1.Early Intervention of Systematic Rehabilitation for Elbow Joint Dysfunction after Trauma

Objective To investigate the early rehabilitation for elbow joint dysfunction after trauma.Methods26 cases who accepted rehabilitation 7~30 d after trauma were determined as Group A,other 20 cases who accepted rehabilitation more than 30 d after trauma as Group B.They were evaluated with Mayo Elbow-Performance Score and range of motor(ROM)after systematic rehabilitation.ResultsThe scores of Mayo Elbow-Performance Score and the ROM were better in group A than in group B(P<0.05).ConclusionEarly rehabilitation is important for the elbow joint function recovery after trauma.
2.Detection of telomerase activity in the specimen lung nodules through CT-guided percutaneous transthoracic needling biopsy

Objective To invstigate the diagnostic value of the detection of telomerase activity in the specimen obtained from CT guided percutaneous transthoracic needle biopsy of lung nodules.Methods The techniques of TRAP silver staining were performed to detect telomerase activity in 43 cases. Results 43 mass cases were studied includirg 32 proved to be primary lung cancer, and 11 benign lesions cytologically and clinically. Telomerase activity was detected as 28(87.5%) of 32 lung carcinomas, whereas in benign lung lesions was 1(9.1%) in 11 cases.Conclusions Detection of telomerase activity in the specimen of lung nodules by CT guided percutaneous transthoracic needle biopsy may be an effective method in the diagnosis of lung cancer and also as tumor marker.
3.Comparing Results with Immature Germinal Vesicle Oocytes From Different In-vitro Fertilization Cycles

[Objective] This study compared outcomes of in vitro maturation (IVM) and in vitro fertilization (IVF) intracytoplasmic sperm injection (ICSI) cycles after IVM of immature germinal vesicle (GV) oocytes.[Methods] ICSI was performed on metaphase II (MII) oocytes retrieved in 163 IVF-ICSI cycles (group I;n = 987) or matured from GV stage oocytes in IVF-ICSI ( group II;n = 132) and 37 IVM cycles ( group III;n = 235).Fertilization and cleavage rates and embryo quality were compared among the three groups.[Results] The fertilization rate,cleavage rate and top quality embryos rate were higher in group I than group II and group III (84.9%,98.1%,and 61.6%;72.0%,90.5% and 22.1%l;75.3%,94.4%,and 25.1%,respectively).Blastomere numbers and morphology scores were highest in group I (P < 0.05),but no significant differences existed between group II and group III.[Conclusion] The morphology of embryos developed from in vivo MII oocytes was superior to those from in vitro matured MII oocytes.No significant difference was observed in embryo morphology from immature GV oocytes in IVF and IVM cycles.

5.Efficacy and safety of routine intraoperative insertion of double-J stents in kidney transplantation;a systematic review

Objective To systematically evaluate the effect of routine insertion of double-J stents to prevent major urological complications(MUCs)in kidney transplant recipients.Methods Medline,Embase,Cochrane Library,and Chinese Biomedicine database were searched to locate relevant randomized controlled trials(RCT).Data extraction and assessment of methodologic quality were performed independently by two reviewers.Meta-analysis was performed by Revman 5.0 software.Results Ten RCT(including 1616 patients)were identified.By comparing the routine stent group with the no stent group,the meta-analysis showed:(1)incidence of urine leak,urinary obstruction and UTI was 4 times lower,6 times lower,increased by 52 % respectively(P<0.0001);(2)Patient and graft survival,rate of acute rejection,delayed graft function and hematuria were of no significant difference.In subgroup analysis,it was found:(1)Compared with the no stent group,the group in which stent duration was≤ 4 weeks had a lower incidence of MUCs and a higher incidence of UTI;meanwhile,the group in which stent duration was > 4 weeks had a much lower incidence of MUCs and the rate of UTI was increased without significant difference;(2)In the RCT of which urethral catheter duration was < 5 days,there were no significant differences between the two groups in MUCs and UTI.In the RCT of which urethral catheter duration was ≥5 days,the stent group had a lower incidence of MUCs and a higher incidence of UTI.Conclusion Routine stenting reduces the incidence of MUCs.Although the double-J stent increases the risk of UTI,it seems that UTI doesn't affect the outcome of transplantation.The stent duration should be within 4 weeks.For the stent recipients,the longer duration of urethral catheter,the lower incidence of MUCs,the higher incidence of UTI;thus,it is up to clinicians to decide the optimal duration of urethral catheter.Long term prescription of 480 mg cotrimoxazole once daily,from the operation day till after stent removal,effectively reduces the risk of UTI associated with stent placement.

Objective The aim of this study is to evaluate the efficacy and safety of preoperative CT-guided hardening agent localization.Methods From December 2010 to January 2012,27 patients with 29 solitary pulmonary nodules who had undergone CT-guided hardening agent localization and video-assisted thoracoscopic surgery (VATS) were studied.Results All cases were underwent CT-guided hardening agent localization successfully,and no patient had serious complication that required any intervention.The diameter of nodules ranged from 3 to 21 mm as measured by CT[mean (11.27 ± 6.32) mm].The distance between the center of nodule and visceral pleural ranged from 4 to 38 mm[mean (14.45 ± 4.32) mm].Conversion from VATS to thoracotomies was not necessary during the diagnostic resection procedure nodules.29 solitary pulmonary nodules underwent thoracoscopic wedge resection,and no intra-or postoperative mortality or morbidity was recorded.Conclusion CT-guided hardening agent localization before video-assisted thoracoscopic solitary pulmonary nodule resection is a safe and effective procedure for accurate diagnosis and resection of indeterminate solitary pulmonary nodules.
7.Preimplantation Genetic Diagnosis for Robertsonian Translocation Carrier

