1.A study on Purification of Water Works by Multi-filter Bed Method with Aeration: Application of Biological Oxidation by Aerobic Microorganism.

5 bed sand filter, applying biological oxidation, was designed and studied on the treatment of water works. Never using any coagulant agent (drugs), which may cause water pollution in pre-treatment of head water, the author attempt a high rate filtration by the microorganism (nitrofication bacteria) end plant which populate in multi layer sand beds. The result are as follows ; In order to evaluate the oxygen effect on filtration, oxygen was injected in aeration tank attached to each filter tank while filtration, and NH3 was tested as a representative ingredient. It was found out that the aeration method was more effective, with over 33% of NH3 removal capacity, than the anerobic and this 5 bed filter showed double removal capacity of NH3 by comparing with conventional sand bed (2 stage bed). According to the examination of two kind of head water, pre-treated with coagulant agent and activated carben, the filtration capacity was affected by the polluted condition of head water, resulting that lower value of pollution and slower velocity of filtration showed more efficiency of NH3 removal. In this experiment NH3 content tested in treated water had a fairly good correlation with others.

2.A study on Public Nuisance in Kwangju City: Surey on Air Pollution and Noise Level.

During the period from June 1st 1971 to November 30th 1971, studies on air polluton were made in Kwangju city. The city was divided into 6 areas : the downtown ares, the simi-downtown area, the heavy traffic area, the commercial area, the residential area, the park area, 13 surveying sites were selected each representing the characteristics of the area. The Measurement methods which were used are described below. Sulfur oxides were measured by PbO2 cylinder method, sulfur dioxides (SO2) and carbon monoxide (CO) by the MSA & Kitakwa detector, dustfall by the Deposit gauge method, and the noise levels by the Kanomax sound level meter. The results obtained are as follows : 1. The mean value of sulfur oxides in Kwangju city was 1.16 mg SO3/day/100cm2 PbO2, ranging from 0.45mg SO3/day/100cm2 PbO2 to 3.10mg SO3/day/100cm2 PbO2. 2. The mean value of sulfur oxides according to its specific area in the city were 1.45mg SO3/day/100cm2 PbO2 in heavy traffic area, 1.36 in downtown area, 1.23 in semi-downtown area, 1.11 in commercial area, 0.96 in residential area, and 1.07 in park area, respectively. 3. The average concentration of sulfur dioxide was 0.063 ppm from 2 to 5 P.M in Kwangju city. 4. The average concentration of sulfur dioxides according to its specific area, from 2 to 5 P.M, in the city were 0.084 ppm in heavy traffic area & downtown area, 0.067 in commercial area, 0.053 in semi-downtown area, 0.052 in residential area, and 0.036 in park area. 5. The average concentration of carbon monoxide was 22.3 ppm from 2 to 5 P.M in Kwangju city. 6. The average concentration of carbon monoxide according to its specific area, from 2 to 5 P.M, in the city were 27.0ppm in downtown traffic area, 26.3 in semi-downtown area, 23.0 in heavy traffic area, 21.7 in commercial area, 20.0 in residential area, and 17.6 in park area. 7. The mean value of dustfall in Kwangju city was 29.28 ton/Km2/month, ranging from 9.85 ton/Km2/month to 66.34 ton/Km2/month. 8. The mean values of dustfall according to its specific area in the city were 50.37 ton/Km2/month in semi-downtown area, 42.76 in heavy traffic area, 34.67 in downtown area, 17.77 in commercial area, 14.40 in park area, and 14.76 .in residential area. 9. The mean value of the soluble dust in Kwangju city was 10.23 ton/Km2/month and that of the insoluble dust was 19.05 ton/Km2/month. 10. The mean values of noise level in Kwangju city was 62 phon, ranging from 37phon to 88 phon. 11. The mean values of noise level according to its specific area in the city were 76 phon in heavy traffic area, 67 in semi-downtown area, 64 in downtown area, 59 in commercial area, 52 in part area, and 50 in residential area.

5.Complications Following Thyroid Surgery.

BACKGROUND AND OBJECTIVES: Despite the fact that the rate of complications is constantly decreasing, the immediate and severe complications of thyroid surgery emphasize the need for precise surgical techniques and to provide patients with the most benefical treatment that results in lower morbidity. To clarify the safety of thyroid surgery, 147 thyroidectomy cases were reviewed. MATERIALS AND METHODS: From April 1998 through March 2000, 147 thyroidectomies were performed at the otolaryngologic department. The risk of recurrent laryngeal nerve lesion is based on the number of nerves at risk, and the risk of hypocalcemia is based on the number of patients undergoing bilateral procedures, or contralateral surgery in patients previously operated on. The risk of superior laryngeal nerve injury is based on the number of all thyroidectomies. RESULTS: The number of nerves at risk was 210. The rate of postoperative permanent recurrent laryngeal nerve palsy was 1.4% (3/210), which is iatrogenic. Four recurrent nerves (1.9%) invaded by cancer were sacrificed intentionally. Transient paralysis was found in 1.4% of the cases (3/210). Transient superior laryngeal nerve injury was found in 0.7% (1/147), all within one month of recovery. Of 89 patients undergoing dissection of both thyroid lobes, 1.1% (1/89) developed postoperative permanent hypocalcemia. Transient postoperative hypocalcemia was found in 7 patients (7.9%), with a recovery period ranging from one to two weeks. Postoperative bleeding, infection, and seroma were not noted in this study. CONCLUSION: The data strongly indicate that careful surgical techniques, understanding of anatomic variation and surgical experiences hold the clue for a low rate of complications following thyroid surgery.

