3.Long-term Results of Silicone Tube Intubation in Incomplete Nasolacrimal Duct Obstruction (NLDO).

PURPOSE: To evaluate the results of long-term follow-up of silicone tube intubation in patients with acquired nasolacrimal duct obstruction (NLDO) that is incomplete. METHODS: A retrospective analysis was conducted of the outcomes of silicone intubation performed between 1998 and 2003. During that period, we performed silicone intubation on 109 eyes, but only 45 eyes that completed at least 6 months of follow-up were included in the analysis. RESULTS: The mean follow-up period was 30.4 months (6~76 months), and silicone tubes remained in place an average of 6.5 months (1~18months). The success rate, which is defined as the improvement of epiphora symptoms with patent nasolacrimal irrigation. CONCLUSIONS: Silicone tube intubation is a simple, cost-effective, and beneficial treatment for patients, but the recurrence rate increases over time, especially several years after the operation.

4.Analysis of Heavy Metals in 201TlTlCl Injection Using Polarography.

PURPOSE: Thallous-201 chloride produced at Korea Cancer Center Hospital(KCCH) is used in detecting cardiovascular disease and cancer. Thallium impurity can cause emesis, catharsis and nausea, so the presence of thallium and other metal impurities should be determined. According to USP and KP, their amounts must be less than 2 ppm in thallium and 5 ppm in total. In this study, the detection method of trace amounts of metal impurities in [201Tl]TlCl injection with polarography was optimized without environmental contamination. MATERALS AND METHODS: For the detection of metal impurities, Osteryoung Square Wave Stripping Voltammetry method was used in Bio-Analytical System (BAS) 50W polarograph. The voltammetry was composed of Dropping Mercury Electrode (DME) as a working electrode, Ag/AgCl as a reference electrode and Pt wire as a counter electrode. Square wave stripping method, which makes use of formation and deformation of amalgam, was adopted to determine the metal impurities, and pH 7 phosphate buffer was used as supporting electrolyte. RESULTS: T1, Cu and Pb in thallous-201 chloride solution were detected by scanning from 300 mV to -800 mV. Calibration curves were made by using TlNO3, CuSO4 and Pb(NO3)2 as standard solutions. Tl was confirmed at -450 mV peak potential and Cu at -50 mV. Less than 2 ppm of Tl and Cu was detected and Pb was not detected in KCCH-produced thallous-201 chloride injection. CONCLUSION: Detection limit of thallium and copper is approximately 50 ppb with this method. As a result of this experiment, thallium and other metal impurities in thallous-201 chloride injection, produced at Korea Cancer Center Hospital, are in the regulation of USP and KP. Polarograph could be applied for the determination of metal impurities in the quality control of radiopharmaceuticals conveniently without environmental contamination.

7.Radiologic Evaluation of Intraabdomenal Masses in Childhood.

The abdominal tumors in children are different from those of adult. These tumors are the third most common one, preceded by leukemia and brain tumors, in children under 15 years. X-ray examination is the most important method among diagnostic approaches. The role of diagnostic imaging is to identify the precise anatomic location and extent of pathologic process with the minimal number of imaging procedures. 23 cases of abdominal tumors were reviewed in respect of age incidence, site of origin, radiologic findings. The results are briefly summarized as follows: 1. Neuroblastoma was the most common (6 cases) and wilm's tumor (5 cases), choledocal cyst (4 cases), ovarian mass (3 cases), hydronephrosis (2 cases), were descending order in frequency. 2. The most common site was retroperitoneum (60%) Kidney was the single most common site of origin. 3. Radiologic findings. The most common findings of plain radiography was ill defined soft tissue mass and this method was helpful in the presence of calcification especially in neuroblastoma. Ultrasonographic pattern was anechoic (cystic), echoic or mixed pattern, but this method provide less precise anatomical details, nevertheless Ultrasonography was particularly useful imaging modality for the pediatric abdominal tumors. IVP findings were renal displacement, caliceopelvic system distortion or nonvisualization of kidney, these information was helpful in determining the location of tumors. CT scan showed homogenous or inhomogeneous, cystic or solid, mass with their anatomic location. 4. Ultrasonography was the most widely used specific diagnostic method, but had limited value in detecting the anatomic location of tumors. CT scan was superior to ultrasound for determining the extent of tumors.

8.Platelet Serotonin Level and Psychological Features of the Suicidal Attempters.

