1.A Clinical Study of the Congenital Hypertrophic Pyloric Stenosis.

A Clinical study was carried out on 39 cases with congenital hypertrophic pyloric stenosis who were admitted to the department of Pediatrics, Catholic Medical College, during the 7 years 8 months from January 1971 to August 1978. The following results were obrained. 1. The age incidence was highest in the 2-4 weeks of age and 82.0% were under the 8 weeks. Af age Male to female ratio was 12:1. 2. The age at onset was within 3 weeks of age in the majority cases(69.2%). 3. In distribution of birth weight, the majority cases(75%) were 3.0~4.0kg. and 18 cases(46%) were first born bady. 4. In the body weight percentile on admission, the jahority cases(75.4%) were under the 25 percentile. 5. The kinds of feeding in order of frequency were breast(41.0%), bottle(30.8%) and mixed feeding(28.2%). 6. The average duration of symptoms prior to operation was 17 days. 7. On the physical examination, gastric peristaltic wave was observed in 30 cases(77%) and the pyloric tumor mass was palpated in 26 cases(67%). And Projectile vomitiong and dehydration were observed in all cases. 8. Barium meal X-ray study showed the string sign in 33 cases(88.9%).

2.A Study on Serum IgE Levels in Patients with Scabies.

Serum IgE levels in 48 patients with scabies were measured by enzyme-linked immunosorbent a.ssay(ELISA) to evaluate the relationship between the scabies and serum IgE. A group of 44 healthy persons was taken as a controls. The results were summarized as follows: In children group(p 15 year-old) of the scabies, serum IgE level was 446 3 +267.2 IU/ml and significantly elevated than in controls, 126, 3 175, 4 IU/rnl, In adult group(over 16 year-old), serum IgE levels of the scabies and controls were 506. 4 + 581. 8 IU/ml, 435, 5+ 548, 8 lU/rnl respetively, and there was no significant difference between them. 2) There were no statistical correlations between the serum IgE levels and intensity of pruritus, duration of illness, number of skin lesions or number of circu1ating eosinophils in patients with scabies.

3.Experimental Studies on the Induction of Cryptorchism with Estrogen.

Previous experimental studies on cryptorchism have been done with surgical manipulation which brought already descended gonads in the abdomen. However, the possibility of tissue damage with surgical manipulation has been discussed as a limitation of this procedure. In an effort to observe the possibility of the induction of cryptorchism with estrogen, Sprague-Dawley rats were injected intramuscularly with depoestradiol cypionate of various dosage schedule and timing of treatment. The results obtained were summarized as follows: 1. In cases treated with 0.5-2.0 mg/rat of depoestradiol cypionate at 2 weeks of pregnancy, stillbirth or death in 24hrs after birth resulted in offsprings. 2. Cryptorchism was not induced in all cases treated with 0.3mg/kg of depoestradiol cypionate irrespective of the age at treatment. 3. Cryptorchism was induced in .all cases treated with 0.3mg/rat of depoestradiol cypionate at 1 week after birth. This results might be used as a basis of the related and consecutive studies on cryptorchism.

4.A Case of Takayasu's Arteritis with Congestive Heart Failure.

A 38 month old girl was admitted with the episode of anorexia, coughing and dyspnea to St. Mary's hospital on Nov. 9, 1977. She had treated as hepatitis at private clinic for one month before admission, however these symptoms were aggrevated with coughing and progressed dyspnea since a week ago. On physical examination, body temperature 36.8C, body weight 11.7kg, heart rate 150/min, BP 130/110mmHg at right arm, 160/90mmHg at left arm and 100/90mmHg at both legs were noted. The right radial pulse was palpated weakly compare to the left radial and both femoral pulses, which were also weak and bruit were heard on the both right carotid artery and abdominal aorta. On cardiac ausculation, no significant murmur was heard except gallop rhythm. The ECG and chest X-ray revealed the left ventricular enlargement and grade I K-W hypertensive retinopathy was found by fundoscopy. On selective left angiographic study mitral regurgitation(grade IV) was proved, and localized obliteration of the right subclavian artery and left carotid artery and diffuse constrictive narrowing of the thoracic and abdominal aorta at the level from D7 to suprarenal(type III) were seen by sequential ascending and abdominal aortorgaphy. The infrarenal abdominal aorta was also hypoplastic. Tuberculin skin test was strongly positive, and VDRL and LE preparation were negative.

