1.Application of micro-ELISA in serodiagnosis of human paragonimiasis.

To observed whether the specific IgG antibody test using ELISA was useful in diagnosis of presently ill patients of paragonimiasis, a total of 95 sera were tested. The sera were collected from 21 egg positive cases, 8 from positive reactors of intradermal test, 7 from Clonorchis infected, 9 from other parasitic diseases and 50 from apparently non-infected cases. By the result, the sensitivity of the test was 86% and the specificity was 100%. There were no cross reactions between Paragonimus antigen and other parastic infections. Specific IgG antibody test by micro-ELISA was concluded to be useful for mass screening of the presently ill paragonimiasis in the field.

Nonunion of fractures of forearm bone occurs frequently. These patients usually suffer from pain and functional disturbance of the hand and forearm. With the introduction of compression plate for the treatment of nonunion at the Campbell Clinic in 1959, a modification of the Nicoll graft was advised. The authors analysed 23 cases of the nonunion of forearm bone, in which military injured patients were admitted and treated by bone graft with internal fixation using various devices in Department of Orthopaedic Surgery of Korea Veterans Hospital from Sep. 1983 to Aug. 1988.

5.Appropriate Sizes of Uncuffed Endotracheal Tubes and Distance from Upper Incisor to Carina in Korean Children under Eight Years Old.

BACKGROUNDS: Various methods have been devised for choosing the correct internal diameter (ID) of endotracheal tubes and the proper lengths for oral endotracheal tubes at different ages in children. This study was performed to evaluate the appropriate sizes of uncuffed endotracheal tubes and distance from upper incisor to carina in Korean children under 8 years old. METHODS: Five hundred thirty five children under 8 years in ASA class 1 and 2 were evaluated for the study by age groups: 0~3 months, 4~11 months and every each year until 8 years. Appropriate sizes were to permit a gas leak at 15~25 cmH2O with positive pressure ventilation. Distances from incisor to carina were at the point which bilateral lung sounds were noticed during slow extubation from endobroncheal intubation. RESULTS: Appropriate tube sizes were 3.0 mm to 3.5 mm for infants under 3 months, 3.5 mm to 4.0 mm for 4 to 11 months and ""4.15+0.28xage (yr) (mm)"" (R2=0.77, p<0.05) for children between 1 and 8 years old. Distances from upper incisor to carina were 12.0 cm for infants under 3 months, 13.7 cm for 4 to 11 months and ""14.5+0.6xage (yr) (cm)"" (R2=0.62, p<0.05) for children between 1 and 8 years old. CONCLUSIONS: We conclude that the endotracheal tube ID for the Korean children are a little greater than those of previous reports in foreign countries and airway lengths for the Korean children are similar to those of foreigners.

7.Underdevelopment of Left Atrial Appendage.

A patient was admitted for catheter ablation of atrial fibrillation. Cardiac computed tomography and transesophageal echocardiography revealed the absence of the left atrial appendage. However, the right atrial appendage looked normal and the level of pro B-natriuretic peptide was within normal limits. Successful catheter ablation was performed without any procedural complications and the sinus rhythm was appropriately maintained for 10 months with an antiarrhythmic drug.

8.Differences between Korea and Japan in Physician Decision Making Regarding Permanent Pacemaker Implantation.

BACKGROUND AND OBJECTIVES: The number of permanent pacemakers (PPMs) implanted in patients in Japan and Korea differs significantly. We aimed to investigate the differences in decision making processes of implanting a PPM. MATERIALS AND METHODS: Our survey included 15 clinical case scenarios based on the 2008 AHA/ACC/HRS guidelines for device-based therapy of cardiac rhythm abnormalities (class unspecified). Members of the Korean and Japanese Societies of Cardiology were asked to rate each scenario according to a 5-point scale and to indicate their decisions for or against implantation. RESULTS: Eighty-nine Korean physicians and 192 Japanese physicians replied to the questionnaire. For the case scenarios in which there was a class I indication for PPM implantation, the decision to implant a PPM did not differ significantly between the two physician groups. However, the Japanese physicians were significantly more likely than the Korean physicians to choose implantation in class IIa scenarios (48% vs. 37%, p<0.001), class IIb scenarios (40% vs. 19%, p<0.001), and class III scenarios (36% vs. 18%, p<0.001). These results did not change when the cases were categorized based on disease entity, such as sinus node dysfunction and conduction abnormality. CONCLUSION: Korean physicians are less likely than Japanese physicians to favor a PPM implantation when considering a variety of clinical case scenarios, which probably contributes to the relatively small number of PPMs implanted in patients in Korea as compared with those in Japan.

Imatinib mesylate (Gleevec(R), formerly known as STI571) is a protein-tyrosine kinase inhibitor that inhibits the bcr-abl tyrosine kinase created by the Philadelphia chromosome abnormality in patients with chronic myeloid leukemia (CML). Between 7 to 21% of the patients treated with imatinib mesylate have been reported to develop adverse cutaneous reactions. However, exfoliative dermatitis related to imatinib mesylate is rare and there is only one previously reported case of this in the Korean literature. We report here on a woman with localized exfoliative dermatitis that developed after administering imatinib mesylate for treating her CML.

10.Predicting Prognosis of Mechanical Thrombectomy in Acute Ischemic Stroke Patients Using Modified DRAGON Score.

BACKGROUND: The DRAGON score is reportedly useful for predicting the outcome of intravenous thrombolysis. This study tested whether the modified DRAGON (mDRAGON) score, in which the onset-to-treatment time is extended, can predict the long-term outcome of transient ischemic stroke (AIS) patients who are candidates for mechanical thrombectomy (MT). METHODS: We assessed 40 AIS patients who were treated by MT alone or in combination with intravenous tissue plasminogen activator. The mDRAGON score is based on the following findings: hyperdense cerebral artery sign/early infarct signs on computed tomography (both=2, either=1, none=0), prestroke modified Rankin scale (mRS) score (>1=1), age (> or =80 years=2, 65-79 years=1, <65 years=0), glucose level (>144 mg/dL=1), onset-to-treatment time (>230 minutes=1), and baseline National Institutes of Health Stroke Scale score (>15=3, 10-15=2, 5-9=1, 0-4=0). Receiver operating characteristics (ROC) curve analysis was used to compare the performance of the mDRAGON score with the performances of other stroke prediction scores. RESULTS: Among 40 AIS patients treated with MT, the proportions with a good outcome (mRS score=0-2) in the groups with mDRAGON scores of 2 or 3, 4 or 5, 6 or 7, and 8-10 were 75%, 20%, 0%, and 0%, respectively, while the corresponding proportions with a poor outcome (mRS score=3-6) were 25%, 80%, 100%, and 100%. For the prediction of a good outcome at 3 months (mRS score=0-2), the area under the ROC curve of the mDRAGON scores was 0.87 (0.76-0.90). CONCLUSIONS: The mDRAGON score can be used to reliably predict the clinical outcome of AIS patients following endovascular treatment.
