1.Application of an intrauterine system for controlled release of levonorgestrel in perimenopausal women

Objective To investigate the effect of an intrauterine system for controlled release of levonorgestrel (LNC-IUS) in perimenopausal women.Methods A total of 105 perimenopausal patients (aged from 42 53 years with a mean age of 45.8±2.9 years) treated with LNGr-IUS in Beijing Hospital were enrolled in this study.Retrospective analysis concerning reasons for device placement,clinical effects and adverse reactions was conducted.Results Of the patients,25 (23.8%) had menorrhagia,38 (36.2%) had dysmenorrhea,16 (15.2%) had a combination of menorrhagia and dysmenorrhea,9 (8.6%) had undergone cystectomy for endometriosis followed by gonadotropinreleasing hormone-analogue (GnRH-a) therapy,13 (12.4 %) had simple endometrial hyperplasia,and 4 (3.8%) had leiomyoma combined with systemic disorders.A statistical difference in the average hemoglobin level was found in menorrhagia patients with anemia before and after treatment [(99.4 ±19.1) g/L vs.(123.3±24.5) g/L,P<0.01].Dysmenorrhea symptom scores decreased six months after treatment[(5.9± 1.0) vs.(3.0±0.7)],P<0.05).No adnexal masses were found on Bultrasound 12 months after treatment in patients who had undergone cystectomy for endometriosis.For patients with simple endometrial hyperplasia,diagnostic dilation and curettage 12 months after treatment showed normal endometrial morphology,with a total effectiveness rate of 100%.No adverse reactions were found.Conclusions The intrauterine levonorgestre[-releasing system is effective in the treatment of menorrhagia,dysmenorrhea and simple endometrial hyperplasia,and can prevent the recurrence of endometriotic cyst in perimenopausal women.
2.The effects of postburn fluid resuscitation on the pathohistological and ultrastructural changes of multiple organs with dysfunction in severely burned dogs.

OBJECTIVETo investigate the effects of postburn fluid resuscitation on the pathohistological and ultrastructural changes of multiple organs with dysfunction in severely burned dogs.
METHODSForty - four mongrel dogs were randomly divided into four groups: (1) immediate infusion (II, n = 8), (2) delayed infusion (DI, n = 15), (3) no infusion (NI, n = 14), (4) normal control (NC, n = 7). The dogs were inflicted with 50% TBSA III degree flame burn produced by napalm in concentration of 30g/L burning for 30 seconds on the back. Small pieces of tissue samples of heart, lungs, liver, kidneys and gastrointestinal tract were taken from injured dogs at 72 postburn hours (PBHs) or moribund stage for the examination with light microscope (LM) and transmission electron microscope (TEM).
RESULTSDifferent degrees of blood circulation disturbance and degenerative changes were found in all above internal organs. These changes were more evident in DI than in II and NI groups.
CONCLUSIONDelayed postburn fluid resuscitation could induce multiple organ dysfunction in early postburn stage.

5.Genetic analysis of a family affected with pulmonary hypertension secondary to hereditary hemorrhagic telangiectasia.

OBJECTIVETo carry out genetic testing for a family affected with pulmonary hypertension (PH) as the initial sign of hereditary hemorrhagic telangiectasia (HHT).
METHODSHigh throughput sequencing was performed to detect potential mutation in the coding regions of endoglin (ENG), activin receptor-like kinase 1 (ACVRL1) and mothers against decapentaplegic homolog 4 (SMAD4) genes.
RESULTSA pathogenic heterozygous c.814C>T (p.Gln272Ter) mutation of the ACVRL1 gene was identified in the proband. Her mother and two sons have carried the same mutation.
CONCLUSIONThe c.814C>T (p.Gln272Ter) mutation of the ACVRL1 gene probably underlies the disease in this family. Genetic testing should be recommended to HHT patient, in particular those with pulmonary hypertension.

