Objective To compare the pass rates of two primary hearing screening times and to determine the appropriate screening time.Methods The newborns born in 2004 and 2005 at our hospital were divided into two groups: group A and group B.Group A received hearing screening with distortion product otoacoustic emission(DPOAE) on the 3rd day after birth,while group B screened in one month after birth.The newborns who failed the initial screening were rescreened one month later.The babies with positive screening findings were referred to full auditory assessments.Results Group A(n=2 305) had pass rate 81.9% at the initial screening and 85.2% at rescreening.Group B(n=1 348) had pass rate 93.9%(?2=103.99,P

Objective To explore the action mechanism of basic fibroblast growth factor (bFGF) on vascular smooth muscle cells (VSMCs) in spontaneously hypertensive rats (SHR).Methods Ts were obtained from SHR and SD rats.The aortic VSMCs were cultured in vitro by tissue explant method.VSMCs were treated with different concentration of exogenous bFGF (0 ng/ml,20 ng/ml,40 ng/ml,60 ng/ml,80 ng/ml,100 ng/ml) for 48 h,then cell proliferation was detected by 3 (4,5 dimethylthiazol)2,5 diphenyltetrazolium bromide (MTT) colorimetric assay.VSMCs from SHR in control group were treated with bFGF (100 ng/ml) for 48h.VSMCs from SHR in treatment group were treated with bFGF (100 ng/ml) plus Proteinkinase C(PKC) inhibitor (staurosporine) for 48 h.Results After treatment with different concentration (0 ng/ml,20 ng/ml,40 ng/ml,60 ng/ml,80 ng/ml,100 ng/ml) of bFGF for 48 h,the values measured by MTT colorimetric method were 0.402 ± 0.103,0.605 ±0.090,0.696 ± 0.131,0.812 ± 0.080,0.901 ± 0.065,1.056±0.078 respectively in aortic VSMCs from SD rats,and 0.404±0.065,0.507±0.078,0.608±0.057,0.704 ± 0.107,0.812 ± 0.097,0.908 ± 0.032 respectively in aortic VSMCs from SHR.Compared with control group,the values measured by MTT colorimetric method were decreased in treatment group (P<0.05).The proliferative effect of bFGF in aortic VSMCs from SHR was attenuated after administration of PKC inhibitor staurosporine.Conclusions Exogenous bFGF administration promotes VSMCs proliferation in SHR and SD rats in a concentration-dependent manner.PKC plays an important role in the signal transduction mechanism in VSMCs proliferation by exogenous bFGF.
5.The relationships among organization climate, psychological empowerment and job embeddedness

Objective To explore the relationships among organization climate,psychological empowerment and job embeddedness by using path analysis.Methods A total of 514 clinical nurses from 26 departments in 3 hospitals were recruited by convenience cluster sampling method and investigated with demography questionnaire,Nurses Organizational Climate Scale,Psychological Empowerment Scale and Job Embeddedness Scale.Results The mean scores of organization climate,psychological empowerment and job embeddedness was (3.01±0.46),(3.27±0.44),(3.08±0.39).The predictors of job embeddedness of nurses were resources support,work experience,human resources management,marital status,job title,management support,quality management,and self-egicacy,explained 48.9% of its variance; organization climate,psychological empowerment had direct positive influence on nurses' job embeddedness,explained 33.3% of its variance.Conclusions Improvement and maintenance of sound organizational climate,increase the sense of psychological empowerment of nurses,are effective ways and means to increase the degree of job embedding of nurses.
6.Influence of narrative therapy on the family environment and values in adolescents with emotional disorder

Objective To explore the effect of narrative therapy on family environment and values in adolescents with emotional disorder.Methods 35 adolescents with emotional disorder and their parents from Affiliated Brain Hospital,Nanjing Medical University were enrolled and treated by narrative therapy.Family environment was assessed using Family Environment Scale-Chinese Version (FES-CV)and was compared between pre-and posttherapy.Results 28 adolescents with emotional disorder and their parents completed therapy.The cohesion,independence,intellectual orientation,active-recreation orientation and organization in FES-CV showed higher scores at post-therapy than at pre-therapy ((6.89 ± 2.56) vs.(4.21 ± 3.21) ; (5.54 ± 1.35) vs.(4.68 ± 1.51) ;(4.75±2.07)vs.(3.50±2.12);(4.07±2.65)vs.(2.50±2.32);(3.68±1.77)vs.(2.50±1.62)respectively).The contradict in FES-CV showed lower scores at post-therapy than at pre-therapy ((2.75 ± 2.44) vs (5.25 ± 2.50)).The values of adolescents with emotional disorder and their parents had a positive change after narrative therapy.Conclusion Narrative therapy is effective on the positive change of FES-CV and values of adolescents with emotional disorder and their parents.

