1.Metabolism of C(14)-acetate by some trematodes.

The adult trematodes, Fasciola hepatica, Eurytrema pancreaticum and Paramphistomum cervi, employed in this experiment were obtained from the cattle slaughtered at the local abbatoir. The worms selected and washed several times in normal sterilized saline solution. Each about ten of intact F. hepatica, fourty of E. pancreaticum, and twenty of P. cervi were incubated in 50 cc volume of special incubation flasks with incubation medium consisting of 10 cc. of Krebs-Ringer phosphate buffer(pH 7.4) The incubation medium was added C(14)-1-acetate and non-radioactive carrier Na-acetate so as to contain acetate concentration of 50 mg per cent . The worms were allowed to incubate for 5 hours in the Dubnoff metabolic shaking incubator at 38 C. After incubation period, respiratory CO(2) samples from central well of incubation flask were analysed for total CO(2) production rate and their specific activity of respiratory CO(2). The lactate and pyruvate appearance rates were determined by analyzing the lactate and pyruvate concentration in a medium after incubation. The glycogen samples isolated from worms were analyzed for the tissue concentration and their radioactivities in order to determine the turnover rate of glycogen pool. Radioactivities of these series of experiments were counted by an endwindow Geiger-Muller counter as an infinitely thin samples. The quantitative analysis of C(14)-acetate utilized by F. hepatica, E. pancreaticum and P. cervi were compared and discussed in this report. According to these data of the experiment, it is suggested that the fatty acid such as acetate may play a part of their oxidative process into the respiratory CO2 and the synthetic process into glycogen in the above species of trematodes.

2.An Epidemiological Study on the Accident resulting from Leakage of Chloride.

During the Period from 22th to November 26th , 1971, An Epidemiological Survey was performed on the Accident which was occurred by Leakage of Chloride Gases in A Chimical Industry, one of Wul San Industrial Areas. The obtained results were as follows ; 1. The largest damage was observed at Yeo Cheon Dong, 5.38 Km2 in Area and 8,192 in the Residents. 2. By the age distribution of Yeo Cheon Dong, The largest damaged area resulting from chloride accident, the age group of between 10 and 39 was highly occupied with 55.2% of all Yeo Cheon residents. 3. The number of patient was 436 totally, 198 in male and 238 in female. 4. By the age group distribution of the patient, in case of male, the largest group was under 20 age (96) and female, between 20 and 40 age group. 5. By the educational level of the patient, No-Education group was 116 (26.6%), and primary school, middle school and high school were respectively 168 (38.5%), 68(15.6%) and 12 (2.8%). 6. By the occupational distribution of the patient, No-Occupation group was 49.7%, students and agriculture were respectively 14.4% and 8.1%. 7. By the complaints of the patient, most of all were for respiratory tract, those are coughing (56.9%), chest tightness (35.6%), sore throat (24.8%), and so forth. 8. By the injured domestic animal, the Dog was most highly injured with 46.2%.

From May lst to November 30th, 1971, a study was conducted by authors on the health status of industrial workers through applying the modified Cornell Medical Index forwards a total of 4,565 labourers working in Busan City. The obtained results were as follows : 1. The largest number of industries and labourers was in Busanjin Gu, 9 and 2,386 (52.2%), respectively. 2. Total numbers of labourers subjected to the survey were 2,355 (51.6% in male, 2,210(48.4%) in female. 3. By age and sex distribution, the largest was seen with the age group of 25-34, 1,260 in male, otherwise with under the age group of 24, 1,994 in female labourers. 4. The highest average number of complaints by section among all age groups and industries was as follows ; In case of male labourers, textile was the highest with numbering 8.7 (35-39 age group) in the section of physical complaints, in the section of mental and total complaints, food industry was the highest with numbering 8.2 and 16.4, respectively, both under 19 age group, otherwise in case of female labourers, food industry was the highest with numbering 12.3 (physical), 10.3(mental) and 22.7 (total complaints) with all in 20-24 age group. 5. The lowest average number of complaints by section among all age groups and industries was as follows; In case of male labourers, other industry was the lowest with numbering 0.4 (55and over) in the physical, 0.3 (19and under) in the mental and 0.7 (55and over) in the section of total complaints, otherwise incase of female labourers, basic metal was the lowest in the all sections with all in 50-54 age groups.

6.Percutaneous Balloon Valvuloplasty of Pulmonary Valvular Stenosis in Adult.

Percutaneous balloon valvuloplasty (PBV) was done in 7 adults of pulmonary valvular stenosis, which was diagnosed with catheterization and cine-angiography of rigtht heart, in Seoul National University Hospital from Jan. 1986 to Dec. 1986. The results were as follows : 1) Among the seven cases six were male and the range of age was from 19 to 40 years. Atrial septal defects associated in 2 cases were not significant clinically. 2) The peak systolic gradient from right ventricle to pulmonary artery before the procedure was below 50 mmhg in 1 cases, 50 to 100 mmHg in 1 case, and above 100 mmHg in 5 cases. 3) Before PBV the mean systolic pressure of right ventricle was 121+/-42 mmHg and the peak systolic gradient was 106+/-41 mmHg. After PBV those were 58+/-30 mmHg and 40+/-31 mmHg, revealing significant improvement. 4) Balloon catheter (Medi-tech, 9F) was used in all cases. single balloon of 20 mm outer diameter was applied in 2 cases and double balloons of various sizes were applied in 5 cases in which single balloon showed inadequate dilatation. It is suggested that percutaneous balloon valvuloplasty is safe and effective alternative to surgical valvotomy of pulmonary valvular stenosis in adults.

7.A Survey of Breast-Feeding.

