1.Clinical Observation of Shenshao Capsules in the Treatment of Unstable Angina of Coronary Heart Disease

OBJECTIVE:To observe clinical efficacy of Shenshao capsules in the treatment of unstable angina(UA)of coro-nary heart disease,and its effects on heart rate,blood pressure,myocardial consumption of oxygen (MCO),serum levels of hs-CRP and MMP-9. METHODS:77 UA patients with coronary heart disease selected from our hospital during May 2014-May 2016 were divided into observation group (39 cases) and control group (38 cases) according to random number tablet. Control group received rountine treatment. Observation group was additionally given Shenshao capsule 1.2 g,tid,on the basis of control group. Both groups were treated for 4 weeks. The heart rate,blood pressure,myocardial oxygen consumption,serum levels of hs-CRP and MMP-9 in 2 groups were observed before and after treatment;stopping and reduced rate of nitroglycerin,and therapeu-tic efficacy of ECG ST segment were compared between 2 groups. RESULTS:There was no statistical significance in heart rate, blood pressure,myocardial oxygen consumption,serum levels of hs-CRP and MMP-9 between 2 groups before treatment (P>0.05). After treatment,above indexes of 2 groups were decreased significantly,and the observation group was significantly lower than the control group,with statistical significance (P<0.05). The stopping and reduced rate of nitroglycerin,total response rate of ECG ST segment were 82.1%,87.2%in observation group,which were better than 63.2%,78.9%in control group,with statis-tical significance(P<0.05). CONCLUSIONS:Shenshao capsules is effective for coronary heart disease UA,and can effectively re-duce heart rate,blood pressure,myocardial oxygen consumption,serum levels of hs-CRP and MMP-9.
2.Doctor-patient Relationship Pattern Research during Resident Doctor's Standardized Training

Objective:To carry out the large sample research of the importance of medical ethics in the stand-ardized resident doctor training. Methods:Randomly selected 100 graduated resident doctors from Jiangsu province, Zhejiang province,Shanghai and Jiangxi province of standardization training. Using the questionnaire survey form to investigate the doctor-patient relationship and carry out correlation analysis. Results:Two groups of subjects after research statistics, the analysis found the doctor-patient relationship in the model show the current mutual partici-pation pattern of the more advanced undergraduate group was 22%, 16% in the graduate student degree group has significant difference;While 66% of bachelor's degree group and 70% graduated degrees performance as the guide-cooperation mode. Conclusion:It should strengthen medical ethics in the stage of standardized training, this study further strengthen standardized training for the future stage of low qualification resident ethics application guide the importance and the influence on the doctor-patient relationship in the future physician has provided the basis, at the same time, also provides certain theoretical guidance for doctor-patient relationship training methodology.
3.Clinical characteristics of tubulointerstitial nephropathy caused by Guanxinsuhe pill (冠心苏合丸):a clinical analysis of 27 cases

Objective:To investigate the clinical characteristics of renal injury caused by Guanxinsuhe pill(冠心苏合丸,including slander dutchanspipe root(青木香)).Methods:The clinical data of 27 patients with renal injury caused by Guanxinsuhe pill were studied.The clinical characteristics of the renal damage caused by Guanxinsuhe pill and the relationships between duration of illness,duration of taking Guanxinsuhe pill,anemia,renal atrophy and the progression of renal injury respectively were also analyzed.Results:Twenty-seven patients all taken Guanxinsuhe pill for suspicious coronary heart disease(CHD) and were diagnosed as chronic tubulointerstitial nephropathy accompanied with renal tubular acidosis,renal glucosuria and hyposthenuria after admission.Anemia was of more severity than renal dysfunction.Atrophic kidney occurred in 26 patients.The main complains were nonspecific symptoms including fatigue,anorexia,nausea,vomiting,etc.and accompanied with different degrees of chronic renal dysfunction.Twentyfour hours urinary protein excretions were 120-900 mg((490?250) mg).There were few red and white cells in uroscopy.Sixteen patients discontinuously took the Guanxinsuhe pill(one capsule,2-3 times a day) according to the pharmacopoeia for a long period.Ten patients took a dosage(two capsules,2-3 times a day) higher than that limited by the pharmacopoeia.Twenty-six patients took Guanxinsuhe pill discontinuously.One patient continuously took Guanxinsuhe pill until got renal dysfunction.The duration of taking Guanxinsuhe pill was 1-120 months(37.38?31.58),and the amount of 50-43 800 pills(4 576.00?8 830.54) were used,but the amount of pills of 3 cases was uncertain.The renal injury occurred within 30-216 months(95.41?56.74) after initiation of the drug taken.The renal injury obviously correlated with the anemia and the duration of taking Guanxinsuhe pill.Conclusion:Taking Guanxinsuhe pill can cause renal injury.The Guanxinsuhe pill dose indicated in pharmacopoeia also causes renal damage.The onset of renal injury is insidious.Most of the patients have the history of CHD and when the chronic nephropathy in most of such patients is detected,it is advanced to stage 5.The renal injury induced by Guanxinsuhe pill usually presents as chronic tubulointerstitial nephropathy,renal tubular dysfunction and anemia.Anemia is of more severity than renal dysfunction.The 24 hours urinary protein excretions of all the patients are lower than 1 000 mg daily.It is suggested to avoid taking Guanxinsuhe pill,adjust the prescription of Guanxinsuhe pill and do not use slander dutchanspipe root as an ingredient.
4.Effect of early enteral nutrition with probiotics on gastrointestinal motility disturbance and nutritional status of patients with severe craniocerebral trauma

