1.Survey on expense of traditional Chinese medicine in the new rural cooperative medical system

Objective To verify that traditional Chinese medicines have certain price advantages compared with western medicines,and to estimate the rational increase in the compensation proportion of traditional Chinese medicine in the new rural cooperative medical system.Methods Using stratified sampling survey and literature review,the average outpatient and hospitalization expenses in the treatment of common diseases in rural areas were calculated and compared among traditional Chinese medicine,western medicine,and integrated traditional and western medicine.Results For average outpatient and hospitalization expenses,traditional Chinese medicine was lower than western medicine,integrated traditional and western medicine was lower than western medicine.and traditional Chinese medicine was lower than integrated traditional and western medicine.For hospitalization expense,traditional Chinese medicine had a 25.2%lower expense compared with western medicine.The ratios of expenses were statistically calculated,and the mathematical model for the compensation proportions of traditional Chinese medicine and western medicine in the new rural cooperative medical system was put forward.Conclusions In the implementation scheme of new rural cooperative medical system,the compensation proportion of traditional Chinese medicine in outpatient and hospitalization expenses should be increased by 25%and 33%respectively compared with western medicine.
2.The value of real-time ultrasonic elastography in rectal tumors

Objective To evaluate the value of real-time ultrasonic elastography in the differential diagnosis of rectal tumors.Methods One hundred patients (30 cases of adenoma and 70 cases of adenocarcinoma) with rectal tumors proved by pathology and 70 normal subjects referred to the First Affiliated Hospital of China Medical University from April 2012 to October 2013 were included in this prospective cohort. All patients underwent real-time ultrasonic elastography. The tumour tissue and reference tissue were chosen for strain ratio measurements. Only tumor tissue was selected for the tumor sample area. Tissue recognized as normal rectal wall and perirectal tissue was selected as the reference sample area. At the same time the elasticity score were calculated. Receiver operating characteristic (ROC) curves of the elasticity score and strain ratio method were used to find the cut-off point of adenoma and adenocarcinoma. The corresponding sensitivity, accuracy and specifi city were calculated. One-way ANOVA was used to comparestrain ratio value among healthy control group, retal adenoma group and rectal adenocarcinoma group and among patients with different preoperative stages of adenocarcinoma group. LSD-t test was used to compare strain ratio value between two groups.Results ROC curve showed that the best cut-off value of elasticity score in diagnosis of rectal tumor was 3, the sensitivity, specificity and accuracy were 85.1%, 73.1% and 82.0%, and the area under ROC curve was 0.780. The best cut-off value of strain ratio in diagnosis of rectal tumour was 2.34, the sensitivity, specifi city and accuracy in diagnosis were 91.4%, 83.3% and 89.0%, and the area under ROC curve was 0.945. Strain ratio of healthy control group, rectal adenoma group and rectal adenocarcinoma group was 0.74±0.44, 1.75±0.58 and 7.48±6.80. There was signifi cantly statistical difference among three groups in strain ratio. Compared with the strain ratio of healthy control group and rectal adenoma group, that of rectal adenocarcinoma group was higher (t=-8.26, P=0.000; t=-6.98,P=0.000). Compared with the strain ratio of healthy control group, that of rectal adenoma group was higher (t=-8.53,P=0.000). Strain ratio of patients with preoperative pathological stages T1, T2, T3 and T4 rectal carcinoma was 4.91±3.60, 7.07±7.23, 8.64±2.62 and 8.58±9.95 and there was no significantly statistical difference among patients with different preoperative stages of adenocarcinoma group (F=0.86,P=0.47).Conclusion The real-time ultrasonic elastography is a promising modalityfor the differential diagnosis of rectal tumors.
3.Effects and mechanisms of trophoblast cells autophagy in preeclampsia

Objective To investigate the occurrence law of autophagy in trophoblast cells from preeclampsia and its underlying mechanisms.Methods Twenty cases of placenta tissues were collected from women suffered from preeclampsia and normal pregnant women respectively.Autophagosome of trophoblast cells were observed by transmission electron microscope.The expressions of LC3-Ⅱ/Ⅰ and Atg4B in placenta tissues were detected by western blot and real-time PCR.Results Compared with the control group,typical autophagosomes of trophoblast cells were observed by transmission electron microscope.The ratio of LC3-Ⅱ / Ⅰ in placenta of PE patients was increased (1.43 ± 0.23) compared with control group (0.59 ±0.12),and the expression of Atg4B was up-regulated in both mRNA [(1.73 ±0.16) folds] and protein levels (0.71 ± 0.13) compared with control group (P < 0.05).Conclusions Autophagy was significantly up-regulated in trophoblast cells from patients suffered from preeclampsia.Thus,all the data suggest that autophagy might be involved in the generation of preeclampsia.
4.Analysis of the diagnosis and treatment of group B hemolytic streptococcus septicemia in 7 neonates

