1.The significance of ISO15189 in the cervical cancer and breast cancer screening

Objective To explore the significance of ISO15189 in the cervical cancer and breast cancer screening.Methods Collected in 2009 and 2010 by the clinical laboratory in Hefei Adicon cervical cytology screening women ensure the submission of Anhui Funan Women BUPA 10050,28436 patients as research subjects,analysis of process management in accordance with ISO15189 Specifications in the detection of smears before and after implementation before,during and after the result of differences in the process of formation of. Results The quality of cervical smears and the positive rate had greatly improved, dramatically improve customer satisfaction. Conclusion ISO15189 implementation of process management standard “two cancer ” screening of cervical cytology screening provided quality assurance.
2.The alteration of CD4+ regulatory T cells in patients with ankylosing spondylitis

Objective To evaluate the alteration of CD4+ regulatory T cells in peripheral blood from patients with ankylosing spondylitis(AS). Methods Seventy-eight AS patients and 50 healthy individuals were included in this study. The proportion of CD4+CD25+CD127lo/- T cell population in CD4+ T cells as well as that cytotoxic T lymphocytes(CTL) and NK cells in lymphocytes was evaluated by flow cytometry. Serum TGF-β and TNF-α were measured by ELISA. The inhibitory function of CD4+CD25+ regulatory T cells was measured by mixed lymphocyte culture. Results The population of CD4+CD25+CD127lo/- T cells in peripheral blood of AS patients accounted for (4.36±1.21)% of CD4+ T lymphocytes, which was significantly lower than that in healthy individuals (P<0.05). The CD4+CD25+CD127lo/- T cells population in AS patients was positively correlated with TGF-β level, but negatively with TNF-α. Compared with healthy individuals, the function of CD4+CD25+ regulatory T cells in the inhibition of alloreactive T cells was lower in AS patients, which was related to the decreased secretion of TGF-β. Conclusion The CD4+ regulatory T cells in peripheral blood of AS patients are significantly decreased and its function is defective, which leads to immune regulatory dysfunction in vivo. It may be one of immune pathogenesis mechanisms of AS.
3.Establishment and evaluation of a high throughput sequencing technology for detection of Salmonella in laboratory animals

Objective To establish a detection method of Salmonella in laboratory animals based on a high-throughput sequencing technology, and to apply it in detection of Salmonella in laboratory animals.Methods DNA samples were extracted from mouse feces.Universal primers for 16S rDNA, 23S rDNA, 16S-23S rDNA, 23S-5S rDNA region, gyrB preferred area were designed, respectively.Each primer was tested and analyzed to determine the best amplification conditions and build a database.Forty-two samples of Salmonella were assayed by Illumina high-throughput sequencing technology and evaluated the specificity and stability of this method.Results The species preferred region of Salmonella was gyrB gene region.The primers for gyrB gene were FP5 ’-AACCACCGCAATCAGACCTT3‘ and FP5 ’-AGCCACGAAACCTTCACYA-3’.The primers were optimized and determined, through a high-throughput sequencing, and the sequence analysis detected very small amount of Salmonella in the 42 samples, indicating that this detection method is stable, highly sensitive, and the limit of detection reached to 0-102 CFU.Conclusions We have established a complete detection system for detection of Salmonella in laboratory animals based on a high-throughput sequencing technology, This system can detect trace amounts of Salmonella in laboratory animals, and this detection method is stable and highly sensitive, which can be also used in detection of other kinds of pathogenic microorganism in laboratory animals.
4.Influence of nocturnal prolonged hemodialysis on nutrition status in uremic patients

