1.Intrathoracic administration of Kushenhuangqi Injection combined with cisplatin in treatment of malignant pleural effusion

Kushenhuangqi Injection is made from Chinese herb medicine flavescent sophora and astragalns roots.Seventy-one patients with malignant pleural effusion were randomly divided into two groups: 36 patients were treated with both intrathoracic Kushenhuangqi Injection and chemotherapeutic agent cisplatin (treatment group) and 35 patients were treated with cisplatin alone.The response rate of treatment group was significantly higher than that of control group (31/36 vs 20/35,u=3.020,P<0.01).The rate of adverse reaction (leucopenia,nausea,vomiting etc.) in treatment group was significantly lower that that of control group.The results indicate that Kushenhuangqi Injection can enhance the therapeutic effect of chemotherapeutic agent and reduce its adverse reaction in treatment 0f malignant pleural effusion caused by carcinoma of the lungs.
2.Study on the efficacy of ganciclovir in viral keratitis and its influence on serum related indicators

Objective To investigate the efficacy of ganciclovir in viral keratitis and its influence on serum related indicators.Methods Patients with viral keratitis from May 2012 to December 2013 in our hospital were randomly divided into control and observation groups, each group had 62 cases.Patients in control group were treated with acyclovir eye drops, and those in observation group were treated with ganci-clovir ophthalmic gel.The difference in efficacy of two groups was compared.The levels of serum inflamma-tory cytokines of interleukin-2 (IL-2), IL-4, IL-6, IL-8, tumor necrosis factor-α(TNF-α), and interferon-γ( IFN-γ) of two groups before and after treatment were compared, the serum levels of nitric oxide ( NO) , super oxide dismutase (SOD), malondialdehyde (MDA), C3, and C-reactive protein (CRP) were been compared.Results The pain relief time and corneal healing time of observation group [(3.41 ±0.89)d and (4.02 ±1.11)d] were all shorter than control group [(4.52 ±1.21)d and (6.30 ±1.45)d].The cure rate and total effective rate of observation group were 58.06%and 90.32%, respectively.It was sig-nificantly higher than control group ( P <0.05).After 3 and 7 days of treatment, the serum levels of IL-4, IL-6, and IL-8 were lower than control group, and IL-2, and IFN-γwere higher than control group (33.87%and 82.26%) ( P <0.05).After 3 and 7 days of treatment, the serum levels of NO and MDA were lower than control group, and SOD, C3, and CRP were higher than control group ( P <0.05).Con-clusions Ganciclovir has a better efficacy than acyclovir for viral keratitis, and can effectively adjust the serum levels of related indicators.
3.Analysis on Reasonability of TCM Injections Infusion Prescription in Outpatient and Emergency Department of Hospitals in Tianjin

OBJECTIVE: To probe into the safety and reasonability of TCM injection infusion prescription in outpatient and emergency department of hospitals in Tianjin.METHODS: Infusion prescriptions were randomly collected from outpatient and emergency department of secondary and three-tier hospitals in Tianjin.Reasonability of TCM injection intravenous infusion prescriptions were analyzed in respect of dosage and usage,compatibility and administration route.General information of patients and prescription were recorded.Primary data were classified,summarized and screened using EXCEL and the statistic data were calculated using SPSS11.0 software.RESULTS: 11 medical institutions were included and 2 309 TCM injection infusion prescriptions were collected within 4 days.Dosage and dosing interval of reasonable prescription accounted for 66.91% and 80.60% respectively.The safety compatibility proportion of prescriptions was only 58.64%.CONCLUSION: TCM injection infusion prescriptions in outpatient and emergency department of hospitals in Tianjin are reasonable while there is still some unreasonable compatibility of injection.

