5.Feasibility of internal curve distractor of the zygoma for maxillary segmental reconstruction

BACKGROUND:The functional reconstruction of large maxillary defects is a significant challenge due to the lack of bony support.In this paper.a new approach for segmental maxillary reconstruction and bony support foundation is put forward and tested by animal experiment.OBJECTIVE:To evaluate the possibility of segmental maxillary regeneration by internal curve distraction osteogenesis of zygoma with the self-developed distractor.DESIGN:Animal experiment.SETTING:Department of Stomatology.the 252 Hospital of Chinese PLA.MATERIALS: Two healthy female hybrid canines of 1-1.5 years old and 20-25 kg were selected. Internal curve distractor was composed of three parts:curve track,movable part and a flexible central thread bar fixed at the center of the curve track.The former two parts,which was made of TC4,(Ti-6AI-4V),and the flexible central thread bar was made of nickel titanium wire. A 360° of counter clockwise rotation of the distraction activator achieved an advancement of 0.35 mm for the movable part;the maximum distraction length allowed by the distractor was 25 mm.METHODS: The experiment was carried out in the animal experimental center of Fourth Military Medical University of Chinese PLA from October 2003 to February 2004. The segmental defect of 18-20 mm in width was made in maxilla and zygoma to simulate maxillary defects.Then an osteotomy distal to the defect was performed to create the variable transport disk. The distractor was fixed to the residual zygoma and zygomatic arch for internal curve distraction osteogenesis.After a latency period of 5 days,the distraction started at a ratio of 1.05 mm per day (0.35 mm once,3 times per day) for 13 consecutive days. Before and after distraction and at the second, fourth and eighth week of consolidation, the bone radiographs of residual zygoma and zygomatic arch were taken routinely.MAIN OUTCOME MEASURES: The general condition of the animals and distractor, and the quality and quantity of new formation bone in the distracted gap.RESULTS:Canines and distractor were in good condition during the experimental period.X-ray film showed that at the end of distraction, the bony transport disks were distracted about 12 mm around arc.At the 8th week of consolidation,the density of new bone was close to that of normal bone,and the curved bony regeneration was almost completed.CONCLUSION: The serf-designed curve internal distractor is fit the condition of major maxillary defects,and could be used for the internal curve distraction of zygoma.

Professor SONG Nanchang's clinical experience and characteristics for treatment of peripheral facial paralysis are introduced. In clinical treatment, professor SONG has adopted staging treatment strategy, and performed acupuncture stimulation with different levels. He attaches great importance to the acupoint selection on distal limbs. For the treatment on the face, he takes temperature as necessity; he inherits from famous Chinese doctor ZONG Ruilin's acupuncture technique of slow-twisting and gentle-pressing. Meanwhile, he excels in combination, of different therapies, using acupuncture, moxibustion, electroacupuncture, auricular point sticking, Chinese herbal medicine, etc. according to individual condition and disease stages. He also emphasizes on psychological counseling and daily life care to achieve rehabilitation within the shortest time.

9.Degradation of PDLLA cage in the intervertebral fusion

[Objective]To observe the degradation process of PDLLA in the intervertebral fusion.[Method]Twenty goats were divided into 4 groups as experimental group,and their L_(3~4)intervertebal spaces were implanted with PDLLA cage containing pieces of graft bone.Animals were sacrificed at 4,8,12,16 wks and specimens were taken for observation of the degradation process and bone fusion by gross observation and electronic microscope.Another 12 goats were used as normal fusion control group.Their L_(3~4)space were grafted with bone block for fusion.[Result]The degradation rate was non-lineal.In the early stage of fusion,the main degradation and decrease of molecular weight was shown at the superficial decomposition.With the proceeding of fusion and degradation,as the kydrolyzation speeding-up,internal decomposition by self-catalyse resulted into the collapse and total disassemble of the PDLLA.The PDLLA cage maintained its shape in the early stage and its biomechanical strength decrease in late stage but was still enough to keep the structure from collapse,till the fusion was achieved in the bone implant area.[Conclusion]The velocity of degradation of PDLLA is slower than the speed of bone regeneration of bone fusion,so the PDLLA cage could provide sufficient support during the process of intervertebal fusion and is a suitable choice of degradative material for cage in the intervertebal fusion.
10.Anesthetic efficacy of epidural ropivacaine mixed with chloroprocaine for hysterectomy

Objective To assess the efficacy of the mixtures of different concentrations of ropivacaine(R) and chloroprocaine(C)for epidural anesthesia in patients undergoing hysterectomy.Methods Sixty ASAⅠorⅡpatients aged 27-56 weighing 45-75 kg undergoing elective hysterectomy were randomly divided into 4 groups(n= 15 each);groupⅠ0.75% R alone;groupⅡ0.5% R-1% C;groupⅢ0.5% R-1.5% C and groupⅣ0.75% R+1% C.The epidural catheter was placed at L_(2,3)interspace and advanced 3.5 cm into the epidural space in cephalad direction.A total of 22 ml of epidural solution was injected in each group.The onset time,block height and duration of sensory block and the onset time,degree and duration of motor block(using Bromage scale)were assessed.The use of supplementary drugs(ketamine and ephedrine)and side effects were recorded.Results The onset time of sensory and motor block was significantly shorter in groupⅡ,ⅢandⅣthan in groupⅠ(0.75% R alone).The duration of sensory and motor block was significantly shorter in groupⅡandⅢthan in groupⅠandⅣ.The incidence of hypotension was significantly increased but the incidence of discomfort produced by traction on the viscera during operation was reduced in groupⅣas compared with groupⅠ.Conclusion The anesthetic efficacy of epidural 0.5% ropivacaine is significantly enhanced when mixed with 1.0% or 1.5% chloroprocaine.
