1.Clinical features of post-ictal headache in patients with epilepsy and the risk factors

Objective To determine the clinical characteristics of post ictal headache (PIH) and clinical risk factors associated with the occurrence of PIH in patients with localization related epilepsy.Methods The subjects were 47 patients with temporal lobe epilepsy (TLE),21 patients with occipital lobe epilepsy (OLE),and 29 patients with frontal lobe epilepsy (FLE).The subjects were directly asked whether headaches occurred just after seizures and its characteristics.Results The characteristics of PIH were:throbbing quality,which was exacerbated by coughing,vomiting,bending,and sudden head movement,accompanied by photophobia,phonophobia,and relieved by sleep.The incidence of PIH was 21 3% for TLE,61 9% for OLE,and 41 4% for FLE.The risk of PIH was significantly higher for OLE than for TLE or FLE,and for patients with generalized tonic clonic seizures.Younger onset age of epilepsy was also a risk factor for PIH.Conclusions PIH possess some characteristics similar to migraine.The occurrence of PIH may be related to the region of epileptic focus and the region of the spread of epileptic discharges.
2.Effects of Dengzhan Shengmai capsule on renovascular hypertension-induced β-amyloid protein deposition and cognitive impairment in rats

Objective To observe the effects of Dengzhan Shengmai capsule on β-amyloid protein (Aβ) deposition in the brain and learning and memory function in renovascular hypertensive rats (RHRSP).Methods Male Sprague-Dawley rats were randomly divided into four groups (5 rats per group):normal group,shamoperated group,hypertension with Dengzhan Shengmai capsule treatment group and hypertension with normal saline (NS) treatment group.Renovascular hypertensive models were created by clipping two-kidney.Dengzhan Shengmai capsules were dissolved in sterile 0.9% NS and were administered (20 mg · kg-1 per day) by daily gavage for 4 weeks.In the NS group,hypertensive rats were given saline in the same volume.Immunofluorescent labeling and western blot were used to detect the expression of Aβ,NF-κB,IL-1β,TNF-α in the brain,respectively.Learning and memory function were detected by Morris water maze.Results RHRSP significantly increased Aβ deposition in the cerebral cortex and impaired memory function in rats.Dengzhan Shengmai capsule treatment significantly lowered the blood pressure compared with NS treatment((157.45±11.58) mmHgvs (197.76±10.12) mmHg).In addition,the levels of Aβ,NF-κB p65,IL-1β,TNF-α protein were significantly reduced,by Dengzhan Shengmai caspule treatment.The escape latency was shortened((24.64±4.57) s vs (37.17±3.87)s),while the frequency of passing through the platform quadrant(5.39±0.12 vs 3.05±0.28) and the dwell time((27.34±3.67) s vs (16.83±5.76)s) (all P<0.01) in the platform quadrant were significantly increased by Dengzhan Shengmai capsule treatment.Conclusions Dengzhan Shengmai capsule may reduce Aβ deposition in brain and improve learning and memory function by anti-inflammatory effects in RHRSP.
3.Optimization of extraction technology of the polysaccharides from sporophyll of Undaria pinnatifida Suringar by orthogonal design

Objective To study the extraction technology of the polysaccharides from Sporophyll of Undaria pinnatifida Suringar and find out the optimum extraction condition.Methods The extraction technology was studied by the orthogonal design.Results The optimum extraction conditions were to add 50 times of water to the Undaria pinnatifida Suringar material at 90℃ for 4 hours.Conclusion This optimum extraction technology based on the orthogonal design was confirmed by the experiment and was available in the future research.
4.Determination of polysaccharide from Ecklonia kurome

Objective To develop a stable and simple method to measure the polysaccharide in Ecklonia kurome Okam.Methods After the conversion coefficient of polysaccharide from Ecklonia kurome Okam to glucose was obtained,the content of polysaccharide was measured by phenol-sulfuric acid method.Results The correlation coefficient of standard curve was(0.9998),the recovery rate was 97.35% and RSD was 4.68%.Conclusion The method was convenient for content determination of polysaccharide from Ecklonia kurome Okam as well as its related products.
5.Clinical analysis of 502 cases of subarachnoid hemorrhage

Objective To summarize the clinical and imaging features of subarachnoid hemorrhage(SAH) and its complications and prognosis. Methods The clinical data of 502 cases of SAH were retrospectively analyzed and summarized. Results There were 84.3 % (423/496) patients sufferring from intense headache, dizziness or vertigo (13. 1%, 66/496 ), disturbance of consciousness ( 27. 5%, 138/496 ), partial neurologic impairment (58.2% ,292/496) ,meningeal irritation sign(72.5% ,364/496). The positive rate of digital subtraction angiogra-phy was 54.1% (93/172). Aneurysm was the common reason of SAH (20. 1%, 101/496). Prognosis in most pa-tients was better,but the old patients suffered from high mortality. Conclusions The cause and clinical manifesta-tion of SAH varies,cerebral angiography often could help find the cause of SAH. Most SAH patients recover well.
6.Efficacy, Prognosis and Safety of Decitabine Combined with Low-Dose Cytarabine in the Treatment of Elderly Patients with Relapsed/Refractory Acute Myeloid Leukemia.

