1.Anthelmintic effect of oxantel and oxantel/pyrantel tablets against intestinal nematode infections.

Present trial was carried out to evaluate the anthelmintic efficacy of oxantel/pyrantel tablets against intestinal nematode infections, and to determine the efficacy of oxantel tablels against Trichuris infection. A total of 34 subjects with the mixed infections were treated with oxantel/pyrantel tablets (100 mg/tablet each) in a single dose of 15 mg/kg body weight, and another group of 22 Trichuris infected cases received oxantel pamoate tablets (125 mg/tablet) in a single dose of 15 mg/kg. All stool examinations were done before the treatment and 3 weeks after the treatment. The cellophane thick smear (Kato's technique) and Stoll's dilution egg counting method were employed. The results of the trial were summarized as follows: The cure rates (egg negative conversion rates) and egg reduction rates for oxantel/pyrantel tablets were 85.3 percent and 97 percent in trichuriasis, 100 percent each in ascariasis and ancylostomiasis. The oxantel tablet treated group demonstrated a cure rate of 90.9 percent and an egg reduction rate of 96.3 percent in the treatment of Trichuris. There were no detectable objective and subjective side effects in this trial. Both oxantel/pyrantel and oxantel pamoate tablets were readily accepted and well tolerated.

2.A case of Fanconi syndrome.

Fanconi syndrome is a complex of renal tubular dysfunction defined by glycosuria without diabetes, generalized aminoaciduria, phosphaturia, bicarbonaturia, uric aciduria, and renal tubular acidosis. It is often associated with hypokalemia, hypophosphatemia, rickets and osteomalacia. We have experienced one case of Fanconi syndrome with chronic tubulointerstitial nephritis. The patient was a 4 year old and his chief complaints were polyuria, polydipsia, and poor weight gain. There were hyperchloremic metabolic acidosis, hypokalemia, glycosuria, generalized aminoaciduria and phosphaturia. We report a case of Fanconi syndrome with brief review of the literatures.

3.Analysis of Fractures of the Foot and Ankle

The results of the treatment upon total 132 cases of ankle and foot fractures treated during the period from January 1969 to August 1978 at the Department of Orthopaedic Surgery, National Medical Center have been analysed and reported in this paper. 1. Sex ratio between male and female was 3.7:1. 68 cases (51.5%) cases out of 132 cases were in the age groups of 2nd and 3rd decades. remarkably higher than any other group. 2. 85 cases out of 132 cases were closed fractures and 47 cases were open fractures, but the incidence of the open fracture in higher in the fractures below the metatarsal bones and multiple fractures. 3. 59 cases out of 132 cases were ankle fractures 34 tarsal bone fractures 14 metatarsal bone fractures, 8 phalangeal bone fractures and 20 multiple fractures Accordingly, the incidence of the ankle fracture is higher than other fractures. 4. Ankle fracture was classified in 4 types according to the fracture mechanism (Lauge-Hansen classification). 20 cases out of 59 cases were pronation-abduction type, 15 cases were pronation-external rotation type, 14 cases were supination adduction type and 9 cases were supination-external rotation type. 15 cases out of 59 cases of the ankle fractures were treated by tension band compression internal fixation. The result of this method was much better than other methods. 5. 25 cases out of 35 cases of calcaneal fracture involved subtalar joint according to Rowe classification. 12 cases out of 25 cases of calcaneal fracture involved subtalar joint were Tongue type-C. In the treatment, no relation between restoration tuber angle or Gissane angle and the incidence of complication was obtained. 6. The incidence of the complication, such as secondary oateoarthritis, non union. malunion, occurred higher in the open and multiple fractures than closed and single fractures.

4.Immunohistochemical and Ultrastructural Observation on Small Round Cell Tumors.

Small round cell tumors such as neuroblastoma, rhabdomyosarcoma, Ewing's tumor, malignant lymphoma and small cell carcinoma are often confused clinically and histologically. To clarify the similarites and differences and to get more information on the histogenesis among the small round cell tumors, we examined histological, immunohistochemical and ultrastructural features of ten cases of neuroblastomas, twenty Ewing's tumors, ten embryonal rhabdomyosarcomas and twelve small cell carcinomas in children and young adults. Antibodies against desmin, vimentin, cytokeratin, neuron-specific enolase, synaptophysin, neurofilament, S-100 protein, chromogranin and HBA 71 were used in biotin streptavidin procedures. The results of the immunohistochemical and electron microscopical examinations yielded virtually identical findings in each group as followings. 1) Among the twenty cases of Ewing's tumors, eighteen cases were positive in staining for HBA-71. The staining for HBA-71 was negative in neuroblastoma, embryonal rhabdomyosarcoma and small cell carcinoma. 2) Neuroblastomas had marked interdigitating cytoplasmic processes containing many microtubules and dense-core secretory granules, however, they were sparse and rare in Ewing's tumor. 3) Embryonal rhabdomyosarcoma showed actin-myosin bundles. According to differentiation, well differentiated rhabdomyosarcoma exhibited Z-band materials and external lamina. 4) Neuroblastoma showed ultrastructural evidence of a neuronal differentiation, but neuronal differentration is a sparse and rare in Ewing's tumor. This ultrastructural feature strengthens the hypothesis that Ewing's tumor is derived not only from an undifferentiated neuroectodermal stem cell but from primitive cell of neuroectodermal origin.

9.A Case of Complication of Expandible Metallic Stent with Endobronchial Stenosis.

To maintain an adequate airway in a patient with tracheobronchial narrowing coming from various causes, prosthetic tracheobronchial stents provide palliative treatment for narrowed airways where surgical resection is inadvisable. After insertion, precious reported complications were granuloma formation, dysphagia, suction catheter entrapment and fatal massive hemoptysis. We report a case of complication associated with expandible metallic scent with endobronchial stenosis due to tuberculosis.

10.Successful Treatment of Auricular Arterio-Venous Malformation.

Auricular arterio-venous malformation(AVM) is relatively rare and few have been reported in the literature. Two cases, presented here showed pulsating lesions of anterior and posterior auricular or even retroauricular mastoid area. They caused irregular macro and prominent ear compared with the opposite normal ear. After embolization of feeding arteries to the arteriovenous malformations, skin and subcutaneous masses were completely removed and grafted with full thickness skin: one was removed one stage and the other underwent a two stage operations. There were no evidence of recurrence for more than 2 years follow up and esthetic results were promising.
