1.Clinical analysis with subacute necrotizing lymphadenitis

Objective To investigate etiological factors, clinical characteristic and related laboratory findings of subacute necrotizing lymphadenitis and improve the acknowledgement of subacute necrotizing lymphadenitis. Methods We analyzed 14 cases of subacute necrotizing lymphadenitis who had been diagnosed by lymph node biopsy retrospectively. Results The most of patients were school children with mean age of 10.4 ± 2.9) years. The ratio of male to female was 1.8∶1 .The main clinical symptoms were that lymph nodes were swelling and pressing-pain in 14 cases (100%) and fever in 13 cases (92.9%), rash in 2 cases (14.3%),hepatosplenomegaly in 2 cases (14.3%).The white blood cell count was (2.3 ~ 7.4) × 109/L in 14 cases and average count was (3.90 ± 0.35) × 109/L. The white blood cell count showed leukopenia in 10 cases (71.4%) and none showed heighten. Twelve cases were tested by CRP and the result of 8 cases elevated slightly (10 ~ 30 mg/dl). Thirteen cases underwent ESR examination and the result of 11 cases showed higher than that of normal level (20mm/h)(82.6%). The average ESR risen value was 40mm/h and topmost value was 70 mm/h. Eight cases underwent anti-nuclear antibody test and 2 cases of them were positive. Twelve cases underwent EB virus antibody test and 4 cases of them were positive. Eleven cases underwent blood Mycoplasma-IgM examination and one of them was positive. Six cases underwent coxsackie virus antibody and one of them was positive.Coxsackie and EB virus infected in one case. Lymph node biopsy was found necrotic fragments in different sizes. The cellular hyperplasia was found around the necrotic area and plasma cells, neutrophils, and eosinophils were found rarely in these areas. The DNA of EB virus was negative in lymph nodes. Eight cases of all were treated by prednisone and effect showed well 1.5 ~ 2.0 mg/(kg·d)). One of all was recurrence and the rest recovered well. Blood routine examination was used when they were under a stable condition, five cases of them showed normal white blood cells. Conclusion Those with fever of unknown origin combined with lymphadenectasis are suggested to have an early lymph node biopsy. The process is self-limited and glucocorticoid therapy is effective to those impatients. The long-term follow up is required as recurrence and development of SLE have reported. EB virus was not an etiological factor of SNL. Leucocyte can be used as one of the indicators of improvement.

OBJECTIVE:To establish a method for determination the genotoxicity impurities (methyl methanesulfonate,ethyl methanesulfonate and isopropyl methanesulfonate) in mesylate nafamostat raw materia. METHODS:GC-MS was conducted,and the genotoxicity impurities were extracted by dichloromethane. The column was DB-5 capillary column by programmed tempera-ture,the inlet temperature was 240 ℃,column flow was 3.0 ml/min,purge flow was 6.0 ml/min,sample mode splitless injection, carrier gas was high purity helium,detector is a mass spectrometer detector,ion source temperature was 230 ℃,the interface tem-perature was 230 ℃,the delay time of solvent was 2.5 min,ionization mode was electron impact,detector voltage was respect to the tuning results,scanning(detection)method was selective ion monitoring,electron energy was 70 eV,and the injection volume was 1.0μl. RESULTS:The separation degree of 3 impurities were greater than 2.0;the linear range of 3 impurities were 0.10-20μg/ml (r≥0.999 5);RSDs of precision,stability and reproducibility tests were lower than 2%;recoveries were 97.7%-104.8%(RSD=2.8%, n=9),102.5%-110.7%(RSD=2.6%,n=9)and 103.0%-107.6%(RSD=1.6%,n=9). CONCLUSIONS:The method is simple, accurate,sensitive and rapid,and can be used for the genotoxicity impurities in mesylate nafamostat raw materia.
4.Therapeutic Observation on Female Rats in Immune Sterility Treated by Zhuyun Decoction