Objective To observe the influence of nocturnal prolonged hemodialysis (INHD) on patients' nutrition status. Methods Thirty-two maintenance hemodialysis patients received INHD (3 times per week and 7.5 hours each session) and thirty-five maintenance hemodialysis patients received conventional hemodialysis (3 times per week and 4 hours each session) as control were observed for 6 months.The nutrition status of these patients on various aspects which concluded physical measurements,laboratory tests,and dietary record at baseline(0month) and exit (6 months) were recorded. Results (1)There were no differences in age,sex,body weight,and primary diseases between two groups.(2)The body weight,triceps skinfold thickness (TSF),and hand grip strength increased at exit point,but no statistical difference compared with the control group.Mid-upper arm circumference (MAC) increased signicantly from (27.1±4.2) to (30.5±6.1) cm (P<0.05).Compared with the control group (26.9±3.4) cm,there was a significant difference (P<0.05).(3)Serum phosphate decreased significantly from (0.5±0.5) to (0.1±0.6) μ mol/L (P=0.001) in INHD group.(4)The nutrition status were improved in INHD group evaluated by subjective global assessment (SGA)(P=0.03).(5) Dietary intake was recorded by a 3-day food record.Dietary intake of energy,protein,lipid,calcium,potassium,and phosphate increased in INHD group.None of the differences achieved statistical significance between two groups. Conclusion As compared with conventional hemodialysis,INHD can increase the dietary intake,decrease serum phosphate level,and improve patients nutrition status.
5.Feasibility of Two-Screw Anterior Fixation for Odontoid Fractures in a Chinese Population:A Morphometric Study Based on Computed Tomography

Background:
To evaluate the feasibility of treating odontoid fractures in the Chinese population with two cortical screws based on computed tomography (CT) scans and describe a new measurement strategy to guide screw insertion in treating these fractures.
Methods:
A retrospective review of cervical computed tomographic scans of 128 patients (aged 18–76 years; men, 55 [43.0%]) was performed. The minimum external transverse diameter (METD), minimum external anteroposterior diameter (MEAD), maximum screw length (MSL), and screw projection back angle (SPBA) of the odontoid process were measured on coronal and sagittal CT images.
Results:
The mean values of METD and MEAD were 10.0 ± 1.1 mm and 12.0 ± 1.0 mm, respectively, in men and 9.2 ± 1.0 mm and 11.0 ± 1.0 mm, respectively, in women. Both measurements were significantly higher in men (p < 0.001). In total, 87 individuals (68%) had METD > 9.0 mm that could accommodate two 3.5-mm cortical screws. The mean MSL value and SPBA range were 34.4 ± 2.9 mm and 13.5°–24.2°, respectively, with no statistically significant difference between men and women.
Conclusions
The insertion of two 3.5-mm cortical screws was possible for anterior fixation of odontoid fractures in 87 patients (68%) in our study, and there was a statistically significant difference between men and women.
6.Effects of mindfulness intervention on the mindfulness, psychological health and quality of life for uremia patients

Objective To analyze effects of mindfulness intervention on mindfulness, psychological health and quality of life for uremia patients. Methods A total of 82 uremia patients in Wuxi No.2 People′s Hospital from January 2013 to July 2015 were randomly assigned to the intervention group ( n=41) and the control group ( n=41). Patients in the control group were given routine nursing, while patients in the intervention group were given additional mindfulness intervention. The classification of mindfulness, the status of psychological health and quality of life scores after 8 weeks intervention and before the intervention were compared between two groups. Results After 8 weeks′ intervention, the grading state of mindfulness in the intervention group was obviously superior to the control group. The scores of interpersonal relationship, somatization, compulsion, depression, anxiety, crankiness, fear, nervousness and the total score were significantly lower than those of the control group ( P<0.05) . Quality of life scores in the intervention group (74.22±11.17) was higher than the control group (65.78±10.45)(P<0.05).Conclusions Mindfulness-based intervention is conducive to improve mindfulness of uremia patients and reduce patients′ negative mental states and it is of great significance to improve the quality of life.

The author reviewed and sorted out the historical evolution process of public hospital reform guided by public welfare, summarized and condensed the core connotation of public hospital public welfare, including " legal obligations, system concepts, people′s positions, and inclusive goals", and introduced the practice of deepening the public welfare reform of public hospitals based on the connotation of Union Hospital affiliated with Tongji Medical College of Huazhong University of Science and Technology based on this connotation, focusing on " party building, medical technology, health emergency response, improvement of services, salary distribution, economic operation, resource expansion and sinking" .
