1.Conservative Management of the Renal and Perirenal Abscesses.

PURPOSE: We critically evaluated the most appropriate management of renal and perirenal abscess and identified the set of patients who had benefited from conservative management. MATERIALS AND METHODS: Between May 1995 and June 1999, fifteen patients with renal (10 cases) and perirenal abscess (5 cases) were treated in our hospital. Among the fifteen patients, six patients were managed by antibiotics alone, the other nine patients were managed by percutaneous aspiration and antibiotics. Percutaneous abscess aspiration and drainage was done under ultrasound guidance and local anesthesia. RESULTS: On the urine culture, 4 cases had positive urine culture (27%); E. coli was in 3 cases, Klebsiella in 1 case respectively. On the blood culture, E. coli was in 2 cases. On the abscess culture (9 cases were performed), the organisms were identified in 8 cases (89%); E. coli was in 6 cases, S. aureus was in 1 case, S. epidermidis in 1 case respectively. CONCLUSIONS: We suggest that proper antibiotic therapy alone or combined with ultrasound guided percutaneous drainage of renal and perirenal abscesses is a choice of reasonable, safe and effective management in selected patients.

The introduction, indication, procedure of the surgery and the use of the self curing acrylic cement were reviewed briefly. 5.A Case of Sparganosis In the Spermatic Cord.

Sparganosis is a parasite infestation caused by the larval stage of Spirometra erinacei. Ingestion of raw or undercooked flesh of numerous intermediate hosts results in human infection. Sparganum infestation of human body was reported over 120 cases including urogenital lesions in Korean literatures since 1924. But, no case of sparganosis in the spermatic cord was reported until now. We experienced a case of sparganosis in the spermatic cord.

6.The change of anti-HBs titer after hepatitis B vaccination in newborn according to dosage and time.

We studied anti-HBs titer, positive and effective rate in relation to dosages(5microgram, 10microgram) and time interval after third vaccination in 23 infants born to HBsAg negative mother. The babies were divided into two groups. In one group(n=12), 5microgram of Hepavax was administered intramusculary at 1 month, 2 months and 6 months of age, in other group (n=11), 10microgram of Hepavax at same time interval. And the anti-HBs was studied at 2 months and 3 year after third vaccination by radioimmunoassay. The results were as follows: 1) The anti-HBs positive rate was 100% in two groups at 2 months and 3 years after vaccination. 2) The geometric mean anti-HBs titer at 2 months after third vacciantion was 9418.3+/-13041.5 IU/L in 5microgram group and 12750.0+/-12750.5 IU/L in 10microgram group, and 3 year after vaccination, 949.4+/-1404.0 IU/L in 5microgram group, 1067.4+/-1067.7 IU/L in 10microgram group. There were no significant difference between two groups, although mean anti-HBs titer decreased significantly after 3 years. 3) The effective rate at 3 years after vaccination was 66.7% in 5 microgram group and 72.8% in 10microgram group. There was no statistical significant difference between two groups.

7.A Case of placenta increta which was found about 50days after induced abortion at 1st Trimester.

During 1st and 2nd trimester, placenta accreta is rarely found and is not easy to diagnosis during these trimester. From time to time placenta accreta is found after induced abortion or spontaneous abortion. We had experienced a case of placenta increta which was found about 50 days after D&C due to missed abortion at local clinic. So we presented it with a brief case history and brief review of the concerned literatures.

8.Clinical Experience of Symptomatic Management of BPH with Terazosin, Doxazosin or Combination of Terazosin and Finasteride.

PURPOSE: We evaluated and compared the efficacy of terazosin, doxazosin and terazosin(alpha-1 adrenoreceptor antagonist) with finasteride(5-alpha reductase inhibitor) in the treatment of patient with benign prostatic hyperplasia (BPH). MATERIALS AND METHODS: The study was single-blind design. The patients were divided 3 groups(terazosin group, doxazosin group, terazosin with finasteride group). Terazosin was administrated with escalating dose of 1 to 5mg once daily for 12 weeks. Doxasosin, fixed dose of 2mg was taken once daily for 12 weeks. Finasteride was taken 5mg once daily with terazosin for 12 weeks. The study enrolled 69 patients, and 60 patients were included in the analyses. RESULTS: The parameters used to assess the effectiveness included international Prostatic Symptom Score(1-PSS), Quality of Life(QOL) index and peak urinary flow rate(Qmax). At baseline, 1-PSS, QOL index and Qmax were 18.8+/-4.3, 3.7+/-1.0, 8.6+/-1.7 in terazosin group, 19.3+/-3.9, 3.6+/-1.0, 7.8+/-1.8 in doxazosin group, 20.1+/-4.4, 3.8+/-1.0, 72 +/-1.6 in combination group, respectively. After 12 weeks trial, 1-PSS, QOL index and Qmax were 12.0+/-2.8, 1.9+/-0.9, 11.4+/-2.8 respectively in terazosin group, 11.3+/-3.0, 1.7+/- 0.7, 10.6+/-2.6 in doxazosin group, 10.9+/-4.0, 1.8+/-0.9, 9.8+/-1.0 in combination group, respectively. CONCLUSIONS: There was clear evidence for the efficacy of alpha-1 blocker in treating patients with bladder outlet obstruction due to BPH. There was no significant difference between alpha-1 blocker therapy alone and combination therapy with finasteride. This study showed beneficial short term results for the safety and efficacy of long acting selective alpha-1 blocker and finasteride in the management of symptomatic BPH, but if symptom and quality of life for patient were not improved, we are 1ikely to consider that early surgical therapy will be required.

9.Combination Chemotherapy with VP - 16 , Ifosfamide , and Cisplatin ( VIP ) in the Advanced Non - Small Cell Lung Cancer.

PURPOSE: We conducted a phase II study in previously untreated patients with unresectable stage IIIB or IV non-small cell lung cancer to evaluate the response rate and toxicity of the combination chemotherapy regimen of etoposide, ifosfamide and cisplatin. MATERIALS AND METHODS: From September 1993 to December 1996, twenty patients with advanced non-small cell lung cancer (stage IIIB 5 and IV 15) (squamous cell 8, adeno- carcinoma 12), were enrolled in this study. There were 13 (65%) males and 7 (35%) females, and median age of patients were 56 years (range: 34~66). Eighteen patients had performance status (ECOG) 0~1, two patients had performance status 2. Treatment was consisted of cisplatin (20 mg/m2 i.v., day 1~4), VP-16 (etoposide) (75 mg/m2 i.v., day 1~4), ifosfamide (1000 mg/m2 i.v., day 1~4) with mesna. This treatment was repeated every four weeks. RESULTS: The overall response rate was 25%. Complete response rate was 5% (1/20) and partial response rate was 20% (4/20). The median cycle of response was 4 (2~6) cycles. The median overall survival time was 28 weeks (9~98 weeks). The median time to progression was 10 weeks (3~50 weeks). Toxicities were evaluated by WHO criteria. Toxicity > GradeIII included: leukopenia 1.6%, thrombocytopenia 3.2%, nausea and vomiting 15%, alopecia 30%, stomatitis 10%. These toxicities were tolerable and reversible. CONCLUSION: VIP regimen was not superior to previous regimens for advanced non-small all lung cancer, and the toxicities were tolerable.
