3.The Work Experiences of Emergency Room Nurses during the COVID-19 Pandemic

Purpose:
The purpose of this study was to explore emergency room nurse's work experiences during the COVID-19 pandemic.
Methods:
Data were collected from 15 registered nurses working in the emergency room between October, 10th and December 15th, 2020, through focus group interviews and in depth interviews.Collected data were analyzed using Colaizzi's phenomenological method.
Results:
The study derived four categories and eight themes. The four categories are ‘sense of cringing with fear’, ‘nursing in chaotic situation’, ‘lonely struggles’, and ‘career growth as a nurse’. Emergency room nurses experienced fear and confusion in the COVID-19 pandemic, but overcame the crisis and had positive experiences of professional growth as a nurse.
Conclusion
To protect emergency room nurses from infectious disease and improve their professional quality of life during the COVID-19 pandemic appropriate education and healthcare resource should be provided. It is necessary to reflect clinical worker's opinions and improve the delivery system of guideline to reduce confusion in the field and develop practical infectious disease control guideline. Furthermore, better working conditions, appropriate rewards and social and mental support should be provided for nurses in emergency room.
4.Cancer Care Facilities Nurses Experience of Infection Control

Purpose:
The purpose of this study was to explore cancer center nurses experience of infection control.
Methods:
From March 26 to 2019 to May 1, 2019 data were collected through focus group interviews and in-depth interviews. Data collected though interviews and field notes were analyzed using Colaizzi's phenomenological method. Participants were 12 registered nurses and 2 head nurses.
Results:
Fifteen themes emerged from four themes clusters. 1) Encountering barriers to infection control. 2) Infection control is still difficult. 3) Complaints about Infection Control Department exist. 4) Need to consider strategies for improving infection control.
Conclusion
The results of the study indicate that in order to facilitate infection control and increase the compliance rate of the guidelines, it is necessary to develop a program to increase the individual's competency associated with infection control and the elimination of negative factors. Further, mediation of external factors related to the hospital environment is necessary.
5.Living experiences of older patients with cancer amid the COVID-19 pandemic: A phenomenological study

Patients with cancer experience psychological and social problems; in particular, older patients with cancer face many difficulties during the cancer treatment process owing to aging and underlying diseases. Furthermore, the lives of individuals may be impacted by the COVID-19 pandemic. Therefore, the purpose of this study is to describe the experiences of older patients with cancer during the cancer diagnosis and treatment process amid the COVID-19 pandemic. Methods: This study employed a qualitative, descriptive phenomenological approach to explore and analyze the experiences of the participants. The participants in this study consisted of patients aged over 65 who were diagnosed with cancer, and data were collected from May 4 to June 30, 2022 through in-depth individual interviews. The collected data were analyzed using Giorgi’s phenomenological analysis. Results: Participants were six older patients with cancer with an average age of 69.66 years. Five theme clusters and 15 themes were generated. The theme clusters were “psychological adaptation to cancer diagnosis,” “receiving social and medical support,” “difficulty in the treatment process,” “fear of the COVID-19 pandemic,” and “living through difficulties.” Conclusion: This study revealed that older patients with cancer demonstrated a unique psychology about how to accept the shock of the diagnosis and the resulting coping patterns and feelings of depression. Additionally, it was verified that older patients with cancer, susceptible to infections due to their advanced age and weakened immunity, also face an increased vulnerability to COVID-19. Therefore, a support system tailored to the characteristics of older patients with cancer should be established.
6.Improved Bioethanol Production Using Activated Carbon-treated Acid Hydrolysate from Corn Hull in Pachysolen tannophilus.

