1.Effects of acupuncture on SATB1/p21 signaling pathway and SASPs in MPTP-induced Parkinson disease model mice

Objective:To observe the effects of acupuncture on the motor function of Parkinson disease(PD)model mice and to investigate the neuroprotective effects of acupuncture on PD from the perspective of cellular senescence.Methods:C57BL/6J mice were randomly divided into a normal control(NC)group,a 1-methyl-4-phenyl-1,2,3,6-tetrahydropyridine(MPTP)group,an acupuncture(ACU)group,and a rasagiline(RAS)group,with 6 mice in each group.Except for the mice in the NC group,all mice were injected intraperitoneally with MPTP[30 mg/(kg·bw)]to establish a PD mouse model.After the models were successfully established,mice in the ACU group received acupuncture at Baihui(GV20)and bilateral Yanglingquan(GB34)for 15 min,once a day for 14 consecutive days.Mice in the RAS group were treated with gavage of rasagiline mesylate[0.5 mg/(kg·bw)],once daily for 14 d.Mouse balance and motor functions were detected using the mouse fatigue rotating rod apparatus.Immunohistochemistry staining was used to detect the number of tyrosine hydroxylase(TH)-positive neurons and the protein expression levels of special AT-rich sequence-binding protein 1(SATB1),p21,and p53 in the substantia nigra(SN)region of the mouse brain in each group.The glutathione peroxidase(GSH-Px)activity of mouse brain SN tissue was detected by enzyme-linked immunosorbent assay.The protein expression levels of interleukin(IL)-6 and senescence-associated β-galactosidase(SA-β-gal)in the SN tissue of mice in each group were detected by Western blotting.The relative expression of SATB1,p21,and p53 mRNA in the SN of each group was detected by real-time quantitative polymerase chain reaction.Results:Compared to the NC group,the overall rod performance(ORP)score,the number of TH-positive neurons,and GSH-Px activity in the SN region were significantly lower in the mice in the MPTP group(P<0.01);compared to the MPTP group,the ORP score,the number of TH-positive neurons,and GSH-Px activity were significantly increased in the ACU group and the RAS group(P<0.01 or P<0.05).Compared to the NC group,the protein levels of IL-6 and SA-β-gal in the SN tissue,the protein and mRNA expression levels of p21 and p53 were significantly increased(P<0.01);compared to the MPTP group,the protein levels of IL-6 and SA-β-gal in the SN tissue,the protein and mRNA expression levels of p21 and p53 were significantly decreased in the ACU group and the RAS group(P<0.01 or P<0.05).Compared to the NC group,the relative expression of SATB1 protein and mRNA in the SN of mice in the MPTP group was significantly decreased(P<0.01);compared to mice in the MPTP group,mice in the ACU group and the RAS group showed significant increases in the relative expression of SATB1 protein and mRNA(P<0.01 or P<0.05).Conclusion:Acupuncture can improve motor function and increase the number of TH-positive neurons in the SN of PD model mice.Its neuroprotective effect may relate to the regulation of the SATB1/p21 signaling pathway and the inhibition of cellular senescence-related biomarker expression in the SN.
2.Study on the value of superb microvascular imaging in prediction of ovarian cancer staging

Objective To evaluate the application value of superb microvascular imaging(SMI)in pre-dicting ovarian cancer staging.Methods One hundred and thirty-six patients with ovarian cancer who under-went surgical treatment in our hospital from March 2019 to May 2024 were enrolled.Transvaginal,transab-dominal low-frequency,and transabdominal high-frequency two-dimensional grayscale imaging,Color Doppler Flow Imaging(CDFI),and SMI were performed before surgery for staging prediction.Taking surgical-patho-logical staging as the gold standard,the sensitivity,specificity,and accuracy of the combined application of transvaginal SMI,transabdominal low-frequency SMI,and transabdominal high-frequency SMI for predicting ovarian cancer staging were calculated.The receiver operating characteristic(ROC)curve was established,the area under the ROC curve(AUC)was calculated,and the diagnostic efficacy of SMI for predicting ovarian cancer staging was determined.Results The combined application of transvaginal SMI,transabdominal low-frequency SMI,and transabdominal high-frequency SMI for predicting ovarian cancer staging showed that sen-sitivity was 89.35%,92.36%,81.65%,and 83.21%for stages Ⅰ,Ⅱ,Ⅲ,Ⅳ,respectively;specificity was 86.93%,84.29%,83.39%,and 82.88%;accuracy was 92.50%,94.38%,80.15%,and 84.96%;and AUC was 0.799,0.760,0.695,and 0.727.Conclusion SMI has a high application value in predicting the stage of ovarian cancer,especially for stage Ⅰ and stage Ⅱ ovarian cancer.

