Chinese Journal of Health Management 2011;05(6):384-386
Objective To understand the prevalence of internet addiction disorder(IAD)and explore its relationship with suicidal tendency of undergraduates. Methods A total of 1517 undergraduates were investigated by using Internet Addiction Scale(IAS),Beck Hopelessness Scale(BHS)and self-made questionnaire. Results The prevalence of lAD was 14.8%.Males,single child, and those at the second grade, with poor performance or higher family income were at a higher risk of internet addiction(P < 0.01).There was significantly positive correlation between internet addiction and suicidal tendency in undergraduates(x2 =76.30,P < 0.01,r =0.40).The risk of suicidal tendency was significantly increased in the high internet addiction score group(OR =12.29,95 % CI 6.33 to 23.84).Conclusion Higher prevalence of IAD was found in the undergraduates, which could contribute to the increasing suicide.
2.Drug Resistance of Pathogenic Bacteria from Blood Culture and the Distribution in Clinical Departments from 2012 to 2014
Acta Medicinae Universitatis Scientiae et Technologiae Huazhong 2017;46(1):84-89
Objective To analyze drug resistance of pathogenic bacteria from the blood culture and distribution in clinical departments, and to guide the rational clinical drug use. Methods We retrospectively analyzed 11 275 samples of blood cultures in The Central Hospital of Wuhan from 2012 to 2014.The blood specimens were cultured by VersaTREK(USA).The pathogenic bacteria were identified and their drug resistance was analyzed by BD-PHOENIX 100 automicrobiological identification systems(USA).Results Among the 11 275 blood cultures,636 bacterial strains were detected. The top four bacterial strains were Escherichia coli,Staphylococcus aureus,Klebsiella pneumonia and Enterococcus f aecium.A vancomycin-resistant Enterococcus faecium strain and a pandrug-resistant Klebsiella pneumoniae strain were detected. The top three clinical departments with distribution of pathogens were Gastroenterology Department,Nephrology Department and Intensive Care Unit (ICU).Pathogens isolated from ICU were evenly distributed. Conclusion Distributions of pathogenic bacteria in the blood culture are different in clinical departments.Identification of pathogenic bacteria and result of drug susceptibility can reduce use of broadspectrum antimicrobials and enhance antimicrobial de-escalation.

4.The pilot study on mutations of HIV-1 envelop V3 loop in AIDS patients in Shenzhen
Chinese Journal of Infectious Diseases 2001;0(05):-
Objective To investigate the characteristics of human immunodeficiency virus(HIV-1)V3 loop amino acid mutations among AIDS patients in Shenzhen. Methods Fragments of HIV-1 env gene were amplified by RT-nested-polymerase chain reaction(PCR)from plasma of the AIDS patients in Shenzhen. The PCR products were then directly sequenced and the sequences were aligned, translated and analyzed. Results Sequence analysis showed that there were B and CRF01-AE HIV-1 subtypes in Shenzhen. The V3 loop terra peptide GPGQ accounted for about 48%,GPGR about 36%,other V3 loop terra peptide forms were 16%;the V3 loop central motif in the majority(76.9%)of the CRF01-AE strains was GPGQ,the majority(75%)of the B strains was GPGR.We also found some unusual terra peptide compositions:DQDR and DQGQ on the tip of V3 loop, some amino acid mutations at specific of V3 loop associated with SI phenotype were very common in AIDS patients(80%). Conclusions The V3 loop amino acid mutations were very common in AIDS patients, the V3 loop central motif in the majority was GPGQ and GPGR in Shenzhen, the mutations shows the change of HIV-1 phenotype and predicts that disease progress to AIDS.

Objective To evaluate the efficiency of levonorgestrel-releasing intrauterine system (LNG-IUS) in the treatment of recurrent endometriosis after conservative surgery or conservative surgery combined with medical treatment. Methods Twenty-three patients with recurrent endometriosis after conservative surgery or conservative surgery combined with medical treatment were treated by LNG-IUS. All patients rejected further operation and had no desire of fertility. The visual analogue scale (VAS) scores of pain, menstrual model, weight and serum CA125 level and the volume of ovarian endometriotic cysts before and after 3, 6, 12, 24 and 36 months of treatment were recorded and compared. Results ( 1 ) VAS score:after 12 months of using LNG-IUS, dysmenorrheal, chronic pelvic pain or dyspareunia were relieved significantly. VAS score were dropped from 5.9 ± 2. 3,4. 3 ± 2.0 to 1.0 ± 0. 7,1.4 ± 1. 1 ( P < 0. 01 ). ( 2 )Volum of cysts :after 6 months of using LNG-IUS, the volume of recurrent ovarian endometriotic cysts in 11 patients were reduced from ( 11.4 ± 6. 1 ) em3 to ( 5. 5 ± 3.4 ) em3 significantly ( P < 0. 01 ). At 12 months of follow-up, it suggested that 2 patients' ovarian endometriotic cysts disappeared. At 24 months follow-up,9 patients ovarian endometriotic cysts disappeared ( 3 ) CA125: serum CA125 decreased from ( 65.5 ± 19. 6 )kU/L to (42. 1 ± 13.6) kU/L at 6 months after treatment remarkably (P < 0. 01 ). Continued to decrease after 12 months and then become steady. Irregular bleeding and spotting was the main side effects, weight gain was also observed in few patients. Conclusions LNG-IUS could be used in treatment of recurrent endometriosis after conservative surgery or conservative surgery combined with medical treatment effectively. It could relieve pain, reduce the level of CA125 and decrease the size of ovarian endometriotic cysts. LNG-IUS seems to be an effective, safe, and long term treatment for endometriosis with fewer side effects and better compliance.

