It is a promising method to use Liposomes as a carrier in the transdermal delivery system.Presently,llposomes,deformable liposomes,and ethosomes are in common use.The transdermal mechanism of the liposomes is to promote the velocity and the amount of the drugs of transdermal absorbance by affecting the keratoderma,acting on the skin appendages and changing the exterior characteristic of the drugs to make them permeate the skins.The Mechanism of the actions includes hydration.penetration,transforamation,and osmotic pressure driving effect,etc.The liposomes have been approved to have abroad prospect of application in the transdermal delivery system.

3.Therapeutic Observation of Surround Needling plusHigh-energy Red Light for Herpes Zoster

ObjectiveTo observethe clinical efficacy of surround needling plus high-energy red light in treating herpes zoster. MethodSixty patients with herpes zoster were randomized into a medication group, a control group and a united group. The medication group was intervened by oral administration of medication, the control group was by surround needling plus electroacupuncture in addition to the medication, and the united group was by the medication, surround needling plus electroacupuncture, and the high-energy red light radiation. The effect on pain and the comprehensive therapeutic efficacy were evaluated on treatment day 5 and day 10.ResultThe efficacies in releasing pain in the control group and united group were significantly superior to that in the medication group on treatment day 5 (P<0.05), while the difference between the control group and united group was statistically insignificant (P>0.05); on treatment day 10, the effects on releasing pain in the control group and united group were more significant than that in the medication group (P<0.01), and the effect in the united group was more significant than that in the control group (P<0.05). The comprehensive therapeutic efficacy in the united group was superior to that in the medication group on treatment day 5 (P<0.01); the difference between the control group and medication group was statistically insignificant (P>0.05); on treatment day 10, the efficacies in the control group and united group were significantly better than that in the medication group (P<0.01), and the efficacy in the united group was superior to that in the control group (P<0.05). ConclusionSurround needling plus high-energy red light can effectively reduce the neuralgia caused by herpes zoster, shorten the pain duration, improve the symptoms, andthus this combination is an effective and safe method.
4.The research of pelvic floor ultrasound in diagnosis of stress urinary incontinence

Objective To investigate the diagnostic parameters,criteria and diagnostic value of pelvic floor ultrasound in female stress urinary incontinence(SUI).Methods Simple factor logistic regression analysis was used to compare the difference of ultrasonic parameters between SUI patients(260 cases) and asymptomatic subjects(60 cases) to find the relevant diagnostic indexes,and to evaluate the diagnostic criteria and diagnostic value by the ROC curve.Results There were significant differences in urethral inclination angle and levator hiatus area in resting and bladder neck position,bladder position,urethral inclination angle,retrovesical angle,levator hiatus area in Valsalva state and urethral rotation angle,bladder neck mobility between the two groups (all P < 0.05).There was no significant difference in age,BMI,bladder neck position,bladder position,retrovesical angle between resting in the two groups (all P >0.05).Using the ROC curve analysis,the cut-off points of urethral inclination angle and levator hiatus area in resting,bladder neck and bladder position,urethral inclination angle,retrovesical angle,levator hiatus area in Valsalva,bladder neck mobility and urethra rotation angle to diagnose SUI were 16.5°,13.5 cm2,3.5 mm,0.5 mm,29.5°,139.5°,19.5 cm2,24.5 mm,45.5°,respectively.The sensitivity/specificity were 54.6%/66.7%,49.2%/80.0%,68.1%/95.0%,64.2%/98.3%,67.3%/93.3%,73.5%/50.0%,68.8%/81.7%,70.0%/95.0%,67.2%/85.0%,respectively.The area under the curve were 0.625,0.668,0.855,0.854,0.817,0.622,0.811,0.866,0.817,respectively.Conclusions Pelvic floor ultrasound is a better way to diagnose stress urinary incontinence,and it provides an objective basis for the diagnosis of SUI.
5.Determination of galanthamine in bulb of Lycoris radiata by RP-HPLC.

OBJECTIVETo establish a method for determination of galanthamine in Lycoris radiata.
METHODHPLC separation was carried on a column of ODS (4.6 mm x 150 mm, 5 microm), with the mobile phase of phosphate buffer (pH = 3-4)-methanol (93:7) at the flow rate of 1.0 mL x min(-1), and the detection wavelength was set at 289 nm.
RESULTGalanthamine revealed linearity within the range of 3-30 microg x mL(-1) (r = 0.9997), the detection limit was 0.3 ng. The average recovery was 99.5% (RSD = 0.5%).
CONCLUSIONThe method is easy to operate and the results of the determination are accurate, it can be used to evaluate the quality of L. radiata.

6.Combined transcatheter arterial chemoembolization and beta-ultrasound guided portal vein embolization in the treatment of hepatocellular carcinoma.

OBJECTIVETo study the effect of transcatheter arterial chemoembolization (TACE) combined with beta-ultrasound guided portal vein embolization (PVE) through fine-needle liver puncture for hepatocellular carcinoma.
METHODS209 patients with primary hepatocellular carcinoma were divided into TACE group (104 patients) and TACE + PVE group (105 patients).
RESULTSThe response rates (CR + PR) were 37.5% in TACE group and 57.2% in TACE + PVE group (P < 0.01). Tumor thrombi became lessened or resolved in the portal vein with incidences of 22.2% in TACE group and 68.8% in TACE + PVE group (P < 0.01). The 1-, 2- and 3-year survival rates were 65.1%, 36.3% and 20.5% in TACE group and 95.6%, 59.6% and 39.1% in TACE + PVE group (P < 0.05).
CONCLUSIONThe effect of TACE combined with PVE is much more effective than TACE alone for hepatocellular carcinoma. Beta-ultrasound guided portal vein embolization through fine-needle liver puncture is effective, easy, safe and should be widely practiced.

