1.A Clinical Study on Childhood Leukemia.

Clinical analysis of 170 patients with childhood leukemia was made, who were admitted to the department of Pediatrics, Pusan Paik Hospital, Inje University, College of Medicine during 10 years from 1981 to 1990. The results were as follows: 1) The annual number of the patients with childhood leukemia was in the range of 6~25 cases. And the annual rate of the number of the patients in the department of the Pediatrics was 0.37~1.60% with the average of 1.06%. 2) Of the 170 patients, 72 cases(42.3%) were in the range of 0~5 years of age, which was the largest age group. In male, of the 97 patients, the largest age group was the age group of 0~5 years, which included 45 cases (46.4%), while in female, the largest was that of 6~10 years, which included 29 cases of 73 patients (39.8%). 3) Of the 170 patients, sex ratio was 1.33:1 with male dominance in all age groups except 1:1.6 with female dominance in the age group of 6~10 years. 4) Of the 170 patients, 113 cases were ALL (66.5%), 50 cases AML (29.4%), and 7 cases CML (4.1%). 5) In ALL, 51 cases of the 113 patients (45.1%), were in the age group of 0~5 years, 35 cases (31%) in the age group of 6~10 years, and 27 cases (23.9%) in the age group of 11~15 years. In AML, 19 cases of the 50 patients (38.0%) were in the age group of 0~5 years, 17 cases (34%) in the age group of 6~10 years, and 14 cases (28%) in the age group of 11~15 years. 6) In ALL, the sex ratio of male: female was 1.21:1, 1,50:1 in AML, and 2.5:1 in CML. 7) According to the FAB classification in ALL, of the 113 patients, 72 cases (63,7%) were in L1 type, 34 cases (30.0%) in L2 type, and only 3 cases in L3 type. In AML, of the 50 patients, 19 cases (38%) were in M2 type, 16 cases (32%) in M1 type, and 6 cases (12%) in M4 type, in order. 8) In CML, of the 7 patients, 4 cases were juvenile type, who were all under 10 years old, while 3 cases were adult type, who were all male above 11 years old. 9) Of the 170 patients, clinical symptoms on admission were pallor in 112 cases (65.9%), fever in 84 cases (49.4%), hemorrhage in 78 cases (45.9%), weakness in 40 cases (25.3%), bone pain in 32 cases (18.8%) in order. 10) Of the 170 patients, physical findings on admission were hepatomegaly in 113 cases (66.5%), lymphnode swelling in 84 cases (49.4%), splenomegaly in 78 cases (45,9%)in order. 11) Of the 170 patients, the hemoglobin level on admission were 4~7gm/dl in 95 cases (55.9%), 8~10gm/dl in 42 cases (24.7%), and 0~3gm/dl in 22 cases (12.9%) in order. 12) Of the 170 patients, the WBC count on admission were 10,000~50,000/mm3 in 58 cases (34.1%), below, 5,000/mm3 in 49 cases (28.8%), and 5,5000~100,000/mm3 in 24 cases (14.1%) in order. 13) Of the 170 patients, the platelet count on admission were below 50,000/mm3 in 93 cases (54,7%), above 100,000/mm3 in 48 cases (28.2%), 20,000~50,000/mm3 in 43 (25,3%), 43 cases (25,3%), and 50,000~100,000/mm3 in 29 cases (17.1%) in order.

2.Interlocking Intramedullary Nailing of the Femoral Shaft Fracture: Technical Problems