OBJECTIVE:
To investigate the efficacy, prognosis and safety of decitabine combined with low-dose CAG regimen in the treatment of elderly patients with acute myeloid leukemia (AML).
METHODS:
The clinical data of 40 elderly patients with relapsed/refractory AML (69-85 years old) admitted to our hospital from January 2014 to August 2016 were analyzed retrospectively. 40 patients were divided into combination therapy group and CAG group according to different treatment methods. 20 patients of the combination therepy group were treated with decitabine combined with low-dose CAG (decitabine, 15 mg/m, d 1; aclarithromycin, 10 mg/m, d 3-6; Cytidine, 10 mg/m, d 1-14; recombinant human granulocyte macrophage colony-stimulating factor (G-CSF) for injection, 200 μg/(m·d), d 1-14). 20 patients of CAG group were treated by the standard CAG protocol (acralmycin 20 mg/m, d 1-4; cytarabine for injection, 15 mg/m, d 1-14; G-CSF 400 μg/(m·d), d 1-14). One course of treatment lasted for 2 weeks, after 2 courses of continuous medication, the complete remission rate (CR), overall remission rate (ORR), overall survival (OS), 1-year survival rate, hemoglobin, white blood cells, platelets improvement, and incidence of adverse reactions were compared.
RESULTS:
In combination therapy group the CR was 55.00% (11/20), OR was 85.00% (17/20), but in the CAG group CR was 30.00% (6/20), and OR was 50.00% (10/20). Till to February 2018, out of 40 patients 17 survived, 20 died, and 3 failed to be followed-up. The median follow-up time was 12 (2 to 35) months; the median survival time in the comtination therapy group was 13 (2-35) months, and the 1-year OS rate was 70.00%, and the median survival time of the CAG group was 10 (2-31) months, and the 1-year OS rate was 50.00%, without staistical significance between the 2 groups (P>0.05). After treatment, the WBC and Plt counts in the combination therapy group were higher than those in the CAG group, but the Hb level was lower than that in the CAG group with statistically significant difference (P<0.05). In the combination therapy group, the incidence of lung infection, nausea and vomiting was higher than that of the CAG group (65.00% vs 25.00%, 50.00% vs 20.00%), with statistically significant difference (P<0.05).
CONCLUSION
Decitabine combined with low-dose CAG regimen is effective for the treatment of relapsed/refractory AML in the elderly. Compared with the standard CAG regimen, the long-term efficacy of this regimen is not different significantly, but its adverse reactions are increase, thus the preventive treatment should be given in time.

7.Comparing between CAUP and UPPP in treatment of OSAHS.

OBJECTIVE:
To evaluate the prospective effect of CAUP and applying midline partial glossectomy with UPPP in the treatment of OSAHS.
METHOD:
One hundred OSAHS patients were distributed into two groups averagely. The patients of the two groups had CAUP and midline partial glossectomy respectively. All patients were followed-up 6 months and 12 months after operation and carried out using PSG every time.
RESULT:
Therapeutic effect was evaluated by standard of Hangzhou (2002). In general, in 6 months, curative effect was excellent in 41 patients of the group of CAUP, good in 8 patients with effect, bed effect in 1 patients; in 12 months, it was excellent in 28 patients, good in 13 patients, bed in 9 patients. In 6 months, the curative effect the other group were excellent in 44 patients, good in 5 patients, bed in 1 patients; In 12 months, it was excellent in 43 patients , good in 6 patients, bed in 1 patients. In 6 month, the results showed that the effect of the two methods was similar (P > 0.05), but in 12 months the effect of group of CAUP was better than the other obviously (P < 0.01).
CONCLUSION
This operation could enlarge the narrow area of palatopharyngeal cavity effectively and decrease postoperative complications. It's a safe effective and acceptable surgical procedure.