OBJECTIVES: Significant progress has been made in understanding psychosocial, psychological, and environmental factors associated with suicide. However it is only recently that attention has been paid to the understanding of the neurobiology of suicide. The aim of this study was to examine the relationship between platelet serotonin level and suicidal behavior and psychological features of the suicidal attempters. METHODS: After a suicidal attempt platelet serotonin level was measured from 21 patients and compared it with those from depression patients and normal controls. Also MMPI, HAM-D, Barratt impulsiveness scale(BIS) were done to evaluate their psychological features. RESULTS: 1) There was no significant difference in sex ratio of the suicidal attempters. 2) There was no significant difference in platelet serotonin levels among three groups. 3) The analysis of MMPI revealed that the scores of D, Hs, Pt in clinical scales were significantly higher in suicide patients and scores of D, Pa, Si were in depression patients. 4)The HAM-D score was significantly higher in depression and suicide patients, especially in depression patients. 5) The analysis of Barratt impulsiveness scale revealed that the scores of nonplanning, motor and cognitive impulsiveness scale were significantly higher in suicide patients. CONCLUSIONS: There was no significant difference in platelet serotonin levels among three groups. However the analysis of psychological features revealed significant differences. Therefore we concluded that psychological examinations are benefit to evaluate the suicidal tendency.

9.The Role of Lateral Buttress in Treatment of Jensen type 4 Intertrochanteric Fractures of the Femur using Dynamic Compression Hip Screw in the Elderly.

PURPOSE: To investigate the effect of lateral buttress on the fracture healing of Jensen type 4 intertrochanteric fracture of the femur treated by dynamic compression hip screw (DCS) in elderly patients, retrospectively. MATERIALS AND METHODS: From March 1999 to February 2003, 54 patients, older than 65 years, of Jensen type 4 intertrochanteric fractures of the femur treated by DCS were included. The relationship between the structual integrity of lateral buttress measured by proximal-medial fragment angle (PMFA) and postoperative neck-shaft angle, penetrating length of lag screw, the sliding length and incidence of fixation loss were evaluated. RESULTS: Among 38 patients of group II (PMFA above 50 degree), 6 cases (15.8%) including 4 cases of cutting out of lag screw from femoral head, 1 case of nonunion and 1 case of excessive extrusion of lag screw failed radiologically. But there was no fixation loss case in 16 cases of group I (PMFA 50 and below 50). The neck-shaft angle in last follow-up was 138.1 degree in group I and 132.1 in group II. The sliding length of lag screw was 7.0 mm in group I and 12. 5 mm in group II (p<0.05). CONCLUSION: Preoperative evaluation of intactness of lateral buttress measured by proximal-medial fragmentary angle is an useful method in treatment of Jensen type 4 intertrochanteric fractures treated by DCS in elderly patients.

10.Surgical Treatment of Pancreatitis.

PURPOSE: The most common cause of acute pancreatitis is alcohol abuse, followed by gallstones and pancreatic trauma. Pancreatitis is treated nonoperatively by inhibiting the causes, which include prohibition of alcohol abuse and cholecystectomy for the removal of gallstones. However, in case of a pseudocyst formation, necrotizing pancreatitis or abscesses remained as the complications of severe pancreatitis. Adequate surgical drainage is very important for the treatment of the complications. Adequate surgical procedure for treatment of chronic pancreatitis, causing intractable pain, pseudocyst, and obstruction of pancreatic or bile ducts, should be considered, not only for the patients physical and psychological recovery from a chronically debilitating status, but also in its differentiation from a malignancy. METHODS: Between July 1997 and January 2002, a review and analysis was undertaken of 33 pancreatitis patients, who had been treated, by surgical intervention, for the complications of acute pancreatitis, chronic pancreatitis, and benign pancreatic disease. RESULTS: For the treatment of the various complications in the 33 patients, 20 drainage procedures and 13 surgical resections were used. There were minor complications in 6 patients (21%), and two mortalities, one from multiple organ failure, due to necrotizing pancreatitis, and the other from a surgical complication, due to a duodenum preserving pancreatic head resection. CONCLUSION: The timely surgical drainage, and/or debridement, is critical for patients with complications due to acute pancreatitis. It is difficult to differentiate chronic pancreatitis from a malignancy. Adequate surgical procedures for the different stati ('type oriented surgery') are required for chronic pancreatitis. A resection of cystic lesions, with the possibility of a malignancy, should be considered. We can confirm the diagnosis, and treat the disease, by adequate operative and drainage procedures, or by resection of the main lesion.
