3.Requirements for the Differentiation of CD4+ Cytotoxic T cells Specific to Mycobacterial Antigens.

The cytolytic activity of CD4' T cells, both human and murine, has been clearly demonstrated in the immune response to mycobacterial infection and suggested to play a significant role in the protection and immunopathology. However, Uttle is known about the differentiation of CD4' CTL. In order to address this issue, we examined the influences of some factors on the generation of CD4' CTL specific to mycobacterial antigens. After 7 days' stimulation of PBMCs from healthy tuberculin reactors with mycobacterial antigens, the cytolytic activity of purised CD4' T cells toward autologous macrophages infected with mycobacteria was measured by Cr release assay. First, we found that both of live M. tubeiculosis and soluble antigens (ST-CF) induced the cytolytic activity of CD4' T cells, although the inducibility of the former was slightly greater than the latter. Second, the cytolytic activity was maximally induced at the relatively low antigen concentration (0.2:1 bacteria:monocyte ratio or 0.5 mg/ml of ST-CF). Finally, in the presence of increasing amounts of neutralizing anti-IL-12 or anti-IFN-r MoAb, the cytolytic activity of CD4+ T cells was decreased in a dose-dependent manner. These results suggest that low dose of antigen, its particulate type give mycobacteria), IL-12, and IFN-r give some positive signals for the generation of CD4+ CTL.

4.Lumbar Spinal Mobility after Anterior Stabilization of the Thoracolumbar Spinal Fractures.

STUDY DESIGN: We analyzed the segmental mobility of the lumbar spine after anterior stabilization in the thoracolumbar spinal fractures, and the data were compared with those obtained from an asymptomatic control group. OBJECTIVE: To determine the effects of anterior stabilization on the unfused segments of the lumbar spine. SUMMARY OF LITERATURE REVIEW: Spinal fusions are commenly used to stabilize unstable motion segments and to help the maintenance of correction afforded by the instrumentation applied in surgery for spinal deformity or injuries. How the presence of the fusion effects upon the remaining infused spine is not well understood. MATERIALS AND METHODS: We measured the segmental deformations of the lumbar spine radiologically, from maximum flexion to maximum extension, right and left maximum lateral bending, in 20 asymptomatic volunteers( the control group ) and 30 patients who had the anterior stabilization of the thoracolumbar spinal fractures(the study group). We assessed the percentage of segmental deformations which were obtained by multipling one hundred after the segmental deformation value was divided by the total lumbar deformation value. We made the comparison between the White and Panjabi's results and the control group, between the control group and the study group, between the segmental deformations and the percentage of segmental deformations. RESULTS: In the control group , the segmental deformations were smaller than those of the corresponding segmenus in the White and Panjabi's results. In the flexion-extension rotation of the lumbarspine, the segmental deformations had a tendency to increase from cephalad to caudal in the controland study group. In the residual lumbar spinal mobility, the segmental deformations of the study group had decreased rather than those of the control group. Especially the segmental deformation below the juxtafused segment had increased more than those of the corresponding segment of the control group in the right lateral bending rotation. The residual lumbar spinal mobility tended to decrease from T12 vertebral stabilization to L2 vertebral stabilization. CONCLUSION: The unfused segments had accommodated a greater percentage of segmental deformations in spite of diminishing the segmental deformations in comparison with those of the corresponding control subjects. It suggests that the greater percentage of segmental deformations predisposes to early degeneration of unfused segments.

The etiology of neural tube defects, a category encompassing spina bifida, anencephaly and encephalocele, remains highly controversial and unclear, However, there is overwhelming evidence supporting a multifactorial etiology for this group of defects. Recent studies have shown that folic acid supplements taken periconceptionally can reduce a woman's risk of having a child with a neural tube defect. Qenetic screening could identify women who will require folic acid supplements periconceptionally to reduce their risk of having a child with a neural tube defects. Recently, we encountered two cases of recurrent anencephaly that had occurted in a same pregnant woman in our hospital. We report these two cases with brief review of the literatures.

The contour of mandibular angle is important for facial esthetics in the oriental population, because the wide and square face is thought to have had an unhappy life. A prominent mandibular angle produces a characteristics quardrangle, coase, and muscular appearance. So, uni- or bi-lateral mandibular angle reduction is usually performed. However, there is little guideline for bi-lateral angle reduction. So, this study was to investigate the pattern of Korean mandibular angle for bi-lateral mandibular angle reduction as guideline. This study was included 66 adult men and women over 19 years old who are having the normal mandibular angle. We measured the posterior border, angle, and inferior border of mandible using cephalometric view. The results of study was as followed : 1. The ratio of posterior mandible was 96.6, 97.3% in male, and 103.0, 106.0% in female. 2. The ratio of mandibular angle(R1-Go/R1-R2) was 120.2% in male, and 117.3% in female. 3. The ratio of inferior mandible(D5, 6, 7, E, F, G/D4) was 97. 3, 90.9, 79.5, 65.2, 57.8, 46.9% in male, and 98.5, 91.2, 80.5, 67.6, 59.1, 50.2 % in female. The results of this study should be useful to decide accurate amount of reduction.

9.The Clinical analysis of Tibial Shaft Fracture at the Primary Trauma Hospital

Tibia fractures are frequently occurred with increased traffic accident and industrial accident recently. And there are controversies in treatment of tibia fractures. From Jan. 1986 to Oct. 1988, tibial fractures of 107 cases in 105 patient, treated at the Department of Orthopedic Surgery of Dae Han Hospital, were analyzed. Results were obtained as follows. l. Of 105 patients, 85 patients were male & 20 patients were female. 2. The most common causes of fracture was traffic accident. 3. The most common level of fracture was middle one-third of tibial shaft. 4. Of 107 cases, conservative treatment was 35 cases and operative treatment in 72 cases. 5. Average union time was 16 weeks in conservative treatment & 18.5 weeks in operative treatment.

10.Extraskeletal Ewing`s Sarcoma on the Duodenum.

Extraskeletal Ewing's sarcoma is a rare primary malignant soft tissue tumor that has the same pathological characteristics of skeletal Ewing's sarcoma. It is known that the differential diagnosis of Ewing's sarcoma from undifferentiated rhabdomyosarcoma, neuroblastoma, reticulum cell sarcoma, and malignant lymphoma by the histologic studies only are not easy because their histologic findings are nearly similar each other. Therefore, immunohistochemical method, electron microscopic examination, cytogenetic study, or molecular gene study are used together to confirm the diagnosis. It chiefly involves the lower extremity, soft tissues of the paravertebral region, and adjacent chest wall. We recently experienced a case of extraskeletal Ewing's sarcoma of duodenum in a 21-year old female. The patient underwent Whipple's operation and postoperative combined chemotherapy were done.