5.I-stage combination operation of anterior and posterior approaches for anterior and posterior compression of cervical spinal cord

Objective To explore the surgical treatments through anterior and posterior approaches for anterior and posterior compression of cervical spinal cord. Methods The clinical data of 25 cases of anterior and posterior compression of cervical spinal cord from June 1999 to November 2003 were summarized retrospectively. There were 18 males and seven females with age range of 28-56 years (average 36.4 years). Of all,18 cases were with tricolumnae fractures of cervical spine caused by traffic injury,five with cervical herniation and two with malignant macrophage tumor in vertebral body and annex and vertebral lamina. Results All 25 cases underwent internal fixation decompression with I-stage combination operation of anterior and posterior approaches. Of 18 cases with tricolumnae fractures,one died and 17 recovered in various degrees. In five cases with cervical herniation,the positive sign disappeared basically. Two cases of malignant giant-cell tumor appeared metastatic carcinoma in the lungs 10 months after operation. Conclusions The I-stage combination operation of anterior and posterior approaches is an ideal way for the patients suffering from anteropsterospinal compression in cervical spinal cord. It can not only decompress completely the cervical spinal cord,stabilize the wounded cervical vertebra,but also facilitate nursing and function training and be beneficial to the recovery of spinal cord function.
6.Microsurgical treatment of leg fracture with severe sofe tissue defects

Objective To investigate the effective method of the treatment of leg fracture with severe sofe tissue defects by microsurgery. Methods Seventy-five cases of the leg fracture with severe sofe tissue defects were treated by completed debrided,used different fracture fixation and surgical flaps. Results Only partial necrosis happened in 1 case,and all the other flaps survived. The appearance and functions were achieved after 6 months to 4 years of follow-up. Conclusion Selecting different fracture fixation and microsurgical techniques flap improve successful rates,markedly reduce the deformity rates and complications of leg fracture with severe sofe tissue defects.

The objective of this study is to explore the application possibility of chitosan/pcDNA-EGFP-TGFPbeta1 nanoparticles in the transfection of synovial-derived mesenchymal stem cells (SDMSCs). Chitosan/pcDNA-EGFP-TGFbeta1 nanoparticles were fabricated through method of ionic crosslinking. The SDMSCs were harvested from rabbit joints and cultured to passage 3. The SDMSCs were then transfected with chitosan/pcDNA-EGFP-TGFbeta1 nanoparticles. Scanning electronic microscope (SEM) was employed to detect the shape and diameter of the nanoparticles. The transfected SDMSCs were examined under the fluorescence microscope and detected through the flow cytometry (FCM). The SEM examination showed that the contour of the fabricated chitosan/pcDNA-EGFP-TGFbeta1 nanoparticles was round and its average diameter was 50 nm. After being cultured for 48 h, the SDMSCs transfected by chitosan/pcDNA-EGFP-TGFbeta1 nanoparticles could be detected under the fluorescence microscope, and the live SDMSCs could also be examined through FCM. The transfection rate was 8% - 10%. Therefore, it suggested that the chitosan/pcDNA-EGFP-TGFbeta1 nanoparticles fabricated through the method of ionic crosslinking could transfect the SDMSCs, but the transfection rate should be improved.

8.Clinical characteristics of severe acute respiratory syndrome coronavirus 2 vaccine breakthrough infections in children

Objective:To analyze the clinical characteristics of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) vaccine breakthrough infections in children, and to provide reference basis for the SARS-CoV-2 vaccination in children.Methods:A total of 97 children aged 3 to 14 years and diagnosed with coronavirus disease 2019 (COVID-19) admitted to Xi′an People′s Hospital (Xi′an Fourth Hospital) from December 27, 2021 to February 7, 2022 were included. According to the COVID-19 vaccination status, the enrolled children were divided into unvaccinated group, partially vaccinated group and fully vaccinated group, and the clinical data of the children in the three groups were collected and compared. Chi-square test, two independent sample t-test and Kruskal-Wallis H test were used for statistical analysis. Results:Totally 97 children including 49 males and 48 females were enrolled, with 87(89.7%) children of mild type, 10(10.3%) children of common type, and no severe or critical case. The proportions of unvaccinated, partially vaccinated and fully vaccinated preschool-aged children (3 to 6 years old) were 56.5%(13/23), 30.8%(12/39) and 17.1%(6/35), respectively, while those of school-aged children (7 to 14 years old) were 43.5%(10/23), 69.2%(27/39) and 82.9%(29/35), respectively. The vaccination proportion in preschool-aged children was significantly lower than that in school-age children ( χ2=9.94, P=0.007). The proportion of the children with fever in fully vaccinated group was 17.1%(6/35), which was lower than that in unvaccinated group (43.5%, 10/23), and the difference was statistically significant ( χ2=4.82, P=0.028). The cycle threshold (Ct) values of the open reading frame ( ORF)1 ab gene in the unvaccinated, partially vaccinated and fully vaccinated groups were 33.77(26.87, 36.58), 35.23 (33.45, 38.57) and 37.12 (34.91, 39.39), respectively, and there was a statistically significant difference among the groups ( H=7.76, P=0.021). The Ct values of the nucleocapsid protein ( N) gene in the three groups were 32.26(25.85, 36.18), 35.12(33.18, 37.96) and 37.26(34.27, 39.24), respectively, and the difference among the groups was statistically significant ( H=7.84, P=0.020). The Ct values of ORF1 ab gene and N gene in fully vaccinated group were higher than those in unvaccinated group, and the differences were statistically significant ( Z=-2.69, P=0.007 and Z=-2.39, P=0.017, respectively). The duration of viral shedding in fully vaccinated children was (9.9±4.1) d, which was shorter than that in unvaccinated children ((12.8±3.7) d), and the difference was statistically significant ( t=2.72, P=0.009). Conclusions:The majority of children with breakthrough infections with SARS-CoV-2 are mild. Vaccination may effectively shorten the duration of viral shedding. And fully vaccination is associated with mild clinical symptoms and lower serum viral load compared to unvaccinated children.
9.Meta-analysis on Xixian Tongshuan Preparation Combined with Conventional Western Medicine in the Treatment of Acute Ischemic Stroke

