Paederus dermatitis is a linear and self-healing blistering disease of the skin caused by contact with insects belonging to genus Paederus(Coieoptera : Staphylinidae). There have been only a few reports of paederus dermatitis in the southern part of Korea since 1968. We recently experienced a case of paederus dermatitis due to contact by P. fuscipes.

3.A Case of Ant Sting By Brachyponera chinensis.

The hypersensitive reactions to ant stings vary from local to systemic. Because repeated stings may induce severe skin reactions or serious systemic allergic reactions, the concerns about minor skin reactions by ant stings have been increasing. A 34-year-old man was stung on the right forearm by an ant. He experienced erythematous papules with pain, burning and dyspnea. The stinging ants collected from the patients skin and the ants resident in his garden were identified as Brachyponera chinensis.

4.Clinical Trials of Galantase for Infantile Diarrhea.

In 1962, Durand suggested the term primary lactose intolerance for the disorder withch resulted from a defect of the lactase activity in the intestinal mucosa. However primary lactose intolerance is rare and it is not commonly encountered in routine pediatric practice. Apart from such a primary lactose intolerance, diarrhea, meteorism, and abdominal discomfort after taking lacose containing food are more frequently encoumtered in case of gastroenteritis in which damage of the intestinal mucosa and coating of the mucosa by inflammatory secretions lead to decreased lactase it form the primary lactose intolerance. It is now considered that secondary lactose intolerance may play an important role in case of infantile diarrhea commonly encoutered in pediatric practice. The auther tried the lactase preparation Galantase?to evaluated it's clinical effect on infantile diarrhea of various kind (50 cases). Evaluation of the effect of Galantase was made on the bases of the following criteria. l Marked effective ; nature of stools and number per day returned to normal within 3 days after Galantase administration. l Effective ; nature of stools and number per day decreased within 3 days after Galantase administration. l Ineffective ; all stools remained unchainged and diarrheic. The results obtsined by this study are as followes l Marked effective ; 78 % l Effective ; 12 % l Slight effective ; 4% l Ineffective ; 6% There was no undesirable side effect observed in all 50 cases. Based upon the above effective rate, we concluded that the Galantase is a quite effective medicament for infantile diarrhea, especially for primary and secondary lactose intolerance.

5.A Case of Bilateral Split Hand and Foot Malformation with Inversion of Chromosome 7.

Split hand and split foot(SHSF) is a human developmental defect characterized by missing digits, fusion of remaining digits, and a deep median cleft resulting in a clawlike appearance of the hands and feets. SHSF is usually inherited in an autosomer dominant fashion. The incidence of SHSF is between 1/10,000 and 1/90,000. Thirteen cases of SHSF and chromosomal aberrations involving 7q21-22 have been described so far in the world. We experienced a case of typical tetramelic SHSF in neonate. Chromosome studies showed a pericentric inversion of chromosome 7:46,XY,inv(7) (p22q22). Inspection of the extremities and chromosome studies in the parents were normal.

Background/Aims: Periampuilary diverticulum has been known to be associated with various bilio-pancreatic diseases such as choledocholithiasis and disturbs performing selective cannuiation of ERCP and EST. This present study aims to investigate anatomical morphology of periampullary diverticulum and to determine whether periampullary diverticulurn influences difficulty and complication performing of ERCP and to analyse the relationship between periampullary diverticulum and choledocholithiasis. Methods: We reviewed records of chart and diverticular pictures in 1389 cases of underwent ERCP and evaluated the incidence, size, direction, location of periampullary diverticulum and its influence against performing of ERCP and investigated its relationship with bilio-pancreatic diseases such as choledocholithiasis.(continue..)

8.The Learning Curve for Laparoscopic Totally Extraperitoneal Herniorrhaphy by Logarithmic Function.

PURPOSE: Totally extraperitoneal (TEP) hernia repair has gained in popularity in the past two decades. Despite the advantages TEP hernia repair, the approach is hindered by the relatively long learning curve of the surgery. We tried to estimate the necessary number of repetitions of TEP hernia repair in the learning curve using logarithmic and exponential function models. METHODS: We performed a retrospective review of all patients who underwent TEP hernia repair by a single surgeon consecutively at a single center. We calculated how many operations were needed to achieve a reduction in the expected operating time to mean operating time using logarithmic and exponential function models. RESULTS: In the 91 patients, the logarithmic function model predicted that 37 cases were needed to overcome the learning curve for TEP hernia repair while the exponential model predicted that 39 cases were needed. CONCLUSION: According to this study, at least 37 to 39 cases are needed in the overcome learning curve of TEP hernia repair. Further studies are needed to optimize surgical education and maximize quality.

9.The Efficacy of Epinephrine Test Doses during Combined Spinal-Epidural Anesthesia in Adult Patients.

BACKGROUND: Combined spinal-epidural anesthesia has been used to reduce the side effect of spinal or epidural anesthesia. The epinephrine test dose to prevent intravascular injection of local anesthetics after subarachnoid block has not been clearly understood. The purpose of present study is to see the efficacy of simulated intravenous test dose during subarachnoid block. METHODS: 20 ASA physical status 1 and 2 patients underwent subarachnoid block with tetracaine 10 mg in hyperbaric solution at the L3-4 interspace and were divided into two groups, Group 1 (n=10) and Group 2 (n=10). 3 ml of Normal saline was injected intravenously to group 1, while 1:200,000 epinephrine 3 ml (15 microgram) was injected intravenously to group 2 at regression of sensory block to T8-10. 1:200,000 epinephrine 3 ml (15 microgram) was given to each volunteer (Group 3, n=10). Heart rate (HR) was measured at 15 seconds intervals for 3 minutes and systolic blood pressure (SBP) was measured at 1 minute intervals for 5 minutes. RESULTS: SBP increased significantly in group 2 and group 3 at 1 minute after epinephrine test dose injection. Maximal HR changes was 39.7 3.7 beat per minute in group 2 and 25.8 5.2 beat per minute in group 3. There was 100% incidence of detection of intravascular injection of 15 microgram epinephrine in both group when HR increase > or = 20 beats per minute is regarded as positive response. CONCLUSIONS: This study demonstrates that the epinephrine test dose is useful method to detect intravascular injection of local anesthetics either in the combined spinal-epidural anesthesia or epidural anesthesia. The heart rate response after injection of epinephrine was greater than the blood pressure response.

10.Laparoscopic Management of Primary Omental Torsion in 11-year-old Boy.

Primary torsion of the greater omentum is a rare cause of an acute abdomen in children, which often mimic other acute abdominal conditions. Resection of the infarcted segment is the treatment of choice. Here, a case of torsion of the greater omentum, diagnosed with preoperative computed tomography and subsequently managed by laparoscopic surgery, is reported. The patient was discharged three day after surgery without any complication. Omental torsion can be accurately diagnosed and safely treated with a laparoscopy.
