2.Treatment of Uncomplicated Male Gonococcal Urethritis: Kanamycin vs . Gentamicin.

Between December 18, 1985 and May 31, 1986 at the VD Clinic, Choong-ku Public Health Center, in Seoul, 274 male patients with uncomplicated gonococcal infection, including 124 penicillinase producing Neisseria gonorrhoeae(PPNG) infection, were entered into the study. They were divided randomly into the group A and the group B. In the group A, treated with kanamycin 2g, im regimen, 126 of 137 patients were followed and 86 patients(68.3%) were cured. In the group B, treated with gentamicin 240mg, im regimen, 125 of 137 patients were followed and 78 patients(62.4%) were cured. The cure rates in PPNG urethritis were 63.9%(39/61) in the group A and 50.9%(29/57) in the group B. The cure rates in non PPNG urethritis were 72.3%(47/65) in the group A and 72.1% (49/68) in the group B. No significant difference was observed in cure rates between two groups. It is suggested that. both kanamycin 2g, im regimen and gentamicin 240mg, im regimen is not suitable for a first line treatment for uncomplicated gonococcal urethritis.

3.Treatment of Uncomplicated Male Gonococcal Urethritis with Norfloxacin.

From November 18 to December 31, 1986, seventy-five male patients with unconplicated gonococcal urethritis at the Venereal Disease Clinic of Choong-ku Public Health Center in Seoul were allocated randomly into one of two treatment regimens, and seventy patients were followed. All thirty-five patients including PPNG infections, treated with single oral dose of norfloxacin 600mg, were recovered(100%). One of thirty-five patients treated with a single oral dose of norfloxacin 800mg, failed. This failed case was one of eighteen nonPPNG infections (failure rate of 5.6%). Susceptibility test with disks containing norfloxacin 10ug showed the inhibition zone greater than 30mm. It is suggested that a single oral dose of 600mg or 800mg norfloxacin has good effect in the treatment of gonococcal urethritis with minimal side effects. Because of high prevalence of PPNG, it can be recommended as the first line treatment for gonorrhoea in Korea.

4.The Value of the Expression of bel-2 and p53 in Colorectal Carcinomas.

There are a lots of evidences that colorectal cancer arise as a result of multiple alterations of genes. Many attempts were made to understand the role of oncogenes and suppressor genes as a prognostic indicator, recently. Although histopathologic staging of tumor is the most important prognostic factor up to now, it is not enough to be used with full confidence. Apoptosis or programmed cell death represents a deletion of damaged or natural cell mechanism. The bel-2 proto-oncogene is known as a inhibitor of apoptosis that may allow accumulation and propagation of cells containing genetic alterations. Overexpression of bcl-2 probably plays a role in colorectal carcinogenesis. The aim of this study was to determine bcl-2 and p53 expression in colorectal carcinoma in correlation with apoptosis, clinical parameters, and histopathology, and to test their prognostic significance in patient with colorectal carcinoma. The bel-2 and p53 protein were identified by immunohistochemical staining using monoclonal and polyclonal antibody. The apoptotic index was detetermined by microscopic examination of hematoxyln and rosin-stained sections at x400. The materials subiected to this study were 54 paraffin-embedded colorectal carcinomas, which were collected randomly from January of 1992 to December of 1994 at Department of Surgery, Chosun University Hospital. Of 54 cases, 21 (38.9%) and 22(40.7%) showed positive expression of bel-2 and p53 protein respectively. Mean apoptotic index(AI) was 2.99% in colorectal carcinoma. Bcl-2 expression did not correlated with p53 expression or apoptotic index. Positive expression of p53 or AI was not correlate with any other clinical and pathologic parameters. An inverse correlation was found between bel-2 expression and increased tumor stage or Iymph node metastasis (P<0.05). In conclusion, these results suggest that bcl-2 expression is significant associated with early stage in colorectal carcinoma. But bcl-2 p53 and AI can`t be a independent prognostic factor in colorectal carcinoma. Further investigations to clarity its possible role in controlling the tumor decelopment and growth of colorectal carcinoma are needed.

5.Disability Scale for Patients with Spinal Cord Injury: Spinal Cord Independence Measure.

