1.Drug Resistance of Pathogenic Bacteria from Blood Culture and the Distribution in Clinical Departments from 2012 to 2014

Objective To analyze drug resistance of pathogenic bacteria from the blood culture and distribution in clinical departments,and to guide the rational clinical drug use.Methods We retrospectively analyzed 11 275 samples of blood cultures in The Central Hospital of Wuhan from 2012 to 2014.The blood specimens were cultured by VersaTREK(USA).The pathogenic bacteria were identified and their drug resistance was analyzed by BD-PHOENIX 100 automicrobiological identification systems(USA).Results Among the 11 275 blood cultures,636 bacterial strains were detected.The top four bacterial strains were Escherichia coli,Staphylococcus aureus,Klebsiella pneumonia and Enterococcus f aecium.A vancomycin-resistant Enterococcus faecium strain and a pandrug-resistant Klebsiella pneumoniae strain were detected.The top three clinical departments with distribution of pathogens were Gastroenterology Department,Nephrology Department and Intensive Care Unit (ICU).Pathogens isolated from ICU were evenly distributed.Conclusion Distributions of pathogenic bacteria in the blood culture are different in clinical departments.Identification of pathogenic bacteria and result of drug susceptibility can reduce use of broadspectrum antimicrobials and enhance antimicrobial de-escalation.
2.Clinical analysis of 233 patients with drug eruption

AIM:To discuss the main clinical characteristics of current drug eruption for reference of prevention and treatment of drug eruption. METHODS: Two hundred and thirty-three with drug eruption in 1999 were collected, and their clinical characteristics, such as age, main causative drugs and eruption types, etc. were analysed. RESULTS: The age of current drug eruption increased. Antibiotics was the most common causative drug, while antiinflammatory analgestics, serum and vaccine, traditional Chinese medicine were next to it. Scarlet fever-like or measles-like type was the most common eruption type, while urticaria type, erythema multiforme type and fixed type were next to it. CONCLUSION: Clinical physicians shoud inquire alergic history of patients thoroughly and avoid unnecessary drugs, so as to decrease the occurrence of drug eruption.
3.Value of serum NT-proBNP in the clinical evaluation of heart failure

Objective To investigate the relationships between the concentration of serum NT-proBNP,New York Heart Association(NYHA) classification,left ventricular ejection fraction((LVEF),)and the value of serum NT-proBNP in the evaluation of cardiac function and the diagnosis of chronic heart failure.Methods One hundred and two subjects were selected,including 30 healthy controls and 72 patients with heart failure.The concentration of serum NT-proBNP was determined by an automated electrochemiluminescence immunoassayan Roche Elecsys 2010.LVEF was measured by the modified Simpson′s equation with echocardiography.The diagnosis of clinical physician was considered to be the golden standard for heart failure.Results The relationships between the concentration of serum NT-proBNP and NYHA,serum NT-proBNP levels(287.7 pg/ml) was significantly higher in heart failure patients(287.7 pg/ml) than that in healthy controls 287.7 pg/ml vs (46.1 pg/ml),P
4.Comparative study on family cohesion and family education style of adolescents with internet addiction disorder

Objective To compare internet addiction disorders' family cohesion and their family educa-tion style, and to help prevent internet addiction disorder and interference treatment in families with internet addic-tion disorder. Methods Ninety adolescents with internet addiction disorder were investigated as the experimental group and one hundred normal adolescents as the control group. Questionnaire and in-depth interview was used to collect data,and the data was analyzed with SPSS 10.0. Results Average scores of control group were significant higher than those of experimental group in family cohesion ((51.75±8.86) vs (38.89±12.17), P<0.01). Av-erage scores of control group were significant higher than these of experimental group in family education style ((50.65±7.66) vs (41.44±10.84), P < 0.01). Average scores of experimental group were significant higher than those of control group in dissatisfaction of family cohesion and family education style ((20.87±4.66) vs (12.33±7.88); (23.30±11.06) vs (14.28±7.29), P < 0.01). Conclusion The adolescents with internet addiction disorder have low family cohesion and high dissatisfaction of family education style,wherefore during in-ternet addiction family intervention and prevention, it should pay particular attention to cultivating close and harmo-nious relations among families members. In aspect of family education, parents should pay special attention to chil-dren's healthy personality development, and guide their children to actively take part in social practice and train-ing.
6.Function of the resolution rate of medical disputes in hospital performance evaluation system