Objective To analyze the clinical characteristics and resistance of group B hemolytic streptococcus septicemia in neonates,and to provide the evidence for clinical treatment.Methods 7 neonates with group B hemolytic streptococcus septicemia hospitalized from Jan.2001 to Aug.2011 were enrolled in this study.The clinical data and laboratory results of antimicrobial sensitivity were analyzed retrospectively.Results There were 4 cases of early-onset infection and 3 cases of late-onset infection,6 cases with leukopenia,6 cases with c-reactive protein elevated,4 cases with suppurative meningitis,3 cases with septic shock and 2 cases with disseminated intravascular coagulation.All strains were sensitive to penicillin, vancomycin and linezolid.Conclusion Attention should be paid to neonatal group B hemolytic streptococcus septicemia.Penicillin and vancomycin are first choice for this type of infection.
5.Epidemiological and etiological characteristics of dengue fever in Hangzhou in 2018

Objective:To analyze the epidemiological and etiological characteristics of dengue fever in Hangzhou in 2018.Methods:RT-PCR was used to detect the nucleic acids and analyze the serotypes of dengue viruses (DENV) in serum samples collected from dengue fever cases. Phylogenetic trees based on the E gene sequences of DENV isolated from the serum samples were then constructed and analyzed. Epidemiological characteristics of these dengue fever cases were analyzed. Results:A total of 80 cases of dengue fever were detected in Hangzhou in 2018 with 55 imported cases and 25 indigenous cases (24 caused by DENV-1 and one by DENV-3). These indigenous cases mainly occurred during late July to early October with people above 50 years old accounting for 68%. Phylogenetic analysis showed that DENV-1 strains isolated from the indigenous cases in Yuhang, Jianggan-Shangcheng and Qiantang districts all belonged to genotype Ⅰ, and were respectively closely related to the strains from Indonesia in 2015, Myanmar in 2017, Ningbo in 2018 and Hangzhou imported cases from Thailand in 2018. The indigenous DENV-3 strain belonged to genotype Ⅲ, and shared 99.5% homology with the Singapore strain in 2013.Conclusions:Imported cases accounted for a large fraction of the dengue fever cases in Hangzhou, which brought a high risk to indigenous outbreak. Due to multiple imported cases, the current epidemic presented a characteristic of multiple small-scale outbreaks.
6.Tissue culture system of Cynanchum otophylum in genetic transformation

Objective To establish the tissue culture system of Cynanchum otophylum in genetic transformation. Methods Asepsis seedling was set up by using different explant parts, disinfectors, and disinfecting time. The callus was induced, breeded, and differentiated by using different media, different hormone categories and combinations, and different hormone concentrations of 2,4-D and KT. Results It was the best method that asepsis seeding was built up by using seed as the explant and using 10% NaClO to treat the seed for 20—30 min or using 0.2% HgCl2 to treat the stem for 3 min. The seed, root, stem, and leaf of C. otophylum can form callus easily. The 2,4-D was important in forming callus. C. otophylum was more sensitivity to KT. The effect was better when KT was used to induce callus or adventitious buds. MS+2,4-D 1.0 mg/L+KT 1.0 mg/L was the better medium to induce callus to form. The callus cant be induced to form adventitous buds without reference to using seed or root, stem, and leaf as the explant, without reference to using hormone 6-BA or KT, ZT, 2ip. Gentamicin 100 ?g/mL is the optimum pressure in genetic transformation of C. otophylum. Conclusion The seed or stem is the ideal material bombarded by particle gun that is induced to form callus for 14 d in the media MS+2, 4-D 1.0 mg/L+KT 1.0 mg/L and without forming adventitious buds.
7.Seizure-susceptible brain regions in patients with low-grade gliomas.

Patients afflicted with low-grade glioma frequently suffer from seizures. The mechanisms for seizure initiation in these patients remain poorly understood. Tumor location is correlated with seizure initiation. However, these correlative studies rely on dichotomized data analysis which is based on arbitrary lobe assignments. As a result, the lesion-symptom correlation may be incorrectly interpreted. Categorizing patients according to tumor involvement in a single brain lobe might cause the neglect of important information, such as lesion location and lesion volume. Tumors that invaded more than one brain lobe may could be counted repeatedly. The anatomic correlation of the tumor-induced seizures is therefore difficult to be identified. Investigations based on voxel-wise quantitative lesion analysis could avoid the above statistical bias. According to the voxel-wise analysis, the increased seizure risks were identified for patients with low-grade gliomas that involved the left premotor area.

