1.Clinical Observation of Xingpi Yang'er Granule in the Treatment of Primary Nocturnal Enuresis in Chil-dren

OBJECTIVE:To observe the efficacy and safety of Xingpi yang'er granule in the treatment of primary nocturnal en-uresis in children. METHODS:97 children with primary nocturnal enuresis were randomly divided into control group (46 cases) and observation group(51 cases). Control group received Racemic anisodamine tablet 0.5 mg/(kg.d),orally,once a day+Vitamin B1 tablet 10 mg,orally,3 times a day+Oryzanol tablet 10 mg,orally,3 times a day + Meclofenac hydrochloride capsule 0.1 g 30 min before going to bed,orally,once a day. Observation group was additionally received Xingpi yang'er granule 0.4 g for 5-6 years old and 0.6 g for 7-14 years old,3 times a day. They were treated for 4 weeks. Clinical efficacy,kidney Yang deficiency score,wake-up threshold,bladder capacity,bladder volume,BVWI,enuresis frequency,AVP,cAMP,cGMP,cAMP/cGMP be-fore and after treatment,recurrence rate and the incidence of adverse reactions in 2 groups were observed. RESULTS:The total ef-fective rate in observation group was significantly higher than control group(P<0.01). After treatment,the kidney Yang deficiency score,wake-up threshold,BVWI,enuresis frequency and cGMP in 2 groups were significantly lower than before,and observation group was significantly lower than control group;bladder capacity,serum AVP,cAMP and cAMP/cGMP in 2 groups were signifi-cantly higher than before,and observation group was significantly higher than control group(P<0.01). After 3 months,there was no significant difference in the recurrence rate in total effective children(P>0.05);after 6 months,the total effective rate recur-rence rate in observation group was significantly lower than control group(P<0.01). And there was no significant difference in the incidence of adverse reactions(P>0.05). CONCLUSIONS:Based on conventional treatment,Xingpi yang'er granule shows good efficacy and safety in the treatment of primary nocturnal enuresis in children,which can improve children's kidney Yang deficiency score,wake-up threshold and bladder capacity and serum AVP.
2.Displacement of steel plate for humeral fractures under the load of 100 N and 500 N measured using digital speckle method

BACKGROUND:There is no unified measure of bone tissue mechanical property test at present. The study concerning traditional sensor for fracture displacement had some problems such as low precision, and high consumption cost.
OBJECTIVE:To measure the displacement of plate and screw after internal fixation for fracture of humerus using digital speckle method.
METHODS:A total of eight humeral specimens were taken. The steel plate fixation model in the humerus 1/2 was made. The specimens were fixed using eight-hole steel plate, and four screws were fixed on each end of fracture line. Five conditions of experimental model were designed for comparative analysis. Condition a:compression plate fixation group (not saw off, to simulate fracture healing). Condition b:on the basis of sawing off in condition a, one screw was removed on the proximal end. Condition c:on the basis of condition b, one screw was removed on the distal end. Condition d:on the basis of condition c, one screw was removed on the distal end. Condition e:on the basis of d, one screw was removed on the distal end. The screws were numbered No. 1-8 from top to bottom. That is, the screws on upper fracture line were numbered No. 1-4, and those on the lower fracture line were numbered No. 5-8. The specimens were instal ed on electronic universal testing machine, and loaded 100 N and 500 N. The displacement was calculated using the related software.
RESULTS AND CONCLUSION:Significant differences in total displacement were detected under different loads (F=49.155, P<0.001). With increased load, the total displacement of the fourth and fifth screws gradual y increased under five kinds of conditions. These indicated that the two screws on the two ends of the fracture line endured more stresses (stress concentration), and easily broke. It would be better to choose the screw with the screw diameter of 1-2.5 mm bigger than the present one in order to increase its stability to avoid the breakage of screws.
3.Correlative analysis of post-stroke depression and pathogenesis in elderly patients with ischemic stroke