8. Relationship between expression of BRAFV600E and Ki67 protein and clinical pathologic characteristics of thyroid carcinoma

Objective:
To explore the relationship between BRAFV600E, Ki67 protein and thyroid carcinoma with different pathologic characteristics, thus to provide clinical evidence on early prognosis and personalized treatment in patients with thyroid carcinoma.
Methods:
From January 2015 to January 2017, 76 patients diagnosed as papillary thyroid carcinoma(PTC), who treated in Yinzhou People's Hospital, were enrolled.Twenty cases with normal tumor-adjacent tissue after operation and 34 patients with non-PTC were also enrolled as control.All the patients enrolled were not treated with endocrine, chemical and radiotherapy.The expression of BRAFV600E and Ki67 protein was detected by immunohistochemical method, and the BRAFV600E and Ki67 expression and their correlation with different pathologic characteristic of thyroid carcinoma was analyzed by SPSS 18.0.
Results:
The positive rate of BRAFV600E and Ki67 protein in 110 patients with thyroid carcinoma was 56.36%(62 cases) and 39.09%(43 cases), with the highest expression of BRAFV600E and the lowest expression of Ki67 in patients with PTC respectively.The expression of BRAFV600E and Ki67 protein was all negative in normal tumor-adjacent tissue.The positive expression of BRAFV600E and Ki67 in patients with PTC demonstrated significant difference in TNM stage, tumor side and lymphoid node metastasis(BRAFV600E: χ2=5.281, 9.771, 9.771, all
9.DNA methylation-mediated epigenetic silencing of miR-720 contributes to leukemogenesis in acute myeloid leukemia.

OBJECTIVETo investigate the expression level and regulation mechanism of miR-720 as well as the association of miR-720 expression with leukemia biological characteristics.
METHODSExpression and promoter methylation of miR-720 were determined by quantitive PCR and pyrosequencing in 38 patients with AML and 20 normal controls. Lentivirous-mediated miR-702 overexpression was constructed in AML cell line kasumi-1. The cell proliferation, apoptosis, cycle, colony formation, migration and P53-mediated apoptosis pathway were determined.
RESULTSAML patients showed significantly lower miR-720 expression compared with normal controls (0.69±0.09 vs 3.00±0.46, P<0.01); The methylation level of miR-720 promoter region in AML patients were significantly higher than normal controls [(75.56±2.35)% vs (47.65±2.78)%, P<0.01]. miR-720 overexpression in kasumi-1 cells induced significantly increased cell apoptosis (P=0.017), elevated apoptosis sensitivity to etoposide (P=0.004), and reduced cell proliferation (P<0.01). miR-720 overexpression also induced reduced colony formation (P=0.005), cell cycle arrest in G(1)/G(0) phase and decreased migration ability in kasumi-1 cells. In addition, overexpression of miR-720 significantly induced increased cell apoptosis-related proteins including P53 and Bax, and activation of NF-κB signal transduction pathway. After kasumi-1 cells were treated with 1uM decitabine for 48 hours, miR-720 promoter methylation reduced significantly, and miR-720 expression significantly increased.
CONCLUSIONThe expression of miR-720 in AML patients reduced significantly, and DNA methylation-mediated epigenetic silencing of miR-720 contributed to maintain the malignant characteristics of AML.

10.Expression of BCR/ABL fusion gene in circulating endothelial cells from chronic myelogenous leukemia patients and its clinical significance.

Several studies have shown that the tumor endothelial cells are different from the normal tissue endothelial cells. These tumor endothelial cells may contribute to tumor neo-vasculogenesis. This study was purposed to analyze the biologic features and determine the expression level of CD133 and BCR/ABL fusion gene in circulating endothelial cells (CEC) isolated from peripheral blood of CML patients, as well as to investigate the role of CEC in disease progression. Mononuclear cells were isolated from peripheral blood by density gradient centrifugation; CEC were sorted by MACS and harvested in the endothelial growth medium. The morphologic features of CEC were observed by microscopy, the cell growth rate was calculated by cell counting, and the cells were identified by immunofluorescence staining for the expression of CD31,CD34,VWF and CD133. The expression of BCR/ABL fusion gene was examined by FISH in 12 CML patients. The results indicated that the isolated CEC displayed the typical cobble-stone morphology. These cells could be identified by the positive immunofluorescence staining for CD31, CD34 and VWF, and showed more increased proliferative potential as compared to that of healthy donors. It was found that the positive rate of CD133 was 31.29% in CML patients, which was significantly different from that of healthy donors (P < 0.05). In 12 CML patients, CEC carried the same chromosome aberration as the leukemia cells (10.77%). Higher expression level of CD133 and BCR/ABL fusion gene positively correlated with progression of disease. It is concluded that the CEC may participate in invasion and angiogenesis in patients with CML and possibly correlate to the spreading and progression of the disease.