Objective To identify subtypes of human immunodeficiency virus 1(HIV-1)strains from the AIDS patients in Shenzhen and determine whether the HIV-1 subtypes differ in disease progression. Methods HIV-1 env gene was amplified by the nest-RT-PCR from plasma obtained from 26 patients with AIDS in Shenzhen. The C2-V3 regions were sequenced to identify subtypes The plasma viral loads and CD4T lymphocyte were measured as the same time. Results Phylogenetic trees showed that the 12 AIDS patients had subtype B in which, one was close with the U.S reference strain and 11 with the Chinese Yunnan reference strain;13 AIDS patients had subtype CRF01-AE from Thailand; There were no differences in the CD4 cell count and plasma HIV-RNA levels between individuals infected with subtypes B and CRF01-AE.Conclusion Our study indicated that HIV-1 subtype B and CRF01-AE strains were present in AIDS patients in Shenzhen. There was no evidence that the subtypes of virus could determine disease progression.

Objective To summarize the experience of adjuvant therapy for primary hepatocellular carcinoma. Methods 316 cases of operable hepatocellular carcinoma were divided into three groups. Only hepatectomy were performed in group one (21 8cases).Preopemtive adjuvant TACE were done in group two (52 cases). Preoperative adjuvant TACE and postoperative trans-portal vein chemotherapy were done in group three (46 cases), which was named hepatectomy sequencing two vessel therapy. Results 1, 3 and 5 year survival rote were 51.2 %, 30.0 % and 20.5 % respectively in group one, 57.2 %, 43.0 % and 31.5 % in group two, 84.0 %, 62.5 % and 51.0 % in group three. The postoperative disease-free survival rate in group three was significantly higher than that in group one and group two (P <0.05). Conclusion Hepatectomy sequencing two vessels therapy in perioperative period might improve the survival rate, which can prevent and delay the incidence of recurrence and may improve the effect of liver resection.

Objective To determine the proper dosage of fentanyl for open heart surgery performed under cardiopulmonary bypass (CPB) without aortic cross-clamping and cardioplegia. Methods Twenty-seven ASA Ⅰ -H patients (10 male, 17 female) with fairly good cardiac function (NYHA Ⅰ - Ⅱ) scheduled for surgical repair of atrioseptal defect ( ASD) or ventricular septal defect (VSD) or mitral valve replacement (MVR) were studied. Age ranged from 18 to 44 years and body weight from 35 to 58 kg. The patients were randomly divided into three fentanyl dosage groups: group Ⅰ 10?kg ; group II 30 ?g kg-1 and group Ⅲ 50 ?g kg-1. Premedication consisted of intramuscular pethidine 1-2 mg kg-1 and scopolamine 0.05-0.06 mg kg-1 . Anesthesia was induced with midazolam 0.2 mg kg-1 and fentanyl 5 ?g kg-1 . Tracheal intubation was facilitated with vecuronium 0.15 mg g-1 . The patients were mechanically ventilated (Vr 8-10 ml kg-1 ,F 10-12 bpm,FiO2 100% ). The rest of the total dose of fentanyl (5 ?g kg-1 in group I , 25 ?g kg-1 in group II , 45 ?g kg-1 in group III) was infused after induction of anesthesia until the initiation of CPB, supplemented with inhalation of 0.6 % isoflurane. During CPB propofol was infused at 5 mg kg-1 h-1 . after discontinuation of CPB, again 0.6% isoflurane was inhaled until the end of surgery. Vecuronium 0.05 mg kg was given every 25-30 min during operation. EGG, HR, BP, CVP, SpO2, PET CO2 and body temperature (naso-pharyngeal and rectal) were continuously monitored during operation. Arterial blood samples were obtained before anesthesia (T0), 5 min after tracheal intubation (T, ) , immediately after thoracotomy (T2) , immediately before CPB (T3), 15 min after CPB was initiated (T4) , 10 min after termination of CPB (T5) and 5 min after chest was closed (T6) for blood gas analyses and determination of blood electrolytes and acid-base balance and blood concentrations of glucose, ACTH, angiotensin Ⅱ (A- Ⅱ ) and cortisol. Time of emergence from anesthesia and extubation were recorded. Results The demographic data, including age and body weight, CPB time and duration of surgery were comparable among the three groups. There were no significant changes in SpO2 , PETCO2 , body temperature, blood gases and electrolytes during operation in the three groups. MAP decreased significantly during CPB. The blood glucose, ACTH, A- Ⅱ and cortisol concentrations increased significantly during and after CPB as compared with the preanesthetic baseline (T0 ) ( P
