1.Lymphatic Cyst of the Adrenal : Report A Case.

A case of lymphatic cyst of the adrenal gland in a 55-year-old woman is described. The patient presented with vague abdominal pain, and abdominal CT and ultrasonography suggested a pancreatic cyst. The cyst, 10x8x6 cm in size, was found in the left adrenal gland; it was multilocular and septated with thin walls that were lined by a single layer of attenuated cells. The lining cells gave a weak positive immunoreactivity against factor VIII-related antigen and Ulex eurapaeus, and ultrastructural study revealed the lining cells to be of endothelial origin. Together with small tiny proliferative cystic spaces were intermixture of the normal adrenal cortical cells in the transition zone between large cysts and adrenal cortex. No histologic distinction between cystic lymphangioma and lymphangiectatic cyst was obtained; instead, a term of lymphatic cyst seems preferred unless otherwise confirmed. With its classification, the pathogenesis of adrenal lymphatic cysts is reviewed.

2.Correlation between Serum Levels of ICAM-1 and Serum Levels of Thyroid Hormones, TSH

Lymphocytic infiltration of the thyroid gland is a hallmark of the human thyroid autoimmune disease. Enhanced expression of immunoglobulin and adhesion molecules are consistently found in patients with autoimmune thyroid diseases. And cytokines are implicated in enhancing the expression of adhesion molecules.It has been suggested that adhesion-molecule expression within thyroid glands mediates lymphocyte homing events to the target of the autoimmune process. The expression of ICAM-1 was shown to be up-regulated on thyroidal perifollicular endothelial cells and thyrocytes in autoimmune thyroid diseases both in vitro and in vivo.Therfore, we investigated the correlation between thyroid hormone, TSH receptor antibodies, Interleukin-6 and soluble ICAM-1 in patients with Graves' disease before and 2 months after treatment with prophylthiouracil(PTU).Serum concentrations of soluble intercellular adhesion molecule-1(sICAM-1), T_3, T_4, TSH-receptor antibodies(TSH-R-Ab) and Interleukin-6(IL-6) of peripheral blood monocytes were measured in patients with Graves' disease.Serum levels of sICAM-1 were elevated in patients with Graves' disease before treatment with PTU, but serum levels of sICAM-1 did not correlate with the serum concentration of thyroid hormone and TSH-R-Ab before and after treatment. In addition, no correlation between serum levels of sICAM-1 and IL-6 of peripheral blood monocyte.We conclude that although the elevated serum levels of sICAM-1 may contribute to the autoimmune process in Graves' disease, we need more future studies for the role of sICAM-1 and correlation between adhesion molecule and thyroid hormone or cytokines in patients with Graves' disease.

3.A Case of Lichen Nitidus Coexisted with Molluscum Contagiosum.

Lichen nitidus is an uncommon chronic dermatosis characterized by its typical clinical and histopathologic findings and has rarely been described in association with other diseases. We observed a patient with an association of lichen nitidus and molluscum contagiosum, which to the best of our knowledge has not been previously reported in the literature. It remains to be further studied about a possible pathogenesis of lichen nitidus secondary to some viral infections such as molluscum contagiosum, etc.

4.Hemodynamics Actions of Lidocaine during Halothane Anesthesia in Dogs.

The hemodynamic responses to lidocaine were studied in eight mongrel dogs during halothane anesthesia. The animals inhaled 1 MAC of halothane (group H). During halothane anesthesia, a small dosage of lidocaine (group H; iv bolus 1.5 mg/kg over 1 min, followed by continuous iv infusion with 0.1 mg/kg/min), and a large dosage of lidocaine (group HL; iv bolus 1.0 mg/kg over 1 min, followed by continuous iv infusion with 0.3mg/kg/min) were administered for 30 min, respectively. One MAC of halothane anesthesia decreased the heart rate (10.9%), systolic blood pressure (13.3%), rate pressure product (22.5%), coronary perfusion pressure (16.5%), cardiac index (17.6%), and left ventricular stroke wark index (22.5%) compared with the control. Compared with group H, a small dosage of lidocaine (group H(1)) only significantly decreased the heart rate by 10%, but it seemed to decrease the above mentioned paramenters further. A large dosage of lidocaine (group H L) decreased the heart rate (27.3%), systolic blood pressure (26.0%), cardiac index (35.1%) and rate pressure product (46.0%), compared with the control value, and significantly increased the PR interval in ECG (30%). Although all the parameters indicating oxygen demand and supply were decreased, the shunt ratio was decreased and the alveolar-arterial oxygen tension difference was maintained at the control level. This study demonstrates that the hemodynamic changes by lidocaine might be induced by direct cardiac effect, not by the effect on peripheral vessels and that oxygenation might be well maintained during lidocaine infusion in a clinical dose.

5.Augmented bony change following platelet rich plasma(PRP) application on maxillary defect.