8.Simultaneous determination of Forsythoside A and other four components in Xiao'er-Ganmao-Keli from multivendor by HPLC

Objective To establish a method for the simultaneous determination of Forsythoside A, Phillyrin, (R,S)-Epigoitrin, Chlorgenic Acid and Isochlorogenic Acid A by HPLC, and test 16 batches of samples from 14 manufacturers. Methods The test was performed on Kinetex EVO C18 column (150 mm × 4.6 mm, 5 μm) with the column temperature at 35 ℃ . The gradient elution was adopted with the mobile phase of acetonitrile and 0.3% phosphoric acid aqucous at a flow rate of 1.0 ml/min. The detection wavelength of (R,S)-Epigoitrin and Phillyrin were set as 236 nm, the detection wavelength of Forsythoside A, Cholorogenic Acid and Isochlorogenic Acid A were set as 327 nm. Results The good linear relationships were displayed within the linear range of 0.050 45-2.018 00 μg for Forsythoside A (r=0.999 9), 0.018 21-0.728 40 μg for Phillyrin (r=0.999 9), 0.010 16-0.406 40 μg for (R,S)-Epigoitrin (r=0.999 9), 0.006 60-0.263 90 μg for Cholorogenic Acid (r=0.999 9) and 0.0040 44~0.161 76 μg for Isochlorogenic Acid A ( r=0.999 5). The RSDs of reproducibility and stability tests were lower than 2%; recoveries were 97.01%, 98.28%, 99.35% and 96.21%, RSD were 3.19%, 1.19%, 0.81%, 2.88% and 2.96%. The content ranges of Forsythoside A, Phillyrin, (R,S)-Epigoitrin, Chlorgenic Acid and Isochlorogenic Acid A from 16 batches of samples from 14 manufacturers were 0.057 43-1.508 71 mg/g, 0.017 72-0.350 15 mg/g, 0.005 68-0.177 13 mg/g, 0.007 53-0.226 33 mg/g and 0.00308-0.11908 mg/g. Conclusions The established method is simple and accurate, and has a good repeatability. It can be used for the quality analysis of Forsythoside A, Phillyrin, (R,S)-Epigoitrin, Chlorgenic Acid and Isochlorogenic Acid A. The content of the tested chemical components from 16 batches of samples from 14 manufacturers have significant differences which indicate that a reinforcement of the quality control is needed.
9. Quantitative Assessment of Disease Severity of Ulcerative Colitis

Background: Assessment of disease severity of ulcerative colitis (UC) is crucial for the decision of drug treatment strategy, the appraisal of response to drugs, and the prognosis evaluation. Aims: To establish and validate a discriminant model for the quantification of disease severity of UC. Methods: The clinical data of 169 active UC cases admitted at the Beijing Haidian Hospital were analyzed retrospectively. Based on Truelove-Witts criteria, seven widely accepted indices including temperature, pulse, defecation frequency, hematochezia, hemoglobin (Hb), erythrocyte sedimentation rate (ESR) and albumin (Alb) were selected and a stepwise discriminant analysis was performed to establish a discriminant model using Fisher's linear discriminant function, and then the discriminant model was validated from different aspects. Objective To analyze the prevalence of influenza A (H3N2) virus in Hangzhou be-tween 2012 and 2017 and to investigate the genetic variations in hemagglutinin ( HA) and neuraminidase ( NA) . -ethods Throat swab samples were collected for viral isolation from 12185 patients with suspected influenza in Hangzhou area from January 2012 to December 2017. Influenza virus subtypes were identified by real-time RT-PCR. HA and NA genes of some isolated Influenza A (H3N2) viruses were amplified with spe-cific primers and then analyzed by sequencing and phylogenetic analysis. Results Influenza A (H3N2) virus was the predominant subtype circulating in Hangzhou during 2012 to 2017. It caused high morbidity in elderly people (Z=81. 039, P<0. 05). Most of the isolated influenza A (H3N2) viruses belonged to the phylogenetic clades of 3C. 3a and 3C. 2a. These viruses shared a homology of 96. 7%-100% in nucleotide sequences of both HA and NA genes, but possessed several HA and NA mutations in antigenic sites. Con-clusions Influenza A (H3N2) virus was an important pathogen causing influenza epidemics in Hangzhou during 2012 to 2017. HA and NA genes showed many mutations in antigenic sites. No drug resistant virus was reported.