To optimally convert corn hull, a byproduct from corn processing, into bioethanol using Pachysolen tannophlius, we investigated the optimal conditions for hydrolysis and removal of toxic substances in the hydrolysate via activated carbon treatment as well as the effects of this detoxification process on the kinetic parameters of bioethanol production. Maximum monosaccharide concentrations were obtained in hydrolysates in which 20 g of corn hull was hydrolyzed in 4% (v/v) H2SO4. Activated carbon treatment removed 92.3% of phenolic compounds from the hydrolysate. When untreated hydrolysate was used, the monosaccharides were not completely consumed, even at 480 h of culture. When activated carbon-treated hydrolysate was used, the monosaccharides were mostly consumed at 192 h of culture. In particular, when activated carbon-treated hydrolysate was used, bioethanol productivity (P) and specific bioethanol production rate (Qp) were 2.4 times and 3.4 times greater, respectively, compared to untreated hydrolysate. This was due to sustained bioethanol production during the period of xylose/arabinose utilization, which occurred only when activated carbon-treated hydrolysate was used.

7.Clinical Usefulness of Otoacoustic Emission Sum in Sudden Sensorineural Hearing Loss Patients.

BACKGROUND AND OBJECTIVES: Sudden hearing loss remain a controversial issue with respect to its prognostic indicators. In a recent study, the change in distortion product otoacoustic emission (DPOAE) has been shown to provide useful information about its prognosis. The purpose of this study is to evaluate the relationship between the changes of DPOAE and hearing improvement in sudden deafness patients. SUBJECTS AND METHOD: Sixty-one patients underwent pure tone audiometry (PTA) and DPOAE on their first hospital day; among them 35 patients underwent DPOAE after 2 weeks. These patients were divided into mild (< or =40 dB) group, moderate to moderately severe (41-70 dB) group and severe (> or =71 dB) group according to their initial hearing thresholds. RESULTS: Of the 22 patients in the moderate to severe group, 11 patients showed a DPOAE response and 5 patients (45%) showed hearing improved. In 11 non-DPOAE response patients, 4 patients (36.4%) had improved. We compared PTA and OAE sum gap results by analyzing 35 patient's initial thresholds with those measured 2 weeks after the therapy. The correlation coefficient between the two group was 0.547 (p<0.05). CONCLUSION: The results show that OAE sum is an efficient and non-invasive objective method and that it can be used to monitor the effects of treatment in sudden hearing loss patients.

8.Surgical Treatment of Piriformis Syndrome.

BACKGROUND: Piriformis syndrome (PS) is an uncommon disease characterized by symptoms resulting from compression/irritation of the sciatic nerve by the piriformis muscle. Uncertainty and controversy remain regarding the proper diagnosis and most effective form of treatment for PS. This study analyzes the diagnostic methods and efficacy of conservative and surgical treatments for PS. METHODS: From March 2006 to February 2013, we retrospectively reviewed 239 patients who were diagnosed with PS and screened them for eligibility according to our inclusion/exclusion criteria. All patients underwent various conservative treatments initially including activity modification, medications, physical therapy, local steroid injections into the piriformis muscle, and extracorporeal shock wave therapy for at least 3 months. We resected the piriformis muscle with/without neurolysis of the sciatic nerve in 12 patients who had intractable sciatica despite conservative treatment at least for 3 months. The average age of the patients (4 males and 8 females) was 61 years (range, 45 to 71 years). The average duration of symptoms before surgery was 22.1 months (range, 4 to 72 months), and the mean follow-up period was 22.7 months (range, 12 to 43 months). We evaluated the degree of pain and recorded the responses using a visual analog scale (VAS) preoperatively and 3 days and 12 months postoperatively. RESULTS: Buttock pain was more improved than sciatica with various conservative treatments. Compared with preoperatively, the VAS score was significantly decreased after the operation. Overall, satisfactory results were obtained in 10 patients (83%) after surgery. CONCLUSIONS: PS is thought to be an exclusively clinical diagnosis, and if the diagnosis is performed correctly, surgery can be a good treatment option in patients with refractory sciatica despite appropriate conservative treatments.

9.Periportal Distribution of Diethylnitrosamine-Induced Gamma-GTP Positive Foci in the Liver of Sprague-Dawley Rat.