【Objective】To perform preimplanation genetic diagnosis by using dual color fluorescent in-situ hybridization (FISH).【Methods】The chemical and mechanical division methods were used to perform embryo biopsy in 3 cases of Robertsonian translocation t (13q14q).Vysis LSI 13q14 and Tel Vysion 14q probes were used to detect the blastomeres biopsied from the IVF embryos of the patients.FISH analysis was performed to select normal or balanced karyotype embryos ,which then were transfered into the uterus.【Results】Total of 23 oocytes were retrieved in 3 treatment cycles.Fertilization rate was 79%.14 embryos were available for embryo biopsy.Among them,9 embryos were biopsied by chemical division method,with further cleavage rate of 67%;5 embryos were biopsied by mechanical division method,with further cleavage rate of 40%.Single embryo was diagnosed as normal karyotype or balanced respectively in 2 treatment cycles.Both of them were transfered into the uterus.One clinical normal on-going pregnancy was achieved,the diagnosis was confirmed by amniocyte karyotype analysis.【Conclusion】Preimplanation genetic diagnosis can be used to resolve the problem of fertility for Robertsonian Translocation Carriers.

Abstract
Cyberbullying endangers the physical and mental development of students, and it is important to carry out research in this area to safeguard their lives and well being. To reduce the incidence of cyberbullying among students, Canada favors a comprehensive approach and has implemented a series of comprehensive treatment measures aimed at cyberbullying. These measures cover the formulation of strict and distinctive comprehensive laws and regulations, the publication of specific anti cyberbullying reports with clear terms, the implementation of a variety of prevention and control programs that also aim to unite social groups. The existing problems and challenges mainly include an imperfect legal system, and a lack in terms of an adequate control and evaluation mechanism that targets cyberbullying. Canada s comprehensive treatment approach to address cyberbullying provides a referential framework for China.
9.Clinical value of seminal paraoxonase-1 activity evaluation in the diagnosis of male infertility.

OBJECTIVETo investigate the changes in seminal paraoxonase-1 (PON-1) activity in infertile male patients and assess the clinical value of seminal PON-1 examination in the diagnosis of male infertility.
METHODSSeminal PON-1 activity was detected by spectrophotometric method in the semen samples from 270 infertile male patients and 50 health fertile males (control), and the semen parameters were analyzed using a computer-assisted semen analysis system.
RESULTSIn the male infertility group, seminal PON-1 activity was 1.22∓0.76 U/L in the patients with normal semen parameters and 0.64∓0.54 in the patients with abnormal semen parameters, both significantly lower than that of the control group (3.17∓0.89 U/L, P<0.01). In patients with asthenospermia, the declined sperm motility was associated with decreased seminal PON-1 activity, which showed significant differences between patients with mild, moderate, and severe asthenospermia. Seminal PON-1 activity was positively correlated with the percentage of sperm viability (P<0.01), but inversely with the percentage of morphologically abnormal sperm (P<0.01). According to ROC curves, the area of seminal PON-1 activity under the curve was 0.907, showing a statistical significance (P<0.01).
CONCLUSIONThe detection of seminal PON-1 activity can provide a laboratory evidence for the diagnosis of male infertility.

10. Mild hypothermia can delay the occurrence of post-stroke infection: a propensity score matched-cohort study

Objective:
To evaluate the effect of mild hypothermia on the incidence of post-stroke infection and explore the relationship between mild hypothermia and outcome of stroke patients by using propensity score matching.
Methods:
Patients hospitalized in department of intensive care unit (ICU), neurology and neurosurgery in the First Affiliated Hospital of Guangxi Medical University due to stroke from March 2012 to April 2018 were retrospectively analyzed. According to whether or not mild hypothermia was provided, they were divided into the normal thermic group (NT group) and mild hypothermia treatment group (MHT group). The MHT group patients were matched with the NT group patients by the propensity score matching method at a ratio of 1∶1. The observation period was within the first 7 days after admission. Baseline characteristics including age, gender, type of stroke, comorbidities, acute physiology and chronic health evaluation Ⅱ(APACHEⅡ) score and Glasgow coma score (GCS) on admission, surgical operation, dysphagia, invasive procedures and outcomes of these patients had been analyzed. The primary outcome was incidence of post-stroke infection, and the secondary outcomes included the time of initial infection (TII, the duration from stroke to initial infection), hospital mortality, sequential organ failure assessment (SOFA) at discharge, incidence of complications such as arrhythmia, coagulation dysfunction and multiple organ dysfunction syndrome (MODS).
Results:
201 stroke patients were enrolled, 41.8% (84/201) of whom underwent mild hypothermia. Comparison with NT group before matching, there were more males in MHT group (71.4% vs. 56.4%), the proportion of surgical operation, mechanical ventilation, deep vein catheterization and gastric catheterization were higher (78.6% vs. 54.7%, 84.5% vs. 39.3%, 90.5% vs. 37.6%, 98.8% vs. 70.9%), and so as incidence of infection (90.5% vs. 72.6%), in-hospital mortality (27.4% vs. 12.8%) and TII [hours: 62.00 (35.25, 93.00) vs. 42.00 (28.50, 69.50)].
