Objective:To evaluate the clinical use rationality of edaravone injection in our hospital to improve the level of rational use of drugs in clinics.Methods:Totally 500 medical records with edaravone injection from January to June in 2016 were selected randomly based on the hospital medical record management system.According to the drug package insert,relative literatures and information,the clinical use rationality was evaluated.Results:Among the 500 medical records,neurosurgery patients accounted for 48%,nerve internal medicine patients accounted for 37%,ICU patients accounted for 7%,orthopedics patients accounted for 5%,and the patients in the other departments accounted for 3%.Among the 500 medical records,108 ones involved off-label drug use,which accounted for 21.6%,180 cases involved improper use frequency,which accounted for 36%,201 cases involved improper treatment course,which accounted for 40.2%,37 cases involved improper drug administration time,which accounted for 7.4%,3 cases involved inappropriate solvent choice,which accounted for 0.6%,and 4 cases involved special population,which accounted for 0.8%.Among the 500 medical records,totally 261 (52.2%) ones were irrational.Conclusion:The overdose use and improper treatment course of edaravone injection in our hospital are severe,meanwhile,improper frequency and notice ignorance are common as well,which should be paid more attention.

Objective To explore the relationship between big-five personality,and public mood of volunteers,and to find out the moderator effect of justice sensitivity.Methods A total of 1 022 volunteers were investigated.The Big-Five Inventory (BFI),Public Mood Scale (PMS),Justice Sensitivity Inventory (JSI) were applied to all subjects.Results The average score was (19.77±4.49) for extraversion,(26.69± 4.13) for agreeableness,(23.33±5.37) for conscientiousness,(12.49±4.91) for neuroticism,(24.95±5.44)for openness,(8.78±3.23) for positive public mood,(3.46±3.06) for negative public mood and correlated with (26.85± 10.55) for observer sensitivity.The extraversion,agreeableness,conscientiousness and openness were correlated with positive public mood(β=0.31,P＜0.01;β=0.25,P＜0.01;β=0.29,P＜0.01;β=0.26,P＜0.0 1),and negatively correlated with public mood (β=-0.14,P＜0.01;β=-0.29,P＜0.01;β=-0.20,P＜0.01;β=-0.08,P＜0.01).Neuroticism was negatively correlated with positive public mood (β=-0.27,P＜0.01),and positively correlated with negative public mood (β=0.31,P＜0.01).The interaction effect of agreeableness and observer sensitivity negatively correlated with positive public mood,negative public mood (β=-0.09,P＜0.01;β=-0.07,P＜0.05).The interaction effect of neuroticism and observer sensitivity was positively correlated with positive public mood (β=0.08,P＜0.01).Conclusion Observer sensitivity plays a regulative role between big-five personality and mental health,and can slow the negative impact of low agreeableness to positive public mood,strengthen the impact of low agreeableness to negative public mood,and slow the negative impact of high neuroticism to positive public mood.

Objective To study the role of cover-stent and embolization in the treatment of hepatic artery pseudoaneurysm following liver transplantation.Methods 5 patients with hepatic artery pseudoaneurysm after liver transplantation were treated with cover-stent and embolization between May 2010 and July 2013.The clinical features,imaging findings and complications were reviewed.Results All the 5 patients with hepatic artery pseudoaneurysm were successfully treated.2 patients with intrahepatic pseudoaneurysm received embolization.Of the 3 patients with extrahepatic pseudoaneurysm,2 received cover-stent treatment,and 1 patient received embolization.No complications related to the interventional treatment were encountered.2 patients died from multi-organ failure one month after the interventional treatment.Conclusion Cover-stent and embolization were effective and safe to treat patients with hepatic artery pseudoneurysm following liver transplantations.

Objective To investigate the early outcomes of reconstruction of severe acetabular bone defects by the use of tantalum trabecular metal cup combined with tantalum augment in total hip replacement.Methods From April 2014 to December 2015,23 patients (24 hips) underwent total hip arthroplasty by the use of tantalum trabecular metal cups combined with tantalum augments.There were 11 males (12 hips) and 12 females (12 hips),with the average age 54 years (range,38-65 years).We tried to reconstruct an annular support for the cup.If an annular support cannot be reconstructed,the cup obtains primary stability based on the three point fixation rationale.Flexible placing of the TM cup combined with the augments helped reconstruction of the socket ring or supporting points,thus achieving rigid primary stability of the cup and restoring center of rotation.Harris hip score and Western Ontario and McMaster Universities (WOMAC) osteoarthritis index score were recorded.Any severe intraoperative complications including vascular or nerve injury and severe postoperative complications such as periprosthetic joint infection,dislocation or periprosthetic fracture were reported.The vertical position of the center of rotation from the interteardrop line and the horizontal position of the center of rotation from the teardrop were measured and analyzed.Results The average follow-up duration was 16 months (range,7-28 months).The mean Harris hip score was 35.3±12.1 preoperatively and 82.7±8.3 postoperatively (t=-16.991,P＜0.001).The mean WOMAC score was 39.1 ±24.0 preoperatively and 87.2± 11.4 postoperatively (t=-7.671,P＜ 0.001).The average vertical location of the center of rotation was 44.02± 11.65 mm preoperatively and 22.96±8.24 mm postoperatively.All the tantalum cups and augments were stable.There was no periprosthetic joint infection,dislocation,nerve injury or periprosthetic fracture.Conclusion Reconstruction of Paprosky Ⅲ acetabular bone defects by the use of tantalum trabecular metal cup combined with tantalum augment can achieve good primary stability and restore the center of rotation with almost normal hip biomechanics.The short-term outcomes are satisfied.

