5.A Case of Neonatal Altoimmune Thrombocytopenia due to Anti-HLA-B7 +1=160 +B61.

We encountered a case of neonatal altoimmune thrombocytopenia(NAIT) due to anti-HLA-B7+B60+B61. Bilateral cephal hematoma and umbilical hematoma were noted at the time of birth. Purpura developed at the third day. Platelet count was 110,000 at birth and decreased to 66,000/micro liter at the day 4. Prothrombin time and partial prothrombin time were within normal limit. The mother's platelet count was 220,000/micro liter and she had no history of abnormal bleeding. Platelet antibody tests empolying mixed passive hemagglutination and immunofluorescence revealed that the mother's serum was reactive against the platelets from the father and the neonate, but was not reactive with her own platelets. Platelets from eight volunteer group 0 donors were tested with the mother's serum; seven were reactive and one was negative. The positive reactions were lost after chloroquine treatment of platelets. Antigen capture ELISA(ACE) and modified antigen capture ELISA employing monoclonal antibodies against platelet glycoproteins In, IIa, IIb, and IIIa were negative. Mother's serum was tested for lymphocytotoxicity against 49 donor ]ymphocytes and the specificity was found to be anti-HLA-B7+B60+B61. At the 9th day, one unit of platelet concentrate from the mother was transfused and the platelet count of the neonate rose up to 340,000/micro liter. The neonate was discharged at the day of sixteenth and the platelet count remained high thereafter.

6.Cases of hypothermia with Osborne's J wave on ECG.

Authors report two cases of hypothermic patients with Osborne's J wave on electrocardiogram. A 46-year-old male with liver cirrhosis was brought to the emergency room by ambulance due to comatous mental status. On exammination, the body temperature was below 35 degree C. By rewarming using warm normal saline intravenous infusion and hot pack application on the chest, his body temperature rose up to 36 degree C and regained consciousness. Later, he expired with afterdrop phenomenon. Another 41-year-old stuporous quadriplegic male with post-traumatic cerebral palsy was brought to the emergency room by his family complaining weakness and vomiting. On exammination, the body temperature was below 35degree C. He was rewarmed using warm normal saline intravenous infusion and hot pack application on the chest. However, he could not regain his normal body temperature and expired due to irreversible shock.

7.Thymofibrolipoma: A Brief Case Report.

Thymofibrolipoma is an extremely rare tumor in the anterior mediastinum, and represents a histologic variant of the usual thymolipoma. Herein, we report a case of thymofibrolipoma in a 9-year-old girl who had a huge mass with fatty attenuation in the right hemithorax on chest computed tomography. She denied any subjective symptoms except mild fever. The surgically resected tumor was ovoid, soft and well-encapsulated, measuring 9.0 x 7.5 x 7.0 cm. The cut surface was light tan in color with yellowish streaks. Microscopically, two distinct areas were admixed in different proportions. One consisted of normal thymic tissue with subinvoluted features and the other was composed of extensive areas of collagenous tissue interspersed in mature adipose tissue. In a high power view, there were thin strands of remnant thymic epithelial cells, separating the pseudolobules. Thymofibrolipoma should be distinguished from other benign or malignant conditions, occurring in the anterior mediastinum, so that unnecessary treatment can be avoided.

9.The effects of desflurane and sevoflurane on the intraocular pressure associated with endotracheal intubation in pediatric ophthalmic surgery.

BACKGROUND: For ophthalmic surgery anesthesia, it is vital that intraocular pressure (IOP) is controlled. Most anesthetic drugs affect IOP dose-dependently, and inhalational anesthetics dose-dependently decrease IOP. In this study, we compared the effects of desflurane and sevoflurane on IOP and hemodynamics in pediatric ophthalmic surgery. METHODS: Thirty eight pediatric patients from the age of 6 to 15 years, who were scheduled for strabismus surgery and entropion surgery, were randomized to be administered desflurane (group D, n = 19) or sevoflurane (group S, n = 19). IOPs and hemodynamic parameters were measured before induction of anesthesia (B), after induction but immediately before intubation (AI), 1 min after intubation (T1), 3 min after intubation (T3), and 5 min after intubation (T5). RESULTS: The mean arterial pressure (MAP) at T1 and heart rates (HRs) at T1 and T3 were significantly higher in group D than those in group S. There was no significant difference between the groups in IOP, cardiac index (CI) and stroke index (SI). There was a significant difference within the group in IOP, SI, MAP and HR. There was no significant difference within the group in CI. CONCLUSIONS: There was no significant difference between the groups in IOP and hemodynamic parameters. The two anesthetic agents maintained IOP and hemodynamic parameters in the normal range during anesthetic induction.

10.Influence of the curing time for the adhesive on the oxygen-inhibited layer thickness and the shear bond strength to dentin.

OBJECTIVES: This study investigated the hypothesis that increasing light-curing time would leave the oxygen-inhibited layer (OIL) of the adhesive thinner, and in turn, result in lower shear bond strength (SBS) than those obtained by the routine curing procedures. METHODS: 120 human extracted posterior teeth were randomly divided into three groups for bonding with three adhesives: All Bond 2(R), One Step(R), and Adper Prompt(R). They were subsequently divided into four subgourps with different light-curing time (10, 20, 30 and 60 s). The assigned adhesives were applied on superficial occlusal dentin according to the manufacturer's instructions and cured with one of the four curing times. Composite resin cylinder, 2.35 mm in diameter, were built on the cured adhesive and light-cured for 40 s. SBS were measured after 24 h from the bonding using a universal testing machine (crosshead speed 1.0 mm/min). The relative thickness of the OIL and the degree of conversion (DC) were determined from the adhesive on a slide glass using FT-NIR in an absorbance mode. Data were analysed with One-way ANOVA and Duncan's multiple test (p < 0.05). RESULTS: With increasing cure time, although there were no significant difference in th SBS of One-step and Adper Prompt (p > 0.05), those of All Bond 2 decreased significantly (p < 0.05). The relative thicknesses of the OIL on each adhesive were not affected by the cure time (p > 0.05). Although the DC of All-Bond 2 were statistically not different with increasing cure time (p > 0.05), those of One-Step and Adper Prompt showed an increasing trends with increasing cure time (p < 0.05). CONCLUSIONS: Increasing light-curing time did not affect on the relative thickness of the OIL of the adhesives, and in turn, on the SBS to dentin.