OBJECTIVE: The objectives of this study were to evaluate the reliability of the Korean version of Spinal Cord Independence Measure (SCIM) and to compare the sensitivity of the SCIM to functional changes of spinal cord injury (SCI) patients with that of the Functional Independence Measure (FIM). METHOD: Seventeen subjects with SCI were studied. The SCIM was translated and modified to convert as SCIM Korean-version. All patients were evaluated with the SCIM and the FIM by two raters every other week. To determine inter-rater reliability, the relationship between the SCIM scores obtained by two raters was evaluated by Kappa coefficient and linear regression. To determine relative sensitivity of the test to functional changes, changes in the scores on the SCIM and FIM were compared by McNemar test. RESULTS: The Kappa coefficient of the various individual tasks in SCIM ranged between 0.63 and 1.00. High correlations were also found between the total SCIM scores for the paired raters (r=0.99, p<0.01). The SCIM detected all the functional changes detected by FIM total scoring, but in 3 (14%) of 22 sequential test batteries, the FIM missed changes detected by SCIM total scoring. CONCLUSION: These results demonstrated that the SCIM is reliable and more sensitive than the FIM in reflecting the functional changes of SCI patients.

7.An Anesthetic Experience for Resection of Pheochromocytoma - 1 cases .

The author reports an anesthetic experience of right adrenalectomy. The patient was 38 years old male, having suffered from pheochromocytoma. Thiopental sodium, 300mg of 2.5% was used for induction, followed by N2O, O2, and halothane for maintenance and 0.1% S.C.C. solution in 500 ml of 5% D/W solution, dripped intravenously for muscle relaxation. AS the time of surgical manipulation of the gland, blood pressure was suddenly raised to 250/160mmHg, and a few minutes later massive bleeding, about 1800ml in amount, was detected in the operating and then the heart was arrested. Resuscitation was performed with adequate oxygenation and medications. The patient recovered well without any sequelae. The importance of preoperative preparation, sufficient sedation, smooth anesthetic induction, complete analgesia, good muscle relaxation, adequate ventilation and proper cardiovascular control are discussed together with the choice of anesthetic agents and practices.

8.An Anesthetic Experienee with Unilsteral Adrenalectomy for Cushing`s Syndrome due to Adrenocortical Adenama - 1 Case .

Anesthesia was induced with intravenous thiopental, followed by succinylcholine, N2O-O2-halothane and pancuronium. An endotracheal semiclosed circle absorption technic with controled ventilation was performed. Under continuous monitoring of blood pressure, pulse, EKG, CVP and urine output, the patient tolerated during the anesthetic period well. And high blood pressure was adequately controlled by increasing the percentage of halothane concentration only.

9.Anal Endosonographic Findings of Internal Anal Sphincter in Normal Adult Korean.

PURPOSE: To describe the appearance and average thickness of the internal anal sphincter with anal endosonography in healthy Korean adults. MATERIAL AND METHODS: 184 subjects(male: 96, female: 88) with no history of anorectal disease or surgery were studied with anal endosonography. The average thickness of internal sphincter was meas ured at the mid-anal canal. For the internal sphincter, which is often asymmetric, the thickness of each 4 part(12, 3, 6 and 9 o'clock direction) were measured in left lateral decubitus position. RESULTS: The anal wall was well visualized in 5 layers(mucosa, submucosa, internal anal sphincter, intersphincteric plane, external anal sphincter) with anal endosonography. The anal endosonogram showed the internal anal sphincter as a homogenous, well-defined, hypoechoic, circular band, and slightly asymmetric. The average thickness of the internal anal sphincter in the area of mid-anal portion was 2.0 0.3 mm(range: 1~3 mm). There was no sexual difference; however, a significant positive correlation with age was found in average thickness of the internal anal sphincter. The correlation with lean body mass was not found. CONCLUSION: The internal anal sphincter is well-visualized, best defined structure by anal endosonography. Average thickeness of the sphincter in Korean appeared to be the same as in the Western.

10.Chronic Arterial Occlusion by Hypercoagulable State 2 cases report (Antiphospholipid syndrome and Polycythemia vera).

Hypercoagulable states (HS) are well recognized as a cause of arterial and venous thromboembolism. These coditions are usually associated with abnormalities in natural anticoagulants, the fibrolytic system, or platelet aggregation. Primary arterial thrombosis is unusual in the young population. Hypercoagulable states are usually acquired, but they may be congenital. The congenital states occur in response to deficiency of protein C, protein S, AT-III. And the acquired states occur in response to disease, in response to tissue injury, or in response to therapy. But in this country, we have no any data of prevalence ratio of HS, which developed symptome of arterial occlusion, cases of bypass and theapeutic modality. We experienced two cases of acquired HS that is antiphospholipid syndrome and polycythemia. Based on our experience with this patients and a review of the literature on the previously reported. And we suggest that there will be made for the purpose of studying non-atherosclerotic lower extremity occlusion registry which developed in our country through the small study group.