[Objective] To observe the effect of Zhuyun Decoction on immune sterility rats.[Method] Set up groups of normal saline,model,Zhuyun Decoction and prednisone,make models of every group by injecting subcutaneously suspension of sperm and Fu's adjuvant,etc.into female SD rats along the groin,NS into normal saline group.After 4 weeks,the groups of normal saline and model take NS,Zhuyun Decoction for Zhuyun Decoction group,prednisone for prednisone group.After 2 weeks,they are closed together with male ones.Before the first immune and 1 week after strengthened immune,take blood and measure AsAb with ELISA method,measure the immune integral of IgG and IgA in uterus tunica intima with SABC method;meanwhile,compare the pregnant rats and average nidation points in all groups.[Result] In the Zhuyun Decoction group,the pregnant rats and nidation points are much more that other groups;Zhuyun Decoction can reduce IgG and IgA integral in uterus tunica intima.[Conslusion] Zhuyun Decoction has definite effect on immune sterility rats,whose mechanism may be related with regulation of IgG and IgA integral in uterus tunica intima.
5.Effect of anti-KDR antibody on the proliferation of hemangioma vascular endothelial cells in vitro.

The suppressive effect of anti-KDR antibody against VEGF on proliferation of hemangioma-derived vascular endothelial cells (HVECs) was investigated. HVECs from one case of hemangioma in proliferative phase were cultured. Both primary culture and sub-culture were conducted in M199 medium. The HVECs of passage 3 were divided into 4 groups based on the concentrations of anti-KDR antibody. Cell count was performed and inhibitory rate of HVECs was measured before and 9 days after interference. The results showed that the number of HVECs in the anti-KDR antibody-treated groups was significantly decreased and the inhibitory rate of HVECs by anti-KDR antibody (50, 10 and 2 microg/mL) was 84%, 63% and 39% respectively at 9th day after interference, with the difference being significant. In the control group, the number of HVECs was increased significantly. In was concluded that the anti-KDR antibody could suppress the activity of VEGF through blocking the KDR, indicating the potential clinical applications of anti-KDR antibody in the treatment of hemangioma.

6.THE RELATION BETWEEN THE OSTEO-FIBROUS CANAL OUTSIDE OF THE LUMBAR SPINAL CANAL AND THE SPINAL NERVE PASSING THROUGH IT. Ⅰ. THE MORPHOLOGY OF THE OSTEO-FIBROUS CANAL OF THE MEDIAL DIVISION OF POSTERIOR RAMUS OF THE LUMBAR NERVE WITH SPECIAL REFER

1. 40 cadavers were dissected in order to observe the position and formation of the osteo-fibrous canal (384 canals) of the medial division of posterior ramus of lumbar nerves together with the occurrence of the articulo-accessory (process) ligament.2. The form of the external opening of the osteo-fibrous canal may be classified into three groups; nearly circular 35.1% (151); approximately ellipsoid 32.3% (124); crack detector 28.4% (109).3. The osteo-fibrous canal consists of four walls, bounded by mammillary process, accessory process, bony wall and superior articular process ligament respectively. According to the constitution of the anterior and posterior walls of the canal, it may be divided into three categories; osteo-aponeuroticoligamentous type 29.4% (113); osteo-membranous type 66.4% (255) and bony type 4.2% (16).4. The length and width of the superior articular process ligament were measured. The sexual difference of the lengths of L_(1~5) lumbar is significant.5. The sulcus between the superior articular process and the alar portion of the sacrum was observed and measured.6. The ligament on the posterior wall of the canal is nominated as the ligament of the superior articular process.7. Based on the anatomical structures thus studied, some clinical significances were discussed.
7.Study of Extraction Process of Polysaccharide from Seeds of Toona sinensis and Antioxidant Activity in Vitro

Objective To optimize the conditions of extraction process of polysaccharides from seeds of Toona sinensis; To preliminary determinate the antioxidant activity in vitro. Methods The content of polysaccharides from Toona sinensis was chosen as evaluation index; the extraction time, material liquid ratio, and extraction times were chosen as factors; L9(34) orthogonal design was used to optimize extraction process; antioxidant activity of polysaccharides from Toona sinensis was investigated by the total reducing power and 1,1-diphenyl-2-picryhydrazyl (DPPH) method. Results From the statistical data of processing, the optimal extracting conditions were as follows:liquid to material was 1:12, extraction time was 120 min, and extracted for 3 times. Polysaccharides from seeds of Toona sinensis had certain reducing capacity and had better scavenging ability on DPPH? and the scavenging rate significantly increased with the concentration. The effect was simulated by curve equation. Conclusion The process is simple, feasible, stable and reliable and can be used for the small-scale study on extraction of polysaccharides from seeds of Toona sinensis. Polysaccharides from seeds of Toona sinensis have antioxidant activity.
8.Topical propranolol hydrochloride gel for superficial infantile hemangiomas.

