1.Expression of X-linked inhibitor of apoptosis in pancreatic carcinoma tissues and its relationship to chemoresistance

Objective To explore the expression of X-linked inhibitor of apoptosis (XIAP) in pancreatic carcinoma tissues and SW1990 cell line, and to investigate its relationship to the chemoresistance. Methods The expression of XIAP in 23 pancreatic carcinoma tissues was detected by immunohistochemistry and the relationship of XIAP expression with the clinical stage and pathological grade was analyzed. 5-FU was used to induce the chemoresistant capability of SW1990 pancreatic cell line. The XIAP expression in SW1990 was examined by RT-PCR and Western blot. The XIAP level in the cells and its chemoresistance to 5-FU were analyzed by linear regression. Results The XIAP expression was detected in 89. 6% (20/23) of pancreatic carcinoma tissues and 33. 3% (4/12) of normal pancreatic tissues,respectively. 2.Preliminary experience of metatarsophalangeal arthroplasty with silicon Swanson prosthesis

Objective To evaluate the short-term results of total metatarsophalangeal arthroplasty (TMPA) for OA, RA, hallux valgus with severe OA, hallux rigidus, and osteonecrosis of metatarsal heads. Methods From March 2002 to March 2005, total metatarsophalangeal arthroplasty was performed in 30 patients (11 males and 19 females, age from 33 to 77 years old, average 62.5 years old) and 47 feet. Fifteen were bilateral. The preoperative underlying diagnosis was RA in 7, hallux rigidus in 5, hallux valgus with severe OA in 21, avascular osteonecrosis in second metatarsal head in 6, old metatarsophalangeal dislocation in 2, complications after McBride operation in 2 feet. Totally, 33 first metatarsoplangeal joints, 11 second metatarsophalangeal joints, 2 third metatarsophalangeal joints and 1 fourth metatarsophalangeal joint were replaced with metal bushed silicon Swanson prostheses. Two patients (4 feet) with abnormal first-second intermetetarsal angle (19?-20?) received simultaneous proximal metatarsal osteotomy. The osteotomed metatarsal were fixed with Biofix absorbable screws. Two patients (3 feet) received simultaneous metatarsal head (2-5) resection arthroplasty. Twenty-nine (46 feet) were followed-up for average 26.5 (3-36) months with Maryland metatarsal joint scoring system. Oriented by the Maryland scoring system, pain, joint function, and objective messurements were recorded and scored. Results The preoperative and postoperatvie Maryland score was 72 (55-82) and 91 (67-96), respectively. Excellent result was achieved in 41 feet (89.13%), good in 2 feet(4.35%), fair in 2 feet(4.35%), and poor in 1 foot(2.17%). The good-excellent result rate in this group was 93.48%. The complications included: delayed wound union in 2 feet, implant dislocation in 1 foot, reactive synovitis in 1 foot. Conclusion The short-term result of total metatarsophalangeal arthroplasty for treatment of OA, RA, hallux rigidus, old dislocation of metatarsophalangeal and avascular osteonecrosis of metatarsal heads is satisfying.

Purpose:To investigate the expression of VEGF and its prognostic significance in ampullary carcinoma.Methods:22 resected tumor specimens from patients with ampullary carcinoma were immunohistochemically stained for VEGF and CD34 (surrogate for vessels) by streptavidin-peroxidase (S-P) method. Results:Expression of VEGF in tumor tissue was found in 50% cases of patients with ampullary carcinoma. The mean MVD for the entire group was 26.4?12.8. 4.Result of modified Mitchell operation for treatment of hallux valgus

Objective To evaluated the result of modified Mitchell operation for treatment of hallux valgus. Methods From March 1998 to October 2001, 39 feet of 20 patients were treated with modified Mitchell procedure. Before operation, HVA averaged 29.5? and IMA averaged 12.5? were measured on standing AP radiographs. The lateral displacement of the capital fragment was depended on the amount of IMA and the capital fragment shifted plantarly 2-3 mm. Fixation was achieved with an absorbable screw. The lateral spike on the distal fragment was not preserved in this operation. Results After follow-up of 11-38 months, patients rated as excellent or good in 97.4%. The average postoperative HVA was 14.5? and IMA was 8.5?, the average HAV correction was 15? and 4? for the IMA. Conclusion The modified Mitchell procedure provides satisfactory deformity correction. The transverse arch of metatarsals is reconstructed since distal part of the first metatarsal is shifted plantarly in this operation.
5.Metabolic changes of osteoarthritis cartilage reflected by special cartilage markers

