1.Lyme disease.

Lyme disease is an infecious disease caused by Borrelia burgdorfieii and has been reported world-wide. The spirochete invades multiple organs and induces manifesations in the skin, joints, neri us and cardiovascular systems. 13espite a considerable amount of serological evidence and isolation of R burgdorferi in ticks, no human case has yet been reported in korea. In this report, we present the first clinical case of Lyme disease in Korea. A female patient, 26 years old, developed clinical signs of eryth m,i chronicum migpans about ten days after traveling to a rural area and responded well to treatment ith entibiotions. Laboratory tests showed elevated IgM and IgG responses to B. burgdorferi, and a spirociete was found in a skin biopsy by electron microscopy.

4.Chondrosarcoma of the Pelvis.

PURPOSE: This study was undertaken to evaluate the long term survival of pelvic chondrosarcomas, optimal reconstructive procedures and functional results. MATERIAL AND METHODS: We studied 14 primary and 3 secondary cases of pelvic chondrosarcomas. All patients had an internal pelvectomy. Resection types were the iliac wing in 4, periacetabular in 9, and there were 4 in the pubic area. Reconstructive types were: no reconstruction in 6, spacer and bone graft in 2, heat treated autogenous bone-total hip replacement arthroplasty (THRA) composite in 4, limited fusion in 2 and the remaining 3 patients had a saddle prosthesis. RESULTS: An actual and continuous disease free survival rate was 93/87% at 130 months. The overall functional score was 22.4. The score for four cases of iliac resection was 27.7, in nine cases of periacetabular it was 18.6, and in four cases of pubic lesion it was 25.5. Associated complications were dislocation of THRA in one case, two cases of infection, sciatic nerve palsy in one and one case of skin necrosis that was secondary to radiation. CONCLUSION: Chondrosarcoma of pelvis shows an excellent long term survival rate. Attempts at pseudoarthrosis rather than anatomical reconstruction in periacetabular resections with massive soft tissue damage may reduce complications and their functional results are acceptable.

10.Plain Radiographic Findings of a Supraspinatus Tear: Correlation with MR Findings.

PURPOSE: To correlate the plain radiographic findings of a supraspinatus tear with the degree and extent of a supraspinatus tear on MR images. MATERIALS AND METHODS: We retrospectively reviewed the plain radiographs of 35 patients with a supraspinatus tendon tear confirmed by MRI and surgery from July 2004 to June 2006. On the plain radiographs, degenerative changes of acromion and the greater tuberosity and acromiohumeral distance were evaluated. Patients were divided into groups according to the degree and size of the supraspinatus tendon tear and the duration of symptoms. We compared the degenerative changes score and acromiohumeral distance among the groups. RESULTS: There was no statistically significant difference for the degenerative changes score for acromion and greater tuberosity among the groups classified by the degree and size of the supraspinatus tendon tear. However, the acromiohumeral distance was shorter as the extent of the tear was larger; this finding was statistically significant. There was no statistical correlation for the degenerative changes score and the acromiohumeral distance with the duration of symptoms. CONCLUSION: On plain radiographs of the shoulder joint, a decreased acromiohumeral distance is useful to predict the degree and size of a supraspinatus tear, but degenerative changes are not useful to predict the degree and size of a supraspinatus tear and do not correlate with symptom duration.