4.Recovery of urinary function in patients with stress urinary incontinence after tension-free vaginal tape procedure and its related influencing factors

Objective lo investigate the recovery of urinary function and related influencing factors in patients with stress urinary incontinence(SUI)after the tension-free vaginal tape procedure at the mid-urethra.Methods A total of 112 SUI patients who underwent the tension-free vaginal tape procedure at the mid-urethra in Beijing Rehabilitation Hospital Affiliated to Capital Medical University from October 2018 to October 2023 were selected as the research subjects.The postoperative urinary function recovery of all patients was recorded.According to the postoperative urinary function recovery time,patients with a recovery time of≤3 days were included in the rapid recovery group,and those with a recovery time of>3 days were included in the non-rapid recovery group.Clinical data including age,body mass index,menopausal status,vaginal delivery history,number of deliveries,disease severity,comorbid hypertension,diabetes,history of pelvic organ prolapse,maximum urinary flow rate,postoperative urinary retention,preoperative bladder residual urine volume,maximum bladder volume,hemoglobin(Hb)level,white blood cell(WBC)count,red blood cell(RBC)count,and platelet(PLT)count of patients between the two groups were compared.Factors affecting the recovery of urinary function in SUI patients were analyzed by multivariate logistic regression.Results The recovery time of urinary function in 112 patients with SUI ranged from 1 to 15(4.10±0.78)days.Specifically,64 patients(57.14%)recovered within 3 days,and 48 patients(42.86%)recovered in more than 3 days.The proportions of patients with menopause,history of vaginal delivery,history of pelvic organ prolapse,and postoperative urinary retention in the non-rapid recovery group were significantly higher than those in the rapid recovery group,and the maximum urinary flow rate was significantly lower than that in the rapid recovery group(P<0.01).There were no significant differences in age,body mass index,number of deliveries,preoperative bladder residual urine volume,maximum bladder volume,Hb level,WBC count,RBC count,PLT count,disease severity,proportion of patients with comorbid hypertension and proportion of patients with diabetes between the two groups(P>0.05).Multivariate logistic regression analysis showed that menopause,history of vaginal delivery,history of pelvic organ prolapse,postoperative urinary retention and maximum urinary flow rate were influencing factors for the recovery of urinary function in SUI patients after surgery(P<0.05).Conclusion The recovery of urinary function in SUI patients after a transvaginal tension-free vaginal tape procedure at mid-urethra is not ideal.Menopause,history of vaginal delivery,history of pelvic organ prolapse,postoperative urinary retention and maximum urinary flow rate are the influencing factors for the recovery of urinary function in SUI patients.
5.Best evidence for the application management of over-the-counter medications for constipation in the elderly

Objectives:To search, evaluate, and summarize the evidence related to the application management of over-the-counter (OTC) medications for constipation in the elderly, providing evidence-based support for the rational use of these medications.Methods:A systematic search was conducted through computerized decision support systems, guideline websites, professional association websites, and databases for clinical decisions, guidelines, evidence summaries, and expert consensus on the management of OTC medications for constipation in the elderly. The search period spanned from January 2018 to March 2023. The quality of the literature was independently assessed, and evidence was extracted. The 2014 version of the Joanna Briggs Institute (JBI) evidence grading system was used to determine the levels of evidence.Results:A total of 18 article were included, consisting of two clinical decisions, five guidelines, seven expert consensus statements, and four evidence summaries. These covered five areas: medication principles for elderly individuals with different characteristics, medication selection, medication monitoring, health education, and precautions, culminating in a total of 30 best evidence.Conclusions:This study summarizes the best evidence for the management of OTC medications for constipation in the elderly, providing evidence-based support for the rational application of these medications.
6.Application and prospect of universal CAR-NK cell therapy for systemic lupus erythematosus