Objective To evaluate the therapeutic results of percutaneous transhepatic stent angioplasty in patients with portal vein stenosis following liver transplantation.Methods From 2005 to 2013,38 patients developed portal vein stenosis following liver transplantation.Percutaneous transhepatic angioplasty of the portal vein stenosis was performed on these patients.The results were monitored by clinical follow-up and imaging studies.Results Percutaneous transhepatic angioplasty was successful in these patients.Self-expanding metallic stents (n=7),balloon-expandable coronary stent (n=29),and membranous stent (n=1) were used.The follow-up period ranged from 3 to 90 months.Portal venous patency was maintained in 34 patients (one patient died due to multi organ failure,1 patient accepted a third liver transplantation because of biliary tract complication,and 1 patient received a repeat placement of a membranous tent because the portal vein stent was blocked by a tumor thrombus,and 1 patient developed stent restenosis).There was 1 patient who developed hemorrhage in the early postoperative period (2.63％).A diagnosis of hepatic artery hemorrhage was made by hepatic artery angiography and the patient was treated by interventional embolization.Conclusion Percutaneous transhepatic stent angioplasty is an efficacious and safe method to treat portal vein stenosis following liver transplantation.

Objective:To investigate the inhibitory effect of RNA interference (RNAi) on the expression of multidrug resistance (MDR1) gene and analyze the altered sensitivities of human renal carcinoma cell line to cisplatin.Methods:Three small interfering RNA (siRNA) sequences targeted MDR1 gene were synthesized and transfected into renal carcinoma A498 cells. The expression level of MDRl mRNA was measured by RT-PCR to identify the most effective siRNA sequence. The recombinant plasmid was packed by lentivirus and transfected into A498 cells. RT-PCR was used to screen the A498 cells with the optimal silencing efficacy. The MDR1 protein expression level in the cloned cells was verified by Western blotting. The inhibitory effect of cisplatin on the proliferation of A498 cells was assessed by MTT assay and the IC_(50) value was calculated. Results:The 3 siRNA sequences suppressed MDR1 gene expression at different degrees. The siRNA 1 sequence silenced MDR1 gene more effectively with a significant reduction of 67%. The MDR1 protein expression greatly decreased in screened A498 cells compared with non-transfected cells (P<0.01), and the IC_(50) value of cisplatin on screened A498 cells was significantly decreased by 83.37% (P<0.01). Conclusion: The RNAi could effectively inhibit the expression of MDR1 gene and increase the sensibility to cisplatin in human renal carcinoma A498 cell line, which make it possible to reverse the resistance of renal carcinoma to chemotherapy.

Objective To observe the efficacy of Mimics finite element analysis software in the gasserian ganglion radiofrequency treatment of trigeminal neuralgia. Methods 180 cases with primary trigeminal neuralgia and VAS score ≥8 were randomly divided into 2 groups (n = 90 each): CT group (group C) and Mimics group (group M). The preoperative skull CT image of the foramen of cranial base could be analyzed in group C. The preoperative cranial CT image could be reconstructed and analyzed by Mimics finite element analysis software in group M. The puncturing success rate, complications rate and the outcomes between two groups were recorded. Results The puncturing success rates were 100% in group M and 92% in group C (P < 0.05). 6 cases with hematoma and 1 case, which were stabbed into the oral cavity were found in group C. No puncture complication was found in group G. There was no statistically significant between the two groups (P < 0.05). The VAS score was 1.6 ± 0.3 in group C and 1.3 ± 0.4 in group G, there was no statistical significance (P > 0.05) between them. Conclusions The Mimics finite element analysis software could improve the success rate of basicranial foramen ovale puncture and reduce the occurrence rate of puncture complications. Therefore , it could be safely applied to the treatment of primary trigeminal neuralgia by gasserian ganglion radio frequency.

AIM:To investigate the effects of resveratrol ( Res) on the proliferation of ARPE-19 cells and to ex-plore the possible mechanisms.METHODS:After ARPE-19 cells were treated with Res at concentrations of 0, 50, 100, 150, 200 and 300 μmol/L for 24 h, 48 h and 72 h, the effects of Res on the proliferation of the cells were tested by CCK-8 assay.The ARPE-19 cells were treated with Res at concentrations of 0, 100, 150 and 200 μmol/L for 48 h.The effects of Res on the cell cycle and apoptosis were detected by flow cytometry with Annexin V-FITC/PI staining.The protein expression of proliferating cell nuclear antigen (PCNA) was detected by immunofluorescent assay.The mRNA expression of PCNA, P21 and P27 was determined by real-time PCR.RESULTS:The results of CCK-8 assay showed that Res inhibited the prolifera-tion of ARPE-19 cells in a time-and dose-dependent manner.The treatment with Res for 48 h resulted in an arrest of cell cycle at S phase without increasing cell apoptosis.Res inhibited the protein expression of PCNA in ARPE-19 cells.The re-sults of real-time PCR showed that Res increased the mRNA expression of P21 and P27, and decreased the mRNA expres-sion of PCNA.CONCLUSION: Res inhibits the proliferation of ARPE-19 cells and induces the cell cycle arrest at S phase.The mechanism may be related to up-regulation of P21 and P27, and down-regulation of PCNA.