Objective To explore the predictive value of brain lobe location of stroke lesion to development of long-term post-stroke depression(PSD)in the first-episode ischemic stroke patients for providing evidence for early intervention.Methods In the prospective study,158 patients aged 60 and over with first-episode ischemic stroke were continuously admitted into Department of Neurology of Shangqiu First People's Hospital from January 2013 to July 2013.The 2 to 3 years follow-up after stroke episode was performed in 126 cases for inquiring into correlation between brain lobe location of stroke lesion and development of PSD.The diagnosis of depression was in accordance with Mental Disorders Diagnostic and Statistical Manual 4(DSM-Ⅳ)standard,and divided into groups of stroke with depression(n=52)and stroke without depression(n=74).The degree of depression was evaluated by 17-item Hamilton Rating Scale for Depression(HAMD17).The location,number and volume of stroke lesions were determined by head MRI.The relationship between PSD and pathogenetic loci was analyzed by unconditional Logistic regression analysis.A difference in hemisphere commensalism.Results The morbidity of PSD was 41.3%(52/126),with 21.4%(27/126),12.7%(16/126),7.1%(9/126)in mild,moderate and severe PSD respectively.The frontal lobe(OR=2.824,95%CI=1.189-6.706)and the temporal lobe(OR=3.579,95%CI=1.233-10.393)cerebral infarction were correlated with the occurrence of PSD.The long-term PSD severity was more in frontal lobe than in temporal lobe(χ2=6.399,P<0.05).The average volume of cerebral infarction was larger in PSD group than in non-PSD group(t=3.271,P<0.05),and the average number of cerebral infarction loci was more in PSD group than in non-PSD group(t=3.176,P<0.05).The more severe the degree of depression according to HAMD17,the larger the average volume of cerebral infarction(F=6.280,P<0.05)and the more the average number of lesions(F=6.132,P<0.05).Conclusions The development of long-term PSD in the first-episode elderly patients is affected by the invasion site.The frontal lobe and temporal lobe infarction are independent risk factors for long-term PSD in patients with ischemic stroke,and the PSD was more severe in frontal lobe infarction than in temporal lobe infarction.
4.HLA-G 14 bp gene tolvmorthism in relation to Epstein-Barr virus infection in children

Objective To explore the relationship between the HLA-G 14 bp insertion/deletion polymorphism and the infection of Epstein-Barr virus(EBV) for children.Methods The study genotyped HLA-G 14 bp insertion/deletion polymorphism of 102 infectious mononucleosis children and 165 normal controls by PCR-PAGE,detected the plasma sHLA-G level of 51 infectious mononucleosis children and 146 normal controls by ELISA.Results A significant difference was observed for the frequencies of the HLA-G 14 bp genotype between the two groups( x2 =6.742,P=0.034 ),and a significant difference was also observed for the 14 bp allele frequencies between the two groups( x2 =6.672,P=0.01 ).The plasma sHLA-G levels in the infectious mononucleosis children were dramatically higher than that in normal controls,and a significant difference was observed between the two groups( Z=-9.472,P<0.01 ).Among the infectious mononucleosis children,levels of sHLA-G was find a significant difference between the three genotypes of HLA-G 14 bp insertion/deletion polymorphism( H=6.09,P =0.048 ),and the level of s HLA-G with 14 bp+/+ genotype was markedly lower than that of the two other genotypes (Z=-2.376,P=0.01 8).Conclusion There was a relationship between the HLA-G 14 bp insertion/deletion polymorphism and the susceptibility to the infectious mononucleosis for children.Children who carried the 14 bp-/- genotype or deleted the 14 bp allele may have a significantly increased risk of the infection of EBV.The plasma sHLA-G might be considered as an index for auxiliary diagnosis infectious mononucleosis.
5.Family History and Stroke Risk in China: Evidence from a Large Cohort Study.

BACKGROUND AND PURPOSE: Large cohort studies on relationship between family history of stroke (FHS) and stroke risk are lacking in Asians. We aimed to systematically evaluate the association of FHS with stroke risk in a cohort study of 0.5 million Chinese adults. METHODS: Information about FHS was self-reported. The median follow-up time was 7.16 years and the end-point of follow-up was incident stroke, which was entered directly into the China Kadoorie Biobank system. Multivariate analyses were performed with Cox proportional hazards model, and interaction analyses were carried using likelihood-ratio tests. RESULTS: Compared with participants without FHS, the hazard ratio (HR) (95% confidence interval, CI) of stroke for participants with FHS was 1.50 (1.46-1.55). The HRs increased with the number of first degree relatives with stroke (HRs=1.41, 1.98 and 2.47 for 1, 2 and ≥3 relatives, respectively, P(trend) <0.001). The HRs were 1.57 (95% CI: 1.50-1.66) and 1.49 (95% CI: 1.45-1.54) for sibling history and parental history, respectively. Similar associations with offspring stroke risk were observed between paternal history (HR=1.48, 95% CI: 1.43-1.54) and maternal history (HR=1.49, 95% CI: 1.43-1.55). Moreover, significant interactions were detected between FHS and health-risk behaviors (tobacco smoking and alcohol drinking). CONCLUSIONS: FHS is an independent risk factor for stroke in Chinese. The more first degree relatives are affected by stroke, the higher are individuals’ risk of suffering from stroke. The management of the health-risk behaviors for reducing stroke should be highlighted, especially for the individuals with FHS.