Objective To systematically evaluate the efficacy of Xixian Tongshuan Capsules/Pills in the treatment of acute ischemic stroke(AIS).Methods Literature about Xixian Tongshuan Preparation combined with conventional Western medicine for the treatment of AIS was retrieved from CNKI,SinoMed,VIP,Wanfang Data,PubMed,Medline,Embase,Cochrane Library and Web of Science from establishment of the databases to February 28,2023.Meta-analysis was conducted for the studies that could be quantitatively analyzed.The effective rate and response indicators were combined.Results A total of 7 articles were included for Meta-analysis.Results showed that there was statistical difference in the effective rate(RR=0.34,95%CI[0.23,0.51],P<0.01),NIHSS score(MD=-2.90,95%CI[-3.74,-2.06],P<0.01),BI score(MD=-10.08,95%CI[-13.47,-6.68],P<0.01),FIB(MD=-1.18,95%CI[-1.59,-0.77],P<0.01)of Xixian Tongshuan Preparation combined with conventional Western medicine for the treatment of AIS.There was no statistical difference in IL-6(MD=-15.4,95%CI[-33.3,2.49],P=0.09).There was no statistical difference in the effects of different dosage forms and treatment courses on the effective rate and NIHSS score.Conclusion The combination of Xixian Tongshuan Capsules/Pills could better improve the NIHSS and BI scores of patients with AIS,recovery the neurological function,and reduce the risk of blood hypercoagulability by reducing FIB content,with good safety.
10.Meta-analysis of Efficacy and Safety of Compound Duzhong Jiangu Granules in the Treatment of Knee Osteoarthritis

Objective To systematically evaluate the efficacy and safety of compound Duzhong Jiangu granules in the treatment of knee osteoarthritis.Methods According to PICOS principle,literature retrieval was carried out in CNKI,SinoMed,VIP,WanFang,PubMed,Medline,Embase and Cochrane Library,and the retrieval time was up to October 2023.Meta-analysis was performed on quan-tifiable studies,and the effective rate and efficacy indicators were combined.Results A total of 5 articles were included for Meta-anal-ysis.The effective rate of compound guzhong Jiangu granules was higher than that of control group,the difference was statistically signifi-cant(RR=1.12,95%CI:1.07-1.18,P<0.001).Compared with the control group,there were significant differences in pain or discomfort scores during bed rest at night,standing from seat score,maximum walking distance score,getting off standard ladder score,walking on uneven road score and WOMAC score between compound duzhong jiangu granules and the control group(P<0.01).There were no significant differences in improving morning stiffness or pain after getting up,boarding standard ladder score and joint function ef-ficacy score between compound Duzhong Jiangu Granules and control group(P>0.05).There were no serious adverse events were repor-ted in the included studies.Conclusion Combined with compound guzhong jiangu granules can improve the therapeutic effect of knee os-teoarthritis,relieve a mild to moderate pain to a certain extent,and promote the recovery of joint function,but it is not good for improving morning stiffness,so it is recommended for early combined treatment with good safety.
