1.Effect of epidural versus epidural anesthesia in cesarean section

Objective To explore effects of spinal-epidural anesthesia combined with epidural anesthesia for cesarean section anesthesia and hemodynamic maternal.Methods 122 cases of mothers for cesarean section surgery in Lyuliang Municipal People's Hospital from January 2012 to January 2014 were selected,and they were divided into two groups by randomized single-blind method.The control group (61 cases) was given simple subdural anesthesia (EA),while the observation group was given CSEA.The maternal hemodynamics,efficacy of the anesthesia and adverse reactions were compared between the two groups.Results The onset time of anesthesia,maximum block level,amount of anesthetic in the observation group were (3.87 ± 0.25) min,(5.04 ± 0.41) T,(6.87 ± 0.92) mL,which were significantly reduced compared with the control group (t =8.55,8.03,6.45,all P < 0.05),and the duration of anesthesia was (116.76 ± 7.89) min,which was significantly increased compared with the control group (t =7.93,P<0.05).The MAP and HR of observation group in T1,T2,T3 showed no significant differences compared with T0 (P > 0.05);while the control group of patients with the MAP at T2 had obvious reduction compared with T0 (t =8.34,P < 0.05),T2,T3 compared with T0 decreased significantly (t =7.87,7.56,all P < 0.05).The incidence rate of adverse reaction in the observation group was 9.84%,which was significantly lower than 24.59% of the control group,the difference was statistically significant (x2 =9.45,P < 0.05).Conclusion The lumbar epidural anesthesia for cesarean section anesthesia had significantly fewer adverse reactions,and it can remain stable maternal vital signs,which was an ideal cesarean for section anesthesia.
3.Value of Doppler sonographic features in diagnosis of ovarian neoplasms

Objective To observe and measure the Doppler sonographic features of benign and malignant ovarian neoplasms,which included morphology and hemodynamics of tumor angiogenesis,and to select the most significant features. Methods Ninety-five patients with one hundred and five ovarian tumors were examined preoperatively by power Doppler sonography to observe the morphology of tumor angiogenesis. The hemodynamic features of tumor angiogenesis referred for pulsed Doppler sonography evaluation were measured,which included peak systolic velocity(V PS ),end diastolic velocity(V ED ),mean flow velocity(V M),pulsatility index(PI),resistance index(RI) and diastolic notch. To assess the variables jointly,stepwise regression analysis was used to identify the group of features that allowed the best prediction of benignity versus malignancy. Results All Doppler features significantly associated with malignancy .The ROC curve of RI revealed that the cutoff of RI( 0.51 ) with the highest accuracy had a sensitivity of 98.0 % and specificity of 96.2 %.Conclusions RI was most significantly associated with malignancy. The combination of RI and gray-scale sonography can maximize the ability to discriminate between benign and malignant ovarian tumors.
4.Comparative Analysis of Different Quantitative Methods for Baicalin in Qinghouyan Mixtures

Objective:To comparatively analyze the effect of different methods on the content determination of baicalin in Qing-houyan mixtures. Methods:The content of baicalin in Qinghouyan mixtures was quantitatively determined by orthogonal function spec-trophotometry and HPLC method, respectively. Results: The regression equation of the proposed method was obtained as follows:P2 =0. 091 7C-0. 005 9(r=0. 999 9). The linear relationship was good within the range of 0. 045-0. 225 mg/ml. The determina-tion results had no statistical difference from those of HPLC(P>0. 05). Conclusion:The orthogonal function spectrophotometry meth-od is simple, fast and efficient, which can be applied in the determination of the main active components in traditional Chinese medi-cine preparations.
5.Influence of gestational age and delivery mode on neonatal pulse oxygen saturation shortly after birth