BACKGROUNR/AIMS: Heterogeneity of liver cell populations within the hepatic lobules can affect xenobiotic reaction. In this study, we attempt to clarify the stereological distribution of the y-GTP positive foci in hepatic lobule initiated by diethylnitrosamine and promoted by phenobarbital. METHODS/METERIALS: To the five Sprague-Dawley rats, one day after birth, diethylnitrosamine (0.15 pmole/pn) was intraperitoneally injected and by the weaning at four weeks of life, 0.035% phenobarbital in drinking water was fed for four weeks, at which time the rats were sacrified to obtain the livers. Livers were fixed in fresh cold ethanol acetic acid. Ihe 200 cotmecutive tissue sections were stained by histochemistry for y-GIP and countastained with toluidine blue ar HkE. Employing Zeiss microprojector, y-GTP foci wae traced. And then the largest cross-sectian (size class: 65-165 pm in diameter) of 52 y-GTP foci were selected. Using microscopic grid, distances fiom center of y-GlP foci to nearest centtal vein and bile ductule was measured. As a control group, out of 52 random points determined by rareken digit table, the distances were measured by the same romr. RESULT: the diameter Aom the central points of y-GIP positive foci to the neatest bile ductules was measured as 0.234620+0.14899mm (mean+SD) and the diameter from random points to the neatest bile ductules was measured as 0.303080+0.19582mm (mean+SD). Y-GTP positive foci located within 0.3mm diameter fram bile ductules were counted as 73.1% of 52 foci, whereas that of random points were only 55.8%. Accoring to Aequency table analysis by Shapiro-Wilk W test, there was significant difference by W-value (0.812065) and p-value (0.0144866). CONCLUSION: Stereologically, y -GTP positive foci initiated by diethylnitros;unine, promoted by phaxkarbital in neonatal Spague-Dawley rats were non-rarxlanly distributed in peripcetal atea (Rappaport zone I) in hepatic lobule during hepatocarcinogenesis.

10.Centrilobular Distribution of Ethylnitrosourea-Induced Hepatocellular Foci in the Mouse.

BACKGROUND/AIMS: Hepatocytes on the hepatic lobule mipate from portal zone to centrilobular mea as the DNA synthesis within it. And also, the xenobiotic reactions reveal characteristic differences associated with zone specific metabolism in the liver acinus. In this study, the zonal distribution of ethylnitrosourea (ENU)-induced hepatic precancerous lesion was stereologically investigated. METHODS: Nine B6C3F1 mices were given I.p. injection of ENU (60 ug/pn body weight) when the pups were 15 days old prior to sacrifices at 8 weeks of life. All the 150 consecutive sections, 3 p m in thickness, were stained with hematoxylin and eosin and identified the basophilic precancerous lesions with 80-165 p m diameter in equatorial plane by the Zeiss microprojector. And then the distances from the center of selected foci to terminal hepatic vein or portal vein branches were estimated under the microscopic fields. As a control group, the same estimations were performed from the random points by the appointments of random digit table. RESULTS: Mean distance between ENU-induced 52 hepatocellular foci and the nearest terminal hepytic vein was 181.15+112.39 p m (Mean+ SD), but that of randomly selected 104 points was 291.73+157.98pm (Mean+5D) (Students t-test, p<0.0005). Substantially, 52.7% of ENU-induced 52 hepatocellular foci were within 300 p m from the terminal hepatic vein, but randomly selected 104 points were only 50.9% (Shapiro Wilk W test, w=0.819857, p=0.048038). Mean distance from ENU-induced 52 foci to portal vein was 398.85+149.98pm (Mean+SD), but that from the randomly selected 104 points was 315.87+145.79 pm (Mean+SD)(Students t-test, p<0.0005). CONCLUSION: Stereologically, ENU-induced mice liver cell foci distribute non-randomly to Zone III, centrilobular zone of mouse hepatic acini where promote invasion toward terminal hepatic veins.
