1.An Experimental Study on Relationship Between Cystitis Glandularis and Vesico-Rectal Fistulas.

An experimental study on cystitis glandularis induced by vesico-rectal fistula formation in rabbit was performed, to analyze relationship between histopathologic complexity of cystitis glandularis 3rd various forms of chronic proliferative cystitis, and to evaluate the possibility that contiguous endovesical growth of intestinal mucosa through fistulous tract may partly participate in the pathogenesis of cystitis glandularis. Followings were the results: 1) Various types and stages of chronic proliferative cystitis including cystitis glandularis were de. monstrable. 2) 51.5% of animals disclosed cystitis glandularis, of both intestinal and subtrigonal types with increasing frequency and severity roughly parallel to experimental duration. Minimum requirement of duration for development was approximately 3 months. 3) Among the cases with cystitis glandularis, intestinal type comprised 52. 9% mainly affecting trigonal regions, whereas subtrigonal type of 17.6% was encountered in the bladder neck. Rest of cases interpreted as mixed type, containing both intestinal and subtrigonal varieties. 4) More than two third of cases with cystitis glandularis accompanied nests of von Brunn, cystitis cystica, squamous metaplasia and small proportion of cystitis follicularis and bullosa, with indications of development of glandular structures followed by chronic non-specific cystitis, formation of von Brunn's cell nests with central liquefaction and glandular metaplasia in order of sequence. 5) Unusually high incidence of intestinal type of cystitis glandularis in the trigonal regions could partly be explained by endocystic contiguous extension or ectopic proliferation of intestinal glands through the fistulous tract. From the above experimental design, cystitis glandularis may develop not only following consequent metaplastic mechanism but also secondary to intravesical extension of colonic mucosa through the fistulous tract. None of cases showed invasive neoplastic growth, though cellular atypism was encountered only in three instances.

2.A Case of Pseudomonas Gangrene.

Forytfour-year-old female inpatient, who was gravely ill, developed vesicobullae on the greenish patches surrounded by narrow red to brown halo in the left lower extremity. Two years ago, she had treated by surgical operation and radiation therapy for cervix cancer. Blood and bullous fluid cultures showed Pseudomonas aeruginosa. She developed gangrenous and mummificated skin lesions after septic shock therapy but died due to disseminated intravascular coagulation.

5.Clinical Results of Phacoemulsification Performed by Ophthalmic Resident.

The clinical results and complications of 27 phacoemulsification which had been performed by ophthalmic resident during the learning curve were studied retrospectively. Mean postoperative follow-up period was 8.9 weeks and 82%(22 of 27) was followed up for 4 weeks or longer. Posterior chamber intraocular lenses were implanted in all cases. Final best corrected visual acuity of 0.5 or better was obtained in 21 eyes(95.5%) and mean value of the surgically induced astigmatism was -0.47 diopter of the 22 eyes who were followed up for 4 weeks or longer. The incidence of vitreous loss was 11.1 %(3/27). Of these, two cases occurred during nuclear emulsification, while one case during cortex aspiration. With proper patient selection, meticulous technique, and under the faculty's supervision, phacoemulsification performed by residents during the learning curve can be an effective method for cataract surgery procedure.

6.The gastrofiberscopic findings in pediatric patients.

We observed 240 cases ranging in age from 3 months to 15 years seen by gastrofiberoscopy in the department of pediatrics of Seoul Red Cross Hospital from March 1990 to October 1992 and following results were obtained. 1) The male to female ratios in total cases and in 89 patients complaining of abdominal pain for more than one month were 0.94:1 and 1.02:1 respectively. The most prevalent age group was 10~12 years in both groups. 2) The indication in the order of frequency were chronic abdominal pain (37.1%), acute abdominal pain (32.5%), vomiting (7.9%), hematemesis and melena (5.4%), follow-up study (6.3%), further evaluation (3.8%), chest discomfort (2.5%), foreign body removal (2.1%), nausea (1.2%), dysphagia (0.8%) and abdominal mass (0.4%). 3) The common endoscopic findings in the 225 cases excluding follow-up ones were gastritis (34.7%), duodenitis (14.2%), ulcer (11.6%), esophagitis (7.6%) and foreign body (2.2%). 4) The common endoscopic findings in cases of chronic abdominal pain were gastritis (42.7%), duodenitis (16.9%) and ulcer (7.9%). 5) There was no complication except mild sore throat.

7.Expression of Osteopontin in Osteoclast.