Objective To explore the trend of preductal pulse oxygen saturation (SpO2) during 10 min after birth in newborns born at different gestational age and through different delivery mode.Methods From May 2014 to August 2014,319 neonates,born in Tianjin Central Hospital of Obstetrics and Gynecology and whose Apgar score at 1 min after birth ≥ 8,were included into the study.Term infants were grouped into vaginal delivery group (n=102) and caesarean group (n=117) according to the delivery mode.Infants born through caesarean section were divided into three groups,including 30-33+6 weeks (n=50),34-36+6 weeks (n=50) and 37-41+6 weeks (n=117) group according to the gestational age at birth.Immediately after the umbilical cord clamped,preductal SpO2 was recorded at each minute from one to ten minutes using the pulse oximetry.Oneway analysis of variance and two independent sample t test were applied for statistical analysis.Results At each time point within ten minutes after birth,the preductal SpO2 for preterm infants of 30-33+6 weeks gestation in the caesarean group were (54± 3)%,(59± 3)%,(65 ±4)%,(70±4)%,(75 ±4)%,(80±4)%,(84± 3)%,(89± 3)%,(91 ± 3)% and (93 ± 2)%,respectively,which were significant lower than those of the 34-36+6 weeks group [(57 ± 5)%,(66 ± 4)%,(72 ± 6)%,(78 ± 6)%,(83 ± 6)%,(87 ± 6)%,(90± 5)%,(92 ± 4)%,(94± 4)% and (95 ± 4)%,respectively] and of the term infants [(58 ± 3)%,(67 ± 4)%,(73 ± 5)%,(78 ± 6)%,(83 ± 6)%,(87 ± 6)%,(90± 5)%,(92± 5)%,(94± 3)% and (95± 3)%,respectively] (all P ≤ 0.05).While the corresponding preductal SpO2 for term infants in the vaginal delivery group were (61 ± 6)%,(69± 7)%,(75 ± 6)%,(81 ± 7)%,(86± 6)%,(90 ± 6)%,(93 ± 5)%,(94± 4)%,(95 ± 4)% and (96 ± 3)%,respectively,which were all higher than those of term infants in the caesarean group (all P ≤ 0.05).Conclusions The preductal SpO2 for preterm infants of 30-33+6 weeks of gestation is lower than that of preterm and term infants of ≥ 34 weeks of gestation at all time points within ten minutes after birth.The preductal SpO2 for term infants born through vaginal delivery is higher and rising faster than that of term infants born through caesarean section at all times.
6.Dual plane ultrasonography study of anal canal

Objective To identify anal canal structures by dual plane transrectal ultrasound. Methods One anorectal specimen was observed by ultrasonography when needles were placed in different anal muscle layers respectively. Fifty patients with no anorectal or perianal diseases were examined by transrectal ultrasound to verify the sonographic features of anal canal, meanwhile the inner and outer sphincters and longitudinal combination muscle were measured. The ultrasound images of anal canal were compared with MR images. Fifty patients were divided into three groups according to the age: 25- 40 years old, 41 -55 years old,and >55 years old. Results The sonographic features of anal canal structures by transrectal ultrasound were identified. Anal canal sonographic images and MR images had consistency. The thickness of inner sphincter in each group was (0.19 ± 0.03)cm, (0.22 ± 0.02)cm, (0.24 ± 0.03)cm respectively,while outer sphincter (0.44 ± 0.03)cm, (0.49 ± 0.04)cm, (0.52 ± 0.04)cm, and longitudinal combination muscle (0.10±0.02)cm,(0.11 ± 0.02)cm,(0.11 ± 0.02)cm. Conclusions Transrectal ultrasound is a simple,efficient and non-invasive method to examine anal canal which can identify anal canal structures clearly.
7.Contrast ultrasound in the assessment of renal cortical perfusion in rabbits

Objective To explore the value of the ultrasound contrast agent in conjunction with advanced contrast imaging techniques in the evaluation of the renal cortical perfusion. Methods The animal model was established with ligating renal artery to cause the renal cortical perfusion decrease of rabbits. Real-time harmonic gray scale imaging was performed to visualize the contrast enhancement of the renal cortex. After bolus injection of contrast agent SonoVue, dynamic image was observed and recorded during the first three minutes. According to the time-intensity curve (TIC), accelerating time (AT),Peak intensity (A), time to half of peak intensity (T), difference between peak and base (PBD), slope rate of the curve (β) and the product of A and β (A·β) were obtained before and after the renal artery operation. The t test and correlation analysis was used to examine the above parameters. Results Before the renal artery operation, renal cortex enhanced rapidly and obviously, the way of the enhancement was renal artery-cortex-pyramids. After ligation of renal artery, AT prolonged evidently (P<0.001), A decreased obviously (P<0.001), T obviously shortened (P<0.001), PBD decreased markedly (P<0.001), while the decline of βinduced (P<0.002) and A·βobvious dropped(P<0.001). AT, BPD and A·βcorrelated with stenosis level of renal artery. Compared with 31%-69% group, AT prolonged evidently (P<0.001). A decreased obviously (P<0.029), PBD decreased obviously (P<0.001), A·β was dropped markedly (P<0.02) in 70%-93% stenosis group. Conclusions Contrast agent combining with developed contrast specific imaging is able to detect changes of renal cortical microcirculation sensitively with the help of analysis of TIC.
8.The establishment of the drug-resistant Candida albicans disseminated infected mice model

