1.The effect of propofol on interleukin-6 induced inhibition of neutrophil apoptosis

Objective To study the inhibition of neutrophil apoptosis induced by interleukin-6 (IL-6) and evaluate the effect of propofol on IL-6 induced inhibition of neutrophils apoptosis. Methods Neutrophils were isolated from 20 healthy volunteers( 20-55 yr) and incubated in RPMI1640 cell culture medium . Neutrophils from each volunteer were divided into 6 parts and incubated with NS (group Ⅰ -control), IL-6(group Ⅱ), propofol( group Ⅲ), intralipid (the lipid vehicle of propofol-group Ⅳ ), IL-6 + propofol(group Ⅴ), IL-6 + intralipid(group Ⅵ ) respectively. After 24 h of incubation, neutrophils were stained with acridine orange and ethidium bromide and observed under fluorescence microscope. 2.Analysis of ERG11 gene mutations in fluconazole-resistant Candida glabrata strains

Objective To study the mutations of ERG11 gene which encodes P450 lanosterol 14-α demethylase, and to explore the possible role of ERG11 gene in inducing fluconazole resistance in Candida glabrata. Methods ERG11 genes of 9 fluconazole-resistant Candida glabrata isolates and 10 fluconazole-sensitive Candida glabrata isolates were cloned into pUC57-T vector. The open reading frame of ERG11 gene were sequenced by two directional sequencing using universal primers. All sequences were compared with the published sequence. Results Ten kinds of synonymous point mutation were found. Neither missense mutation nor frame-shifting mutation was found. Among the 10 kinds of synonymous point mutation, 5 were found in both fluconazole-resistant and fluconazolesensitive Candida glabrata isolates, and 3 were only found in fluconazole-resistant isolates, 2 were only found in fluconazole-sensitive ones. The majority of the point mutations were located between 1320-2200 base pair of ERG11 gene. Conclusions There are ERG11 gene polymorphisms in clinical strains of Candida glabrata. ERG11 gene mutations are not found to be involved in the development of fluconazole resistance in Candida glabrata.
3.Laparoscopic surgery for middle or lower rectal cancer

Objective To compare and analyze the effect of laparoscopic surgery and open toperation of rectal can﹣cer in middle or lower positions. Methods 128 patients with middle or lower rectal cancer were randomly divided into observation group and control group, 64 cases in each one, Laparoscopic surgery for observation group and con﹣trol group using traditional open surgery. Operative time, blood loss, ambulation time, length of stay, postoperative complications, anus-preserving, the recurrence during follow-up were compared between the two groups. Results Blood loss, ambulation time, length of hospital stay, the observation group were significantly less than the control group (P< 0.05), operation time between the two groups have no obvious differences (P> 0.05). Nine patients in control group with complications, the rate was 14.1 %; postoperative complication were observed in 2 cases, the rate was 3.1 %; complication rate in observation group was significantly lower than the control group (P< 0.05). Anus-preserving rate was significantly higher in the observation group (P<0.05);followed up for 8-47 months with an av﹣erage follow-up (23.1 ± 6.4) months, relapse rates of the two groups have no significant difference (P> 0.05). Con﹣clusions Compared with open operation,laparoscopic surgery is safe and effective, can reduce the surgical trauma and complications, shorten hospitalization time, improve the anus-preserving rate, worth clinical promotion.
4.Orthodontic treatment of malocclusions by lower incisor extraction

Objective:To investigate the indications of lower incisor extraction in orthodontic treatment of malocclusions. Methods:42 patients (males 16,females 26) were divided into four groups: abnormal Bolton index; class III malocclusions with anterior teeth cross bite; one congenital lower incisor losing; and anterior teeth area periodontal disease.One lower incisor extraction, one lower incisor and other teeth extraction and other accessorial therapy were used in the treatment. Results:According to these four different conditions, alternative treatment plans of lower incisor extraction should be determined. For the former two conditions, one lower incisor was extracted; A lower incisor and two maxillary first premolars were extracted in the third condition, in which lower canines and first premolars should be grinded to improve esthetics; In the last condition, some maxillary teeth need stripping besides extracting one lower incisor. All of the 42 cases achieved satisfactory results, normal overbite and overjet and stable occlusal relationship. Conclusion:To be well acquainted with the indications of the extraction of the lower incisor in orthodontic treatment of malocclusions can help to achieve satisfactory clinical effect.
5.The protective effects of verapamil and SOD on ESW-induced kidney damage

Objective To investigate the protective effects of calcium channel blockade and oxygen free radical catalase on ESWL-induced kidney damage. Methods 120 female uninephrectomized rat models were randomizedly divided into six groups.After verapamil and(or) SOD injection,the kidneys were shocked 1 000 times at 14kV by ESW.Then ET level in kidney tissue,levels of NAG,MDA,ET in urine,variation of creatinine clearance (Ccr) and the kidney histopathological changes were evaluated. Results The variation of NAG and MDA level in urine and the Ccr in the therapeutic groups were significantly lower and the histopathological changes were also much slight than those in the control group. There was no significant difference of the biochemical and pathological changes when the dose of SOD increased from 20 thousand units to 40 thousand units per kg body weight. If verapamil was used, the levels of ET in kidney tissue and in urine both decreased.Combined use of SOD and verapamil can induce better protective effects. Conclusions Calcium channel blokade could decrease not only the degree of lipid overoxidation, but also the level of ET-relieve,so it can induce a marked protective effect on ESW-induced kidney damage and the effect could be much enhanced if combined with SOD.
6.Effect of orthodontic and surgical treatments of skeletal crossbite on the electromyographic activity of human masticatory muscles