5.Usefulness of Questionnaires, Physical Examination and Median Mixed Nerve Conduction Studies in Patients with Diabetes Mellitus.

OBJECTIVE: To find simple screening method for assessment of patients with diabetic neuropathy by use of brief questionnaire, scored physical examination including vibration perception threshold, and mixed median conduction study. METHOD: Subjects were 24 patients with diabetes mellitus. Conventional nerve conduction study was performed in 24 patients and the patients were divided into two groups: 11 patients with polyneuropathy; 13 patients of normal findings. The questionnaires were given for evaluation of sensory function in all the subjects. The questionnaires were followed by physical examination including two point discrimination, vibration perception threshold by using biothesiometer, 10 gram filament test, pin prick test, DTR check, and muscle strength test. Abnormality was determined by the number of positive responses or abnormal clinical findings. Finally, we recorded the conduction velocity and amplitude of median mixed nerve and compared these parameters to values of 20 normal adults. RESULTS: The questionnaire and physical exam scores were higher in patients with polyneuropathy (p<0.01). The patients with polyneuropathy showed higher vibration perception threshold values (p<0.01) and slower conduction velocity and smaller amplitude of median mixed nerve (p<0.05). In correlation with conventional nerve conduction study, the median mixed nerve conduction velocity had higher kappa value than amplitude in screening for diabetic polyneuropathy. CONCLUSION: We suggest the questionnaires, scored physical examination, vibration perception threshold, and median mixed nerve conduction velocity to be a simple screening method for assessment of patients with diabetic plyneuropathy.

6.A Case of Esophageal Perforation by an Endoscopic Biopsy.

The esophagus or stomach can be perforated during diagnostic upper endoscopy in 0.03 to 0.1 percent. Instrumentation injury, as a whole, is probably the most common single cause of all cases of esophageal perforation. Most of the esophageal perforation result from either therapeutic maneuvers(dilation, sclerotherapy, foreign-body removal etc.) or underlying esophageal lesion(such as strictures or diverticular or neoplasm). Endoscopic perforation of the esophagus may be obvious immediately or within a few hours. Cervical pain, subcutaneous emphysema, fever, tachycardia, and characteristic radiographic appearances make the diagnosis easy, but some distal esophageal injuries are subtler, An immediate esophagogram should be obtained if peirforation is suspected. To select an appropriate course of management, precise delineation of location and the extent of perforation is necessary. The esophageal perforation can be managed conservatively by close observation, esophageal rest, and antibiotic coverage, but the mortality rate of medical treatment is near 12%. Causes of death are sepsis and multisystem organ failure. So we report a case of esophageal perforation by an endoscopic biopsy.

7.Two Cases of Needle and Toothpick Stuck in the Stomach.

Foreign bodies in the stomach are usually ingested by children under 5 years of age, persons with dentures, alcoholics, mentally disturbed individuals or prisoners with a purpose of secondary gain. Many patients will be asymptomatic and will be brought in by parents with a history of having swallowed something. The ingested foreign bodies are mainly coins, particles of metals, fish bones and etc. A needle or a toothpick in the stomach has been rarely reported in an adult.. We present two cases of patients: one case of patient with needle stuck in the stomach, who swallowed for the purpose of secondary gain, and the other case of patient with toothpick stuck in the stomach, who did not realize having swallowed it. A 23-year-old man, prisoner, was admitted to hospital due to epigastric pain after ingestion of a needle. Endoscopy showed a black needle, 1.2cm long stuck at the prepyloric antrum. It was removed by the endoscopic biopsy forceps through the overtube used in endoscopic variceal ligation and proved to be a needle with a length of 4.9 cm and a diameter of 0.1 cm. A 58-year-old woman developed an acute onset of hematemesis and epigastralgia, and presented to the emergency room. On admission, her blood pressure was 60/40 mmHg. An emergency endoscopy showed a yellowish, thin, stiff material 3.5cm long stuck at the greater curvature of the antrum with meat materials. It was removed by the above method and proved to an wooden tooth pick with a length of 6.5 cm and a diameter of 0.2 cm. No active bleeding was noted from the removed site. She did not realized she had swallowed the toothpick. She was discharged without complication.