Systemic lupus erythematosus(SLE)is characterized by a large number of anti-autoantibodies produced by abnormally activated B cells,which form immune complexes with autoantigens to induce systemic inflammation,leading to the involvement of multiple systems and organs.Existing treatment strategies targeting B cells(such as belimumab,telitacicept,and rituximab)had limited efficacy.Recently,CD 19 targeting chimeric antigen receptor(CAR)-T cell has shown excellent efficacy in the treatment of SLE.However,autologous CAR-T cell therapy has potential risks such as cytokine release syndrome(CRS),T cell tumors,and infections,and it is expensive.CAR-natural killer(NK)cell therapy,on the other hand,is an off-the-shelf cellular therapy with high safety,no neurotoxicity or tumorigenic risk,and with low treatment costs.Currently,CAR-NK cell has achieved remarkable research results in treating hematological malignancies,and preliminary clinical studies of CAR-NK cell therapy for SLE have showed good efficacy,excellent safety,and predictable durability.This article focuses on the characteristics of the new generation of universal CAR-NK cell and the latest clinical research results for the treatment of relapsed and refractory SLE,offering insights into its promising future in the treatment of SLE and B-cell-related autoimmune diseases.
7.Tumor necrosis factor-α inhibitors in treating ankylosing spondylitis complicated with osteoporosis:a 5-year follow-up study

Objective To evaluate the long-term efficacy of tumor necrosis factor-α(TNF-α)inhibitor(TNFi)in the treatment of ankylosing spondylitis(AS)complicated with osteoporosis(OP)and the impact on bone metabolism,bone density,and inflammatory factors.Methods The data of 158 patients with AS and OP,who were admitted to Department of Rheumatology and Immunology of The First Affiliated Hospital of Naval Medical University(Second Military Medical University)from Jan.1,2010 to Dec.31,2017,were retrospectively collected.The patients were divided into bisphosphonate group(n=54),TNFi group(n=58),and TNFi+bisphosphonate group(n=46)according to the treatment methods.All patients were treated with calcium combined with calcitriol as the background treatment.After 5 years of treatment,Bath ankylosing spondylitis disease activity index(BASDAI)and Bath ankylosing spondylitis functional index(BASFI)scores were evaluated,and inflammatory indexes,bone metabolism markers,and bone mineral density were detected.Results After 5 years of treatment,the BASDAI and BASFI scores,erythrocyte sedimentation rate(ESR),C reactive protein(CRP),TNF-α,and interleukin-17A of the TNFi+bisphosphonate group and TNFi group were significantly lower than those before treatment(all P<0.05);in the bisphosphonate group only ESR and CRP were significantly lower than those before treatment(both P<0.05),and the other inflammatory indexes and BASDAI and BASFI scores showed no significant changes(all P>0.05).The bone mineral density of the 3 groups after 5 years of treatment was significantly higher than that before treatment(all P<0.05),and the bone mineral density of the TNFi+bisphosphonate group was significantly higher than that of the other 2 groups(both P<0.05).After 5 years of treatment,the levels of parathyroid hormone(PTH),procollagen type 1 N-terminal propeptide(P1NP)and β-C-terminal telopeptide of type Ⅰ collagen(β-CTX)in the TNFi+bisphosphonate group and bisphosphonate group were significantly decreased compared with those before treatment(all P<0.05),while the levels of N-terminal midfragment of osteocalcin(N-MID)and 25-hydroxy-vitamin D(25VitD)were significantly increased(all P<0.05);in the TNFi group only PTH and P1NP levels were significantly decreased(both P<0.05),while β-CTX,N-MID and 25VitD levels showed no significant differences(all P>0.05).Conclusion Long-term use of TNFi in patients with AS and OP can effectively reduce disease activity,improve physical function,decrease the level of inflammatory factors,alleviate abnormal bone metabolism,and increase bone mineral density;and the combined use of TNFi and bisphosphonates has better efficacy.
8.Investigating efficacy mechanism of electroacupuncture in treating Parkinson disease through TMT proteomics