6.Effect and mechanism of dexmedetomidine on lungs in patients of sepsis complicated with acute respiratory distress syndrome

Objective To investigate the mechanisms of protective effects of dexmedetomidine on lungs in patients of sepsis complicated with acute respiratory distress syndrome (ARDS). Methods The adult patients with sepsis complicated with ARDS, the oxygenation index (PaO2/FiO2) was 150-200 mmHg (1 mmHg = 0.133 kPa), acute physiology and chronic health evaluationⅡ (APACHEⅡ) score was 10-20, need mechanical ventilation (MV) treatment > 72 hours, and admitted to intensive care unit (ICU) of the First Affiliated Hospital of Guangxi Medical University from September 2013 to June 2017 were enrolled. According to the random number table method, the patients were divided into three groups (n = 80): no sedation group, propofol group (0.3-4.0 mg·kg-1·h-1) and dexmedetomidine group (0.2-0.7 μg·kg-1·h-1). The three groups were adequately analgesic treated with remifentanil. The sedation target was -1-0 of Richmond agitation-sedation score (RASS). The levels of interlenkin-6 (IL-6) and tumor necrosis factor-α (INF-α) were determined by enzyme linked immunosorbent assay (ELISA) before sedation, and 24, 48, 72 hours after sedation. The expressions of inflammatory signaling proteins in bronchoalveolar lavage fluid (BALF) were determined by Western Blot before sedation and 72 hours after sedation. Results There were no significant changes for inflammatory factors in serum, and inflammatory signaling proteins and anti-apoptotic signaling proteins in alveolar exfoliated cells in no sedation group. The levels of IL-6 and TNF-α in serum and the expressions of Toll-like receptor 4 (TLR4), myeloid differentiation factor 88 (MyD88) and phosphorylated c-Jun N-terminal kinase (p-JNK) in alveolar cells in propofol group and dexmedetomidine group were all significantly reduced after sedation, moreover, it was more significantly in the dexmedetomidine group compared with propofol group [48 hours: TNF-α (ng/L) was 153.76±29.16 vs. 179.82±30.28;72 hours: IL-6 (ng/L) was 272.18±42.76 vs. 304.49±44.93, TNF-α (ng/L) was 102.18±30.25 vs. 140.28±28.92, TLR4 (IA value) was 0.288±0.034 vs. 0.648±0.029, MyD88 (IA value): 0.356±0.030 vs. 0.752±0.044, p-JNK (IA value): 0.256±0.027 vs. 0.303±0.034, all 1 < 0.05]. The expression of p-Akt in alveolar cells in propofol group and dexmedetomidine group was all significant increased after sedation, moreover, it was more significantly in the dexmedetomidine group compared with propofol group (IA value: 1.032±0.030 vs. 0.743±0.028, 1 < 0.05). Conclusion Dexmedetomidine exerts the protective effects on lungs in patients of sepsis complicated with ARDS through the TLR4-MyD88-JNK signaling pathway.
7.Robot-assisted training can improve the bladder and intestinal functions of paraplegic patients