Recent years found a series of severe incidents of injuring or killing medical workers in several places in China,deteriorating patient-physician relations,and disturbing medical orders.Beijing Administration of Hospitals,since its founding,has attached great importance to the management of medical disputes,as evidenced in the full-process management covering pre-during-post disputes.The administration guided the hospitals in their dispute resolution,and included resolution rate of disputes as an index of municipal hospital performance evaluation.By means of the guidance of such evaluation, medical disputes and major medical accidents have been resolved satisfactorily,and fine management of service quality of such hospitals in terms of medical service,pharmacy,nursing and medical technology has been intensified all the time.
7.A new method to evaluate hip joint damage in ankylosing spondylitis

Objective In order to assess the structure damage of hip joint in ankylosing spondylitis (AS), a new radiograph-based scoring method was developed according to the radiological characteristics of hip involvement in AS, as well referring to prior existing scoring indexes. Methods A new scoring method consti-tuted of erosion, sclerosis and joint space narrowing was developed, pelvis anterior-posterior plain films acquired from patients with AS at baseline and follow-up were collected and assessed by two physicians who were trained in image reading by radiologists. All films were scored independently and blindly. Intra- and inter-reader reliability were assessed by intra-class correlation coefficient (ICC), the feasibility of this new scoring method was assessed by the mean time acquired to score a plain (two hips), its ability to detect the change of structure damage was assessed by the comparison of score differences between baseline and different follow-ups. The date were analyzed by paired-t test or nonparametric tests. Analysis of Variance (ANOVA) or nonparametric tests were utilized for the comparison of means of quantitative variables among the three groups, while Chi-square test for rates of categorical variables. Results No statistically significant differences existed in demographic data and suspected risk factors among the three groups at baseline (P>0.05). Intra-observer reliability was good (0.84 and 0.89), as well as the inter-observer reliability (0.72), the mean time needed to score was (33 ±10) seconds. Score changes were not statistically significant in the groups with follow-up duration of 1-2 and 3-4 years, but in the group of over 5 years, baseline/final scores assessed by the two observers were (6.0±2.7/7.5±3.7) and (5.6±2.1/7.1±3.6), respectively, both changes were statistically significant (t=2.86, Z=-2.99; P<0.01). Conclusion This new method is not only reproducible and easy to operate in clinic practice, but also can tell the changes of hip joint structure damage in the interval of over 5 years, further validation is requeired to demonstrate its discriminability in large populations.
8. Clinical characteristics of patients with ankylosing spondylitis and inflammatory bowel disease

Objective:
To analyze the clinical characteristics of patients with ankylosing spondylitis (AS) with inflammation bowel disease (IBD).
Methods:
AS patients fulfilling the 1984 modified New York diagnostic criteria were recruited in Chinese AS Prospective Imaging Cohort (CASPIC) consecutively from April 2016 to June 2017 in Chinese People′s Liberation Army General Hospital by using smart management system for spondyloarthritis (SpAMS). The diagnosis of IBD was confirmed by tissue pathology via ileocolonoscopy. Demographic, clinical and biochemical data were collected.
Results:
In total, 893 patients with AS were recruited with the mean age 30.8 years. The majority were men (739, 82.8%). There were 64 (7.2%) patients concomitant with IBD. The mean age [(34.5±7.5) years vs. (30.5±8.8) years,
9.Sequence of reduction and fixation in the treatment of middle and lower tibiofibular fractures on the same plane using tibial intramedullary nailing plus fibular plating

