2.Blepharoptosis Correction with Muller Tuck Method.

There are several methods for blepharoptosis correction. Many complication of blepharoptosis induced by operation. Lid lag and lagophthalmos is especially, a difficult problem to resolve. So, many plastic surgeons make an effort to prevent lid lag or lagophthalmos. One of blepharotposis correction methods, we have used Muller's muscle tucking method since 2004. The author's method is Muller's muscle tuck with fixation to the tarsal plate and pretarsal fascia with nylon #6-0 and aponeurosis sutured with pretarsal orbicularis muscle or pretarsal area dermis. We have found this method to show good results, with resulting no or only mild lagophthalmos than previous ptosis corrective methods. This method yield results with more post-operative predictability than aponeurosis surgery due to preservation of the aponeurosis gliding plane. If this method is applied to mild and moderate ptosis, it is one of good method of ptosis correction.

4.A Case of Internalization of Thracoamniotic Shunt of Fatal Bilateral Chylothorax.

Using 3D ultrasound, bilateral chylothorax was diagnosed antenatally in the second trimester. Apparently stable, bilateral pleural effusion progressed rapidly to severe hydrops with facial edema during observation, and then we decided bilateral pleural-amniotic shunt operation. Here we present a case where drainage of pleural effusion by a double reverse pig tail stent made by ourself was achieved, although placement of the thoracoamniotic shunt resulted in near complete drainage of bilateral pleural effusion with normalization of intrathoracic anatomic relationships, subsequent resolution of fetal hydrops, but the ultimate outcome was unsuccessful due to the internalization of one catheter and unknown sudden death. We think that ongoing research is required to further evaluation about complications associated with this procedure, specifically failure of function due to obstruction, migration of the catheter,

The gastrointestinal polyp is a premalignant lesion in varying degree. According to the size, the pathology and the location of polyps, it should be removed. Polyps in the gastrointestinal tract can be removed by surgical excision, electrosurgery, cryotherapy, regional radiotherapy and laser therapy with some limitations. Considerable advance in therapeutic endoscopy as well as in diagnostic endoscopy along with newly developed instruments has now made it possible to excise gastrointestinal polyps by means of high frequency generator. We have underwent endoscopic polypectomy, strip biopsy and "O" ring ligation on gastric polyps in 112 patients who visited in this hospital from July l988 to January 1994, and the clinical characteristics including histopathology has been evaluated. Removed polyps were variable in size from less than 0.5 cm up to 4cm. Finally, we concluded that endoscopic polypectomy is a safe and relativly simple procedure and postpolypectomy complication can be considerably reduced by conservative antiulcer therapy.

9.Comparison of Medical Therapeutic Efficacy in the Patinet with Diabetic Cystopathy According to Prevalence Period of Diabetes Mellitus.

PURPOSE: Patients with diabetic cystopathy ultimately undergo morphometric and functional changes in their bladder. Therefore, various voiding symptoms such as frequency, nocturia, urgency, hesitancy, retention and weak stream were evaluated. Irritative symptoms usually develop in cases where the prevalence period of diabetes mellitus (DM) is short, and the obstructive symptoms develop in the opposite case. Improving the voiding symptoms might differ according to prevalence period. Therefore, the therapeutic efficacy of the treatment was compared. MATERIALS AND METHODS: One hundred ten patients were divided into three groups according to how long they had suffered from DM, and were treated with medical therapy over a 4 week period: group A (<5yrs), group B (5-10yrs), group C (>10yrs). The patients were asked to write their subjective symptoms in a voiding diary and a urodynamic study was undertaken. RESULTS: After medical therapy, the frequency, nocturia and urgency in group A had improved to 72%, 77% and 88% respectively (p<0.05). The frequency, nocturia, urgency, hesitancy, retention and weak stream in group B had improved to 51%, 57%, 66%, 59%, 69% and 61% respectively (p<0.05), and in group C these symptoms had improved to 15%, 12%, 10%, 17%, 12% and 8% respectively (p>0.05). In a urodynamic study of group B, the maximum detrusor pressure had increased from 20.3+/-6.7cmH2O to 53.5+/-8.1cmH2O and the post void residuals decreased from 62.3+/-36.4ml to 21.2+/-17.1ml (p<0.05). In addition, there were little improvements in the maximum bladder capacity, the first sensation and compliance (p>0.05). In group C, there was little improvement in the urodynamic study (p>0.05). CONCLUSIONS: The medical therapeutic efficacy for these patients reduces as the DM revalence period is longer.

10.A Statistical Standard for Detecting Epidemic of Notifiable Acute Communicable Diseases in Korea.

Many problems have been stated in the surveillance system of notifiable acute communicable diseases in Korea. Lack of objective tools for detecting epidemic is one of the most fundamental. We propose a statistical standard for detecting epidemic of those diseases that could be easily and promptly applicable to the existing data. Suggested standard measure is computed from the median and the spread of upper and lower hinge(spr(H)) which is robust to the assumption of normal distribution, so frequently used in exploratory data analysis as a measure of variation, and the results are compared with those of existing method using recent 3 years from January 1994 to December 1996 of monthly data of 8 notifiable acute communicable diseases in Korea. Monthly pattern of statistical epidemic between the proposed (median) and existing(mean) methods is similar. Therefore, we propose that the statistical epidemic should be defined when the current occurrence exeeds the standards of both methods. When the data collection is made weekly than monthly, the proposed method of determining the time of epidemic will be much helpful for the management of notifiable diseases.