Objective To investigate effect of early enteral nutrition supplemented with probiotics on gastrointestinal motility disturbance and nutritional status in patients with severe craniocerebral trauma.Methods Forty patients with severe craniocerebral trauma were randomized into study group (18patients) and control group (22 patients).Patients of both groups received enteral nutrition via nasogastric tube at 24-72 hours after admission,but the patients of study group were also supplemented with probiotics simultaneously.Rate of abdominal distention,vomiting,gastro-oesophageal reflux,gastric retention,constipation and diarrhea were recorded during the whole study.Time to first defecation and time to targeted nutritional goals were also recorded.Prealbumin and transferrin in serum were detected at days 0,4,7,and 15 after the beginning of enteral nutrition.Length of ICU stay was compared between groups.Results There were no significant differences of the two groups in terms of rate of abdominal distention,vomiting,gastric retention and diarrhea.However,less gastro-oesophageal reflux or constipation patients were observed in study group,as compared with control group (P < 0.05).Time to first defecation and time to targeted nutritional goals were shorter in study group,as compared with control group (P < 0.05).Levels of prealbumin and transferrin had no significant differences between the two groups at days 0,4,and 7,but study group showed both were higher than control group at day 15 (P<0.05).Moreover,length of ICU stay showed no significant difference between the two groups.Conclusions Compared with simple enteral nutrition,early enteral nutrition with probiotics improves gastrointestinal motility,facilitates the delivery of enteral nutrition,and further ameliorates nutritional status in patients with severe craniocerebral trauma.
5.Dosimetric comparison of three intensity-modulate radiation therapy treatment modules for nasopharyngeal carcinoma

Objective To compare the dosimetric differences of fixed field intensity-modulated radiation therapy(IMRT),volumetric modulated arc therapy (VMAT) and helical tomotherapy (HT) for nasopharyngeal carcinoma (NPC) patients.Methods Eighteen NPC patients previously treated with VMAT were retrospectively included and re-planned using HT and IMRT (7 fields) techniques utilizing the same dose prescription and optimization objectives.The following parameters were compared across the three types of plans:homogeneity index (HI),conformity index (CI),maximum dose (Dmax) and mean dose (Dmean) of targets ; the Dmax and Dmean of organs at risk (OARs) ; the doses and volumes of volume of interest; the treatment delivery time and monitor units (MU).Results Clinically acceptable target coverage could be achieved by IMRT,VMAT and HT plans.The HT plans were the best yet IMRT plans were the worst in HI and CI of targets.IMRT imposed highest doses to OARs while HT deposited least doses to the spinal cord,brainstem and parotid.However,the VMAT plans displayed the lowest doses on optic nerves,chiasma and lens while highest doses were found in IMRT plans.The average delivery time per fraction of IMRT (8.0±0.5) min were more than that of HT (7.4 ±0.9) min and VMAT (3.9 ±0.1) min plans.The MUs of IMRT plans (711.4 ±78.7) were larger than that of VMAT plans (596.4 ±33.7).Conclusions Three types of plans can all achieve the clinical dosimetric demands,but HT has the best performance on CI and HI.VMAT is most efficient regarding the delivery time and total MUs.
6.Relationship between brachial-ankle pulse wave velocity and metabolic syndrome among the elderly in a Beijing community and the gender difference

