1.Correlation of Adiponectin and Chronic Obstructive Pulmonary Disease

Objective To study the relationship between adiponectin and pulmonary function, age and gender in pa-tients with chronic obstructive pulmonary disease (COPD). Methods A total of 120 COPD patients with normal body mass index (BMI<24 kg/m2 ) were selected and divided into acute exacerbation group (n=60) and stable group (n=60). And 60 healthy people were included as controls. There were equal numbers of male and female in each group. The serum level of ad-iponectin was sdetected by ELISA in there groups. The pulmonary function was collected,and values of FEV1/FVC, FEV1%predicted and RV/TLC were calculated in three groups. Results The serum level of adiponectin was significantly higher in acute exacerbation group than that of stable group and control group (P<0.05). And the serum level of adiponectin was significantly higher in stable group than that of control group (P<0.05). The serum levels of adiponectin were signifi-cantly higher in female patients than those of male patients (P<0.05). The values of FEV1%, FEV1/FVC and FEV1%pre-dicted were significantly lower in acute exacerbation group and stable group than those of control group. The value of RV/TLC was significantly higher in acute exacerbation group and stable group than that of control group (P<0.05). There was a negative correlation in serum levels of adiponectin, FEV1%predicted and FEV1/FVC between acute exacerbation group and stable group. But there was a negative correlation in serum levels of adiponectin and RV/TLC between these two groups ( P<0.01). There was a positive correlation between serum level of adiponectin and age in male patients (r=0.943,P<0.01). Con-clusion The serum level of adiponectin was significantly increased in patients with COPD, which suggested that adiponec-tin played an important part in proinflammatory and might be closely related with airway resistance. The serum level of adipo-nectin might exist gender differences and may be related with age.
2.Distribution of drug resistance mutations and genotypes in patients with chronic hepatitis B in Taicang city

Objective To study the distribution of drug resistance mutations and genotypes in the patients with chronic hepatitis B(CHB)in Taicang area.Methods 350 cases of CHB were selected and detect the drug resistance mutations and genotypes before using nucleoside(acid)anti-viral drugs.The correlation between the drug resistance mutations with genotypes in CHB patients was analyzed.Results Among 350 cases of CHB,the distribution of genotypes was genotype B in 172 cases,accounting for 49.14%, genotype C in 164 cases,accounting for 46.85%,genotype D in 9 cases,accounting for 2.57%,mixed genotype of B and C in 5 ca-ses,accounting for 1.42%,no other genotypes were found.In this study,the drug resistance mutations were detected in 58 cases,in-cluding 22 cases of genotype B,35 cases of genotype C and 1 case of genotype D,no drug resistance mutations were found in mixed genotype B and C.The drug resistance mutations sites:lamivudine resistance-associated mutations in 36 cases(10.28%),and adefo-vir resistance-associated mutations in 16 cases(4.57%)and entecavir associated resistance mutations in 6 cases(1.71%).Conclusion The genotype of CHB patients in Taicang area was dominated by genotype B and C,which accounting for more than 90% and the proportion of these two kinds of genotype is similar.The proportion of drug resistance mutation occurrence in the patients with gen-otype C is higher than that with genotype B.The lamivudine resistance-associated mutation has the highest proportion of drug re-sistance occurrence.It is suggested that the CHB patients with undefined medication history and recurrence treatment should be per-formed the drug resistance detection for selecting the targeted therapeutic schemes before accepting nucleoside(acid)drug therapy.

It is a promising method to use Liposomes as a carrier in the transdermal delivery system.Presently,llposomes,deformable liposomes,and ethosomes are in common use.The transdermal mechanism of the liposomes is to promote the velocity and the amount of the drugs of transdermal absorbance by affecting the keratoderma,acting on the skin appendages and changing the exterior characteristic of the drugs to make them permeate the skins.The Mechanism of the actions includes hydration.penetration,transforamation,and osmotic pressure driving effect,etc.The liposomes have been approved to have abroad prospect of application in the transdermal delivery system.

5.Research progress of single Chinese exterior-relieving medicine in the treatment of osteoporosis

In recent years, pharmacological researches of Chinese medicine have found that single Chinese exterior-relieving medicines such as Pueraria, Semen Sojae Praeparatum and Cimicifuga had very good anti-osteoporosis effect. They can significantly improve the bone mineral density (BMD) and bone mineral content(BMC), promote bone formation, and prevent bone absorption. Based on relative documents from home and abroad, this paper analyzed the current situation and provided new ideas and methods for prevention and treatment of osteoporosis.
6.Peptide Induced Conformational Changes of E. Coli DegP (HtrA) Protease

The DegP protein, functioning as both chaperone and protease, plays a critical role in degrading and removing denatured or damaged proteins in the cellular envelope during heat shock and other stresses. So far, several proteins have been identified as its natural targets. A carboxyle-terminal peptide derived from the PapG pilus, one of the in vivo substrates for DegP, has been shown to activate the protease. Nevertheless, neither the details nor the physiological implications of such activation have been studied. The evidence that DegP undergoes conformational changes upon binding the peptide derived from C-terminal sequence of pilus subunit PapG has been presented. It demonstrated that upon binding this peptide, detectable changes can be observed for both secondary and tertiary structures of DegP, as examined by CD spectroscopy. Gel filtration and dynamic light scattering analysis also revealed that the size of DegP becomes smaller to a minor extent. Moreover, both the hydrophobic surfaces and catalytic sites of DegP were found to expose slightly in the presence of the peptide. Upon peptide binding, a less flexible and more rigid conformation of DegP was obtained as analyzed by fluorescence anisotropy. The physiological implications of these observations for DegP are discussed.
7.The effect of femoral component design on anterior knee pain after patella reserving total knee arthroplasty

