6.A Case of Porokeratosis Arising in Previous Burn Areas.

Porokeratosis is a specific disorder of keratinization characterized histologically by the presence of a cornoid lamella. Clinically, the basic lesion is a sharply demarcated hyperkeratotic plaque which may be linear, punctated, or annular with central atrophy. The etiology of the various types of porokeratosis is unknown. However, heredity, UV radiation, immunosuppression, trauma, burns, and occult infection are known to be precipitating or exacerbating factors. A 32-year-old female was burnt on her right arm and chest at the age of 12. Several years later, brownish plaques developed in these burn areas. Seven years prior to visiting our clinic, nu- merous match-head sized, peripherally elevated macules developed on the forearms and have gradually spread to the other areas of her upper and lower extremities. The histological findings of two lesions from the burn areas showed the same features including cornoid lamellae in the epidermis and fibrosis in the dermis. Howerer, the histological finding of a lesion from a non-burn area showed cornoid lamellae in the epidermis without evidence of dermal fibrosis. We believe our case is the first to be reported in Korea in which porokeratosis arose in previous burn areas.

8.Congenital Heart Disease and Extracardiac Anomalies.

Purpose: The diagnostic usefulness of fetal echocardiography in congenital heart disease is well known. One of the indications for fetal echocardiography is presence of extracardiac anomalies and such coexistent congenital heart disease may have important implications for obstetric and neonatal management. MATERIALS AND METHODS: 496 women with singleton pegnancies with amgenital anomalies that underwent penatal care and follow-up in Severance Hospital for 5 years from 1991 to 1995 were evaluated for the incidence of congenital heart disease. RESULTS: The incidence of congenital heart disease in fetuses with the associated extracardiac anomalies were as follows; central nervous system 6.9%(11/160), gastro-intestinal system 10.7%(8/75), genito-urinary sysem 4.7%(4/85), others 5.6%(4/72). Thirty-one of eighty-ane fetuses with congenital heart disease were found to have the associated extracardidac anomalies and ten of them chenoscenal abnormalities. CONCLUSION: We recommend that fetuses with congenital extracardiac anomalies sbould be checked for the presence of congenital heart disease and chmmosomal abnormalities, if needed.

10.Eukaryotic Kxpression of the Major Antigenic Determinants Evoking Neutralizing Antibodies in Human Cytomegalovirus ( HCMV ) Isolated in Korea.

Human cytomegalovirus (HCMV) isolated from Korean patients is different in the antigenic and genomic structure of gB from the laboratory-adapted strain. To dissect the reactivity to HCMV glycoprotein B (gB) domains, each domain gene of gB of HCMV SNUCH1, Korean isolate, was amplified from the extracted DNA of the virus-infected fibroblasts with the specific primers by polymerase chain reaction (PCR). Amplified DNA was cloned into pcDNA3. Immunofluorescent staining and western blot analysis revealed that the expressed gB in mammalian cells was immunoreactive and equivalent to the naturally expressed gB in virus-infected fibroblasts. The antigenic component reactive with monoclonal antibodies, MCMVA 57, 88, and 98 appeared at the D3 domain of gB molecule, and that with MCMVA 66 and 135 at the D2b domain. Antibody titer was measured with HCMV-infected fibroblasts and the domains of gB expressed in mammalian cells. There was no correlation between the antibody titer to the whole HCMV and neutralizing antibody titer, and between the antibody titer to whole HCMV and whole gB. It was more reasonable to use whole gB than whole HCMV in the comparison with the neutralizing antibody titer. D3 was representative domain in gB molecule in the anti-gB reactivity. Conclusively it is highly recommendable to use the representing isolates in Korea and its domains for the detection of antibody or the analysis of antigen in the aspect of immunological properties and molecular structures.