Objective: To investigate the effect of orthodontic and surgical treatments of skeletal crossbite on the electromyographic activity of human masticatory muscles. Methods: Electromyogram was used to test the changes of the electromyographic activity of masseter muscles, temporal anterior and diagastric anterior in 20 patients with skeletal crossbite before and after treatments. Results: Orthodontic and surgical treatments showed different effect on the activity of masseter muscle, temporal anterior and diagastric anterior. The change of the activity of the masseter muscle was the most significant among them. But, during chewing the masseter muscle was less active after treatments. Conclusion: Abnormal activity of the masticatory muscles can be partly corrected by the orthodontic and surgical treatments. The treatments cannot fully recover the function of the masseter muscle.
7.The relative renal safety of iso-osmolar contrast media compared with low-osmolar contrast media in intravenous contrast-enhanced CT: a Meta-analysis

Objective To compare the nephrotoxicity of the iso-osmolar contrast media (iodixanol) to low-osmolar contrast media (LOCM) in intravenous contrast-enhanced CT.Methods Randomized controlled trials (RCTs) of iodixanol or low-osmolar contrast media in intravenous contrastenhanced CT were searched in the database of VIP,CBM,CNKI,Wanfang,PubMed,EMBASE,Web of Science,Cochrane Library from their start year to July 2012.Screening and information extracted were did by two researchers independently.The quality of the included documents was evaluated by the criterion of Cochrane handbook.Revman software (version 5.0) of the Cochrane collaboration was used in data analysis.Results There was no significant difference in the incidence of contrast-induced nephropathy (CIN) among 6 trials recruited 907 patients between the iodixanol group and the LOCM group [RR =0.64,95%CI (0.31-1.32),P=0.22] by using serum creatinine increased by more than 44 μmol/L (0.5 mg/dl) as the diagnostic criteria.No considerable difference was existed by using serum creatinine increased by more than 25% as the diagnostic criteria between the two groups [RR =0.79,95%CI (0.48-1.30),P =0.35].Subgroup analysis showed there was no obvious difference [RR =0.57,95%CI (0.30-1.10),P =0.09] between the two groups in patients with increased baseline of serum creatinine.No obvious difference were gained in normal baseline group [RR =1.28,95%CI (0.57-2.86),P =0.55].Conclusion Compared with low-osmolar contrast media,iodixanol is not associated with less CIN in intravenous contrast-enhanced CT.
8.Characteristics of opportunistic infection in hospitalized acquired immunodeficiency syndrome patients

Objective To investigate the characteristics of opportunistic infections in acquired immunodeficiency syndrome (AIDS) patients.Methods A comprehensive retrospective chart review study was conducted of 603 AIDS patients with different opportunistic infections in a special hospital authorized for AIDS patients.Results In all 603 patients,327 cases (54.2%) were infected with Pneumocystisjiroveci,251 cases (41.6%) were infected with fungus and 210 cases (34.8%) were infected with mycobacterium.Totally 367 cases (60.9%) had only one pathogen and 236 cases (39.1%) were co-infected with more than one pathogen.A total of 390 cases (64.7%) had a single site of infection with the respiratory tract being most frequently involved (271 cases,69.5%),and 213 cases (35.3%) had more than one infected site.The most common coinfected sites were oropharynx and respiratory tract (111 cases,52.1%).Conclusions AIDS patients are usually coinfected with multiple pathogenic organisms and multi-systems involved.Most of the opportunistic infections can be cured by timely diagnosis and effective treatment to prolong life and improve the quality of life of AIDS patients.
9.The tongue image in AIDS patients

We retrospectively reviewed the tongue images and related clinical data of 118 AIDS patients treated in Shanghai Public Health Clinical Center Affiliated to Fudan University. The tongue images included: cyanotic and purplish tongue ( 33, 28.0% ), light-reddish tongue ( 32, 27.1% ), light-whitish tongue (21, 17.8% ) , bulgy tongue(19, 16.1% ), dark-red tongue (7, 5.9% ) and fissured tongue (6,5.1% ). There were significant differences in counts of CD4+T lymphocytes, white blood cells and red blood cells among different tongue imagine groups. The tongue image can indicate the pathogenic factors of disease as well as the functions of viscera in AIDS patients.
10.Combination of gemcitabine with cisplatin in the treatment of advanced or metastatic transitional cell carcinoma of urinary tract (report of 18 cases)

ObjectiveTo evaluate the therapeutic effect and toxicities of gemcitabine combined with cisplatin in the treatment of advanced or metastatic transitional cell carcinoma (TCC) of urinary tract. Methods Eighteen patients with advanced or metastatic TCC of urinary tract (15 cases of bladder TCC,2 of renal pelvic TCC,and 1 of ureteral TCC),who were pathologically confirmed,were treated with GC regimen (gemcitabine 1000 mg/m 2,iv infusion on day 1 and day 8;cisplatin 30 mg/m 2,iv infusion on day 2,day 8 and day 9).Before treatment, Karnofsy score for each patient was evaluated with a result of ≥60;liver and renal functions and blood routine test were normal.GC regimen was repeated for 2-3 cycles every 3-4 weeks and the response rate was evaluated.ResultsThree patients (16.7%) had complete response; 7(38.9%), partial response;5(27.8%),no response;and 3(16.7%) had progression.The overall response rate was 55.6%. The main toxic effects included decrease in white cell count (10 cases),anemia (7),nausea and vomiting (10) and constipation (8),which were mild or moderate and disappeared after stopping treatment.No chemotherapy-associated death occurred.ConclusionsGemcitabine combined with cisplatin in the treatment of advanced or metastatic transitional cell carcinoma of urinary tract is effective,and the toxicity is mild and tolerated. It is suggested that GC regimen can be used as the first line chemotherapy.