Objective:To evaluate the association between brachial-ankle pulse wave velocity ( ba-PWV) and metabolic syndrome ( MetS) among the Chinese elderly and the gender difference .Methods:We conducted a population-based cross-sectional study in a representative urban area of Beijing , China. A sample of 2 102 community elderly (848 males, and 1 254 females) aged 60 to 95 years were included in the study .MetS was defined according to the 2009 harmonizing definition .Results:The prevalence of MetS was 59.1% (50.1% in males and 65.2% in females, P<0.001).The baPWV value was be-tween 8.2 to 45.6 (20.0 ±4.4) m/s, and showed an increasing trend with age (P<0.001).The par-tial correlation showed baPWV was positively associated with BMI (r =0.076, P =0.037), systolic blood pressure (r=0.380, P<0.001), diastolic blood pressure (r=0.276, P<0.001), triglyceride (r=0.040, P=0.046), fasting blood glucose (r=0.140, P<0.001), 2-hour post-meal blood glu-cose (r=0.121, P<0.001), and negatively associated with HDL-C (r=-0.128, P=0.048).There was a strong association between baPWV and prevalence of MetS and its component number in females but not in males .Compared with the lowest quartile of baPWV , the adjusted ORs were 1 .22 ( 95%CI 0.83-1.79), 1.32(95%CI 0.90-1.93), 1.46(95%CI 1.00-2.14) in males and 1.28(95%CI 0.93-1.77), 1.55(95%CI 1.12 -2.16), 1.86(95%CI 1.32-2.61) in females for the second, third and top quartiles .Conclusion:The prevalence of MetS increases substantially with increasing levels of baPWV among the Chinese elderly , especially in females .
7.An improved method for isolation of human atrial cardiomyocytes

Objective To study an improved isolated method of single human atrial myocytes.Methods Enzyme digestion method was used to isolate single myocytes from human atrial and whole-cell patch clamp technique was used to record small conductance calcium activated potassium current.Results This method obtained a large number of atrial myocytes.The total amount of atrial myocytes in SR group was 320±30 while AF group was 230±20 and the difference was statistically significant(P<0.01).In this study,a large number of simple and striated single atrial myocytes were obtained,and a typical small-conductance calcium-activated potassium channel current was recorded on the isolated atrial myocytes.Conclusion The established isolated method is simple,stable and effective.We can acquire a large amount of single atrial myocytes with good quality.
8.Correlation between mitochondrial DNA control region variations and keloid formation

Objective:To investigate the correlation between variations in mitochondrial DNA (mtDNA) control region (D-loop) and keloids.Methods:A total of 216 patients with keloids were collected from Department of Dermatology, the First Affiliated Hospital of Kunming Medical University from 2016 to 2019. Total DNA was extracted from peripheral blood samples of all the patients, as well as keloid tissues and perilesional normal skin tissues of 25 patients with keloids. Peripheral blood samples were collected from 299 health checkup examinees without keloids in Health Examination Center, the Affiliated Hospital of Yunnan University, who served as controls. PCR amplification and Sanger sequencing were performed on the mtDNA D-loop region, and mutation sites in each sample were analyzed by comparisons with the revised Cambridge Reference Sequence (rCRS) . Haplogroups were assigned in the 2 groups by using Phylotree-mtDNA tree Build 17. Mutations in the mtDNA D-loop region were compared among keloid tissues, perilesional normal skin tissues and peripheral blood samples. A median-joining network was constructed via network 5.0 software. Binary logistic regression analysis was performed to investigate the correlation between haplogroup frequencies and the occurrence of keloids, and chi-square, t and t′ tests were used to analyze clinical data. Results:Among the 216 patients with keloids, variations in mtDNA D-loop region were classified into 10 haplogroups, including A, B, D, R9, G, M*, M7, M8, M9 and N9, with the haplogroups R9 and M9 showing the highest (21.3%, 46/216) and lowest (0.9%, 2/216) frequencies respectively. The frequencies of haplogroups M7 ( P=0.040, OR=0.248, 95% CI: 0.066 - 0.937) and N9 ( P=0.048, OR=0.191, 95% CI: 0.037-0.986) were significantly lower in the patients with keloids than in the controls. The median-joining network plot showed that the distribution pattern of the haplogroup M7 differed between the patients with keloids and controls. Significantly less number of lesional sites and younger age of onset were observed in the patients with haplogroup M7 compared with those with non-M7 haplogroups ( P=0.000 1, 0.045, respectively) . Conclusion:The haplogroup M7 is correlated with the occurrence of keloids, and may be a potential protective factor for keloid formation.
9.Genetic analysis and prenatal diagnosis of a pedigree with oculo-facio-cardio-dental syndrome: a case report and literature review

