Peroxisome proliferators-activated receptor-gamma(PPAR?),one of members of the nuclear hormone receptor superfamily of ligand-activated transcription factors,may affect the ocurrence and development of atherosclerosis(AS) through effective modulate lipocyte differentiation,improve glucose and lipoprotein decompensation and control inflammation reaction and so on.This review will focus on the recently development that relation on PPAR? and AS.

Since the information supplied by the paternity testing of alleged parents was less than that of standard triplet parentage testing,so the paternity index (PI) calculating methods of standard triplet parentage testing was not suitable for calculating the PI value of alleged parents.In order to establish a more precise method for calculating PI value of alleged parents with STR typing results,the first thing is to summarize the standard triplet PI calculating formulas according to the Essen Mller theory.These formulas are 1/p,1/2p,1/p+q,1/2p+2q.This article reports a new PI calculating method in case of paternity testing of alleged parents.Compared with other methods,the new method for calculating Y value either considering random man and random female or considering the alleged father(mother)and random female(man).

Objective To observe the effects of White Mange Mixture on the content of VEGF and the genetic and protein expressions of VEGFR-2 of HaCaT cells;To explore its mechanism. Methods HaCaT cells were cultured in vitro to establish psoriasis experimental model. SD rats were randomly divided into the blank serum group, white mange mixture of low-, medium- and high- dose groups and acitretin group, meanwhile to set up blank control group. Cultured HaCaT cells model were intervened with different medicine serum. ELISA was applied to detect the content of VEGF. RT-PCR and Western blot were used to detect the genetic and protein expressions of VEGFR-2.Results Compared with the blank control group, White Mange Mixture of low-, medium- and high-dose groups interventing HaCaT cells could obviously inhibit the secretion of VEGF and decrease the genetic and protein expression of VEGFR-2 in a dose-dependent manner. Conclusion White Mange Mixture plays a role in the treatment of psoriasis possibly through inhibiting the content of VEGF and decreasing the genetic and protein expressions of VEGFR-2.

Objective To summarize the experience of endoscopic thyroidectomy for hyperthyroidism.Methods Endoscopic total or subtotal thyroidectomy was performed through anterior chest wall approach in 7 patients with primary or secondary hyperthyroidism.Results The operation was successfully performed in all the 7 patients.The operation time was 130~260 min(mean,168 min),and the intraoperative blood loss was 10~200 ml(mean,70 ml).No recurrent laryngeal nerve or superior laryngeal nerve injuries,or postoperative hemorrhage,or conversions to open surgery were encountered.The postoperative recovery was uneventful.Short-term follow-up observations demonstrated satisfactory cosmetic results and no recurrence.Hypothyroidism occurred in 2 patients and thyroid functions restored to normal levels in 1 of them 2 months after operation.Conclusions Endoscopic thyroidectomy is a safe and effective procedure for hyperthyroidism.Apart from conventional pre-operative preparation,CT examination is also necessary for identifying the measurements of thyroid glands and determining the proportion and location of residual glands.

Objective To investigate the feasibility and significance of laparoscopic choledocholithotomy with T-tube drainage. Methods A total of 60 cases of common bile duct stones complicated with gallbladder stones were treated with laparoscopic cholecystectomy and common bile duct exploration with T-tube drainage. Results The operation was performed successfully in all the cases,without conversions to open surgery.Bile leakage occurred in 2 cases and was cured with conservative treatment.Residual stones were found in 6 cases and a postoperative choledochoscopy was required.The operative time was 90~180 min(mean,110 min),the blood loss was 10~50 ml(mean,20 ml).Follow-up checkups in 53 cases(88.3%) for 2~33 months(mean,13.2 months) revealed no abdominal pain,fever,or jaundice.Conclusions Laparoscopic cholecystectomy and common bile duct exploration for the treatment of common bile duct stones complicated with gallbladder stones is feasible and minimally invasive.