Objective Establishing the drug?resistant Candida albicans disseminative infected mice model for new drug screening. Methods The disseminative infected mouse model was generated by intravenously injecting a clinical Drug?resistant Candida albicans strain ( CaR) to immunosuppressive ICR mice. The features of model was evaluated by clinical symptom, survival condition, fungal burden in tissue, histopathology, cytokines assay and medication. Results After infected with CaR (0 day), the death of mice started at the first day, though, compared to clinical drug sensitive strain ( CaS) infected group, the difference of mortality rate in 16?day observation period was not significant in two groups (CaR, 90?7%;CaS, 86?2%, P =0?158), mice in CaR group died faster than those in CaS group at the early stage;On the fourth day of infection, Candida albicans could be detected in the different tissues, and we found fungal burden in kidney and brain was a significant difference. The typical granuloma caused by fungal infection was the main histopathological feature observed in the kidney, brain and heart. Cytokines in renal tissue were detected by flow cytometry, The changes of IL?1α,IL?6,TNF?αand IFN?γin kidney were significant. Compared with CaS group, IL?1 and IFN?γ were significantly higher and TNF?αdecreased significantly in CaR group. The mice of groups CaR and CaS were treated with 10 mg/kg fluconazole, the mortality rates were 83?3% and 37?5%, which have a significant difference. Conclusions In this study, we successfully established a drug?resistant Candida albicans disseminative infected mice model which is potential tool for the development of new anti?infectious agent.
9.Investigation on the natural infection of Theiler's murine encephalomyelitis virus and study on experimental infection of the virus in mice

Objective To investigate the natural infection of Theiler's murine encephalomyelitis virus (TMEV) in mice,and to survey the distribution of virus in tissues and the changes of serum antibody in the experimentally TMEV-infected mice.Methods Enzyme linked immunosorbent assay (ELISA) and fluorescence quantitative RT-PCR (qRT-PCR) assay were used to detect the antibody and nucleic acid of TMEV in clinical samples.These samples included SPF mice collected from Guangdong area in 2010-2015,mice obtained from a non-barrier laboratory rodent colony,and wild Rattus norvegicus live-trapped around the non-barrier laboratory rodent colony.36 ICR mice were intracerebrally inoculated with TMEV BeAn strain.The clinical signs of the animals were observed daily post-inoculation.Three mice were euthanatized at day 0,3,7,10,17,21,31,39 and 46 post-inoculation (dpi),respectively.Tissue and serum samples were collected for TMEV detection.Results The TMEV antibody and nucleic acid positive rates of SPF mice collected from Guangdong area in 2010-2015 were 5.29% (n=2834) and 27.27% (n=457),respectively.The TMEV antibody and nucleic acid positive rates of the mice obtained from a non-barrier laboratory rodent colony were 71.95% (n=82) and 53.66% (n=82),respectively.The TMEV nucleic acid positive rate of wild Rattus norvegicus was 25.93% (n=27).In the TMEV positive mice,only two mice showed obvious clinical symptoms.The cecal contents,feces and brain samples were the best candidates for qRT-PCR assay.The viral nucleic acid could be detected in the brain,heart,liver,lung and stomach of ICR mice at 3 dpi,but no viral nucleic acid was detected in the spleen,kidney,and cecum.The viruses in liver,heart,lungs and stomach were completely cleared at 10 dpi,and the viruses persisted in the brain throughout the experiment.The TMEV antibody could be detected at 7 dpi,and then the antibody positive rate reached 100% at 17 dpi.The antibody level increased gradually and maintained up to 46 days.ICR mice showed latent infection after TMEV inoculation,with no obvious symptoms and eye pathological changes.Conclusions The experimental mice and wild Rattus norvegicus in Guangdong area are both infected with TMEV,and the infection rate is high.The mice inoculated with TMEV BeAn strain show latent infection.The TMEV antibody produced in mice can be detected at 7 dpi and persisted until the end of the experiment.The viruses are found in the liver,heart,lung and stomach for a short time,but are persisted in the brain for a long time.There is a good consistency of TMEV detection between qRT-PCR and ELISA.The qRT-PCR assay can be used as a powerful complement method for the national standard of laboratory animals.
10.Establishment and analysis of a mouse model of Staphylococcus aureus-induced arthritis

Objective To establish and analyze a mouse model of Staphylococcus aureus?induced arthritis ( Staphy?lococcus aureus septic arthritis, SA) , and provide an animal model for arthritis mechanism research and drug development. Methods Mice were immunosuppressed with cyclophosphamide, then intravenously inoculated with Staphylococcus au?reus. The gross characteristics of the joints were observed, the arthritis indexes were analyzed, and the pathological scores of the model mice were evaluated. Results From the first day after bacterial inoculation, the mouse joints were swollen. Pathological examination revealed lesions varying from mild and disarranged joint synovial hyperplasia to synovial thickening and intra?articular invasion, and increased neutrophil infiltration. Conclusions A mouse model of Staphylococcus aureus?induced arthritis is successfully established in this study. This model can be developed in a relatively short time, can not only simulate the clinical symptoms and signs and disease progression of human arthritis, but also to a certain extent reflects the etiology, infection and immunological mechanisms of human arthritis.