6.Gallstone ileus inducing obstructive jaundice at the afferent loop of Roux-en-Y hepaticojejunostomy after bile duct cancer surgery: a case report.

The diagnosis of gallstone ileus is occasionally challenging due to the variability of its presentation. We herein present a very rare case of gallstone ileus inducing obstructive jaundice at the afferent loop of Roux-en-Y hepaticojejunostomy after 10 years of bile duct cancer surgery. We describe the case of a 74-year-old Korean woman with obstructive jaundice, treated conservatively. She showed severely impaired liver function test and obstructive jaundice. The computed tomography (CT) scan led to a diagnosis of very rare type of gallstones ileus at the afferent jejunal loop. Since the clinical manifestation was improved, we decided to observe her closely. On the next follow-up CT scan, the gallstone disappeared with mild distension of the afferent bowel loop, implicating spontaneous passage of the gallstone. She recovered and returned to normal life after 10 days of initiation of clinical manifestations. We presume that the gallstone may enter the afferent jejunal loop through the hepaticojejunostomy and later increase in size. The presence of narrow tract of intestine may facilitate the incidence of gallstone ileus. It appears to be the first report on this rare type of gallstone ileus inducing obstructive jaundice.

7.A Case of Sudden Sensorineural Hearing Loss in a Patient with Axial Spondyloarthritis.

Sudden sensorineural hearing loss (SNHL) is rarely presented in patients with several immune-mediated inflammatory diseases. We report a case of sudden SNHL in a patient with axial spondyloarthritis (aSpA). A 29-year-old male with aSpA was admitted for sudden unilateral hearing loss. His aSpA symptom was stable; however, pure tone audiometry revealed that he had SNHL in the left ear at low frequency. His hearing was fully recovered with systemic and local steroid therapy. This case suggests that sudden SNHL may be an extra-articular manifestation of aSpA and thus need prompt steroid therapy in order to restore hearing.

8.Beneficial Effects of Stroke-Unit Care in Stroke Patients with Atrial Fibrillation.

PURPOSE: Continuous cardiac monitoring in a stroke unit (SU) may improve detection of atrial fibrillation (AF), and SU care may improve the rate of anticoagulation by better adherence to a standardized treatment protocol in patients with AF. We investigated the effects of the SU on the detection of AF and the rate of warfarin therapy in patients with AF. MATERIALS AND METHODS: Acute stroke patients who had been admitted before or after the opening of the SU were included in our study. SU patients were monitored continuously with electrocardiography. Rates of AF and warfarin therapy were compared between patients admitted to the SU (SU group) and those admitted to the general ward (GW) prior to the opening of the SU (GW group). RESULTS: Total 951 patients had been admitted to the GW prior to the opening of the SU (from January 2000 to November 2002), and 2349 patients to the SU (from January 2003 to December 2008). AF was found in 149 patients (15.7%) in the GW group and in 487 (20.7%) in the SU group. Most of AF detected during admission was paroxysmal AF (84.8%). The frequency of newly detected AF was higher in the SU group than the GW group (2.5% vs. 0.7%, p=0.001). The rate of anticoagulation consideration was also higher in the SU group. CONCLUSION: SU care improved the detection of AF and the rate of anticoagulation consideration in acute stroke patients. Our findings support the benefits of continuous cardiac monitoring in the SU for stroke patients.

9.Esophageal Ulcer Induced by Alendronate.

Alendronate sodium (Fosamax(R)) is an aminophosphonate used in treatment of postmenopausal osteoporosis by inhibition of osteoclastic activity. Esophageal ulcers related to alendronate use have been reported at the rate of 2~6%. However, there have been no reports of esophagitis or esophageal ulcer by alendronate in Korea. We present a case of esophageal ulcer caused by alendronate. The patient presented with retrosternal pain and severe odynophagia for a few days. Esophagogastroduodenoscopy revealed multiple deep ulcers at the esophagus. Her symptoms resolved completely after stopping alendronate. A follow-up esophagogastroduodenoscopy 6 weeks later revealed complete healing of the ulcers.

10.Diagnostic Value of Preoperative Serum Thyroglobulin Measurement for the Diagnosis of Malignancy in Follicular or Hurthle Cell Neoplasms of the Thyroid Gland.

OBJECTIVES: The aim of this retrospective study was whether serum Tg predicts malignancy in follicular or Hurthle-cell neoplasms on fine needle aspiration. METHODS: A chart review of 111 patients (90 females, 21 males; mean age 46.8 +/- 11.9 years) with follicular or Hurthle-cell neoplasms on fine needle aspiration, who were surgically treated between Sep. 2001 and Sep. 2011, was performed. Predictive factors for malignancy were identified by the chi-squared test and multivariate logistic regression. RESULTS: There were no differences between 41 malignant and 70 benign lesions in serum Tg or any of the normalized indexes. Receiver-operator characteristic analysis revealed no cut-off value. Lesions with serum Tg levels greater than 500 g/L had no significant difference. And also there were no independent predictors of malignancy by multivariate logistic regression. CONCLUSIONS: In this study, the author found that serum Tg has poor accuracy for predicting malignancy in follicular or Hurthle cell neoplasms on fine needle aspiration.