The use of the interlocking intramedullary nails expand the possibilities for treatment of femoral shaft fractures, as illustrated by consistent and satisfactory clinical results. However, the technique is relatively complex compared to conventional nails and frequently poses technical problems. A retrospective review was undertaken of twenty-eight patients with femoral shaft fractures which had technical problems after undergoing the closed reduction and internal fixation with interlocking intramedullary nail procedure. We studied the types of the problems. Their causes, the frequency of problems with various kinds of nails, and discussed solutions of the problems. There were 21 additional comminutions and intraop erative splinterings, 8 angular deformities, 3 malpositioned nails, 3 loosened distal locked screws, 2 brocken drill bits, and 1 femoral neck fracture. Bone-union was attained in all cases (28 patients), but the period of treatment to attain bone-union was protracted to an average of 6.8 (5-12) months. In 3 cases, the knee joint range of motion was of limited flexion, 70 to 90 degrees, but in all other cases full range of motion was realized. 7 cases required 2 or more operations. The hospitalization period was for an average of 3.6 months. In conclusion, the importance of careful preoperative planning and experienced surgical technique cannot be overemphasized because we could see no significant difference in the occurence of technical problems in several kinds of the nail.

3.Removal patterns of smear layer according to application temperature and time of EDTA.

The purpose of this study was to evaluate the effect of EDTA irrigant according to application time and temperature. 31 human mature extracted teeth with a single canal were sectioned with microtome in 3mm thickness and gained 62 samples of root canals. They were distributed randomly into 6 groups of 10 specimens each and control group of 2 specimens. Each specimen was prepared with GT rotary file (Dentsply, Maillefer Co., Swiss) and irrigated with 3 ml sodium hypochlorite every minute. Then smear layer was removed with EDTA solution (PULPDENT(R), PULPDENT Co., USA.) except two control specimens. Specimens of each group were irrigated with 17% EDTA. All specimens were split longitudinally and prepared for examination by scanning electron microscopy. A set of reference micrographs was used to award a debris score as follows: 0 = no smear layer, all tubules clean and open; 1 = no superficial smear layer, tubule openings visible, but some contain debris plug or soft tissue remnants; 2 = moderate smear layer, some tubules open and others closed; 3 = heavy smear layer, most/all tubule openings obscured. Results were evaluated with Kruskal-Wallis test to determine whether there was statistically significant difference among six groups. Pairs of groups were analyzed using the Student-Newman-Keuls Method and Mann-Whitney test. The results were as follows: 1. Control specimens showed heavy smear layer at the canal walls. 2. Among the groups applied with EDTA for 2 minutes, group 1 showed the heaviest smear layer, and there was statistically significant difference between group 1 and the other groups(p<0.05). 3. Among the groups applied with EDTA for 5 minutes, group 4 and group 6 showed smear layer, but there was no significant difference between them. 4. Among the groups applied with EDTA for the same temperature, group 1 showed heavier smear layer than group 4, and there was statistically significant difference(p<0.05). 5. Among the groups applied with EDTA for the same temperature, group 2 showed heavier smear layer than group 5 and group 3 showed heavier smear layer than group 6. But there was no statistically significant difference among them. From the results above, it could be concluded, EDTA solution is effective in removing of smear layer when it is applied for 5 minutes. If EDTA is applied for 2 minutes, it should be applied above room temperature.

4.Pulmonary mucormycosis with an appearance of consolidation.

The mucormycosis is a group of serious opportunistic infections caused by fungi of the class Zygomycetes and order Mucorales. Pulmonary mucormycosis is a relatively rare disease but typically manifested by a rapidly progressive, often fatal pneumonia in patients with diabetes mellitus, hematologic malignant neoplasms, or organ transplants. The radiologic manifestations of pulmonary mucormycosis are nonspecific and include progressive lobar or multilobar consolidations, pulmonary masses and pulmonary nodules. Recently, we experienced a pulmonary mucormycosis in 32-year-old man with uncontrolled diabetes. He complained of cough, left pleuritic chest pain and generalized weakness. Initial chest X-ray finding was the consolidation on the lower lobe of the left lung. On the sixth hospital day, bronchoscopic examination with lung biopsy revealed broad, non-septate hyphae with right-angle branching, diagnostic of mucormycosis, and consequently amphotericin B therapy was started. We performed a lobectomy of the left lower lobe of the lung on 29th hospital day.

7.A Case of Supravalvular and Valvular Aortic Stenosis.

