1.Suturing pattern of the scleral incision affect the postoperative intraocular pressure after 23G vitrectomy

Objective To observe the relationship between the suturing patterns to close the scleral incision and postoperative intraocular pressure (IOP) in 23G minimally invasive vitrectomy.Methods Eighty eyes of 80 patients with vitreoretinal diseases,who were treated with primary 23G minimally invasive vitrectomy,were enrolled in this prospective clinical study.Patients with poor closed scleral incision which need suturing were excluded from this study.The corrected visual acuity ranged from hand movement to 0.2.The IOP ranged from 7.9 to 19.8 mm Hg (1 mm Hg=0.133 kPa),with the mean of (13.9±1.8)mm Hg.The eyes were randomly divided into three groups:group A (20 eyes),suturing all three scleral puncture after vitrectomy; group B (20 eyes),suturing only two upper scleral puncture,but not the lower infusion puncture after vitrectomy; group C (40 eyes),no suturing for all 3 scleral puncture after vitrectomy.All patients underwent 23G vitrectomy only.The corrected visual acuity and IOP were observed after surgery.Results The corrected visual acuity were 0.1 0.3,0.2-0.5,0.3-0.8 in one,seven and 14 days after surgery,respectively.No one in group A,B experienced hypotony in one,three,seven and 14 days after surgery.Thirteen (32.5%),five (12.5 %),two eyes (5.0%) in group C experienced hypotony in one,three and seven days after surgery.Seven eyes (17.5%) experienced severe hypotony (<5 mm Hg)in 14 day after surgery in group C.The difference was statistically significant compared the incidence of hypotony in group C with group A,B respectively at different time points after surgery (xx2 =16.82,P=0.007).The difference was statistically significant compared the incidence of hypotony in group C at different time points after surgery (x2 =11.64,P=0.003).The difference was no significant compared the IOP between group A and B at different time points after surgery (F=1.618,P=0.205).Compared the IOP of group C to group A and B,the difference was statistically significant in one and three days after vitrectomy (F=9.351,P=0.000); but not statistically significant in seven and 14 days after vitrectomy (F=0.460,P=0.633).Conclusions Whether or not suturing the scleral punctures is closely related to postoperative hypotony in 23G vitrectomy.Suturing only the two upper scleral punctures can reduce the occurrence of postoperative hypotony.

Objective To investigate the prevalence of voice disorder and voice fatigue mental state in the pri-mary school teachers in a district of Chengdu.Methods A random sampling survey included 389 teachers from a dis-trict of Chengdu after they filled out the throat symptoms questionnaire,carried out voice fatigue tests,and strobo-scopic laryngoscopy.Results The most common voice of discomfort symptoms was hoarseness,followed by sore throat,dry throat and vocal fatigue.There were 189 teachers,48.5%,with voice disorders as the voice disorder group)and 200 teachers without throat and voice disorders as the control group.The failure rate was significantly higher at 80 dB than 75 dB,and for the study group,the failure rate was higher than that of the control group.The difference was statistically significant in 1,2 and 10 minutes between the two groups(P <0.05).Conclusion Voice fatigue is very common in the primary school teachers and when high volumes are required,the voice fatigue is more serious.To reduce voice fatigue and the incidence of primary school teachers'voice disease,we should strengthen the teacher's voice health care.
4.The development of a bioartificial liver and its application in acute liver failure patients

Objective Using polysulfon fibers, a new bioartificial liver was developed. This study was to evaluate the efficacy of this bioartificial liver in the support of a disfunctioned liver. Methods Hepatocytes were procured from swine using Seglen′s methods. Objective: To design the proficiency testing (PT) project (No. NIFDC-PT-183) for assays of bismuth potassium citrate capsules and organize to assess the proficiency of complexometric titration in laboratories, and provide some technical analyses and advices.
Methods: Two groups of samples with different concentration were prepared. The uniformity was evaluated with one-way analysis of variance and the stability was confirmed with t-test, whose results all conformed the requirements. The samples with three combinations were randomly distributed to 279 laboratories. The determination was performed according to the assays of bismuth potassium citrate capsules in Volume Ⅱ of the Chinese Pharmacopoeia 2015. The median value and normalized interquartile range (NIQR) of robust statistical analysis was adopted and Z-scores were used to evaluate the results from each of laboratories.
Results: Among 279 laboratories, 240 laboratories results were satisfactory, 23 were questionable, and the other 16 were unsatisfied. The satisfaction rate was 86.0%.
Conclusion: The overall capacity of national laboratories for assays of bismuth potassium citrate capsules is good while a portion of participants require further improvement.
8.Primary clinical study of a novel multi-layer flat-plate bioartificiai liver for patients with liver failure