PURPOSE: This study was aimed to determine the relationship between osteopontin(OPN) and osteoclast, especially focused on whether ostecolast could produce osteopontin or not. MATERIALS AND METHODS: Osteoclasts were isolated from the giant cell tumor of proximal tibia and seeded on the 13 mm round cover slip resided in 24 multi-well plates for culture. After 2 days, osteclasts on the cover slip were fixed with cold acetone for 3 minutes and immunocytochemistry was done with rabbit osteopontin antibody. For in situ RT-PCR, osteoclasts on the cover-slips were fixed with 4% paraformaldehyde for 4 hours and were treated to pepsin. PR-PCR was done and the PCR producst were stained with anti-digoxigenin-AP. RESULTS: Osteopontins were found on the surface of the osteoclast by immunocytochemistry, and intense osteopontin mRNAs were found by in situ RT-PCR. CONCLUSION: We have identified that osteoclast could synthesize the osteopontin, and confirmed that in situ RT-PCR was a very useful method in expressing small amount of mRNA in case of mixed cell culture. Further study was needed to identify the action of the osteopontin produced by the osteoclast.

8.Clinical Evaluation of Laparoscopic Appendectomy.

BACKGROUND: This study was performed to evaluate our experience on laparoscopic appendectomy. METHODS: Retrospective analysis was performed on 2,856 patients who had been operated by laparoscopic appendectomy under diagnosis of acute appendicitis at the Sung-Ae Hospital and Kwang-Myoung Sung-Ae Hospital from October 1991 to July 1998. RESULTS: Among 2,856 patients who had undergone laparoscopic appendectomy,2,379 patients (83.3%) were operated due to simple acute appendicitis, 275 patients (9.6%) due to perforated appendicitis. Operation time was 44.6 minutes for simple acute appendicitis and 60.3 minutes for perforated appendicitis. In perforated appendicitis, intra-peritoneal irrigation and drain insertion was performed. The length of hospital stay in patient with simple acute appendicitis was 3.7 days (5.82 days in conventional appendectomy) and patients with perfotrated appendicitis was 6.1 days (9.91 days in conventional appen-dectomy). Complications such as wound infection, intra-abdomen abscess, trocar site bleeding, subcutaneous emphysema developed in 43 (1.5%) patients (79/1,947, 4.5% in conventional appendectomy). In 202 (7.1%) patients, appendix was normal, but another diseases were detected, including acute pelvic inflammation, ovarian cyst, mesenteric lymphadenitis, enteritis, diverticulitis in order. CONCLUSION: Overall complication rate was lower in laparoscopic appendectomy compared with conventional appendectomy and the length of hospitalization of laparoscopic appendectomy was shorter. When the acute appendicitis is suspected, especially in the reproductive women, the laparoscopic approach would be better diagnostic and therapeutic value than conventional method. Therefore laparoscopic appendectomy would be replaced with conventional appendectomy.

9.An experimental study on recovery of renal function using 99mTc DMSA scintigram after percutaneous nephrostomy in unilateral hydronephrosis.

Obstruction on the urinary tract eventually results in damage to the kidneys and loss of function. The questions that concern the clinician are the degree of nephron loss in that kidney and the potential for recovery following the relief of obstruction. 99mTc DMSA accumulates in tubule cells and has been proposed as a marker of the tubular mass. The authors estimated the renal uptake ratio of 99mTc DMSA for the degree of nephron loss corresponding to duration of hydronephrosis following left ureteral ligation in 5 NewZealand white rabbits and the potential for recovery following percutaneous nephrostomy of hydronephrosis in 24 rabbits. While the renal uptake ratio of 99mTc DMSA of the kidney with unilateral hydronephrosis following ureteral ligation reduced dramatically within 24 hour, that of the opposite healthy kidney increased, and the total renal uptake ratio was same as normal functioning kidneys before ureteral ligation. Upon ureteral release, there was no evidence of definite recovery or impairment in the experimental kidneys for 5 days. The authors conclude that a combination of ureteral release and administration of some drugs such as renal vasodilator or diuretics is an appropriate treatment for the recovery of function in unilateral hydronephrosis.

10.A case of Congenital patent urachus.

The complex group of changes which occurs during the intrauterine maturation of the urinary tract gives rise to several anomalies including those of the urachus. The occurrence of patent urachus is very rare and 67 cases has been reported by world literature since 1550, first description of patent urachus by Bartholomaeus Gabriolus A brief discussion of embryology, diagnosis and treatment is included for completeness. And a case of congenital patent urachus was seen at Han Ill Hospital.
