1.Pituitary Stalk Transection Syndrome

We experienced one case of severe pituitary dwarfism in a 16 years old male boy that magnetic resonance image(MRI) revealed transection of the pituitary stalk with the ectopic location of posterior pituitary gland and hypoplastic anterior pituitary gland. The serum growth horrnone(GH) response to clonidine and L-dopa revealed severe GH deficiency. The patient revealed normal response in TSH, prolactin and LH, FSH to TRH and LHRH respectively. So far, the primary cause of idiopathic pituitary dwarfism in many patients was due to hypoxic injury to hypothalmus by perinatal insults. In this patients, there was no history of perinatal insults and postnatal head trauma. We report a case of severe dwarfism due to hypogenesis of anterior pituitary gland.

3.Late Pa.nophthalmitis after Filtering Operation for Glaucoma.

The authors present that was diagnosed late panophthalmitis histopathologically and that was a case of 17 scheie's filtering operated eyes during last 2 years. The patient was 63 year-old male who had chronic narrow angle glaucoma associated with moderate optic nerve cupping and atrophy in both eyes a year ago. Good filtering bleb showed and intraocular pressure controlled well normally after Scheie's operation in right eye. One year and five months later acute suppurative endophtbalmitis occured in operated eye during catarrhal conjunctivitis in both eyes. The authors concluded that the route of infection was filtering fleb. Culture of excreting pus through ruptured filtering fleb was negative.

4.Treatment of Tibial Medial Bone Defect in Primary TKA.

There are several rnethods to correct the tibial bony defect including resection, cement filling, autograft or allograft and metal augmentation. The purpose of this study is to find the adequate treatment method of tibia bony defect through analysis of the result with above methods. From Sep. 1993 to Dec. 1997, the authors analyzed 93 cases of tibial medial bony defect corrected by overresection of lateral condyle, allograft and metal wedge or block among 358 cases of primary total knee arthroplasty(TKA) operated at Asan Medical Center. All cases were devided into four groups according to the treatment method; group A(31 cases) with overresection of lateral condyle, group B(37 cases) with metal wedge, group C(21 cases) with metal block and group D(4 cases) with allograft. The mean follow up period was 23.6 months(12 56 months). The results were as follows, 1. There were no definite statistical difference between group A, B, C and D in HSS knee score, ROM, correction of deformity. But in group D, there is one case of loss of the correction. 2. Loosening of the implant was not noted, but 17 cases of mild bony resorption was found just beneath the implant. It was particularly prominent in group B(9 cases) than group A(5 cases) and C(3 cases). 3. Among 4 cases of allograft, one has developed collapse of allograft. 4. 3 cases of deep infection developed only in group C, which were followed by revision TKA. Although further follow up study should be carried out, we concluded that resection of lateral tibial condyle, allograft, metal augmentation is a good substitute to the correction of the tibial bony defect in primary TKA.

5.Immunoblot observation of antigenic protein fractions in Paragonimus westermani reacting with human patients sera.

: In order to observe the antigenic fractions in saline extract of adult Paragonimus westermani, proteins in the crude extract were separated by sodium dodecyl sulfate-polyacylamide gel electrophoresis (SDS-PAGE) in reducing conditions. The separated protein fractions were transferred to nitrocellulose paper on which 20 sera from human paragonimiasis were reacted and immunoblotted. Out of 15 stained protein bands in SDS-PAGE, 7 reacted with the sera. Of 14 reacted bands, 30 kilodalton(kDa) band was the most frequently reacted (95%) and was a strong antigen. Protein bands of 23 and 46 kDa were also strong antigens. Bands of over 150 kDa, 120 kDa, 92 kDa, 86 kDa, 74 kDa, 62 kDa, 51 kDa, 32 kDa, 28 kDa, 16.5 kDa and 15.5 kDa were also reactive but their frequencies of the reaction were variable.

6.Clozapine Induced Neuroleptic Malignant Syndrome.

Neuroleptic malignant syndrome is a rare, but potentially life-threatening adverse event associated with the use of neuroleptic agents. We describe the case of a 47-year-old schizophrenic woman who was treated with clozapine for years. The patient developed acute renal failure with pulmonary edema, and underwent mechanical ventilation and hemodialysis.

7.The Effect of Room Temperature on the VDRL Test Result.

The Venereal Disease Research Laboratory microflocculation (VDRL) test, especially the quantitative test is well standardized, simple, rapid and, at present, the most widely used non-treponernal test in serodiagnosis and follow up sturly of syphilis. However, the UDRL test result could be influenced by room ternperature, especially in summer and winter in Korea in the abscence of proper air-conditioning and heating. This study was undertaken to evaluate the effect of room temperature on the VDRL test result and to determine the critical temperatures which alter the test result. Eighty six VDRL-reactive serum samples collected at the Soonchunhyang Medical College Hospital and the V.D. Clinic of the Chung-Ku Public Health Center in Seoul during the 7 months periods from September 1982 to March 1983 were used. The results obtained were as follows: The change in the VDRL titer was not obscrved at room temperature of between 18 C and 32 C 2. The VDRL titer became weakly reactive on the same titer or lowered to half titer at room temperature of below 18C. R. The VDRL titer increased two fold at room temperature of above 32C.(countinue..)

8.A Case of Primary Cutaneous Mucinous Adenocarcinoma.

We report a case of primary cutaneous mucinous adenocarcinotna in a 64-year-old female. The tumor was about 2cm in size, forminga round dome-shaped alopecic scalp mass which had gradually increased in size over about 5 years, Histalogic examination revealed that the tumor was divided into numerous compartments by strands of fibrous tissue. In each compartment, abundant amounts of pale-staining mucin surrounded nests or cords of moderately anaplastic epithelial cells. We couldn't find any evidence of internal neoplasms as a source of metastasis. We resected the tumor with approximately a 1 cm margin and then performed a split thickness skin graft.

9.A case of Congenital patent urachus.

The complex group of changes which occurs during the intrauterine maturation of the urinary tract gives rise to several anomalies including those of the urachus. The occurrence of patent urachus is very rare and 67 cases has been reported by world literature since 1550, first description of patent urachus by Bartholomaeus Gabriolus A brief discussion of embryology, diagnosis and treatment is included for completeness. And a case of congenital patent urachus was seen at Han Ill Hospital.

10.Atypical Carcinoid of the Larynx: A report of two cases.

Atypical laryngeal carcinoid is a rare tumor with a poor prognosis, mostly occurring in the supraglottic larynx. It is a subtype of neuroendocrine carcinoma which should be separated from typical carcinoid and small cell carcinoma. The histogenesis and standardized classification of laryngeal neuroendocrine carcinoma have not been clearly defined. In this report, we present two cases of atypical laryngeal carcinoid, one occurring in a 67 year old male and the other in a 54 year old female. Indirect laryngoscopy revealed a polypoid supraglottic mass, approximately 3 cm in diameter. Microscopically, each case showed a moderate degree of pleomorphism, tumor necrosis and frequent mitoses. The immunohistochemistry revealed a strong positive reaction for chromogranin, neuron specific enolase and cytokeratin. Each patient had distant metastasis, noted within 4 months after resection (liver and stomach), and died postoperatively at 5 and 20 months, respectively. A brief review of the literature concerning the biological behavior, histogenesis and pathology of atypical laryngeal carcinoid was performed.