Objective:To analyze the clinical phenotypes and prenatal diagnosis of a pedigree with oculo-facio-cardio-dental (OFCD) syndrome.Methods:A pregnant woman at 17 gestational weeks was admitted to the Nanjing Drum Tower Hospital, the Affiliated Hospital of Nanjing University Medical School in 2017 for genetic counseling. Genetic tests as performed for the proband (the pregnant woman), her husband, and the induced fetus of last pregnancy genetic test and the detected variants were analyzed and verified by chromosomal microarray analysis (CMA), multiplex ligation-dependent probe amplification (MLPA) and quantitative real time-polymerase chain reaction (Q-PCR). The detection platform established by MLPA and Q-PCR technology was used to perform prenatal diagnosis of the present pregnancy. Other family members were screened for BCOR gene mutation. Related mutation types were retrieved from ClinVar database with term of " BCOR", and related literature from CNKI and PubMed with terms of "OFCD syndrome", " BCOR gene", and "oculo facio cardiac dental syndrome" to summarize the clinical manifestations, mutation type and pathogenesis of this disease. Results:The proband has congenital cataracts, long face, congenital atrial septal defect, and severe dental malformations, which were consistent with the clinical features of OFCD syndrome. WES suggested that the proband and her induced fetus were suspected of having a large submicroscopic deletion of the exons of BCOR gene, which was confirmed by CMA, MLPA and Q-PCR, with a 105 kb deletion containing BCOR exons 1-15. The amniotic fluid genetic analysis of the present pregnancy showed that the fetus has a normal female karyotype, and did not carry the same BCOR gene copy number abnormality as the proband. The child grew and normally developed without any characteristic manifestations of OFCD syndrome during follow-up. Other families of the proband did not show clinical features of OFCD syndrone, and no BCOR gene copy number abnormality was detected. A total of 35 cases of BCOR gene mutation types related to OFCD syndrome were retrieved from ClinVar database. The data analysis revealed that the differences in clinical manifestations between Lenz microphthalmos syndrome and OFCD syndrome were mainly caused by different mutation types of BCOR gene. Among the 90 retrieved cases of OFCD syndrome obtained through literature, only one case was reported in China. Analysis of these 90 cases showed that the characteristic manifestations of OFCD syndrome, involving the eye, face, heart, teeth, and skeletal system. OFCD syndrome were confirmed in the proband and her induced fetus according to the clinical manifestation and the mutation type of BCOR gene. Conclusions:The clinical manifestations of OFCD syndrome are complicated, caused by various mutation types of BCOR. Systematic molecular genetic technology can be effectively applied for gene and prenatal diagnosis of OFCD syndrome.
10.Meta-integration of qualitative researches on nursing students' learning experience in high-fidelity simulation teaching

Objective:To integrate the qualitative researches on nursing students' learning experience in high-fidelity simulation teaching, so as to provide a basis for the improvement of high-fidelity simulation teaching.Methods:The qualitative researches on nursing students' learning experience in high-fidelity simulation teaching was searched by computer in Embase, Web of Science, CINAHL, PubMed, Cochrane Library, Scopus, Wanfang Database, China Biology Medicine disc, CNKI and VIP. The search deadline was from establishment of the database to September 30, 2022. The quality of the literature was evaluated using the 2017 JBI Critical Appraisal Checklist for Qualitative Research, and the results were integrated by a pooled integration method.Results:A total of 13 articles were included, 29 research results were extracted, which were summarized into 6 categories, and 3 results were obtained by integration (positive and negative experience brought by high-fidelity simulation teaching, improvement of students' ability by high-fidelity simulation teaching and influencing factors for the effectiveness of high-fidelity simulation teaching) .Conclusions:Nursing students hold a relatively positive attitude towards high-fidelity simulation teaching, believing that it can closely combine theoretical knowledge with practice and strengthen their professional abilities. Future research needs to strengthen student-centered approaches and build a highly simulated and sustainable simulation environment to improve the effectiveness of high-fidelity simulation teaching for nursing students.