Objective:To explore the sequence of reduction and fixation in the treatment of middle and lower tibiofibular fractures on the same plane using tibial intramedullary nailing plus fibular plating.Methods:A retrospective analysis was performed of the 58 patients with middle and lower tibiofibular fractures on the same plane from January 2016 to December 2017. They were 38 males and 20 females, aged from 20 to 65 years (average, 40 years). The left side was affected in 30 cases and the right in 28. By the AO classification, 27 cases were type 42-A, 18 ones type 42-B and 13 ones type 42-C. Of them, 33 had the fibula reduced and fixated first (the fibular group) while 25 had the tibia reduced and fixated first (the tibial group). The 2 groups were compared in terms of operation time, rate of tibial closed reduction, rate of dynamization of intramedullary nails, fracture healing time and postoperative complications.Results:All the patients were followed up for 12 to 24 months (average, 17.2 months). The operation time in the fibular group was 96 minutes ± 15 minutes, significantly shorter than that in the tibial group (116 minutes ± 19 minutes)( P<0.05). The rate of tibial closed reduction was 84.8% (28/33) in the fibular group and 60.0%(15/25) in the tibial group, presenting a significant difference ( P<0.05). There were no significant differences between the 2 groups in the rate of dynamization of intramedullary nails, fracture healing time or postoperative complications ( P>0.05). Conclusions:Tibial intramedullary nailing plus fibular plate osteosynthesis is an effective treatment for the middle and lower tibiofibular fractures on the same plane. When the fracture line is not located in the narrow segment of the tibia, reduction and fixation of the fibula first is advantageous over reduction and fixation of the tibia first, because it can facilitate tibial reduction and nail placement and improve surgical efficiency without increasing the fracture healing time.
10.Kinematic parameter characteristics of treading manipulation based on three-dimensional dynamic capture technique

Objective To study the kinematic characteristics of Luo's treading manipulation,and provide evidence-based data reference for unique manipulations of prominent senior experts. Methods Three complete treading motions manipulated by Professor Luo Caigui and beginners were recorded by using 3D motion capture technique,and their motions of hip, knee and ankle joints were recorded by using high speed camera. The dynamic changes of angle, angular speed and barycentric velocity were observed in the total cycle and at important time points, and overall and local kinematic characteristics of Luo's treading manipulation were analyzed. Results Compared with beginners,the overall cycle characteristics of Professor Luo's treading motions showed that the difference in angles of hip and ankle joints had statistical significance (P <0.01). The local characteristics showed that angular speed of knee joint changed slightly at the moment of barycenter descending to the lowest point in cycle 1 (P<0.05),and hip angle was smaller(P<0.05) and angular speed of ankle joint changed slightly(P<0.01) in cycle 3. At the moment of barycenter ascending and knee joint straightening,angular speeds of knee joint and ankle joint changed slightly(P<0.01) and barycentric velocity was smaller(P<0.01) in cycle 1. The angular speeds of hip joint and knee joint changed slightly (P <0.05), and angle of ankle joint was smaller (P<0.05) in cycle 2, and angular speeds of hip joint and knee joint changed slightly (P <0.05) in cycle 3. At the moment of barycenter descending and maximum flexion of knee joint,Professor Luo's angle of knee joint was larger, angular speed changed slightly (P <0.01), and barycentric velocity changed slightly (P<0.05) in cycle 1. The barycentric velocity was smaller (P <0.05) in cycle 2,and angles of hip joint and knee joint were smaller(P<0.01) in cycle 3. Conclusion Professor Luo's motions have obvious local characteristics, i.e. both angles and angular speeds of joints change slightly during treading period,and at the same time of keeping body stable, his motions are constant, steady, dexterous and strengthened associated activity and inertia. Luo's treading manipulations lay emphasis not only on overall stable,but also on dexterous and trigger force,and reflect the characteristics of associating activity and inertia and combining softness and hardness.