Objective To investigate the safety and therapeutic effects ot a novel multi-layer flat-plate bioartificial liver (BAL) for patients with liver failure.Methods Thirty-eight patients with liver failure from Dec.2010 to Dec.2011 were treated with a novel BAL based on multi-layer flatplate bioreactor and the co-cultured cells of the porcine hepatocytes and mesenchymal stem cells.A total of 48 treatments was performed,4 h each time.The clinical signs and symptoms,liver function,ammonia,coagulation function and complete blood count were evaluated before,during and after the treatment.DNA in the collected PBMCs was extracted for PCR with PERV specific primers and the porcine specific primer Sus scrofa cytochrome B.The RT activity was detected as well.Levels of xenoantibodies (IgG,IgM) were determined by using ELISA kit. Titers of complement were quantified by CH50 kit.Results All treatment procedures were completed successfully without any adverse reaction. All samples presented negative PERV DNA and RT activity. The levels of antibodies were similar before and after treatment.Treatment was associated with a temporary decline in levels of complement,and then the levels were recovered quickly.The clinical symptoms such as acratia,anorexia and abdominal distension were improved.The stage of hepatic encephalopathy in 16 patients was decreased. The liver function and ammonia was reduced disproportionately. Seven patients in all were bridged to liver transplantation,2 patients died and 2 patients gave up the treatment,and the others were turned better.After the outcome judgment according to the standard developed by the Artificial Liver Group,and Chinese Association of Infectious and Parasitic Diseases,there were 9 patients with clinical healing,25 patients with improvement and 4 patients with no effect,and the cure-improvement rate was 89.5%.Conclusion The novel multi-layer flat-plate BAL could be used as a safe and effective therapy for patients with liver failure.
9. Experimental study of miRNA200a regulating Wnt/β-catenin signaling pathway in silica-induced mouse lung epithelial cells

Objective:
To observe the effect of overexpression of miRNA200a (miR-200a) recombinant lentivirus on the expression of Wnt/β-catenin signaling pathway in mouse lung epithelial cell line MLE-12 induced by silica (SiO2) .
Methods:
The mice were divided into SiO2 control group (SiO2) , virus control group (SiO2+Lv-NC) group and overexpressing miR-200a virus group (SiO2+Lv-miR-200a). The expression of β-catenin, MMP2, MMP9, TCF-4 and Cyclin D1 mRNA and protein were detected by realtime-PCR and western blot after incubating cells for 18 h stimulating at the final concentration of 200 μg/ml of SiO2.
Results:
The expression of miR-200a in MLE-12 cells of SiO2+Lv-miR-200 a group was significantly higher than that in SiO2 group and SiO2+Lv-NC group. The mRNA and protein expression of β-catenin, MMP2, MMP9, TCF-4 and Cyclin D1 in MLE-12 cells of SiO2+Lv-miR-200a group were significantly lower than those in SiO2 group and SiO2+Lv-NC group (
10.The clinical analysis of severe complications induced by esophageal foreign bodies.

OBJECTIVE:
To explore and analyze the clinical characteristics and treatment strategy of severe complications caused by esophageal foreign bodies.
METHOD:
A retrospective study was carried out on 49 cases with foreign bodies in esophagus through careful analysis of their clinical data to explore the associated problems with etiology and therapy. Among this complications, there were cases of 13 periesophageal abscess, 20 cases of abscess in the neck, 11 cases of mediastinal abscess, 3 tracheoesophageal fistula, 1 case of aorta injury and 1 septicemia.
RESULT:
Forty-eight (97.96%) of the patients recovered while one died.
CONCLUSION
Hard esophagoscopy under general anesthesia is the main therapeutic strategy to take out the esophageal foreign bodies. When it failed or severe complications such as perforation or others emerged, open surgery like lateral neck incision or thoracotomy supplemented with positive and timely supporting therapy are vital and essential.
