1.Effect of High-Dose Steroid Treatment on Adipogenesis and Secretion of PAI-1 in Rats.

This study was conducted to invetigate the effect of high-dose steroid administration on adipogenesis and secretion of PAI-1 in rat. Sixty healthy Wistar rats were divided into three groups. Group A consisted of 20 control animals received 5 ml/kg isotonic saline for 1 weeks. Group B consisted of 20 animals received 10 ml/kg of methylprednisolone intraperitoeally for 1 week. Group C comprized 20 animals received 10 ml/kg of methylprednisolone intraperitoneally for 1 week. After the completionof treatment, blood sampling was performed for measurenement of PAI-1 and the animale were sacrificed for histopathologic examination and immunohistochemical study. The PAI-1 protein expressin of femoral heads were evaluated with Western-blotting. Blood level of PAI-1 increased significantly in high-dose steroid treated group(Group B and C) compared to control group. In histopathologic examination, significant fat morrow conversion, fat cell hypertrophy and fat cyst formation were observed in high-dose steroid treated group. Immunohistochemically, PAi-1 expression was sprominent in the fat cells. Protein expression of PAI-1 of femoral heads increased significantly in highdose steroid treated group compared to control group. In lights of above results, it is postulated that increased secretion of PAI-1 from fat cells could play a major role in the pathogenesis of osteonecrosis of the femoral head in rats treated with high-dose steroid.

2.Immunohistochemical Study of S-100 Protein in Salivary Gland Tumors.

An immunohistochemistry for S-100 protein by biotin avidin system technique was done to evaluate the existence and distribution pattern of S-100 protein positive cells in various obtained were as follows. 1) Positive immunostaining for S-100 protein was observed in myoepithelial cell, serous acinar cell and nervous bundle in normal salivary gland. 2) Strong immunoreactivity for S-100 protein was shown in plemorphic adenoma, which was localized not only in myoepithelial cord or sheets of epithelial portion but also in chondrocytes, stellate cells of myxoid stroma and in squamous keratin pearl of mesenchymal metaplastic foci. 3) The S-100 protein was demonstrated in the tumor cells of tubular adenoma, acinic cell tumors and in epidermoid area of mucoepidermoid tumors. 4) Immunoreactivity for S-100 protein, however, was not found in the tumor cells of adenoid cystic carcinoma and adenolymphoma except for stroma reticulum cells. 5) Intensity of positive reaction for S-100 protein varied from cell to cell: Some had intense immunoreactivity, whreas others were only weakly positive or completely negative, even in myoepithelial cell nest of the same pleomorphic adenoma.
3.PVL in preterm infants:correlation of MR & US.

MRI (magnetic resonance imaging ) had been the most up-to-dated modality in evaluating white matter disease in recent years, whereas US (ultrasonogram) has been used extensively in diagnosis of neonatal PVL(periventricular leukomalacia) conventionally. We evaluated the diagnostic value of MRI by reviewing the MR findings and correlation of MR and US of PVL in II preterm infants. Evaluation criteria were MR signal intensity and discrimination of PVL on each pulse sequences land comparision between MR and US findings performed simultaneously, on the extent of PVL, size of the largest cyst and detectability of hemorrhagic lesion. MR findings of 11 cases of PVL were of low signal patterns on T1WI (T1weighted image) in 7, low signal patterns of PDWI (proton density weighted image) in 9, iso signal patterns on T2WI(T2weighted image) in 8 and low signal patterns on STIR(short time inversion recovery) in 7 cases. The lesions of 11 PVL were well discriminated in all 11 cases of T1WI, 7 cases of STR, 5 case of PDWE and 2 cases of T2WI. The lateral ventricle was diffuse dilated (n=1) and focally dilated (n=3) in atrial area. In the comparative study, MR presented more extensive lesions in 7 cases, larger cysts in 6 cases out of 7 PVL and more definitive hemorrhage in 3 cases out of 4 cases than sonography. In conclusion MR was more valuable than sonography in evaluating the extent of lesions, size of the largest cystic lesion and detetion of hemorrhage. TIWI and STIR images were more useful in detection of PVL than T2WI and PDWI.

4.The Surgical Treatment of Osteoporotic Vertebral Collapse Caused by Minor Trauma.

