During the period of 9.5 years from September, 1985 to March, 1995, 32 cases with spontaneous closure in simple ventricular septal defect(VSD) were observed and evaluated at Yeungnam University Hospital, and those were confirmed by 2D-echocardiogram. The results were as follows: 1. Among these 32 cases, there were 23 cases(71.9%) of perimembranous VSD, 8 cases(25.0%) of muscular VSD and 1 case(3.1%) of subarterial VSD. Septal aneurysms appears to be main mechanism of spontaneous closure of VSD because of the presence of septal aneurysm in all cases with spontaneous ',;losure of a perimembranous VSD. 2. The size of the defect was variable in diameter, but 27 cases(81.2%) were less than 5 mm. 3. The mean age was 12.1 months at spontaneous closure with the range from 1 month old to 72 months. 4. The mean weight was 9.0 kg at spontaneous closure. 5.--Among these 32 cases, 3 cases had the clinical evidence of cardimegaly or congestive hert -failure during infancy. 6. The purpose of this study was to determine whether workers at a factory next to a lead recycling factory in Pusan, were affected by lead contamination. The mean air lead concentration of lead recycling factory was 0.21mg/m3(TWA=0.05mg/m3). Thirty-nine male workers of Factory A, Cr. plating factory next to the lead recycling factory were exposed group and a comparison group, 62 male workers of Factory B were selected from another Cr. plating factory about 8.5km away from lead recycling factory. Air lead concentration of each workplace was checked for 4 times from August 5 to August 20 in 1995 by low volume air sampler. Each subject was interviewed about age, life-style, smoking, work history, and residence etc, and venous blood was drawn for lead measurement by graphite furnace atomic absorption spectrometry. We have observed that air lead concentration and blood lead concentration of Factory A was higher than Factory B(2.6 +/- 1.6 Vs. 1.2 +/- 0.2 microgram/m3, 14.9 +/- 1.6 Vs. 12.2 +/- 1.6 microgram/dl). We believe that other environmental lead sources such as transportation and residence did not affect air lead and blood lead concentration differences of both factory. We concluded that high air lead and blood lead concentration of Factory A were caused by lead contamination generated by the neighboring lead recycling factory.

It is often difficult to achieve bony union and eradicate infection in treating infected non-unions of the fractured long bones. Authors carried out a clinical analysis of the 35 infected non-union cases, all aged 21 years or older, treated and followed for 6 months to 6 years from July 1979 to July 1988 in order to determine the possible factors which might influence the time required for attaining union. The results were as follows: l. In 22 of the 35 infected non-unions, the primary fracture site was open (62.9%). The remaining 13 cases (37.1% ) were the closed fractures associated with postoperative infection. 2. The sites of infected non-union were tibia in 19 (54.3%), femur in 13 (37.1%), and upper extremity in 3 (8.6%) cases. Tibia was more often involved in open fractures (63.6%). Femur, on the other hand, was more involved in closed fractures (61.5%). 3. After an average of 3.3 times of bony procedures, primary fixation was left in place in 15 (42.9%), external fixation was required in 11 (31.4%), and internal fixation was performed in 9 (25.7%). Bone graft was performed in 26 cases (74.3%). 4. Time required for achieving union was 7.1 month with stable primary fixation and 21.7 months with unstable primary fixation. Femur and tibia united at 10.5 (mean; ranged from 4 to 29) and 7.0 (mean; ranged from 3 to 19) months, respectively. 5. Bony union was delayed as the number of prior surgical procedures before successful treatment increased; the time for union was significantly shorter when either stable primary fixation or external fixation was combined with bone grafting. 6. Firmer bony union was achieved after performing Osteoperiosteal decortication with autogenous cancellous bone graft even in the presence of remaining infection at the non-union site.

The authors evaluated the 56 patients who received the diagnosis of myopia at the department of ophthalmology in Koryo General Hospital in Seoul from May 1975 to August 1985. In this study, we reviewed the statistics of the progression of myopia with age and with the number of visits to the hospital and also the differences between male and female and left eye and right eye. The results are as follows: 1. Myopia progresses with increase of age. 2. The mean diopter of myopia over 21 years is not so remarkable compared with below 20 years. But it is not absolutely true because the number of patients are so small. 3. On the average, males are slightly higher in the degree of myopia than female, but this seems to have no statistical meaning. 4. In terms of the degree of myopia, it is higher after the age of 12 than before 12.

Gliomas of the optic nerve are relatively uncommon. We have experienced one case of glioma of the optic nerve recently. The patient was 17 year old boy who had exophthalmos and visual disturbance in his left eye for three years. The vision was CF/30cm. in his left eye and the optic nerve head was extremely atrophic. The eyeball was displaced infero-temporally. The exophthalmic value was 14mm. in right and 18mm. in left eye. On July 31, 1986, the left eyeball was exposed through Kroenlein method and the tumor mass was excised totally but the eyeball remained intact. The gross appearance of the tumor was large fusiform shape(3.5 X 2.0 X 1.8cm.) with well encapsulated grayish-white surface. The cut-section of the tumor revealed grayish-white solid area and hemorrhagic cystic area. The histopathologic slides showed that the tumor was composed of multiple pilocytic astrocytic cells in the solid area and in microcystic area. The cytoplasm of the tumor cells were bipolar spindle shape and the nucleus was uniform in general and there was no sign of mitotic figure. Numerous dense and amorphous eosinophilic materials so called "Rosenthal fibers" were also seen in the microcystic areas. Finally we have diagnosed this tumor as the Optic nerve glioma.

Chronic dermatological problems associated with lower limb amputation sites include local maceration, intertrigo, blistering and ulceration, contact dermatitis, tissue atrophy, verrucous hyperplasia, lichenification, callosities, epidermoid cyst formation, and malignant neoplasia. A 60 year-old male was presented with a verrucous oozing eruption of the amputation stump of the left leg. He had been involved in a railroad logging accident 30 years before, resulting in the loss of the left lower limb. He were a below-knee prosthesis (patellar-tendon hearing). We diagnosed him as verrucous hyperplasia by clinical and histological findings and then referred him to the division of rehabilitation for a refitting of his prosthesis, after which his eruption cleared two months later.

We report a case of cutaneous T-cell lymphoma in a 40-year-old man. He has suffered from generalized pruritus for 10 years. Seven months ago, multiple nodules developed over the scalp, face, and thigh. The physical examination showed thickened dystrophic twenty nails with brownish discoloration and lichenified, eczematoid lesions over the trunk and extremities. The skin biopsy revealed diffuse, extensive infiltrates of atypical lymphoid cells in the dermis, which appeared as T lymphocytes by immuno-histochemistry; approximately 90% of the tumor cells were CD3-positive, 80% were CD4-positive, and 60% CD45RO-positive. Laboratory analyses in the peripheral bload showed 18% atypical lymphocytes (Sezary cell), an elevated LDH(746 U/liter), and abnormal lymphocyte subset proportions(ratio of Th/Ts is 4.08). A bone marrow puncture revealed the infiltrations of lymphoid cells and computed tamo-graphy showed no evidence of other organ involvement except the swelling of bilateral inguinal lymph nodes. Spontaneous partial regression of the tumors ensued on the way of oral antihistamines and topical corticosteroids for the relief of the pruritus. The tumorous skin lesions and onychodystrophy disappeared completely with 6 cycles of systemic chemotherapy of CHOP(cyclophosphamide, doxoru-bicin, vincristine, and prednisone).