PURPOSE: The superiority of breast-feeding to artificial feeding of infants aged 6 months or less has been well established. Due to the low rate of breast-feeding in our 1994 hospital survey we recommended and educated about breast-feeding to mothers who delivered in our hospital since January 1995. The purpose of this survey is to assess if the rate of breast-feeding increased in our hospital thereafter. METHODS: One hundred twenty-five infant born at National Medical Center (NMC) after January 1. 1995 were surveyed through questionnaire about breast feeding at the well-baby clinic of pediatric department of NMC from May, 1996 to September, 1996. RESULTS: The rate of breast-feeding was 26.4% in the nursery after birth, 40.8% at 1 month of age. Breast-feeding at 1 month of age was more common among children fed breast milk in the nursery after birth (P<0.05), who initiated breast-feeding within 3 days of age (P<0.05), born by vaginal delivery (P<0.05) and positive maternal response to the guideline of breast-feeding (P<0.05). The decision of breast-feeding was made mainly by the mother and 12.0% were by doctor or nurse's suggestion. The main reason for giving up or failure of breast-feeding was "insufficient breast milk". CONCLUSION: Compared with baseline data surveyed in our hospital in 1994, breast-feeding at the nursery increased from 1.3% to 26.4% and at 1 month of age, increased from 35.1% to 40.8%. So recommendation for breast-feeding at hospitals is very important and may play a role to increase the rate of breast-feeding.

8.Characteristics of Sleep Apnea Syndrome in the Elderly in a Clinical Setting.

OBJECTIVES: Much attention has been paid to sleep apnea syndrome (SAS) in the elderly because of its high prevalence. It is expected that SAS in the elderly has both similarities and differences compared to SAS in the young or middle-aged populations. The aim of this study was to elucidate the characteristics and consequences of SAS in the elderly. METHODS: In this study we included 210 young or middle-aged adults between 23 and 59 years (20 women and 190 men) and 65 older adults between 60 and 83 years of age (16 women and 49 men). Respiratory disturbance indices (RDIs) of the study subjects were more than 5 in an overnight polysomnography. They completed the Epworth Sleepiness Scale (ESS) and Pittsburgh Sleep Quality Index (PSQI). Informations about body mass index (BMI), neck, waist, and hip measurements, and blood pressure were obtained. RESULTS: No difference was observed between older adults with SAS (older SAS) and adults aged under 60 with SAS (SAS aged under 60) in RDI, apnea index, % time of oxygen saturation less than 90%, and PSQI. Obstructive apnea index and oxygen desaturation index (ODI) were lower in older SAS. Compared to SAS aged under 60, lowest oxygen saturation and central apnea index were higher in older SAS, but they were statistically not significant. BMI and neck circumference were significantly lower in older SAS compared to SAS aged under 60. Diastolic blood pressure was lower in older SAS compared to SAS aged under 60 with no difference in systolic blood pressure. Older SAS showed lower scores in ESS than SAS aged under 60. Significant correlation was observed between RDI and BMI in SAS aged under 60, but not in the case of older SAS. The relationships between RDI and neck circumference, systolic and diastolic pressure, and ESS were similar. CONCLUSIONS: The elderly with SAS were not over-weight and there was no relationship between body weight and the severity of SAS. Also, the behavioral and cardiovascular effects of SAS were not marked in the elderly, which might be partly explained by decreased ODI and relatively higher lowest oxygen saturation in older SAS. The normal aging process, aside from increased body weight, might contribute to the development of SAS in the elderly with modest complications.

9.The Correlation between the Cutaneous Silent Period and Ultrasonographic Findings of Median Nerve in Patients with Carpal Tunnel Syndrome.

OBJECTIVE: To determine the relation between the latency and duration of the cutaneous silent period (CuSP) and ultrasonographic findings of patients with carpal tunnel syndrome (CTS). METHOD: Subjects included 50 hands of 33 patients with CTS with electrophysiologic evidence of CTS and 50 hands of 39 adults with no evidence of CTS. CuSP was measured on abductor pollicis brevis (APB) and adductor digiti minimi (ADM) by stimulation of digit 3. All subjects were examined with ultrasonography (US). Using US, the cross-sectional area (CSA) and flattening ratio (FR) of the median nerve were calculated under carpal tunnel. Analysis of differences between the control group and the CTS group was performed using ANOVA. RESULTS: Differences of CSA, FR, latency, and duration of CuSP in both APB, ADM muscles were observed between the two groups. Correlations were observed in the patient group in latency and duration of the APB muscle and only in duration of the ADM (p=0.048, r=0.159; p=0.035, r=-0.315; p=0.039, r=-0.293) muscle. Correlations were found only in duration of ADM (p=0.011, r=0.358) in the control group with respect to CSA. However, there was no correlation with FR. CONCLUSION: There seems to be a significant correlation between the CSA of the median nerve and the latency and duration of CuSP in patients with CTS. In addition, there seems to be a significant difference of CSA, FR, and latency in both APB and ADM between the two groups. CuSP and CSA might be useful for study that reflects intact small fibers in patients with CTS.

10.Recurrent Cardiac Tamponade Complicated by Coronary Intervention.

Coronary perforation is a rare, but potentially catastrophic complication of percutaneous coronary intervention. It rarely results in cardiac tamponade with the use of standard anticoagulant regimens. But spontaneous cardiac tamponade was reported recently during balloon angioplasty under the large dose of heparin infusion, and after stent implantation under powerful antiplatelet therapy including platelet IIb/IIIa receptor inhibitor administration. We report a case of delayed cardiac tamponade complicated by guidewire manipulation under powerful anticoagulant and antiplatelet therapy, which recurred early after the first pericardiocentesis.