6.Study on the mechanism of herb cake-partitioned moxibustion inhibiting tumor growth in colitis-associated colorectal cancer based on KDM4D receptor

Objective:To explore the inhibitory mechanism of herb cake-partitioned moxibustion on tumor growth in colitis-associated colorectal cancer(CAC)based on histone lysine demethylase 4D(KDM4D). Methods:Inbred male Sprague-Dawley rats were randomly divided into a normal group,a CAC group,a herb cake-partitioned moxibustion group,and an inhibitor group.Except the normal group,rats in the other three groups were treated with azoxymethane(AOM)combined with dextran sulfate sodium(DSS)to make CAC rat models.Rats in the normal group and the CAC group did not receive interventions;rats in the herb cake-partitioned moxibustion group received moxibustion at Qihai(CV6)and bilateral Tianshu(ST25),2 cones for one point each time,once a day for 30 d with 1-day rest every week;rats in the inhibitor group received intraperitoneal injection of KDM4D inhibitor,5-chloro-8-hydroxyquinoline(5-c-8HQ),once a day for 30 d.After intervention,the general condition,colon length,tumor number and volume,and histopathological colon changes were observed.The expression of adenomatous polyposis coli(APC),axis inhibitor(Axin),cyclin D1,matrix metalloproteinase(MMP)-7 and MMP-9 mRNAs were detected by real-time quantitative polymerase chain reaction.The proliferating cell nuclear antigen(PCNA),cleaved caspase3,KDM4D,APC,and Axin proteins were detected by immunohistochemistry. Results:Compared with the normal group,the general condition was poor,the colon length was significantly shortened(P<0.01),the number and volume of colonic tumors were increased(P<0.01),the structure of glandular duct was obviously disordered with"back-to-back"and cowall phenomenon,and also high-grade adenocarcinoma formed;the protein expression levels of PCNA and KDM4D were significantly increased(P<0.01),while cleaved caspase3,APC,and Axin were significantly reduced(P<0.01);the mRNA expression levels of cyclin D1,MMP-7,and MMP-9 were significantly increased(P<0.01),while APC and Axin were significantly reduced(P<0.01)in the CAC group.Compared with the CAC group,the general condition was improved,the length of colon was significantly increased(P<0.01),the number and volume of the colonic tumors were reduced(P<0.05),and the colon tissues showed epithelial cell proliferation with enlarged and deep staining nuclei,dysplasia and inflammatory cell infiltration;the protein expression levels of PCNA and KDM4D were significantly reduced(P<0.01),while the cleaved caspase3,APC,and Axin were significantly increased(P<0.01);the mRNA expression levels of cyclin D1,MMP-7,and MMP-9 were reduced(P<0.05),while the APC and Axin were increased(P<0.05)in the colon tissues of rats in the herb cake-partitioned moxibustion group and the inhibitor group. Conclusion:Herb cake-partitioned moxibustion regulated abnormally expressed KDM4D in CAC rats,activated APC and Axin,the upstream molecules of Wnt/β-catenin pathway,inhibited abnormally activated downstream molecules of Wnt/β-catenin pathway.This may be a key mechanism of herb cake-partitioned moxibustion in inhibiting CAC tumor growth.
7. Comparison of two imaging diagnosis methods in the diagnosis of pulmonary thesaurosis induced by dust of iron and its compounds

OBJECTIVE: To compare the diagnostic value of high-k V X-ray chest photography and chest high-resolution computed tomography( HRCT) in the diagnosis of pulmonary thesaurosis induced by dust of iron and its compounds(hereinafter called the iron pulmonary thesaurosis). METHODS: A total of 80 workers exposed to iron oxide dust in a magnetic material factory were collected as the study subjects by convenience sampling method,and they were examined with high-k V X-ray chest photography and HRCT. Differences between the two methods in the diagnosis of iron pulmonary thesaurosis were compared. RESULTS: Of the 80 workers,only 5 of them(6. 2%) showed no abnormal changes in chest HRCT,others showed varying degrees of diffuse distribution of air-cavity nodules and ground-glass lesions. High-k V X-ray chest photography and chest HRCT diagnosis of iron pulmonary thesaurosis accounted for 8. 8%( 7/80) and 37. 5%(30/80) with extreme mild degree,and 21. 3%(17/80) and 26. 3%(21/80) with mild degree respectively. The diagnostic rates for iron pulmonary thesaurosis were 30. 0%( 24/80) and 63. 8%( 51/80),respectively. There was consistency between the two methods for the diagnosis of iron pneumoconiosis( Kappa = 0. 411,P < 0. 01). The chest HRCT has a higher diagnostic classification and diagnosis rate compared with the high-k V X-ray chest photography( P <0. 01). CONCLUSION:s The chest HRCT has a higher diagnostic grade and higher diagnostic rate for lung siderosis compared with the high-k V X-ray chest photography,which is helpful for the early diagnosis of the disease.