The clinical efficacy and safety of topical propranolol hydrochloride gel in the treatment of superficial infantile hemangiomas (IHs) were assessed. Fifty-one cases of IHs from Oct. 2010 to Sept. 2011 were subjected to the topical propranolol hydrochloride gel intervention in Fuzhou General Hospital of Nanjing Military Commands, China. Changes in size, texture, color, peak systolic velocity of the hemangiomas, resistance index and adverse effects were observed. The results were evaluated by using Achauer system, and responses of IHs to pranpronolol were considered scale II (poor) in 4 patients (17.24%), scale II (moderate) in 18 patients (24.14%), scale III (good) in 22 patients (44.83%) and scale IV (excellent) in 7 patients (13.79%). The response of superficial hemangiomas was significantly better than other hemangiomas (P<0.05), and no differences in response were found among different primary sites (P>0.05). Our study indicates that topical application of 3% propranolol hydrochloride gel is effective and safe in treating IHs.

1. 40 cadavers were dissected in order to observe the position, formation and morphology of the osteo-fibrous canal (400 canals) of the posterior rami of the lumbar spinal nerves.2. The average lengths of the osteo-fibrous canals for the posterior ramus of the lumbar nerves in both sexes are: L_1 4.88mm, L_2 4.71 mm, L_3 4.61 mm, L_4 4.57mm, L_5 4.39 mm.3. The average longitudinal diameter of the outlets of the osteo-fibrous canals of the posterior rami of the lumbar nerves in both sexes are: L_1 6.38mm, L_2 6.73 mm, L_3 6.34mm, L_4 5.75mm, L_5 2.61mm.The average transverse diameters of the outlets of the osteo-fibrous canals of the posterior rami of the lumbar nerves in both sexes are: L_1 2.69mm, L_2 2.66mm, L_3 2.68mm, L_4 3.14mm, L_5 2.06mm.4. The shapes of the outlets of the osteo-fibrous canals of the posterior rami of lumbar nerves may be classified into four types: nearly circular 64.75% (259 sides); approximately ellipsoid 30.25% (121 sides); nearly triangular 3.00% (12 sides); nearly oblate 2.00% (8 sides).5. 320 inter-transverse ligaments were observed and measured.6. There was always an iliolumbar ligament interposed between L_5 and S_1.7. Further more, the authors discussed the relationship between the morphology of the nerves and the osteo-fibrous canals in connection with the possible causes of low back pain.
10.The value of renal CT arteriography in the pre-operation evaluation of patients with renal tumors

Objective To investigate the value of renal CT arteriography in the preoperation evaluation of patients with renal tumors.Methods From Jan 2007 to June 2012,372 patients with renal tumors took renal CT arteriography (with Philips Brilliance 16) to evaluate the variations of their renal arteries.We executed plain scan first,then artery phase,venous phase scan and delaying scan by turns after injections of Omnipaque.All images were delivered to imaging workstation and reconstructed to 3D views of renal arteries to supply imageological supports for surgeons.Results Tumors in solitary kidneys were detected in 21 patients,and masses in both kindneys were detected in 4 cases.We collected data of 376 kidneys finally.We found premature branching renal arteries on 46 kidneys (12.2%) and accessory renal arteries on 74 kidneys (19.7%).Double accessory renal arteries were seen on 10 kidneys (2.7%),and there were 84 accessory renal arteries in summary.7 accessory renal arteries (7.2%) came from arteria phrenica and 77 (91.7%) came from abdominal aorta (x2 =65.68,P < 0.01).We found the regions where the accessory renal arteries entered kidneys:65.5% (55/84) in upper poles,19.0% (16/84) in central regions and 15.5% (13/84)in inferior poles.Routine CT scans found 44 variations of renal arteries in 218 cases who underwent surgeries,and then renal CT arteriography found 63 variations in the same cases (20.2% vs 28.9%,x2 =4.47,P < 0.05).Finally,surgeons found 60 variations of renal artries in operations in the 63 cases with variations found by renal CT arteriography before surgeries.Conclusions There is limitations of routine CT scan (plain scan and with contrast medium) in detection of renal artery variations.Renal CT arteriography can supply reliable supports for establishment of surgical plans for renal tumors.