7. Study on the status of fear of cancer recurrence and its correlation with quality of life in patients with bladder cancer

Objective:
To investigate the current status of fear of cancer recurrence in bladder cancer patients, and to explore the correlation between fear of recurrence and quality of life.
Methods:
A total of 96 bladder cancer patients were recruited by convenience sampling method from April 2017 to July 2017. They were investigated through general information questionnaire, Chinese version of Fear of Progression Questionnaire Short Form and Functional Assessment of Cancer Therapy-Bladder cancer form respectively.
Results:
The total score of fears of recurrence was 28.76±10.33, the total score of quality of life was 123.66±20.63. A total of 28 patients had a dysfunctional level of fears of recurrence (total score ≥34), the incidence was 29.2% (28/96). There were significant differences in the total scores of fears of recurrence at different ages, fears of recurrence were not related to another sociodemographic factors (gender, educational attainment and income) or disease, treatment related factors. 8. Application of profunda artery perforator flap in breast reconstruction

Objective:
To explore the clinical application of the profunda artery perforator flap in breast reconstruction of patients with early-staged breast cancer.
Methods:
Six cases of early breast cancer patients received radical or modified radicaI surgery.The PAP flap was used to reconstruct breast at the same time or in the second phase.Four cases of breast cancer were in stage I and 2 cases was in stage II.
Results:
The length of flap was (25.2±0.2) cm, the width of flap was (6.4±0.8) cm, the thickness of flap was (2.6±0.2) cm. The length of pedicle was (7.2±0.6) cm, the outer diameter of pedicle was (1.6±0.3) mm.The average weight of flap was 345 g(ranged from 225 g to 575 g). All profunda artery perforator flaps were successful. In one flap fat necrosis occurred and healed with dressing treatment. The reconstructed breasts′ shape, texture and elasticity was good and no flap contracture deformation happened. Only linear scar left in the donor sites, the function of thighs was not affected. All 6 patients were followed up for 2-3 months (2.5 months on average) with satisfied result. No local recurrence happened.
Conclusions
PAP flap is suitable to reconstruct breast for breast cancer patients.
9.Relaying anterolateral thigh (ALT) perforator flap in resurfacing of the donor defect after anteromedial thigh (AMT) perforator flap transfer

Objective To introduce the clinical experience of relaying anterolateral thigh (ALT)flap in the resurfacing of the donor defect after anteromedial thigh (AMT) flap transfer.Methods From February 2014 to December 2015,16 cases with oral carcinoma underwent radical resection,leaving tongue or mouth floor defects which were reconstructed by AMT perforator flaps.The flap size ranged from 7.5 cm ×4.5 cm to 13.0 cm × 7.5 cm [the patients was 45.6 years (range 31-72 years),body mass index (BMI) range 17.5-24.3 kg/m2].Flaps'width was on average 6.6 cm (ranging from 5 to 9 cm) with flap width-to-thigh circumference ratio being 12.5% on average (ranging from 9.8% to 15.7%).The flap donor sites were reconstructed with relaying ALT flap at the same stage,the flap size ranged from 7.5 cm × 4.0 cm to 12.0 cm x 7.0 cm.Results The AMT and ALT perforators existed consistently in all cases of this serie.All free AMT flaps and relaying ALT flaps survived uneventfully.All patients were followed up for 8-24 months with satisfied esthetic and functional results in recipient and donor sites.2-point discrimination distance of AMT flaps ranged from 7 to 14 mm,2-point discrimination distance of relaying ALT flaps ranged from 8 to 15 mm,the function of thighs were not affected.Conclusions The relaying ALT flap is an ideal choice to reconstruct the donor site of free AMT flap.
10.Clinical value of Aβ1-42, tau protein and thyroid hormone levels in predicting cognitive impairment after ischemic stroke.

Objective To investigate the relationship between serum markers β amyloid (Aβ), tau and thyroid hormone levels and post-stroke cognitive impairment (PSCI) in the acute phase of cerebral infarction. Methods A total of 214 patients with acute cerebral infarction were enrolled. The baseline data and serological indicators were collected and the cognitive function of patients was evaluated. All patients were divided into cognitive impairment group and normal group based on follow-up results. The differences of Aβ1-42, tau protein and thyroxine levels between the two groups and their relationship with disease progression were analyzed. The Cox regression analysis and ROC curve were used to compare the above parameters to predict the development of PSCI. Results The total protein level of Tau (210.6 ±98.9 pg/mL) was higher and Aβ1-42 (426.1 ±123.5 pg/mL) and triiodothyronine (T3) (1.43 ±0.57 nmol/L), free thyroxine (FT4) (13.15±2.23 pmol/L) was significantly lower in the cognitive impairment group than in the normal group (P<0.05). Tau protein (r=-0.457), Aβ1-42 (r=0.348), T3 (r=0.211), and FT4 (r=0.306) were all associated with disease progression (P<0.05). Cox regression analysis showed that Aβ1-42 and T3 were important influencing factors in the occurrence of PSCI. The area under the curve of Aβ1-42 combined with T3 was 0.841. The specificity and the sensitivity were 74.8% and 85.3%, respectively, with a diagnostic cutoff value of 0.572. Conclusion Aβ1-42 and T3 levels in the acute phase of cerebral infarction may predict the progression of PSCI.