Objective To investigate the effect of femoral design on anterior knee pain after patella reserving total knee arthroplasty. Methods From 15 January to 15 February 2002, 44 patients and 59 knees were followed up. Among these patients, 40 patients and 51 knees were included in this study, the rest cases were excluded for one or more of following factors. 1)Knee flexion was less than 80 degrees;2)Radiography showed that there were surgical errors such as: femoral component flexion more than 30, anterior femoral cortex notching, and patellar baja or alta. According to current generally accepted criteria, femoral components were classified as "Patella Friendly" group and "Patella Unfriendly" group. The percentage and severity of anterior knee pain was measured with patient visual pain index and Knee Society Functional Score System. Results The aforementioned two groups consisted of "Patella Friendly" group 21 knees and "Patella Unfriendly" group 31 knees. The average anterior knee pain score of "Patella Friendly" group and "Patella Unfriendly" group was respectively 1.58 (0-8) and 3.32 (0-6). Objective To research the effect of Xiaoguruanjian pill on liver cirrhosis ascites patients and the regulation on blood NO and ET-1. Methods 133 liver cirrhosis ascites were divided into two groups randomly. 45 cases of the control group were treated with routine western medicine according to the patients state of illness, 88 cases of the treatment group were treated with Chinese herbal compounds Xiaoguruanjian Pill on the basis of routine western medicine. The total effective rate, liver function test, ultrasonogram of liver and spleen, the contents of blood NO and ET-1 of two groups were detected before and after the treatment, and the effect on blood NO and ET-1 of patients with SBP and renal dysfunction were observed too. 9.Venous thrombosis following laparoscopic cholecystectomy: Analysis of 12 cases

Objective To investigate the etiology and prevention of venous thrombosis (VT) following laparoscopic cholecystectomy (LC). Methods A retrospective analysis on clinical records of 12 cases of VT following 258 cases of LC in this hospital from January 2002 to June 2004 was carried out. Results [WTBZ]The operation time was 17~58 min (mean, 38 min), and the intraoperative blood loss was 15~100 ml (mean, 45 ml). In 11 cases of superficial vein thrombosis, on a treatment for 6~8 days the symptoms of redness, swelling, heat, and pain vanished. Re-examinations of color Doppler ultrasonography revealed disappearance of thrombosis, and an anticoagulation therapy with Warfarin was given. Follow-ups for 6~12 months found no recurrence. In 1 case of deep femoral vein thrombosis accompanying pulmonary embolism, an anticoagulation therapy was employed for 4 months and a follow-up for 6 months showed no recurrence. Conclusions The development of VT following LC is correlated to patients' high-risk factors, carbon dioxide pneumoperitoneum, and anesthesia administration. Emphasis should be on active countermeasures during peri-operative period.
10.The coupling of tibial rotation and extension-flexion motion of knee joint

Objective To investigate the coupling of tibial rotation and extension-flexion motion of knee joint both before and after total knee arthroplasty. Methods Six fresh-frozen left lower limbs from cadavers were tested using a modified Oxford motor rig for the knee joint. Anatomically based multiple loadings of the quadriceps mechanism were applied (Vastus medialis: 24.5 N, Vastus intermedius/rectus femoris: 29.4 N, Vastus lateralis: 19.6 N). The tibial rotation was tracked using a custom-made laser tracking device while the tibial-femoral flexion was measured with goniometer. Total knee arthroplasty (TKA) was performed using ?PFC-PS posterior cruciate ligament substituting knee system. Results Before TKA, when the knee flexed 10?, 20?, 30?, 60?, 90?, 120? and 150?, tibia rotated 5.1??2.3?, 9.3??3.3?, 10.2??4.8?,11.7??3.5?, 13.5??8.4?, 22.0??8.6? and 29.6??9.3? respectively. After TKA, tibia rotated -2.2??1.7?, -2.5??1.5?, -2.0??1.3?, 1.7??2.5?, 3.9??2.4?, 13.5??6.1? and 18.6??4.7? respectively (Tibial rotation was defined as 0? in knee extension; internal rotation as positive and external rotation as negative). Conclusion The tibial internal rotation is coincident with tibial-femoral flexion, its magnitude of tibial rotation varies in different degrees of knee flexion. The tibial internal rotatation is significant in early knee flexion(0? to 30?), then enters into a platform stage on knee bending from 30? to 90?, Once the knee bends over 90?, the tibia rotates internally markedly again, maximum internal rotation is achieved in fully flexion, probably due to relaxation of soft tissue. After TKA, in early flexion (0? to 30?) the external rotation occurs abnormally, but later on 30? to 150?, it rotates again intenally as normal.
