression and Ki-67 expression were measured by immunohistochemistry, and each p53 expression and smoking amount were compared with Ki-67 expression and other clinical prognostic factors. RESULTS: Positive p53 expressions were found in 52 (58%) specimens, including 38 (69%) squamous cell carcinomas, 11 (39%) adenocarcinomas, and 3 (50%) large cell carcinomas, and closely associated with male and squamous cell carcinoma. Also close correlation was observed between smoking amount and p53 expression by the regression analysis. But p53 and Ki-67 expression showed no associations in pathologic stage and survival, and there was no association between p53 expression and survival after adjuvant radiotherapy. CONCLUSION: Smoking seems to affect p53 mutations in non-small cell lung cancer, and additional efforts are needed to evaluate the carcinogesis of lung cancer.

5.A Case of Herpetiform Appearance of Digital Mucous Cysts.

A digital mucous cyst (DMC) is clinically characterized by a round to oval, translucent, smooth nodule localized to the dorsal aspect of the distal digits near the distal interphalangeal joint. It usually presents as a solitary lesion, and multiple lesions are uncommon. An 88-year-old man presented with herpetiform translucent papules on the right thumb. We first diagnosed the lesion as molluscum contagiosum or herpetic whitlow. Histopathology showed a cystic space containing mucinous material and numerous fibroblasts surrounded by mucinous stroma in the upper dermis. The lining of the cyst wall was not apparent and mucinous material was stained with Alcian blue, indicating a diagnosis of DMC.

6.Intraoperative bleeding control during cesarean delivery of complete placenta previa with transient occlusion of uterine arteries.

There are few methods to control heavy intra-operative bleeding during cesarean delivery of placenta previa. Transient occlusion of uterine arteries (TOUA) during operation has previously been reported as a quick and safe method to control intra-operative uterine bleeding. We reported 2 cases of cesarean delivery with complete placenta previa in which TOUA was performed to safely reduce intra-operative complication, especially heavy intra-operative bleeding. In the 2 cases, cesarean deliveries were safe and without any complications under the TOUA method. TOUA can be a good method to control heavy intra-operative bleeding during cesarean delivery of complete placenta previa with risk of heavy bleeding.

7.A Clinical Review of 16 Cases of Renal of Perirenal Abscess.

We reviewed retrospectively 16 cases of renal or perirenal abscess who were admitted to the St. Mary hospital during the period from January 1977 to December 1986. The results were as follows : 1. The average age of the patients was 27 years. 2. The average duration of symptom was 15 days. 3. On laboratory findings, 6 cases (31%) of pyuria and 2 cases (13%) of positive urine culture were seen. 4. Abscess culture was performed in 10 patients and 5 cases of staphylococcus and 2 cases of E. coli were cultured. The remained 3 cases were sterile culture. 5. Predisposing factors were involved in 6 cases (38%). Urologic predisposing factors were 5 cases (31%) and non-urologic predisposing factors were 4 cases (25%). 6. Surgical treatment was performed in 10 cases (63%) by 9 cases of incision & drainage and 1 case of nephrectomy. Non-surgical treatment was successful in 6 cases (37%) by 5 cases of antibiotics only and by 1 case of antibiotics with ultrasound guided percutaneous drainage.

8.BP Values Difference Depending on the Height of Hand Position in Oscillometric Electronic Digital BP Monitor and Its Comparison with Mercury Sphygmomanometer.

BACKGROUND: There is pressure difference depending on the height of hand position when blood pressure is taken, applying the oscillometric electronic digital BP(blood pressure) monitor. Authors have calculated the degree of BP differences, and evaluated the accuracy and effectiveness of the digital BP monitor comparing with the conventional Mercury Sphygmomanometer. METHOD: Randomized sixty cases consisting of in-patients and hospital workers were subjected for this study. BPs were taken at three different levels-nose level, heart level and knee level-on conventional sitting position applying OMRON Digital Automatic Blood Pressure Monitor and conventional Mercury Sphygmomanometer, and statistical analysis was made. RESULTS: At the nose level, systolic BP and diastolic BP were lower with 23.5mmHg and 18.9mmHg respectively, than at the heart level, while at the knee level, they were higher with 21mmHg and 17.5mmHg respectively, than at the heart level. No actual statistical difference of systolic and diastolic values between OMRON Digital Automatic Blood Pressure Monitoring method at the heart level and conventional Mercury Sphygmomanometeric method. CONCLUSION: Present study demonstrated significant discrepancy of BP values, in systolic and diastolic values, depending on the height of measured finger, when OMRON Digital Automatic Blood Pressure Monitor is applied in taking BP on conventional sitting position. However, no actual discrepancy of BP values was noted between two methods when BP is taken at heart level by OMRON Digital Automatic Blood Pressure Monitor and on brachial artery BP by conventional Mercury Sphygmomanometer. Thus OMRON Digital Automatic Blood Pressure Monitor could take the place of Mercury Sphygmomanometer in taking peripheral BP, which subsequently enable us to estimate central artery BP, which is believed to be better clinical index, through this much more handy electrical oscillometric device in the future.

9.Successful term delivery cases of trans-abdominal cervicoisthmic cerclage performed at more than 18 weeks of gestation.

A 38-year-old nulliparous woman was referred to our clinic because of cervical incompetence at 19 weeks of gestation. Trans-abdominal cervicoisthmic cerclage was performed after failure of modified Shirodkar cerclage operation in the patient at 21 weeks of gestation via a laparotomic approach. Another 38-year-old patient, who underwent loop electrosurgical excision procedure conization for treatment of cervical dysplasia 4 years ago, presented for cervical incompetence. At 18 weeks of gestation, we performed trans-abdominal laparotomic cervicoisthmic cerclage without any post-operative complications. During antenatal follow-up, there were no obstetrical co-morbidities and finally she gave birth to a healthy infant at full term by cesarean section. We report two cases of women who underwent trans-abdominal cervicoisthmic cerclage surgery because of cervical incompetence as they were not suitable for transvaginal cervical cerclage. Both patients successfully maintained their pregnancy until full term after undergoing transabdominal cervicoisthmic cerclage at more than 18 weeks of gestation.