Supravalvular aortic stenosis may be defined as an obstructive congenital deformity of the ascending aorta which originates just distal to the level of the origins of the coronary arteries. It may be localized or diffuse, and includes a wide spectrum of pathologic changes. A 16 years old school boy was admitted because of exertional dyspnea for 6 years. Clinical diagnosis of supravalvular and valvular aortic stenosis with bicuspid aortic valves and myocardial hypertrophy was made by echocardiography and angiocardiography. Surgical correction was performed successfully. We presented a case of supravalvular and valvular aortic stenosis with a review of literatures.

8.Treatment of Fracture of the Thoraco

The main goals of surgical teatment of thoracolumbar vertebral fractures are to achieve reduction, stability and early, painless mobilization. A further aim is to avoid late malposition and to preserve mobility and lordosis. The main disadvantage of Harrington instrumentation and similar methods are that this device fixate five to seven vertebrae. The Cotrel-Dubousset(C-D) instrumentation provides a stable construct for fixation of spine fracture. The advantages of C-D pedicle screw for thoracolumbar spinal fractures are that compression or distraction force can be applied in the same rod and used to achieve anatomical reduction of spinal fractures and rigid fixation can be obtained with short instrumentation. The C-D instumentation was done for 15 unstable thoracolumbar spinal fractures at Seoul Red Cross Hospital from January, 1988 to January, 1990. The results were as follows. 1. The most common cause of the spine fracture was fall down, and majority are within 20-40 old age. 2. The most common involved site was T12 and Ll vertebrae (66.7%). 3. The most common type of injury was burst fracture by according to Denis classification(60%). 4. The complication was developed mostly in neurological deficient patient. 5. The advantage of the C-D instrumentation: (1) rigid and short osteosynthesis of spine fracture, (2) compression or distraction force can be applied in the same rod, (3) early mobilization with a brace is possible. 6. The rigid fixation can be obtained.

The mechanical derangement, developmental anomaly, infection amd tumor should be considered as a cause of low back pain in adolescents. Disc herniation in adolescents is very rare and some of that are due to fracture of the epiphyseal ring. We report a case of fracture of the first sacral epiphyseal ring simulating disc rupture.

10.Piezoelectric Extracorporeal Lithotripsy of Gallbladder Stones: New Inclusion Criteria.

PURPOSE: To establish the optimal inclusion criteria for the patients with gallbladder stones to extracorporeal shock wave lithotripsy(ESWL) by retrospectively analyzing our current results. MATERIALS AND METHODS: Data obtained from 201 patients with gallbladder stones treated with ESWL and oral chemolytic agent from November 1989 to July 1992 were restroepectively analyzed. Ninety-six had radiolucent stones and 105 had radiopaque stones. We used piezoelectric lithotriptor(EDAP LT. 01) and there were no limitation in number of sessions or total number of shock waves. ESWL was repeated until the size of the largest fragment is smaller than 4 mm. Follow up ultrasound was done in every three months after the successful fragmentation. Average length of the follow up was 205 days. We analyzed the rate of successful fragmentation, number of shock waves needed to achieve successful fragmentation according to the size, number of stones as well as the presence of the calcification. Stone-free rate after 6 months was also calculated from all subgroups and compared to each other. RESULTS: The rate of successful fragmentation was 76.2% for radiolucent stones and 66.6% for radiopaque stones(p > 0.05) after 46,731 and 56,111 shock waves respectively(p > 0.05). The rate of successful fragmentation was highest in patients with single, radiolucent stone smaller than 2cm(91.7%) followed by single radiolucent stone larger than 2cm(83.3%), multiple calcified stones smaller than 2cm(77.4%) and single calcified stone smaller than 2cm(72.1%). The rate of complete stone clearance after 6 month follow-up was highest in patients with single radiolucent stone smaller than 2cm(63.3%) and followed by multiple calcified stones smaller than 2cm(37.3%), single calcified stone smaller than 2cm(33.9%)(p <0.05). CONCLUSION: To obtain better results with ESWL in patients with gallbladder stone, the authors propose a more strict inclusion criteria, which is the patient with a single, radiolucent stone smaller than 2cm.