Objective To investigate the relationship between the levels of proteoglycan and collagen type Ⅱ in serum and severity of knee osteoarthritis, and to determine whether the two markers could reflect the metabolic changes of cartilage after different knee operations. Methods Serum samples and roentgenographic data of 65 patients with knee osteoarthritis and 22 normal persons in controls were collected. Three different operations were performed for 45 patients respectively. These patients were followed up for half a year. The serum levels of proteoglycan and collagen type Ⅱ epitope were analysed using an enzyme-linked immunosorbent assay(ELISA). Results Epitope levels of proteoglycan and collagen type Ⅱ in serum of osteoarthritis patients were higher than those in control group. The serum levels of both markers in the group that the roentgenograph show minor narrowing of joint space were highest. 8.Proteomic identification of chemosensitivity-associated proteins in human colorectal carcinomas

Objective:The study aims to screen chemosensitivity-associated proteins in colorectal carcinoma tissues by using two-dimensional gel electrophoresis (2-DE) and mass spectrometry,then identify some differentially-expressed proteins. Methods:The patients with advanced colorectal carcinoma were confirmed by clinical diagnosis. Fresh carcinoma specimens were collected by biopsy and preserved in liquid N2. The tissues were classified into two groups: high sensitivity group (HS) and low sensitivity group (LS) based on drug sensitivity test. The total proteins were extracted and separated by 2-DE. The images were composed, compared, and differentially analyzed to identify the proteins with differential expression in HS and LS groups. Then the differentially-expressed protein spots were incised from the gels and digested by trypsin. The peptide mass fingerprintings (PMF) was acquired after matrix assisted laser desorption/ionization time of flight mass spectrometry (MALDI-TOF-MS) and the proteins were identified by data searching in the Mascot database. Two proteins with differential expression were detected by Western blotting.Results:The 2-DE spectrum of HS and LS groups were established. Most protein spots were distributed in the area with pH 4-8 and relative molecular weight of (20-100)×10~3. The average number of the protein spots was 842±23 in HS group and 793±19 in LS group,respectively. The mean matching rate was 90.7%. The number of differentially-expressed dots between HS and LS group was 79.00±13.56. Thirty protein dots were selected for mass spectrum and bioinformatic analysis, and 9 proteins were identified. Conclusion:Colorectal carcinoma with different chemosensitivity had differential protein expression profiles. The differentially expressed proteins may be associated with chemosensitivity and could be used for prediction of chemosensitivity of colorectal carcinoma.
9.Reversion of multidrug resistance of hepatoma cell line SMMC-7721/ADM by adriamycin-loaded immuno-nanoparticles

Objective To explore the effects of adriamycin-loaded immuno-nanoparticles on multidrug resistance (MDR) of hepatoma cell line SMMC-7721/ADM. Methods The cytotoxicity of the adriamycin-loaded immuno-nanoparticles on the bepatoma cell line SMMC-7721/ADM in vitro and the tumor cell-binding ability of adriamycin-loaded immuno-nanoparticles were detected. Results The effect of the cytotoxicity of adriamycin-loaded immuno-nanoparticles on the hepatoma cell line SMMC-7721/ADM was significantly better than that of adriamycin-loaded nanoparticles. Adriamycin-loaded immuno-nanoparticles had the specific binding ability with the hepatoma cell line SMMC-7721/ADM. Conclusions Adriamycin-loaded immuno-nanoparticles can overcome the MDR of the tumor in vitro. The mechanism may be that immuno-nanoparticles could adhere to the tumor cell membrane, and the release of the loaded adriamycin creates a high local concentration in the extracellular medium. The increased concentration gradient improves the diffusion of adriamycin from the extracellular medium to the intracellular medium.
10.Expression and significance of VEGF-D and MMP-2 in pancreatic carcinoma

Objective To investigate the expression of VEGF-D and MMP-2 and explore their relationship with tumor invasion and local metastasis in pancreatic carcinoma.Methods The expression of VEGF-D and MMP-2 in 30 patients with pancreatic carcinomas and 6 cases with normal pancreatic tissues were detected by SABC immunohistochemical method.Results The positive rates of VEGF-D and MMP-2 proteins were 60%(18/30)and 66.7%(20/30)respectively,no expression of VEGF-D and MMP-2 was detected in normal pancreatie tissues.The expression of VEGF-D was significantly correlated with lymph node metastasis(P<0.05),and the expression of MMP-2 was significantly correlated with lymph node metastasis and tumor local invasion(P<0.05).The expression of VEGF-D protein Was positively correlated with MMP-2 protein(rs=0.54,P<0.01).Conclusions There is a significantly positive correlation between high expression of VEGF-D and MMP-2 proteins in pancreatic carcinoma,and they may contribute to the invasive properties of pancreatic cancer.