With an aging population, osteoporotic vertebral collapse is an increasingly common condition. This compression fractures has been considered a benign entity, quite responsive to conservative treatment. In a rare patients, however, a major neurologic complication and painful kyphosis despite conservative treatment can develop. Therefore, the purpose of this present study is to analyze the surgical results of 14 patients with severe back pain, an increasing kyphosis and neurologic deficits caused hy osteoporotic vertebral collapse following minor trauma, who were treated surgically. Presenting signs and symptoms included severe back pain with progression of kyphosis in 6 patients and increasing neural deficit in 8 patients. Of 14 patients, eight patients had an intravertebral cleft sign (vacuum sign). Indications for surgery included increasing kyphotic deformity, intractable pain, or increasing neurologic deficit. There was no correlation between intravertebral cleft sign and neurologic deficit. However, patients who had intravertebral cleft sign had not well respond to conservative treatment. As treatments, combined anterior and posterior fusion in 8, anterior fusion in 4, posterior instrumentation, and wide decompressive laminectomry in one patient, respectively, were carried out. The final correction of the deformity averaged 0.3 degrees. Therefore. correction of kyphosis was not favorably maintained because of variable surgical methods, and sinking of graft bone or instrumentation into the osteoporotic vertebral bodies. However, pain was reduced significantly in all patients. In addition neurological symptoms improved in 7 patients. One patient underwent reoperation with nnterior inierbody tusion together with anterior instrument because of an increasing kyphosis, neurologic. iymptoms and scvcre hack pain following wide decompressive laminectomy. There was no complication relatecl to instruments. The authors strongly helieved that surgical intervention has highly satisfactory results in patients who have intravertehral cleft sign with persistent back pain despite conservative treatment, and proressive or persistent neurologic deficits following osteoporotic vertebral collapse.

5.Each Case of Benign and Malignant Mucocele of the Appendix.

Mucocele of the appendix is uncomrnon and rarely diagnosed preoperatively. The malignant rountpart-i.e. mucinous cystadenocarcinoma--has the same grross appearance and many micro scopic features in cornmon wilhe the benign form. It rnay be associated with ovarian mucinous cystadenoma of strikingly similar microscopic appearence. A serious complication is a rupture of the mucoeele resulting in pseudomyxoma peritonei. Each case of benign and malignant mucocelr of the appendix falsely diagnosecl as an oovarian tumor before laparotxumy are presented wilh a brief rieview of the literatures.

6.DNA Ploidy Heterogeneity in Primary an Metastatic Lesion of Epithelial Ovarian Cancer.

Tumor DNA content measured by flow cytometry may be a predictor in the prognosis of epithelial ovarian cancer, but the results have been inconsistent. It is recognized that these conflicting results are at least partly due to the variation of DNA content between the samples from the same patient(i.e., intratumoral DNA heterogeneity). The purposes of this retrospective study were to investigate the frequency and the nature of DNA heterogeneity in epithelial ovarian cancer and to evaluate the prognostic significance of DNA heterogenetiy itself. Thirty-two patients with stage II to IV epithelial ovarian cancer who were managed at Asan Medical Center between May 1993 and April 1996 were analysed. Measurements of the nuclear DNA content were performed on samples from primary and metastatic lesion using paraffin embedded archival tissues by Epics(Coulter Inc.) flow cytometry. In two cases, the metastatic tumor was minute and did not reveal a separable peak on repeated examination. DNA heterogeneity was defined as different ploidy pattern or difference of the DNA indices than 0.15 between primary and metastatic tumors. DNA heterogeneity was found in 11 cases(36.7%), and the number of cases with homogeneous diploid and that with homogeneous aneuploid tumor were 5(16.7%) and 14(46.7%) respectively. In evaluation of prognostic significance of DNA heterogeneity using correlation with serum CA 125 level after second course of chemotherapy and residual tumor size after cytoreductive surgery among these three groups, the patients with DNA heterogeneity were considered to show intermediate prognosis between those with homogeneous diploid and homogeneous aneuploid tumor. In conclusion, DNA heterogeneity in epithelial ovarian cancer is considerable in frequency and may have prognostic value.

7.Serum CA 125 Levels in Patients with Impaired Renal Function.

Measurement of the serum CA 125 level as a tumor marker in patients with epithelial ovarian cancer has been widely used to monitor disease status and predict survival of patient. While a number of benign gynecologic as well as benign or malignant non-gynecologic conditions are associated with elevations of serum CA 125 levels, the established normal range describes a healthy population of women. Because the metabolism and clearance of CA 125 is not well understood and mild or moderate degrees of renal impairment frequently occurs in ovarian cancer patients during treatment or course of disease, it is valuable to investigate the effect of impaired renal function on serum level of CA 125. Eighty-nine women on hemodialysis who had no other definite cause to elevate serum CA 125 level were selected at random. The age of the patients ranged from 19 to 83 and renal disease was secondary in most cases to diabetes mellitus, hypertension or glomeru-lonephritis. The creatinine clearance was less than 10cc/min for all patients. (continue)

8.A Case of Desmoplastic Malignant Melanoma.

Desmoplastic malignant melanoma (DMM) is an uncommon variant of malignant melanoma featuring a proliferation of spindle cells with pronounced desmopaasia. DMM commonly occurs on the sun-damaged skin such as head and neck in older persons, and it can be associated with lentigo maligna. We report an unusual case of desmoplastic malignant melanoma that developed de novo on the trunk in a young patient.