Objective To explore the clinical effect of training assisted by a lower limb rehabilitation robot on the bladder and intestinal function of paraplegic spinal cord injury survivors. Methods Thirty-eight paraplegic patients with spinal cord injury were divided according to their admission order into an experimental group ( n=19) and a control group (n=19). Both groups were given conventional rehabilitation training, while the experimental group was additionally provided with robot-assisted lower limb training in three stages:adaptation, training and con-solidation. It lasted 30 minutes daily, 5 days per week for 12 weeks. Before and after the training, an urodynamics examination system was used to evaluate the maximum urine flow, bladder capacity, residual urine volume, bladder pressure and detrusor pressure. Colon transit time, mean rectal pressure and intestinal function were measured using the colon transit test, a mean rectal pressure test, and the Functional Independence Measure ( FIM) scale respective-ly. Results The average bladder volume, maximum urine flow rate, average urine flow rate, detrusor pressure, bladder compliance, average rectal pressure and intestinal FIM score of the robot training group after training were all significantly better than before the training, as were the average residual urine volume and colon transit time. After the training, the average bladder volume, maximum urine flow rate, average urine flow rate, detrusor pressure, bladder compliance and average rectal pressure of the robot training group were all significantly higher than those of the control group, while the average residual urine volume and colon transit time were significantly smaller. Then, 32% of the patients in the experimental group achieved no less than 6 points for their average FIM score, significantly higher than in the control group. Conclusion Robot-assisted lower limb training combined with comprehensive rehabilitation training can effectively improve the bladder and intestinal function of paraplegic patients after a spinal cord injury.
8.Clinical outcomes in renal allograft recipients switched to long-term immunosuppressive therapy with mycophenolate mofetil after renal transplantation

Objective To document the impact of conversion to mycophenolate mofetil (MMF)at different time points after transplantation on the renal function of renal function.Methods A longterm,multicenter,non-interventional and observational study was done.Two cohorts were included:One was Switch cohort (340 cases) including renal allograft recipients who switched to MMF at least 6 months after renal transplantation and followed up for 4 years after switch; The other was Stay cohort (123 cases),including renal allograft recipients who received MMF treatment after transplantation and followed up for 4 years after enrollment.Results GFR values of patients in Switch cohort was significantly increased after switch,and the change in GFR slope was 3.1 mL· min-1 · year-1 (P<0.01).GFR values of patients in Stay cohort kept steady before and after enrollment,and the change in GFR slope was 0.44 mL·min-1 ·year-1 (P>0.05).Statistically significant difference in the onset time of GFR decline (defined as 20% decline from the baseline) was observed among subgroups within Switch cohort (P<0.01),but there was no significant difference among subgroups within Stay cohort (P>0.05).Stay cohort was 12% higher than in Switch cohort every year.Conclusion Conversion to MMF >6 months or even many years after transplantation can obviously improve the renal function of recipients.The earlier conversion can benefit improvement of the renal function.
9. Application of biological factor in cementum regeneration

At the present day, curettage and periodontal surgery comprise the main strategy for the treatment of periodontitis, however, these methods are limited in regenerating cementum. It has been found that some biological factors such asenamel matrix derivative (EMD), transforming growth factor-β (TGF-β) and insulin-like growth factor (IGF) could promote cementum regeneration. In the cementum regenerationstudies, there has been a lack of criteria to distinguish cementum from alveolar bone and other types of cementum. Therefore, this article will briefly review the biological factors that affect the cementum regeneration and the molecular markers used to judge the regenerating cementum.
10.Clinical study of Guishao Liujunzi Decoction with TCM mouthrinse for the treatment of Hp-associated gastritis

Objective To observe the clinical efficacy of Guishao Liujunzi Decoction combined with traditional Chinese medicine(TCM)mouthrinse in the treatment of helicobacter pylori(Hp)associated gastritis,and its effect on the Hp conversion rate.Methods Eighty patients with Hp-related gastritis were selected and randomly divided into the control group(standard quadruple therapy)and the treatment group(standard quadruple therapy combined with Guishao Liujunzi Decoction and TCM mouthrinse),with 40 patients in each group.The clinical efficacy,TCM syndrome scores,and Hp conversion rate of the two groups were observed.Results The total effective rate of the treatment group(95%)was higher than that of the control group(72.5%)(P<0.05).The Hp conversion rate in the treatment group(90%)was higher than that in the control group(72.5%)(P<0.05).After treatment,the main symptom scores and total scores of the two groups decreased compared to before treatment,and the treatment group was lower than the control group(P<0.05).Conclusion The combination of modified Guishao Liujunzi decoction and TCM mouthrinse in the treatment of Hp-related gastritis can significantly improve clinical efficacy and Hp negative conversion rate compared to conventional treatment.