Objective:To explore the therapeutic mechanism of electroacupuncture(EA)in treating Parkinson disease(PD)using Tandem mass tag(TMT)quantitative proteomics technology. Methods:Forty-eight PD patients were randomly divided into a control group and an observation group,with 24 patients in each group.The control group received routine drug treatment,while the observation group received EA in addition to the routine drug treatment.EA was administered for 30 min per session,3 times a week,for a total of 12 weeks.Nine patients from each group were randomly selected to provide peripheral blood serum samples before and after treatment for TMT quantitative proteomics analysis.Differentially expressed proteins between the two groups were compared,and bioinformatics analysis was performed.The screened differentially expressed proteins were validated using enzyme-linked immunosorbent assay(ELISA). Results:In the observation group,scores on the unified Parkinson disease rating scale(UPDRS),UPDRS Ⅱ,and UPDRS Ⅲ were significantly reduced after treatment(P<0.05).In the control group,these scores tended to increase,but the changes were not statistically significant(P>0.05).After treatment,the UPDRS and UPDRS Ⅲ scores in the observation group were significantly lower than those in the control group(P<0.05).The observation group showed 62 differentially expressed proteins,while the control group had 36.Compared to the control group,the observation group had 142 differentially expressed proteins.These proteins were primarily involved in the cyclic adenosine monophosphate(cAMP)signaling pathway,T helper(Th)1 and Th2 cell differentiation,ATP-binding cassette transporter,vascular endothelial growth factor signaling pathway,and high-affinity immunoglobulin E receptor(FcεRI)signaling pathway.ELISA verification indicated that after EA treatment,the levels of α-Synuclein(αSyn)and heat shock protein beta 1(HSPB1)in the observation group were significantly lower than those in the control group(P<0.05),while the regulator of G-protein signaling 10(RGS10)level was significantly higher(P<0.05). Conclusion:EA,combined with routine drug therapy,can significantly improve clinical symptoms of PD,potentially through the regulation of the cAMP signaling pathway and the contents of differentially expressed proteins of αSyn,HSPB1,and RGS10.
9.Progress in clinical research of Claudin-18.2 targeted therapy for advanced gastric cancer

Gastric cancer is one of the most common malignant tumors of the digestive system.Most gastric cancer patients are already in advanced stage when initially diagnosed.Advanced gastric cancer has a poor prognosis and has received limited benefit from chemotherapy-based therapies.In the recent years,with the development of biomolecular technologies,more and more researches have set their focus on the search for key molecules that affect the progression of gastric cancer,prognostic indicators,and therapeutic targets.Claudin-18.2 is a promising novel tumor therapeutic target which brings new hope for targeted therapy for gastric cancer.Various types of Claudin-18.2-targeting drugs have been developed,including monoclonal antibody,bispecific antibody(BsAb),antibody-drug conjugate(ADC),chimeric antigen receptor T cell(CAR-T),and so on.At present,several clinical trials have been carried out and the results are encouraging.This review summarizes the progress in clinical trials of Claudin-18.2 targeted therapy,in the hope to provide reference for improving the prognosis of patients with advanced gastric cancer.
10.Preparation and application of novel photosensitive nanoparticles in tumor photodynamic therapy

Objective To prepare a novel photosensitive nanoparticle and to evaluate its physicochemical properties,and effect on the efficacy of photodynamic therapy.Methods 5,15-dibromo-10,20-diphenylporphine(DBN),tetrafluoroterephthalonitrile(TFN),and the amphiphilic polymer methoxy-polyethylene glycol-distearoylphosphatidylethanolamine(DSPE-MPEG2000,PEG)were dissolved in tetrahydrofuran(THF)by co-precipitation method to prepare novel photosensitive nanoparticles,named DBN/TFN@PEG.The physicochemical properties of DBN/TFN@PEG were characterized.Both novel and conventional nanoparticles were continuously irradiated with a 660 nm laser,and the fluorescence intensity of nanoparticles,representing reactive oxygen species(ROS)production levels,was measured using a fluorescence spectrophotometer at different irradiation times.Tumor cells were co-incubated with the nanoparticles and irradiated with a 660 nm laser.ROS levels within the tumor cells were detected using immunofluorescence,and the ratio of dead to live tumor cells was determined using PI/Calcein-AM staining.Results Prepared DBN/TFN@PEG nanoparticles with hydrated particle size of approximately 107.8 nm were uniformly distributed in the solution.Compared to conventional nanoparticles,the ROS production capacity of DBN/TFN@PEG was significantly higher(P<0.01).Immunofluorescence results showed that the generation of ROS levels in the tumor cells of DBN/TFN@PEG group were significantly higher than in the conventional nanoparticles group under laser irradiation(P<0.01).PI/Calcein-AM staining results indicated a significantly higher ratio of dead tumor cells in the DBN/TFN@PEG group compared to the conventional nanoparticle group(P<0.01).Conclusions DBN/TFN@PEG has stable physicochemical properties and uniform distribution in the solution.As effective photosensitizers,DBN/TFN@PEG can exhibit stronger ability to induce ROS generation in tumor cells,and may enhance the efficacy of photodynamic therapy in cancer.