Objective To compare the curative effect of tension-free hernioplasty and transabdominal preperitoneal herniorrhaphy(TAPP)in adult patients with inguinal hernia.Methods A prospective randomized controlled study.A total of 240 patients with inguinal hernia were randomly divided into TAPP group and tension-free hernioplasty group(open group),120 patients in each.Results In both the groups,the surgical treatments were completed.The mean operation time in TAPP group was significantly longer than that in open group [(77?15)min vs(48?9)min,t=18.147,P=0.000],and the average cost for surgery in TAPP group was significant higher than that in open group [(7363?351)yuan vs(4635?242)yuan,t=72.663,P=0.000].However,the patients in TAPP group returned to activity and was discharged earlier than those in the open group [(6?1)h vs(10?2)h,t=-19.385,P=0.000;and(3?1)d vs(5?1)d,t=-15.300,P=0.000;respectively].No significant difference was detected in the severity of postoperative pain,complications,and recurrence rate between the two groups.Conclusion Both TAPP and tension-free hernioplasty are worth being wildly used.

Objective To investigate the preventive and therapeutic effectiveness of Yisheng herbal injection against chronic allograft nephropathy in rats.Methods Renal transplantation was performed from SD to Wistar closed colony strain rats. Before the transplantation, the kidney was preserved in 0- 40 ℃ heparin sodium chloride solution for one h. The rats were divided into 4 groups: Group A only received CsA 10 mg/kg every day; Groups B, C and D received low dose of Yisheng+CsA, high dose of Yisheng+CsA and CsA+MMF, respectively. Objective To investigate the effect of different immunosuppresants on CAN by affecting TGF-?_ 1 and Smads signal pathway. Methods The SD→Wistar rat accelerated kidney sclerosis model was made following the procedure of Kamada with our modification. The rats were divided into 6 groups (each group n=8): Group A, psudo-OP group; Group B, control group; Group C, CsA 6 mg?kg~ -1?d~ -1; Group D, FK506 0.15 mg?kg~ -1?d~ -1; Group E, MMF 20 mg?kg~ -1?d~ -1; Group F, RPM 0.8 mg?kg~ -1?d~ -1. After 4, 8, 12 weeks, the animals were killed. The immunohistochemistry and real-time fluorescence quantitative PCR were used to detect localization and expression of TGF-?_ 1, Smad2 and smad7 in transplant kidney. Results CsA and FK506 simultaneously stimulated the gene expression and protein production of TGF-?_ 1 and Smad2 and inhibited the expression of Smad7 in transplant kidney, whereas MMF and RPM could down-regulate the gene expression and protein production of TGF-?_ 1 and smad2 and up-regulate the expression of smad7. There was no significant difference between CsA group and FK506 group, as well as MMF group and RPM group.Conclusion Different immunosuppresants can affect TGF-?_ 1 and Smads signal pathway in kidney graft. CsA and FK506 can cause CAN, owning to up-regulate the expression of Smad2, and down-regulate the Smad7 expression. MMF and RPM can prevent the progression of CAN because of down-regulating the expression of Smad2 and increasing Smad7 production.

Objective To analyze the reasons for accidental extubation in infants during mechanical ventilation and to explore effective intervention countermeasures.Methods A total of 148 infants with accidental extubation from May 2013 to February 2014 who given routine nursing care were selected as control group.Another 152 infants with accidental extubation in pediatric intensive care unit (PICU) who given optimization nursing care from March to December 2014 were selected as observation group.Retrospective analysis of accidental extubation,artificial airway way and intervention care were conducted.Results The total intratracheal tube time was 283 d in control group and 253 d in observation group.Nineteen cases (6.7％) suffered accidental exudation in control group and that of 8 cases(3.2％) in observation group,and there was significant difference between two groups,x2=5.25,P＜0.05.The primary reason of unplanned extubation was that infants with unconsciousness,the second cause was the improper catheter fixed.The third cause was the patient's comfort level and so on the improper nursing operation leading to accidental extubation too short or too shallow.Conclusion Infants given mechanical ventilation should pay more attention to the intervention of accidental extubation,appropriate calm,physical constraints and correct fixed pipeline.