Objective To study the effect of praziquantel on cells within sehistosomal ovum granuloma in the lung of mice.Methods Forty-eight mice were divided into 4 groups.Group A:first,the mice were injected with sehistosomal ova hypodermically in abdomen,and 10 days later,injected with schistosomal ova intravenously in the cauda;Group B:in addition to the injection of schistosomal ova as the same of Group A,the mice were administered with praziquantel[300 mg/(kg·d)]for 3 days,one day before the intravenous injection of the ova;Group C:in addition to the injection of schistosomal ova as the same of Group A,the mice were administered with praziquantel(75 mg/kg,B.i.d.) for 5 days weekly until the mice were sacrificed;Group D:the same as Group C but praziquantel was given to the mice from the 29th after the intravenous injection of the ova.Three mice of each group were sacrificed on the 7th,14th,28th,56th day after the intravenous injection of the ova and the lung tissues were fixed with formalin and the slices were HE stained.Twenty-five to thirty pieces of schistosomal ovum granuloma were examined and the neutrophilic granulocytes,eosinocytes,lymphocytes,fibroblasts and macrophages within the ovum granulomas were counted and the mean numbers of them of each group were calculated and compared.Results Compared with Group A,the mean numbers of neutrophilic granulocytes,eosinocytes and macrophages within the ovum granulomas were decreased significantly,and the extend of the increase of fibroblasts reduced significantly in the three groups administered with praziquantel,and especially in Group C.On the 56th day after the intravenous injection of the ova,the mean numbers of neutrophilic granulocytes,eosinocytes and macrophages decreased by 54.4%、87.0% and 23.1%,and the extend of the increase of fibroblasts reduce by 59.4%,respectively in Group C,compared with Group A.The numbers of lymphocytes did not change very much in 4 groups.Conclusion Praziquantel can restrain inflammatory cells and the hyperplasia of fibroblasts within schistosomal ovum granulomas.
8.Effects of inhaled budesonide on airway inflammation,reticular basement membrane thickness and bronchial hyperresponsiveness in asthma

Objective To study the relationships between airway inflammation,reticular basement membrane (RBM) thickness and bronchial hyperresponsiveness (BHR) before and after treatment with inhaled corticosteroids (ICS) in patients with asthma.Methods Twenty-eight patients with asthma (asthma group) were treated with inhaled budesonide (200 μg,twice a day for 12 months).Lung function,bronchoalveolar lavage (BAL) fluid and airway biopsy pathology were carried out before and after treatment.And 10 patients without asthma were acted as control group.Results BAL fluid eosinophils,epithelial cell and mast cell proportions before treatment in asthma group were significantly higher than those in control group [asthma group:(1.60 ±0.11)%,(2.50 ±0.29)%,(0.1100 ±0.0100)% ;control group:(0.50 ±0.02)%,(1.20 ±0.08)%,(0.0200 ±0.0001)%,P <0.01].RBM and smooth muscle thickness were increased in asthma group than those in control group [ asthma group:( 14.25 ± 2.15 ),(0.36 ± 0.01 ) μ m;control group:(4.45 ± 1.22),(0.20 ± 0.01 ) μ m,P < 0.01 ].BAL fluid eosinophils,epithelial cell and mast cell proportions,RBM and smooth muscle thickness decreased after treatment in asthma group [ (0.80 ±0.04)%,(1.60 ±0.07)%,(0.0500 ±0.0020)%,(6.18 ± 1.30) μm,(0.26 ±0.02) μm,P <0.01].RBM thickness was negative correlation with methacholine provoking dose which made forced expired volume in one second reduce by 20% (r =-0.672,P < 0.01 ).Conclusions Airway inflammation,RBM thickness and BHR in patients with asthma are interrelated and improved with ICS.The results of this study support the need for early and long treatment with ICS.
9.Comparison of dose distribution between hypofractionated IMRT and SRT plans in lung tumor

Objective To compare the characteristics of dose distribution between hypofractionated intensity modulated radiotherapy (IMRT) and hypofractionated stereotactic radiotherapy (SRT) plans in lung tumor and to select an optimal clinical approach. Methods SRT plans were designed for 16 patients with lung tumors who had received IMRT between April 2007 and April 2008. The dose distribution of target volume and normal tissues, conformal index (CI) and heteregenous index (HI) were analyzed using the dose-volume histogram (DVH) for the IMRT and SRT plans. Results The mean dose and equivalent uni-form dose of planning target volume (PTV) in IMRT were similar to those in SRT. SRT had significantly better CI and HI than IMRT (t = 2.77, P < 0.05 and t = - 4.38, P < 0.01 ). The mean lung dose of IMRT and SRT was (492.4 ±368.5) cGy and ( 310.0 ± 73.1 ) cGy, respectively ( t = 1.68, P > 0.05 ). The lung V20 of IMRT and SRT was 6.9% ± 2.1% and 4.2%± 1.9%, respectively ( t = 3.30, P < 0.01 ). No sig-nificant differences were found in the mean dose to the heart or the spinal cord between IMRT and SRT. Conclusions When PTV is less than 57 cm3 or the long diameter of tumor is less than 4.7 cm, hypofrac-tionated SRT has similar dose distribution to hypofractionated IMRT, while the lung dose was lower in the former.
10.Clinical application of rotating platform high flexion total knee replacements

Objective To discuss the characteristics and clinical results of rotating platform high flexion(RDHF)total knee replacement TKR. Methods A total of 61 patients(78 knees)including 13males(15 knees)and 48 females(63 knees)at age range of45-79 years(mean 64.3 years)were treated with Sigma(RP-HF)rotating platform high flexion total knee replacement.Osteoarthritis was found in 56patients and rheumatoid arthritis in 5.By medial parapatella approach,denervation and facet repair of the patella were done,without replacing them. Results All 78 knees were followed up for 3-16 months (mean 10.5 months).the X-ray film demonstrated no prosthesis loosening,patella dislocation or subluxation.According to the knee society scoring system,the preoperative score was 48 points(33-72 points)and the knee functional score was 41 points(28-63 points)on average.After surgery,the knee score was 97points(88-100 points),the functional score 96 points(84-99 points),and the range of motion(ROM)130.(110.-150.)on average.No spin-out or infection were detected. Conclusions The Sigma RP-HF knee system provides sufficient contact area.full range of patella locus and sufficient axial rotation,which helps reach a stable and durable high flexion performance.Our short-term clinical follow up result is good but the advantages of low wear and longevity of the Sigma RP-HF prosthesis still need further study.