PURPOSE: Platelet Rich Plasma(PRP) application is increasing with sinus inlay bone graft, but there is few research with radiographic ananlysis on effect of PRP in maxillary sinus. The author investigated the amount of bony changes of maxillary sinus for dental implantation among the patients with maxillary inlay graft. MATERIALS AND METHODS: With 10 patients who were treated with sinus inlay autologous bone grafting combined with PRP technique, and with 5 patients who were treated with sinus inlay grafting only without PRP, the panoramic radiographys which were taken at preoperation, immediate postoperation, 3months postoperation, and 4 months postoperation(a month after dental implantation)periods were analysed. The films had been scanned, and then proceeded throughout image analysis system. The bone density of maxillary grafted sites was compared with adjacent tooth enamel density and remeasured according to density luminosity of each film. The density changes on PRP group and bone graft only group were an lysed with non-parameteric statistics method. RESULTS: In PRP combined patients group, bone density on postoperation periods was increased totally. The remarkable enhanced change of bone density was observed on 3 months postoperation period, thereafter the increasing rate was slightly reduced . In only bone graft patients group, bone density on postoperation periods was also increased compared with preoperation period, but the bone density of 4 months postoperation period was decreased compared with 3 months postoperation period. The amount of bone density on PRP group was significantly changed according to periods in contrast to bone graft only group. CONCLUSION: The bone density on PRP group was remarkably increased at 3 months postoperation compared to bone graft only group and it was seemed to be associated with more new bone formation, less grafted bone resorption at bone grafted sites with PRP.

6.Biophysical therapy and biostimulation in unfavorable bony circumstances: adjunctive therapies for osseointegration.

Dental implants using titanium have greatly advanced through the improvement of designs and surface treatments. Nonetheless, the anatomical limits and physiological changes of the patient are still regarded as obstacles in increasing the success rate of implants further, even with the enhancement of implant products. So there have been many efforts to overcome these limits. The intrinsic potential for bone regeneration can be stimulated through adjuvant treatments with the continuous improvement of implant properties, and this can play an important role in achieving optimum osseointegration toward peripheral bone tissue and securing ultimate long-term implant stability in standard surgical procedures. For this purpose, various chemical, biological, or biophysical measures were developed such as bone grafts, materials, pharmacological agents, growth factors, and bone formation proteins. The biophysical stimulation of bone union includes non-invasive and safe methods. In the beginning, it was developed as a method to enhance the healing of fractures, but later evolved into Pulsed Electromagnetic Field, Low-Intensity Pulsed Ultrasound, and Low-Level Laser Therapy. Their beneficial effects were confirmed in many studies. This study sought to examine bone-implant union and its latest trend as well as the biophysical stimulation method to enhance the union. In particular, this study suggested the enhancement of the function of cells and tissues under a disadvantageous bone metabolism environment through such adjunctive stimulation. This study is expected to serve as a treatment guideline for implant-bone union under unfavorable circumstances caused by systemic diseases hampering bone metabolism or the host environment.

8.A Comparative Study on the Doses of Epinephrine Causing Arrhythmia during Enflurane and Halothane Anesthesia .

While halothane, cyclopropane, chloroform and trichloroethylene have been clearly implicated to sensitize the myocardum and increase the risk of ventricular fibrillation, newer inhalation anesthetics have shown relatively less arrhythmogenecity. Isoflurane has been suggested to be compatible with epinephrine, while controversial data suggest enflurane, an isomer of isoflurane, may or may not sensitize the myocardium, which is to be clarified by the authors study. By constant intravenous infusion using VIP pump at the rate of 2.5ug/kg/min, the dosage of epinephrine causing premature ventricular contractions was measured in ten male mongrel dogs during halothane and enflurane anesthesia. While premature ventricular contractions were observed in all dogs anesthetized with halothane, the cardiac arrhythythmia was seen in only two dogs anesthetized with enflurane. Epinephrine dosage causing premature ventricular contractions and the resultant increase in mean arterial pressure at which arrhythmias occurred were significantly higher(p< 0. 05) during enflurane anesthesia than durinh halothane anesthesiae. These results suggest that enflurane, in comparison with halothane, is relatively less arrhythmogenic.

10.Interleukin-8 Expression in Psoriatic Skin of Different Disease Activities.

Psoriasis is a relatively common disorder that affects at least 1-2% of the population, however, the pathogenesis is not fully understood yet. Histopathologically, psoriatic lesion is characterized by hyperplasia of keratinocyte, intraepidermal neutrophil accumulation and activated T lymphocyte infiltration in the dermis. Among many cytokines, IL-8 is known to play a role for the activation and migration for neutrophil and T lymphocyte and for the proliferation of keratinocyte. We performed immunohistochemical staining with anti-IL-8 monoclonal antibody for the active lesion, stable lesion and normal-looking skin of ten psoriatic patients and ten healthy controls. Strong expression of IL-8 in the epidermis and dermis was noted from the active as well as stable psoriatic skins. In the normal-looking skin of the psoriatic patients, only faint expression which is not different from healthy control skin, was noted. These observations indicate that the expression of IL-8 is increased in psoriatic lesion, and IL-S is important for the initiation and maintenance of psoriasis.
