1.Analysis of the patterns of bronchial obstruction at bronchography

Of the bronchographic findings of 408 patients, performed in our hospital for recent 5 years, 108 cases showed definite bronchial obstruction, and 61 cases of those were selected and their obstructive findings were evaluated. All that not confirmed were abandoned. For evaluation of the reliability of 9 bronchographic obstruction signs onappplying to diagnose malignant or benign pulmonary diseases, each sign was identified and applied to each of the61 confirmed cases. In addition, obstructed bronchi, distance of obstruction from the bifurcation site, and thedirection of meniscus, if peresent, were evaluated. The reuslts were follows; 1. The most frequent cause ofbronchial obstruction was lung cancer (59.0%), and that of the benign obstruction was pulmonary tuberculosis(13.1%). 2. Amputation, asymmetric narrowing, thumbprint indentation, rat-tail narrowing and encasement signs werethe most accurate signs of malignancy and were practically diagnostic ones, 3, The most frequent sign in lungcancer was sharp cut off one, but it could be seen in lung abscess and in unresolved pneumonia, too. 4. Circumferential symmetric narrowing and regular concavity with a small central profection signs were specific onesto benignancy. 5. The most frequent obstruction sign in benign lung disease was gradual tapering sign, but it alsocould be seen inbronchogenic epidermoid and alveolar cell carcinoma. 6. Of all bronchial obstructions, 55.4% occurred at lobar bronchus and 77.4% of those were caused by lung cancer. 7. 77.2% of those obstruction which located within 3 times distance of the bronchial diameter at the nearest proximal bifurcation site, were lungcancer, but 75.0% of those located at over 3 times distance were benign pulmonary diseases. 8. There were nocorrelation of the direction of the meniscus at the obstructing and in differential diagnosis between benign andmalignant pulmonary diseases.

2.A case of atypical fibroxanthoma.

A 43-year-old male patient showed well defined, irregular, dark eythematous, thick plaque and tumor with ulceration on the right chest, right axilla, pubis and scrotum, Histopathological findings rervealed atypical lymphocytes showing epidermotrophism and heavy infiltrations in the deep dermis and subcutaneous tissue. The infiltrative cells were positively stained with leukocyte common antigen and T-cell UCHL-1 antigen, but not with L26 antigen. This clinicohisto-pathological features were consstent with dermblade type of mycosis fungoides. The authors report clinicopathological findings of the case with its thrapeutic responses to systemic chemotherapy and radiation treatment.

3.A Study of Lectin Histochemistry in Allergic Contact Dermatitis of Guinea Pig.

The alterations in the localization of keratinocyte membrane glycoconjugates in allergic contact dermatitis were investigated in guinea pig skin treated with topical application of 2.4-dinitro-chlorobenzene. We employed the avidin-biotin complex(ABC) method for the detection of localization of 10 commercially available lectins labelled with biotin: Con-A, SBA, WGA, DBA, UEA-1, RCA-1, PNA, HP, MPA, and ECA. Staining with WGA showed a remarkably decreased intensity in basal and spinous layers of the allergic skin in comparison to those of the control skin, suggesting loss of terminal sialic acids in cell membrane glycoconjugates. The other lectins showed no remarkable difference in the staining patterns between the normal and the allergic ski. The results suggest that epidermal cell membrane glycoconjugates undergoes selective perturbations in acute allergic contact dermatitis, and that the keratinocytes might be an active part of the cutaneous immune system.
4.Application of micro-ELISA in serodiagnosis of human paragonimiasis.

To observed whether the specific IgG antibody test using ELISA was useful in diagnosis of presently ill patients of paragonimiasis, a total of 95 sera were tested. The sera were collected from 21 egg positive cases, 8 from positive reactors of intradermal test, 7 from Clonorchis infected, 9 from other parasitic diseases and 50 from apparently non-infected cases. By the result, the sensitivity of the test was 86% and the specificity was 100%. There were no cross reactions between Paragonimus antigen and other parastic infections. Specific IgG antibody test by micro-ELISA was concluded to be useful for mass screening of the presently ill paragonimiasis in the field.

5.CT demonstration of extrapleural pedunculated thymoma.

A case of extrapleural pedunculated thymoma is presented and its CT features are described. A large ovoid extrapleural pedunculated thymoma in the right lower lung field clearly demonstrated a focal tapered pedicle at the level of inferior border of right main bronchus on CT scan, thus establishing the mediastinal origin of the mass in a 61-year-old man.

8.Clinical Analysis of Interhemispheric Subdural Hemorrhage and Tentorial Hemorrhage.

A propecive analysis of 50 patients with acute interhemisphric subdural hemorrhage and tentorial hemorrhage, an unusual pattern of acute subdural hematoma, who were managed in a uniform way was analyzed to related outcome to hemorrhagic site, initial Glasgow Coma Scale(GCS) and combined injuries. The incidence of acute interhemisphric subdural hemorrhage(ISH) and tentoria hemorrhage(TH) after head trauma was 3.83%, 50 cases among 1303 head injured cases. And 80% of the above hemorrhage disapperaed within two weeks after trauma. There was no significant relationship between feature of hemorrhage and intial GCS(P>0.05), but there was highly significant relationship between initial GCS and Glasgow Outcome Scale(GOS) (p<0.001). There noted significant relationship between initial combined injury and GOS(p<0.01), and also brainstem injury and GOS(p<0.001).