8.Outcomes and Adverse Events After Bariatric Surgery: An Updated Systematic Review and Meta-analysis, 2013–2023

Purpose:
Bariatric surgery is an increasingly common treatment for obesity and related comorbidities. This meta-analysis aimed to compare the outcomes of bariatric surgery and medical treatment (MT).
Materials and Methods:
A systematic search of articles published from January 2013 to May 2023 identified 20 studies. The treatment arms included Roux-en-Y gastric bypass (RYGB), sleeve gastrectomy (SG), gastric banding, and MT. The assessed outcomes included body weight loss, diabetes mellitus (DM) remission, changes in dyslipidemia and hypertension markers, and adverse events.
Results:
Bariatric surgery resulted in significantly better short- and long-term weight loss than MT, with RYGB and SG showing the most substantial reduction. The DM remission rates were notably higher in the surgery group, with marked improvements in hemoglobin A1c and fasting glucose levels. Improvements in dyslipidemia were inconclusive, whereas hypertension showed modest improvements, particularly with RYGB. Complication rates varied, with RYGB reporting higher rates of early complications, and SG reporting increased rates of late complications. The perioperative reoperation rates were low across all surgical treatments. Specific adverse events, such as intestinal obstruction and anastomosis site problems, were more common in the RYGB group, whereas reflux symptoms were more common in the SG group.
Conclusion
Bariatric surgery, especially RYGB and SG, provided superior weight loss and DM remission outcomes compared to MT, although with varied complication profiles. These findings underscore the need for careful patient selection and postoperative management in bariatric surgery. Future studies should aim to refine these processes to improve patient outcomes.
9.Comparative Study on Awareness Topical and Oral Steroid Use Awareness.

BACKGROUND: Topical and oral steroids are widely prescribed in dermatology. While interest in steroid use has increased rapidly, people have also acquired misinformation that causes rampant fears. OBJECTIVE: To identify fears and misunderstandings about steroid use by comparing doctors' attitudes and treatment and non-treatment group experiences with steroid use. METHODS: A total of 346 participants including outpatients who visited the Department of Dermatology at Hallym University Sacred Heart Hospital, physicians who work at the hospital, and visitors to its Department of Occupational and Environmental Medicine completed questionnaires. RESULTS: The topical treatment group had a high response rate of 46.7% regarding feeling “somewhat anxious” about using topical steroids, while the oral treatment group had a high response rate of 53.8% regarding feeling “somewhat anxious” about using oral steroids. With respect to information about the adverse effects of steroid use, those in the non-experience group obtained their information mostly from the Internet (67.5%) or the media (47.5%), whereas those in the topical treatment group obtained their information from product guides (48%) and doctors (38.7%). Meanwhile, the oral treatment group acquired information from acquaintances (39.7%) or doctors (32.1%). CONCLUSION: Dermatologists must evaluate the validity of the information that patients have about the adverse effects of topical and oral steroid use as well as the resulting phenomenon of steroid phobia. If the information possessed by patients is inaccurate, clinicians must provide a full understanding and proper education to enhance patient satisfaction and treatment compliance and success rates.

10.A Case of Vulvar Milium-like Syringoma in a Child Treated with Tranilast.

Syringoma is the most common type of benign intraepidermal eccrine sweat gland tumor in Korea, and is usually found in women in their forties. It presents mostly as a localized lesion, preferring the lower eyelid, cheek, or forehead, and rarely invades the vulval area, and in the case of children, vulvar invasion is even more rare. Tranilast is an antihistamine used for atopic dermatitis and asthma, and has recently been used for the treatment of keloid. A few previous studies have reported both localized and generalized forms of syringoma being effectively resolved with tranilast. Herein, we report a rare and interesting case of milium-like syringoma, which manifested on the vulval area of 10-year old girl that was successfully treated with tranilast.
